The Influence of Staffing Change on Quality of Care in Emergency Room¢wAn Example of Three Hospitals

In light of the competitions of different medical services and the National Health Insurance¡¦s patient-oriented goal, the quality of medical care in the emergency room has been a top priory for improvement in many major medical centers. The purpose of this research is to evaluate the differences of medical quality after adjustment of physician manpower in three emergency departments of three separate hospitals. We will attempt to suggest ways to improve medical quality and make good use of medical resources. This is a retrospective survey using data from three emergency departments of three separate hospitals. The date ranged from March to April of 2002 and March to April of 2003. A total of 66,025 cases were gathered, minus 311 cases with incomplete data, the total valid data were 65,714 cases. The three hospitals A, B, C have 24,010 cases, 17,690 cases, and 24,014 cases, respectively. The result of this study showed that hospital A had increased the number of medical staff when comparing 2003 to 2002 data, however, the quality of care did not improve. Hospital B had increased the number of physicians on duty, decreased the waiting time for the patients, and the number of patients taken care per doctor had been increased too. However, within the 72-hour clinic follow-up patient numbers have increased. Hospital C have decreased the number of physicians on duty, but the waiting time have also decreased. The number of patients being taken care of by physicians, and 72-hour clinic follow-up are all increased. When compare the month between March and April of 2002, the highest ratio of 72-hour clinic follow-up is hospital A, followed by hospital B and C, in descending order. When compare the two study period of March and April of 2002 and 2003, waiting time and the ratio of waiting time in emergency department more than 6 hours is hospital A more than hospital C more than hospital B; the ratio of waiting time less than 2 hours is hospital C more than hospital B more than hospital A. The ratio of waiting time more than 2 hour but less than or equal to 4 hours and waiting time more than 4 hours but less than or equal to 6 hours is hospital A more than hospital B more than hospital C. This study suggests that the hospitals must establish a complete quality indicators, and must conduct periodic evaluation. At the same time, we must incorporate these indicators into our hospital information system and monitoring the effectiveness, in order to promote the quality of care.

The purpose of this research is to develop a systematic methodology for the design of the transmission system of the manpower-drive vehicles. First, analyzing the patents of manpower-drive vehicles. Drawing the function structure of the transmission and concluding the basic characteristic. Second, getting several innovative ideas of transmission systems by systematic methodology and comparing with products in patents. Third, according to the requirement charts to design steering systems and frames. Fourth, all design concept variant can be created by using morphological chart method. Several design concepts can be created for new manpower-drive vehicles. Finally, using computer software to assist drawing the models and manufacturing the models. And to establish a simple systematic procedure.

The Impacts of Psychiatric Professionals Human Resource Inputs on Service Performance-The Experience of Seven Public Psychiatric Hospitals in Taiwan

Abstract There is no literature explores the relationship between manpower and performance of the Taiwan¡¦s psychiatric hospitals yet. This research takes samples from seven public psychiatric hospitals in Taiwan to investigate: 1. Performance measurement of psychiatric hospitals. 2. The influence of psychiatric hospitals¡¦ manpower inputs on service performance. The orientations of data analysis in this research are research framework and research hypotheses, and then collect data from seven domestic public psychiatric hospitals. According to the nature of gathered data, we applied SPSS software on Descriptive Statistics, Pearson Correlation Analysis, K-Means Cluster Analysis, and t-test for difference of two samples mean, to explore the relationship between manpower input and service performance. Research outcomes: 1¡BAlthough service items don¡¦t increase in high professional manpower structure compare to low professional manpower structure, the service amounts increased significantly. Test results show that the hypothesis 1-1 is not supported, but hypothesis 1-2 is supported. 2¡BGroups with higher manpower quality input have significant increasing in service items¡¦ performances, but not significant statistically. Even though groups with higher manpower quality increased service items, the service amounts do not raise. However, groups with lower manpower quality input have significant increasing in service amounts¡¦ performances. It means that the significantly negative correlation between manpower quality and service amounts¡¦ performances. Test results show the hypothesis 2-1 is not supported, neither is hypothesis 2-2. 3¡BEven though the service items¡¦ performances have no significant increasing in the groups with high formal and contracted personnel percentages input, the service amounts are increased significantly. Therefore, the difference of formal and contracted personnel percentages input has significant influence on service amounts. Test results show the hypothesis 3-1 is not supported, but hypothesis3-2 is supported. 4¡BPsychiatric hospitals with high manpower costs input do not increase service items, but increase service amounts. Test results show the hypothesis 4-1 is not supported, but hypothesis 4-2 is supported. ¡@The relationship between psychiatric hospital¡¦s manpower input and service performance is clearly realized through this research, which could be the reference for hospital managers to adjust profession manpower and to employ personnel in the future. Meanwhile, it can facilitate managers to control correct developments, to use every manpower portfolio engaging in medical operations, and to exert maximum efficiency of human resources with most economical manpower costs to improve psychiatric hospitals¡¦ service performances. ¡iKeywords¡j: Psychiatric hospital, Manpower input, Service performance

<p>Nowadays many companies are interested to improve their production system. It has become important in globalize and competitive market for the survival. The companies are looking to their production systems in the different direction to get the competitive advantages. But the most important is to find out the problem of the production system to make improvements.</p><p>In this thesis, a part of the production system of a company is studied to find the problems of the production system to make the improvements and to recommend some points to the company for the achievements of its goals. For this purpose, the overall equipment effectiveness calculations have been used to see the current situation of the production system of the company. These calculations of the O.E.E. of the different production work stations make the clear picture of the problems. It calculates the availability of the production system which shows that maintenance system’s effectiveness. The quality rate calculations of the work stations show the conditions of the machines and the worker’s skill and the calculations of the performance efficiency of the work stations show the utilization of the machines. The result of this thesis after all the calculations of O.E.E. is that there is a problem in the utilization of the machines. Most of the time the machines remain idle or wait for the maintenance if it fails.</p>
