Milk Intake in Early and Late Adulthood and Risk of Osteoporotic Hip Fractures in Utah

The relationship between milk intake and risk of osteoporotic fractures is uncertain. Associations between milk intake and milk avoidance in relation to osteoporotic hip fracture were examined in the Utah Study of Nutrition and Bone Health (USNBH), a statewide case-control study. Cases were ascertained at Utah hospitals treating 98 percent of hip fractures during 1997-2001 and included 1188 men and women aged 50-89 years. Age- and gender-matched controls were randomly selected from Utah driver's license and Medicare databases (N= 1324). In-person interviews were conducted and participants reported frequency of milk intake per week at age 18 and during pregnancy among women who reported being pregnant. Milk avoidance for a period of more than one year and duration of milk avoidance were also reported. Diet and supplement intake in the one-year period before fracture (cases) or the interview (controls) was assessed using a picture-sort food frequency questionnaire. Milk consumption frequency was categorized into four levels of intake at each life stage. Total calcium intake was categorized into quintiles of distribution of intake. Logistic regression models were used to examine associations between milk intake and milk avoidance and risk of hip fracture while controlling for the potential confounding effects of gender, age, body mass index, alcohol use, smoking, physical activity, estrogen use, and total calorie, protein, calcium, and vitamin D intake. Recent milk intake, milk intake during pregnancy, and milk avoidance duration were not associated with risk of hip fracture. A borderline association was found at age 18 showing a decreased risk of hip fracture among those in the highest quartile (2: 15 cups of milk per week) of milk intake (odds ratio (OR): 0.86, 95 percent confidence interval (Cl): 0.75, 1.00; P = 0.046). Milk avoidance for a year or more was associated with an increased risk of hip fracture compared to those who did not avoid milk (OR: 1.38, 95 percent CI: 1.07, 1.78). A significant interaction was found between milk avoidance and quintile of total calcium intake (P = 0.02). Milk avoidance was associated with a significantly higher risk of hip fracture at the lowest two quintiles of calcium intake (OR: 1.72, 95 percent CI: 1.26, 2.17; P = 0.02 and OR: 1.58, 95 percent CI: 1.01, 2.15; P = 0.01, respectively) but was not associated with elevated risk among those with higher calcium intakes. In conclusion, milk intake during pregnancy for women, and in the year before hip fracture (for cases) or before interview (for controls), was not associated with hip fracture risk. The highest level of milk intake at age 18 was associated with decreased risk of hip fracture. Avoidance of milk for one year or more was associated with hip fracture risk, but only among those with low calcium intake (Q1 and Q2).

The 50+ generation has become an age group that is aging according to the statistical data and the number of individuals in this age category is rising. The aging process is accompanied by numerous physical, mental and social changes. As a consequence of these processes new and modified needs occur in these individuals, to which the provided care has to be adapted. We have linked this issue with conceptual models in nursing. This is because a conceptual model will help us integrate patient information into coherent and comprehensible blocks. An important goal of conceptual models is to simplify the work of nurses, who can see a patient as a whole thanks to them. We chose two models in relation to this issue, namely Calista Roy's model and Betty Neuman's model. We chose two research goals on the basis of the topic of the thesis. The first one was to map the knowledge of selected conceptual models among nurses. And the second goal was to find out the specifics of the nursing care in the 50+ group with the application of selected conceptual models from the point of view of nurses. We applied both, qualitative and quantitative methods to the research. Within the quantitative research we used a non-standardized questionnaire. In the qualitative part we applied the focus group method. Three hypotheses and one research question were set in relation to the research. We are able to confirm two of the three hypotheses. The statistical results have shown that the knowledge of the open-systems model and the adaptation model corresponds with the education of the respondents. Respondents with tertiary education show better knowledge of these models. On the other hand, no relation in taking the specifics in the satisfaction of the needs into account in the provision of nursing care in the 50+ generation was confirmed. A procedure for collection of the nursing anamnesis in the 50+ generation was created on the basis of the research. On the basis of this procedure we can enable healthcare staff to classify the patient information into logical systems and therefore to obtain a quality and comprehensive view of a patient. After the completion of the qualitative research we evaluated the proposals of our respondents for the theoretical as well as the practical part of the anamnesis collection. These respondents showed knowledge of the models. They most often criticized bad intelligibility or even redundancy of a question. Among benefits they named simplicity, reasonability and quality evaluation of the nursing care.

