Impact of Self-Controlled Practice on Motor Learning for Individuals with Post-Stroke Upper Limb Hemiparesis

Upper limb (UL) impairments are common after stroke and can lead to loss of independence and long-term disability. Motor learning interventions for individuals with stroke can improve UL function and engagement in daily activities. Self-controlled practice (SCP) involves allowing choice during motor skill practice and has been shown to improve motor learning in healthy adults as well as individuals with Parkinson's disease. Additionally, SCP may impact psychological outcomes, including intrinsic motivation, self-efficacy, and positive affect. Although SCP has been incorporated into complex stroke rehabilitation interventions, little is known about its specific impact on motor learning for individuals with stroke. The purpose of this study is to investigate the effect of SCP on motor learning and psychological outcomes for adults with chronic UL impairment after stroke. Sixteen participants with chronic UL impairment due to stroke were paired and randomized into two groups. All participants practiced a standardized motor task with both their more affected and less affected hands for two consecutive days. During each day of practice, participants completed 50 30-second trials (10 blocks of 5 trials each) with their more affected hand and 20 30-second trials (10 blocks of 2 trials each) with their less affected hand. The experimental group was given control over their practice schedule and chose the order they practiced blocks of trials. Participants in the control group followed a pre-determined practice schedule based on the choices made by the participant they were paired with in the experimental group. Outcome measures were assessed on Day 1 before practice (Pre-Test), Day 2 after practice (Post-Test), and Day 3 (Follow-up). Motor learning outcomes included 24-hour retention of the trained motor task and immediate (10-minute) and delayed (24-hour) transfer of learning to an untrained motor task. Psychological outcomes including intrinsic motivation, self-efficacy, and positive affect were also assessed. Both groups demonstrated significant improvements from baseline on both the trained task and the untrained task at the Day 3 follow-up assessment. There were no significant between-group differences on any motor learning or psychological outcome measures. However, there was a significant association between perceived choice that was assessed on Day 2 following practice and amount of change on the untrained transfer task at Day 3. This suggests that perception of choice may be a relevant factor for motor learning after stroke. However, more research with larger sample sizes is needed to further investigate this finding.

The development of sensory products to stimulate children with learning problems (LP)

Early intervention is desirable to remediate LP. One strategy is to use visual aids as a cue during an intervention strategy. A visual aid can be tactile and can appeal to the child’s senses, such as hearing and touch. Three main cornerstones which influence Sensory Product Development (SPD) were identified. These are design factors such as illustrations, colour, and book themes, the senses of the children and supporting factors which include therapeutic practices and cultural sensitivity. It is envisaged that if these three cornerstones are integrated into a sensory product such as a book, that it will be able to stimulate a child with LP through play, touch and sound. The aim of the study is the development of a qualitative tool that can be used to asses sensory products and the development of a sensory product that are tested for the specific target group. The sensory product was designed according to the guidelines that were identified in a literature review. This qualitative tool includes the various cornerstones and serves as a checklist that teachers and therapists can use to evaluate a sensory product to establish its suitability for a child with LP. The second article in this study recorded the responses from children with LP towards the sensory product and recommendations for the improvement of the sensory product.

Perceptual-motor development for children who show signs of attention deficit hyperactivity disorder (ADHD)

The five case study individuals (ranging for Grade 1- 7) took part in a six-week Purposeful Play Programme. Baseline, pre-test and post-test scores were obtained from the Bruininks- Oseretsky Test of Motor Proficiency (BOTMP) and Attention-Deficit Hyperactivity Disorder Test (ADHDT). All five case study individuals showed a significant improvement on their motor proficiency during retention tests as well as improvements on some of the behavioural soft signs of hyperactivity, impulsivity and inattention. / AFRIKAANDE OPSOMMING: Die invloed van ʼn sesweek perseptueel-motoriese en aandagvaardigheidsprogram, gekombineer met onderrigstrategieë, is op die motoriese vaardighede en aandagvermoëns van kinders gediagnoseer met ATHS, bestudeer. Die vyf gevallestudie individue (tussen Graad 1- 7) het aan „n sesweek Purposeful Play Programme deelgeneem. Die basislyn-, voor- en natoetstellings is verkry vanaf die Bruininks- Oseretsky Test of Motor Proficiency (BOTMP) en Attention-Deficit Hyperactivity Disorder Test (ADHDT). Al vyf gevallestudie individue het ʼn beduidende verbetering in motoriese vaardigheid getoon tydens retensietoetse asook verbetering in sommige van die gedragstekens van hiperaktiwiteit, impulsiwiteit en afleibaarheid.

