Low mood, visual hallucinations, and falls – heralding the onset of rapidly progressive probable sporadic Creutzfeldt–Jakob disease in a 73-year old: a case report

Background Creutzfeldt–Jakob disease is a rare and rapidly fatal neurodegenerative disease. Since clinicians may see only very few cases during their professional career, it is important to be familiar with the clinical presentation and progression, to perform appropriate investigations, and allow for quick diagnosis. Case presentation A 73-year-old British Caucasian woman presented with acute confusion of 2 weeks’ duration on a background of low mood following a recent bereavement. Her symptoms included behavioral change, visual hallucinations, vertigo, and recent falls. She was mildly confused, with left-sided hyperreflexia, a wide-based gait, and intention tremor in her left upper limb. Initial blood tests, computed tomography, and magnetic resonance imaging of her brain showed no significant abnormality. Following admission, she had rapid cognitive decline and developed florid and progressive neurological signs; a diagnosis of prion disease was suspected. A lumbar puncture was performed; cerebrospinal fluid was positive for 14–3-3 protein, real-time quaking-induced conversion, and raised levels of s-100b proteins were detected. An electroencephalogram showed bilateral periodic triphasic waves on a slow background. The diagnosis of probable Creutzfeldt–Jakob disease was made. Conclusions This case report highlights key features in the initial presentation and clinical development of a rare but invariably rapidly progressive and fatal disease. It emphasizes the importance of considering a unifying diagnosis for multifaceted clinical presentations. Although it is very rare, Creutzfeldt–Jakob disease should be considered a diagnosis for a mixed neuropsychiatric presentation, particularly with rapid progressive cognitive decline and development of neurological signs. However, to avoid overlooking early signal change on magnetic resonance imaging, it is important to take diffusion-weighted magnetic resonance imaging for all patients with neuropsychological symptoms. Importantly, early diagnosis also ensures the arrangement of suitable contamination control measures to minimize the risk of infection to health care professionals and other patients.

Im Jahr 1690 und in der ersten Hälfte des Jahres 1691 bis zu Speners Wechsel von Dresden nach Berlin setzen sich die Problemstellungen fort, die im Jahr vorher begonnen hatten: Der Konflikt mit dem sächsischen Kurfürsten Johann Georg III. lässt sich nicht lösen, so dass Spener, der bis dahin auf Grund seiner Überzeugung von Gott nach Dresden berufen worden zu sein, erst die sächsische Residenz verlässt, als er sich eines neuen Rufes als Propst nach Berlin gewiss ist. Die durch die erweckliche Bewegung unter Leipziger Studenten begonnenen Streitigkeiten mit den dortigen Geistlichen eskalieren weiter und finden ihre Fortsetzung an anderen Orten wie etwa in Erfurt. Parallel zu den Ereignissen in Sachsen spiegeln die Briefe Speners Auseinandersetzungen in Gießen, Hinterpommern und Hamburg, die am Anfang der dort entstehenden als „Pietismus“ bezeichnete Frömmigkeit stehen. The disputes with the clergy of Leipzig, which began as a result of the awakening movement among students escalated further and continued in other places such as Erfurt. The Vault with Curvilinear Ribs in the "Hall of Arms" in the Albrechtsburg Meissen: Studies on the Concept, Design and Construction of a Complex Late Gothic Rib Vault

The current state of research on the design and construction of late Gothic vaults is principally based on the interpretation of sources, but rarely on surveys of existing vaults. In particular the idea for the criteria upon which the radii of the single ribs and arches were determined (so-called "principal arch"), is found to derive from sources dating from later periods when Gothic vaulting was no longer generally practised. In fact, this interpretation cannot be supported by geometric surveys on existing vaults: the plain contradiction to the actual geometric properties of some of the vaults studied so far have called for a critical revision of this interpretation. For investigating the design criteria and procedures starting from the built object, and also debating the methodological aspects of using the "building as source", a rewarding subject is the vault with curvilinear ribs in the "Hall of Arms" in the Albrechtsburg at Meissen (Germany), which was built by Jacob Heilmann in 1521. This is due to its ambitious design, high quality of execution and good state of preservation. A detailed survey of this structure is presented, discussing different aspects of the design, construction, and detailing, e.g.: Investigations on the geometrical concept of the vault by means of "reverse geometric engineering", on the background of the problem mentioned above. Further, the stone-cutting of the single members of the vault, which offers interesting aspects of standardization and its limits in nerves with double curvature: standardization in medieval constructions has been object of discussion, but not yet studied in the ambit of late Gothic vault construction. The question whether and how the vault surface was determined in the design, being the principle structural feature working as a shell structure; this is of particular interest as the relation between the rib system and the shell is complex and has consequences on the stone-cutting of the arches. The question how the design specifications for the single rib members could be derived from the general concept of the curves (clarified in the survey); the context to the graphic sources and technical prose related to late Gothic vault design and structural design. The aim is to understand the entire design process, the criteria of the decisions taken, their relation to the contemporary theory, and the communication process which took place for the creation of this extremely demanding structure.

