Die vorliegende retrospektive Studie bezieht sich auf ein Patientenkollektiv von 344 Patienten, die in der Zeit von Januar 1997 bis einschließlich Dezember 2002 in der Klinik und Poliklinik für Urologie und Kinderurologie der Universitätsklinik der Julius-Maximilians-Universität Würzburg sich aufgrund eines nachgewiesenen Prostatakarzinoms einer radikalen Prostatektomie unterzogen. Die Operation erfolgte bei 79 Patienten als radikale perineale (RPP) und bei 265 Patienten als radikale retropubische Prostatovesikulektomie (RRP). Ziel dieser Arbeit war ein Vergleich dieser beiden Operationszugänge hinsichtlich ihrer Ergebnisse unter besonderer Berücksichtigung der postoperativen Kontinenzrate und eine Gegenüberstellung dieser Daten mit den Ergebnissen aus anderen Studien. / In this retrospective study we examined a patient population of 344 patients who underwent radical prostatectomy in the Department of Urology at the University Hospital of the Julius-Maximilians-University in Würzburg from 1997 to 2002. The operation was performed in 79 patients as radical perineal prostatectomy (RPP) and in 265 patients as radical retropubic prostatectomy (RRP). The aim of this work was a comparison of these two surgical approaches in terms of their complications and postoperative results with special reference to postoperative continence rates and a comparison of our findings with results from other studies.

Postoperative Lebens- und Kontinenzqualität nach totaler mesorektaler Exzision bei Rektumkarzinom / Quality of life and continence after total mesorectal excision for rectal cancer

In der vorliegenden Arbeit wurde retrospektiv die Lebens- und Kontinenzqualität von Patienten, die aufgrund eines Rektumkarzinoms mittels totaler mesorektaler Exzision operiert wurden, erfasst. Ein Zusammenhang zwischen Lebens-und Kontinenzqualität wurde untersucht. Die Erhebung der Lebensqualität erfolgte mittels GIQLI (Gastrointestinal Quality of Life Index) und der Kontinenzqualität mit Hilfe des Wexner- und Cleveland Clinic Constipation-Score. Ergänzend dazu wurden Patientendaten erhoben und kombiniert mit den Antworten der Fragebögen ausgewertet. Postoperativ zeigte sich eine zunächst verminderte Lebensqualität, die sich über die Jahre wieder den Ergebnissen gesunder Probanden annäherte. Patienten mit einem im mittleren Rektumdrittel befindlichen Tumor erzielten bessere Ergebnisse im Lebensqualitätsbogen. Die Kontinenz war vor allem von der Art der Anastomose abhängig. Die Tumorlokalisation zeigte sich ohne große Einflussnahme auf die Kontinenzleistung. Die Rekonstruktion mittels J-Pouch oder Latero-terminaler Anastomose war der geraden Anastomosierung vorzuziehen. Postoperative Komplikationen traten vermehrt bei einem im unteren Rektumdrittel lokalisierten Tumor auf. Signifikant häufiger wurden Stenosen diagnostiziert. Eine gute Kontinenzleistung wirkte sich auf alle fünf Bereiche, die der GIQLI abfragt aus und korrelierte somit mit einem höheren Maß an Lebensqualität. / The present study deals with the quality of life and continence regarding rectal cancer patients stare after mesorectal excision. It primarily focuses on the correlation between the given parameters. Methods used to showcase were the GIQLI (Gastrointestinal Quality of Life Index), Wexner- Score and Cleveland Clinic Constipation- Score. Patients answers and data were combined to achieve an objective view on the researched subject. The average result of the GIQLI showed a 20 % lower quality of life compared to examples given in literature of healthy people. The initial decrease of life quality was remissive over the following years up to the results of healthy people group. A tumor located in the mid part of the rectum had a lower negative impact on patients postoperative life quality. Regarding the continence the type of anastomosis is the most important predictor for postoperative function. J-Pouch and side-to-end-anastomosis achieved much better results compared to end-to-end-anastomosis. Tumors located in the lower part of rectum showcased increased amount of complications and significantly more often stenosis. High continence ability correlated with high level on the GIQLI and therefore an increased quality of life.

Age-Stratified Outcomes after Radical Prostatectomy in a Randomized Setting (LAP-01): Do Younger Patients Have More to Lose?

Objective Age is known to have an impact on outcomes after radical prostatectomy (RP). However, age differences can be investigated from a cross-sectional as well as from a longitudinal perspective. This study combines both perspectives. Materials and methods LAP-01 is the first multicenter randomized patient blinded trial comparing outcomes after robotic-assisted and laparoscopic RP. This study stratified the entire population that received nerve-sparing surgery and was potent at baseline by the following ages: ≤ 60 years, 61–65 years, and > 65 years. Potency was assessed using the IIEF-5. The EORTC QLQ-C30 was used for global health perception and the EORTC QLQ-PR25 for urinary symptoms. Continence was assessed by the number of pads used. Longitudinal change was assessed using either validated anchor-based criteria or the 1 or 0.5-standard-deviation criterion. Worsening of continence was measured by increasing numbers of pads. Results 310 patients were included into this study. Older patients had a significantly higher risk for worsening of continence at 3 and 6 months (OR 2.21, 95% CI [1.22, 4.02], p = 0.009 and OR 2.00, 95% CI [1.16, 3.46], p = 0.013, respectively); at 12 months, the odds of worsening did not differ significantly between age groups. Potency scores were better in younger patients from a cross-sectional perspective, but longitudinal change did not differ between the age groups. In contrast, global health perception was better in older patients from a cross-sectional perspective and longitudinal decreases were significantly more common among the youngest patients, at 12 months (36.9% vs. 24.4%, p = 0.038). Conclusion From a cross-sectional perspective, function scores were better in younger patients, but from a longitudinal perspective, age differences were found in continence only. In contrast, global health scores were better in older patients from a cross-sectional and longitudinal perspective.

Urinary Tract-Related Quality of Life after Radical Prostatectomy: Open Retropubic versus Robot-Assisted Laparoscopic Approach

Background: The best technique of radical prostatectomy – open retropubic versus robot-assisted surgery – is a subject of controversy. Patients and Methods: Between January 1st, 2007 and December 31st, 2011, 2,177 men underwent radical prostatectomy at our department. 252 (12%) cases were laparoscopic robot-assisted, the remainder open retropubic procedures. In Germany, certified prostate cancer centers are required to collect urinary tract-related outcome data after radical prostatectomy using the International Consultation of Incontinence Questionnaire Male Lower Urinary Tract Symptoms. The questionnaire data were used to compare both surgical approaches concerning the urinary tractrelated outcome 1, 2 and 3 years postoperatively. Results: Neither the voiding score nor the incontinence score or the bother scale sum differed between the two cohorts at any of the measurement times. Conclusions: Concerning continence recovery, in this series, there were no detectable differences between robot-assisted and open radical prostatectomy.
