Fragen und Antworten zu invasiven Pneumokokkenerkrankungen bei Kindern und Jugendlichen

Background: S. pneumoniae is a major cause of meningitis, pneumonia and sepsis in children. In 2006 universal pneumococcal vaccination was recommended in Germany for all children up to their second birthday. We have compared the prevalence and outcome of IPD at a single hospital before and after the introduction of vaccination. Findings: 55 cases of IPD were identified over an 11 year period. Almost half of the patients were younger than 2 years of age. Most of the children were affected by pneumonia. The second highest incidence seen was for meningitis and sepsis. 17 patients exhibited additional complications. Significant pre-existing and predisposing disorders, such as IRAK 4 defect, ALPS or SLE were identified in 4 patients. Complete recovery was seen in 78% of affected children; 11% had a fatal outcome and 11% suffered from long term complications. Only 31% overall had been vaccinated. The most common serotype was 14. Serotypes not covered by any of the current vaccines were also found. Antibiotic treatment commenced with cephalosporins in over 90%. Conclusion: Frequency of IPD in our hospital did not decrease after initiation of the pneumococcal vaccination. This might be due to vaccinations not being administered satisfactorily as well as to poor education about the need of the vaccination. Pre-existing diseases must be monitored and treated accordingly and rare deficiencies taken into account when IPD takes a foudroyant course. In addition, antibiotic stewardship has been initiated at this hospital centre as a consequence of the high cephalosporin use detected in this study.

Highly efficient linear CMOS power amplifiers for wireless communications

The rapidly expanding wireless market requires low cost, high integration and high performance of wireless communication systems. CMOS technology provides benefits of cost effectiveness and higher levels of integration. However, the design of highly efficient linear CMOS power amplifier that meets the requirement of advanced communication standards is a challenging task because of the inherent difficulties in CMOS technology. The objective of this research is to realize PAs for wireless communication systems that overcoming the drawbacks of CMOS process, and to develop design approaches that satisfying the demands of the industry. In this dissertation, a cascode bias technique is proposed for improving linearity and reliability of the multi-stage cascode CMOS PA. In addition, to achieve load variation immunity characteristic and to enhance matching and stability, a fully-integrated balanced PA is implemented in a 0.18-m CMOS process. A triple-mode balanced PA using switched quadrature coupler is also proposed, and this work saved a large amount of quiescent current and further improved the efficiency in the back-off power. For the low losses and a high quality factor of passive output combining, a transformer-based quadrature coupler was implemented using integrated passive device (IPD) process. Various practical approaches for linear CMOS PA are suggested with the verified results, and they demonstrate the potential PA design approach for WCDMA applications using a standard CMOS technology.

Ce travail de thèse a porté sur la détection et localisation de défauts dans les réseaux de distribution HTA par les<br />indicateurs de passage de défaut (IPD). Les études ont été effectuées dans le cadre du développement attendu et<br />croissant des GED (sources de Génération d'Energie Décentralisées).<br />La première partie du mémoire est consacrée à l'analyse du comportement des IPD. En ce qui concerne<br />l'influence du contexte de fonctionnement sur la réponse des IPD, une partie est destinée à vérifier le<br />fonctionnement des modèles IPD développés et les règles d'utilisation des IPD prévus. Une autre analyse<br />l'influence des GED sur l'utilisation des IPD sur la détection et localisation de défauts. Pour l'amélioration de la<br />robustesse du diagnostic avec IPD en présence de fausses indications, une méthode de détermination de la<br />section en défaut (limitée par des IPD) est proposée.<br />La deuxième partie du mémoire est consacrée à une méthode d'optimisation du placement des IPD dans les<br />réseaux HTA sur la base d'algorithmes génétiques. Nous avons défini différents critères pour l'optimisation ; ils<br />sont validés par un programme de calcul des indices de fiabilité. L'influence de la GED dans le départ HTA sur<br />le placement optimal des IPD est analysée en tenant compte du coût de l'énergie non fournie par la GED et du<br />fonctionnement envisageable comme un secours de la GED.