A motor skills development programme for 10 to 12 year-old children

In a school setting these developmental delays mostly remain unnoticed and are scarcely remediated sufficiently, if at all. Children with developmental deficits often experience movement difficulties and are labelled as clumsy, sloppy or having two left feet. The development of, and proficiency in specific motor skills have been found to play a critical role in the participation of physical activity in children, as well as the maintenance of health and well-being later in life, when entering adulthood. There are, however, countless other factors that affect the optimal development of motor skills and physical activity participation. One such factor is ascribed to instances where children experience problems associated with their environment or the circumstances in which they grow up. Poor socio-economic circumstances and a culturally poor environment, lacking sufficient developmental opportunities, may hinder a child‟s motor skills development and skill learning to such an extent that they cannot reach their full developmental potential. Research suggests that the school environment can provide ample opportunity for the development of motor skills and that all schools should consider implementing motor skills development programmes during the Physical Education (PE) time slots. The mastery of motor skills may influence and benefit the participation in various school sports and may also enhance the ability of children to learn and master new and more complex movement skills within and outside the classroom environment. Thus, due to the fact that children spend a great part of their day at school and in the classroom setting, teachers, especially those facilitating PE, have the opportunity to play a vital role in the acquisition and mastery of important motor skills and subsequently affect the physical activity and developmental future of children. The purpose of this study was to design and implement a motor skills development programme to improve the balance and bilateral coordination of children between the ages of 10 and 12 years in the Stellenbosch region. Two existing classes, from a previously disadvantaged school, were recruited and randomly selected as an experimental (n=35) and control group (n=32). The children completed the Short Form as well as all the Long Form activities for the balance and bilateral coordination subtests of the Bruininks-Oseretsky Test of Motor Proficiency-2 (BOT-2). This was done to provide an overview of the overall motor proficiency and more specifically the level of balance and bilateral coordination. A 12-week motor skills development programme, with a specific focus on balance and bilateral coordination, was designed and implemented by the researcher. The effect of this programme was determined by statistically analysing and comparing the pre- and post-test results with the use of Statistica 2010. The main findings of this study indicated that the intervention programme had a positive, although not statistically significant, effect on the overall motor proficiency and balance and significantly improved bilateral coordination. This study suggests that some of the children, between the ages of 10 and 12, from a previously disadvantaged school in the Stellenbosch region and with access to an established school PE programme, experienced movement difficulties. Consequently, there is great room for improvement and motor skill development in these children. This study can, therefore, be a stepping stone into future research regarding further motor skills research in primary school children, the implementation of expanded motor skills intervention programmes and to improve all the motor skills as opposed to selected motor skills as in this study. / AFRIKAANSE OPSOMMING: Ontwikkelingsprobleme word, sonder dat ʼn mens daarvan bewus is, deur vele kinders ervaar. In die skool-omgewing bly hierdie ontwikkelingsagterstande meestal ongesiens en word min kinders, indien enige, voldoende geremedieer. Kinders met ontwikkelingsagterstande ervaar gereeld bewegingsprobleme en word as lomp, slordig of as iemand met twee linkervoete beskryf. Daar word beweer dat die ontwikkeling van, en vaardigheid in, spesifieke motoriese vaardighede van kinders ʼn kritieke rol in hul deelname aan fisieke aktiwiteit, asook die onderhoud van gesondheid en welstand tydens volwassenheid, speel. Daar is egter talle ander faktore wat die optimale motoriese ontwikkeling en fisieke aktiwiteit deelname van kinders kan beïnvloed. ʼn Voorbeeld van so ʼn faktor word toegeskryf aan gevalle waar kinders probleme, wat met hulle omgewing of die omstandighede waarin hul grootword geassosieer word, ervaar. Swak sosio-ekonomiese omstandighede en ʼn kultureel-arme omgewing, wat tekort skiet aan voldoende ontwikkelingsgeleenthede, mag die motoriese ontwikkeling en die aanleer van nuwe vaardighede tot so ʼn mate inhibeer dat kinders nie in staat is om hulle volle ontwikkelingspotensiaal te bereik nie. Navorsing dui daarop dat die skoolomgewing vele geleenthede vir die ontwikkeling van motoriese vaardighede kan bied en dat daar onderneem moet word om motoriese vaardigheid-ontwikkelingsprogramme tydens Liggaamlike Opvoeding (LO) periodes te implementeer. Die bemeestering van motoriese vaardighede mag die deelname aan verskeie skoolsporte beïnvloed en bevoordeel, asook die vermoë om nuwe en meer komplekse bewegingsvaardighede binne en buite die klasomgewing aan te leer en te bemeester, bevorder. Die primêre bevindinge van hierdie studie dui daarop dat die intervensieprogram ʼn positiewe, alhoewel nie statisties beduidende, effek op die bevordering van algehele motoriese vaardigheidsvlak en balans, asook ʼn statisties beduidende effek op bilaterale koördinasie gehad het. Hierdie studie dui daarop dat kinders, tussen die ouderdomme van 10 en 12, vanuit ʼn voorheenbenadeelde skool in die Stellenbosch omgewing en wat toegang tot ʼn gevestigde LO program het, steeds bewegingsprobleme ervaar. Dus, is daar groot ruimte vir die verbetering en ontwikkeling van motoriese vaardighede by hierdie kinders. Hierdie studie kan dus as ʼn beginpunt vir toekomstige navorsing in verdere motoriese vaardigheids-navorsing van laerskool kinders, die implementering van uitgebreide motoriese intervensieprogramme, asook die bevordering van alle motoriese vaardighede in vergelyking met geselekteerde motoriese vaardighede soos in die huidige studie dien.