Объектом исследования стали психологические особенности процессов профессионализации в возрасте средней и поздней взрослости. Предметом исследования является биографическая рефлексия и готовность к освоению возрастно-временных изменений во взаимосвязи с самооценкой уровня работоспособности. Магистерская диссертация состоит из введения, двух глав, заключения, списка литературы (138 источников) и приложения, включающего в себя бланки применявшихся методик, описательные статистики, таблицы сравнительного и корреляционного анализа. Объем магистерской диссертации 109 страниц, на которых размещены 5 рисунков и 15 таблиц. Во введении раскрывается актуальность проблемы исследования, теоретическая и практическая значимость работы. разработанность проблематики, ставятся цель и задачи исследования, определяются объект и предмет исследования, формулируются основные гипотезы, указываются методы и эмпирическая база, а также этапы проведения исследования. В первой главе были определены теоретические основания исследования биографической рефлексии, готовности к освоению возрастно-временных изменений, самооценки уровня работоспособности на поздних этапах профессионализации. Был проведён теоретический анализ литературы по проблеме профессионализации на стадии средней и поздней взрослости, рассмотрены различные подходы к классификации профессионального становления специалиста. Выводы по первой главе представляют собой итоги по изучению теоретического материала. Вторая глава посвящена эмпирической части исследования. В ней представлено описание организации и методов проведенного исследования и результатов, полученных по всем использованным методикам: Опросник «Готовность к освоению возрастно-временных изменений» Т.Б. Сергеевой, Н.С. Глуханюк, опросник «Дифференциальная диагностика состояний сниженной работоспособности» (ДОРС) А.Б. Леоновой, С.Б. Величковской, Методика М.В. Клементьевой «Оценка уровня развития биографической рефлексии». Также в главе представлен сравнительный и корреляционный анализ результатов исследования. Выводы по главе 2 включают в себя основные результаты эмпирического исследования. В заключении в обобщенном виде изложены результаты теоретической и эмпирической частей работы, а также выводы по выдвинутым гипотезам, обоснована практическая значимость исследования и описаны возможные перспективы дальнейшей разработки данной проблематики. / The object of the study was the psychological characteristics of developmental processes at the age of middle and late adulthood. The subject of the research is biographical reflection and readiness to master age-time changes in relation to self-assessment of the level of operability. The master's thesis consists of an introduction, two chapters, a conclusion, a list of references (138 sources) and an appendix, which includes the forms of the methods used, descriptive statistics, tables of comparative and correlation analysis. The volume of the master's thesis is 109 pages, which contain 5 figures and 15 tables. The introduction reveals the relevance of the research problem, the theoretical and practical significance of the work. the elaboration of the problematics, the goal and objectives of the research are set, the object and subject of the research are determined, the main hypotheses are formulated, the methods and empirical base, as well as the stages of the research, are indicated. In the first chapter, the theoretical foundations of the study of biographical reflection, readiness to master age-temporal changes, self-assessment of the level of operability at the later stages of professionalization were determined. A theoretical analysis of the literature on the problem of professionalization at the stage of middle and late adulthood was carried out, various approaches to the classification of the professional development of a specialist were considered. Conclusions on the first chapter are the results of the study of theoretical material. The second chapter is devoted to the empirical part of the study. It contains a description of the organization and methods of the study and the results obtained by all the methods used: Questionnaire "Readiness to master age-time changes" T.B. Sergeeva, N.S. Glukhanyuk, questionnaire "Differential diagnosis of states of reduced performance" (DORS) A.B. Leonova, S.B. Velichkovskaya, M.V. Klementyeva "Assessment of the level of development of biographical reflection". The chapter also provides a comparative and correlation analysis of the research results. The findings of Chapter 2 are the main results of the empirical study. In conclusion, brief results of the theoretical and empirical parts of the work are presented, as well as conclusions on the hypotheses. The practical significance of the study is substantiated and possible prospects for further development of the problematics are described.
