Prospective Evaluation of Antibiotic Sensitivities for Uncomplicated Cystitis at Banner – University Medical Center Tucson’s Emergency Department

Class of 2016 Abstract / Objectives: To determine if the uncomplicated cystitis population at Banner - University Medical Center Tucson’s (BUMCT) emergency department (ED) is significantly different from the hospital-wide antibiogram. Methods: A prospective, analytical, observational study was performed in the ED at BUMCT. Electronic health records were used to include or exclude patients in the study. Cultures were then compiled and compared to the hospital-wide, 2014 BUMCT antibiogram using a one sample Chi-square goodness of fit test to determine if there was a significant difference between the two proportions. Results: 159 cultures and susceptibilities were analyzed with E. coli and K. pneumoniae being the most common organisms. For E. coli, it was observed that 46.92% were susceptible to ampicillin while 45% was reported by the hospital’s antibiogram (P=0.772), 93.08% compared to 88% for cefazolin (P=0.185), 94.62% compared to 93% for ceftriaxone (P=0.611), 83.08% compared to 78% for ciprofloxacin (P=0.332), 89.23% compared to 90% for nitrofurantoin (P=0.850), and 63.85% compared to 66% for TMP/SMX (P=0.735). For K. pneumoniae, it was observed that 100% of isolates were susceptible to cefazolin, ceftriaxone, and ciprofloxacin compared to 95%, 96%, and 95%, respectively (P=0.254; P=0.309; P=0.254); 40% compared to 28% for nitrofurantoin (P=0.243); and 88% compared to 91% for TMP/SMX (P=0.649). Conclusions: The observed and predicted susceptibilities were not different for any of the drugs and organisms. Our study indicates that the hospital-wide antibiogram used at BUMCT is adequate in treating patients with uncomplicated cystitis coming into their ED.

Bakgrund: Statistik från Försäkringskassan (2016) visar att stressrelaterad sjukskrivning i form av utmattningssyndrom har ökat markant de senaste åren där ökningen är störst bland kvinnor, och dubbelt så vanligt jämfört med män. Det handlar om krav och förväntningar i vardagen, och om svårigheten att hålla balansen mellan dessa. Att arbeta förebyggande för att förhindra sjukskrivning på grund av stress är viktigt och livsstilsmottagning på hälsocentral kan vara en arena för det. Syfte: Syftet var att belysa sjuksköterskors erfarenheter av arbetet på livsstilsmottagning med kvinnor som har stressrelaterade symtom. Metod: Studien är empirisk och utgår från en kvalitativ design med induktiv karaktär. Sex sjuksköterskor deltog i studien där semistrukturerade intervjuer användes. Kvalitativ innehållsanalys har använts vid analys av data. Resultat: Följande kategorier framkom; Sjuksköterskans insatster vid stressymtom, där tydliggörs sjuksköterskans insatser i mötet med kvinnan. Teamsamverkan, där beskrivs befintlig/obefintlig teamsamverkan mellan professioner. Preventivt arbete efterfrågas, där sjuksköterskors erfarenheter av hur ett preventivt arbete med stress skulle kunna utformas. Behov av kompetensutveckling gällande stress, där framkom brist på kompetensutveckling och Utmaningar i arbetet med stressymtom där det framkom sjuksköterskors erfarenheter av svårigheter med att tydliggöra hos kvinnan om stressens påverkan. Slutsats: Sjuksköterskors erfarenhet visar att det idag saknas kompetens på Livsstilsmottagning gällande stresshantering. Resultatet av studien belyser vikten av att involvera sjuksköterskan i arbetet med stress. / Background: Statistics from Försäkringskassan (2016) show that stress-related illness claims due to burnout have significantly increased over the last few years. Whereas this increase is greatest amongst women, it is also twice as common amongst them when compared to men. This is mainly due to the demands and expectations of everyday life, as well as the difficulties in being able to balance them. Working preventively to prevent sick leave due to stress is important and lifestyle medical center can be an arena for it. Purpose: The purpose is to illustrate the experiences of nurses at a lifestyle medical center when it comes to working with women that have stress-related symptoms. Method: The study is empirical and based upon a qualitative design with an inductive nature. Six nurses participated in the study, and semi-structured interviews were performed. Qualitative content analysis has been used to analyze the data. Results: The following categories emerged; The efforts of nurses in the case of stress-related symptoms, which clarifies the nurse's efforts in the meeting with the woman, Teamwork, which describes existing/non-existent teamwork between professions. Preventative work is requested, where nurses´ experience of how preventive stress work could be designed and The need for development of competencies pertaining to stress and exhaustion when it comes to work concerning stress-related symptoms, where nurses´ experience of difficulties in clarifying the stress of stress on the woman. Conclusion: The experiences of nurses show that there is a lack of competence when it comes to dealing with the treatment of stress at lifestyle medical centers. The results of the study highlight the importance of getting nurses involved when working with stress.

Understanding patient experiences with epilepsy monitoring during the COVID-19 pandemic

BACKGROUND: Epilepsy monitoring units (EMUs) provide a safe environment for forming a more illustrative understanding of the patient’s seizure disorder. Patients are admitted to EMUs usually for several days at a time. Upon admission, electroencephalogram (EEG) electrodes are placed and patients are continuously watched via EEG, video, and audio means. By weaning patients off anti-epileptic medications and monitoring brain activity with EEGs, the data will typically allow for a stronger appreciation of the seizure activity. Therefore, it will provide information to develop a more targeted clinical approach for the patient. OBJECTIVE: The purpose of the study is to gain a better understanding of patients’ expectations and experiences with being monitored for seizure activity in an EMU, especially during the Coronavirus Disease 2019 (COVID-19) pandemic METHODS: Patients were interviewed with regards to their inpatient EMU admission for continuous EEG monitoring at Beth Israel Deaconess Medical Center (BIDMC). Phone interviews were conducted both before and after the EMU admission for each patient, using a structured questionnaire that focused on topics such as proclaimed knowledge of personal seizure disorder, quality of life, EMU experience, and hospital admission during the COVID-19 pandemic. Patient responses were documented and analyzed in an exploratory manner to identify relevant themes. The study was conducted according to a protocol approved by the BIDMC Committee on Clinical Investigations. RESULTS: From September 2020 through December 2020, 15 patients were enrolled and interviewed (11 female; age range 26-68 years [median 48]; length of stay range 2-12 days [median 5]). The majority of the population was admitted for event capture or seizure characterization (13/15) and had a history of seizure activity (14/15). The majority of patients had a history of focal seizures (12). Only 4/15 patients had a family history of seizures. Overall, patients felt extremely comfortable speaking with providers. A third (4/12) did not have any notable negative experiences. There were no overarching patterns to the negative experiences that were reported; most responses were specific to the individual. The vast majority (83.3%) applauded providers and staff involved in their EMU admission. CONCLUSIONS: Despite the COVID-19 pandemic, epilepsy patients had mostly positive experiences with their EMU hospitalization at BIDMC. Continuous EEG monitoring remains an important aspect of clinical epilepsy evaluation for some patients, and was a feasible and well-tolerated procedure even during pandemic-altered circumstances.