Das Jahr 1689 stellt Spener vor zwei besondere Herausforderungen. Erstens fällt er wegen eines freiherzigen Briefes, den er an Kurfürst Johann Georg III. von Sachsen als dessen Beichtvater schreibt, bei diesem in Ungnade. Seine Stellung als Oberhofprediger in Dresden steht damit zur Disposition. Zweitens kommt es im Sommer 1689 zu heftigen Auseinandersetzungen an der Universität und in der Stadt Leipzig. Junge Theologen, die sich mit Speners Theologie und Frömmigkeit verbunden fühlen, veranstalten exegetische Collegia, bei denen die Bibelauslegung zur Applikation im persönlichen Leben der Teilnehmer hingeführt wird. Neben Theologiestudenten nehmen im Laufe der Zeit auch Studierende anderer Fakultäten und Bürger der Stadt teil. Von den Gegnern werden die Anhänger dieser Bewegung „Pietisten“ genannt und Spener wird als deren Inspirator ausgemacht und angegriffen. Die Briefe des Jahres 1689 befassen sich naturgemäß ausführlich mit Erklärungen und Stellungnahmen zu diesen Fragen. Die politische Situation des Alten Reiches, die von der militärischen Invasion Ludwigs XIV. von Frankreich bestimmt wird, wird zudem in den Briefen Speners aus diesem Jahr häufig thematisiert und in First of all, he fell out of favor with Elector Johann Georg III. due to a candid letter he wrote to him as his confessor. Consequently, his position as court preacher in Dresden was put into jeopardy. Secondly, in the summer of 1689, violent clashes took place at the University of Leipzig and in the city of Leipzig. Young theologians, who felt spoken to by Spener’s theology and piety, organized classes in exegesis, in which the biblical interpretation was applied to the personal life of the participants. In the course of time, students from other faculties and citizens of the town also joined the classes alongside the theology students. The opponents of this movement called the members “Pietists” and Spener was recognized as their leader and was attacked. In the letters from 1689, Spener addresses these events in great detail with comments and explanations. With an eye to its affect on the future of the Lutheran Church, Spener also frequently addresses the political situation in the Old Empire, expecially Louis XIV. military invasion of the Rhine region Role of PRNP codon 129 genotype in defining strain transmission properties of human transmissible spongiform encephalopathy

The human prion protein (PrP) gene (PRNP) codon 129 (M/V) polymorphism is a susceptibility factor for variant Creutzfeldt-Jakob Disease (vCJD) and a major determinant of clinico-pathological phenotype in sporadic CJD. The role of codon 129 in defining susceptibility and strain transmission properties has been investigated in three lines of transgenic mice that express human PrP. The human PRNP gene has directly replaced the murine version, by gene targeting, and variation at codon 129 has given the three genotype lines (HuMM, HuMV, and HuVV). The genetics of these three mouse lines are otherwise identical, and therefore differences in transmission properties can be directly attributable to the codon 129 genotype. vCJD inoculation has shown that all three codon 129 genotype mice are susceptible with a ranking of transmission efficiency of HuMM>HuMV>HuVV. HuMM mice develop the most widespread neuropathology with features similar to human vCJD. Subclinical infection was noted in each mouse line. These data suggest that the vCJD strain is transmissible to humans of each of the three codon 129 genotypes, implying that non-MM cases of human infection with bovine spongiform encephalopathy (BSE) may exist but with long subclinical incubation periods. Inoculation of material from blood transfusion associated vCJD showed no change in transmission properties suggesting that the threat of a future epidemic of human-to-human vCJD infection has not been increased by adaptation of the vCJD strain. However the route of infection, for example via blood transfusion or surgery, may be more efficient that the original oral route of BSE infection. sCJD is classified into six subgroups according to clinico-pathological features, and defined by codon 129 genotype and electrophoretic mobility type (1 or 2) of disease associated PrPSc (MM1, MM2, MV1, MV2, VV1, VV2). Typical cases from each subgroup have shown specific transmission properties suggesting that the subgrouping is defining separate disease strains. The commonest subgroup (MM1) was the most transmissible and the HuVV mouse line the most susceptible host. These data outline the transmission risk from all sCJD types to recipients of each codon 129 genotype should an infection event occur, and show the significant role of recipient codon 129 genotype in defining the clinical or subclinical state and the success or failure of transmission. This is important for determining individual risk following known exposure, and for modelling the potential of iatrogenic infection from sCJD patients.

Title: The Lilja case – a study about discourse and media representations of violence within ice hockey The purpose of this study is to examine the discourses that influenced the debate in Swedish sport media around the professional ice hockey player Jakob Liljas 10-game suspension and subsequent assault conviction by the Swedish legal system. The aim is to see how Lilja’s violence was defined and what voices were the most prominent in the debate. The study is based on a theoretical framework consisting of Laclau and Mouffe’s discourse theory and Agamben’s ideas of the Homo Sacer and the State of Exception. Using Laclau and Mouffe, an analytical toolbox was assembled to deconstruct the discourses present in the debate. The analysis found two dominant discourses within the debate: a sports discourse and a law discourse. The study found two nodal points that defined how the discourses treated Lilja’s violence; the nodal point “crime” within the law discourse, and the nodal point “rule violation” within the sports one. The sports discourse argued against the legal process maintained that Lilja had already received a sufficient punishment through his suspension. The law one was centred around the premise that legal action was required to properly punish Lilja. The analysis found that the sports discourse unsuccessfully tried to position the sport of ice hockey as a State of Exception where the laws of regular society should not apply. There were also similarities between the underlying masculine norms that informed how the sports discourse treated player health and Agambens Homo Sacer, how people’s life worth is reduced in order to justify certain conditions imposed on them. While a true State of Exception or Homo Sacer does not exist in this scenario, as Lilja was ultimately convicted according to the rules of the law discourse, it is interesting that ideas that align with these concepts were well represented in the medial debate.