Effect of haptic guidance and error amplification robotic training interventions on the immediate improvement of timing among individuals that had a stroke / Effet de l’entrainement robotisé par réduction de l’erreur et augmentation de l’erreur sur le timing du mouvement chez la personne ayant eu un accident vasculaire cérébral

Abstract : Many individuals that had a stroke have motor impairments such as timing deficits that hinder their ability to complete daily activities like getting dressed. Robotic rehabilitation is an increasingly popular therapeutic avenue in order to improve motor recovery among this population. Yet, most studies have focused on improving the spatial aspect of movement (e.g. reaching), and not the temporal one (e.g. timing). Hence, the main aim of this study was to compare two types of robotic rehabilitation on the immediate improvement of timing accuracy: haptic guidance (HG), which consists of guiding the person to make the correct movement, and thus decreasing his or her movement errors, and error amplification (EA), which consists of increasing the person’s movement errors. The secondary objective consisted of exploring whether the side of the stroke lesion had an effect on timing accuracy following HG and EA training. Thirty-four persons that had a stroke (average age 67 ± 7 years) participated in a single training session of a timing-based task (simulated pinball-like task), where they had to activate a robot at the correct moment to successfully hit targets that were presented a random on a computer screen. Participants were randomly divided into two groups, receiving either HG or EA. During the same session, a baseline phase and a retention phase were given before and after each training, and these phases were compared in order to evaluate and compare the immediate impact of HG and EA on movement timing accuracy. The results showed that HG helped improve the immediate timing accuracy (p=0.03), but not EA (p=0.45). After comparing both trainings, HG was revealed to be superior to EA at improving timing (p=0.04). Furthermore, a significant correlation was found between the side of stroke lesion and the change in timing accuracy following EA (r[subscript pb]=0.7, p=0.001), but not HG (r[subscript pb]=0.18, p=0.24). In other words, a deterioration in timing accuracy was found for participants with a lesion in the left hemisphere that had trained with EA. On the other hand, for the participants having a right-sided stroke lesion, an improvement in timing accuracy was noted following EA. In sum, it seems that HG helps improve the immediate timing accuracy for individuals that had a stroke. Still, the side of the stroke lesion seems to play a part in the participants’ response to training. This remains to be further explored, in addition to the impact of providing more training sessions in order to assess any long-term benefits of HG or EA. / Résumé : À la suite d’un accident vasculaire cérébral (AVC), plusieurs atteintes, comme un déficit de timing, sont notées, et ce, même à la phase chronique d’un AVC, ce qui nuit à l’accomplissement de tâches quotidiennes comme se vêtir. L’entrainement robotisé est un entrainement qui est de plus en plus préconisé dans le but d’améliorer la récupération motrice à la suite d’un AVC. Par contre, la plupart des études ont étudié les effets de l’entrainement robotisé sur l’amélioration de l’aspect spatial du mouvement (ex : la direction du mouvement), et non l’aspect temporel (ex : timing). L’objectif principal de ce projet était donc d’évaluer et de comparer l’impact de deux entrainements robotisés sur l’amélioration immédiate du timing soit : la réduction de l’erreur (RE), qui consiste à guider la personne à faire le mouvement désiré, et l’augmentation de l’erreur (AE), qui nuit au mouvement de la personne. L’objectif secondaire consistait à explorer s’il y avait une relation entre le côté de la lésion cérébrale et le changement dans les erreurs de timing suivant l’entrainement par RE et AE. Trente-quatre personnes atteintes d’un AVC au stade chronique (âge moyen de 67 ± 7 années) ont participé à cette étude, où ils devaient jouer à un jeu simulé de machine à boules. Les participants devaient activer une main robotisée au bon moment pour atteindre des cibles présentées aléatoirement sur un écran d’ordinateur. Les participants recevaient soit RE ou AE. Une ligne de base et une phase de rétention étaient données avant et après chaque entrainement, et elles étaient utilisées pour évaluer et comparer l’effet immédiat de RE et AE sur le timing. Les résultats ont démontré que RE permet d’améliorer les erreurs de timing (p=0,03), mais pas AE (p=0,45). De plus, la comparaison entre les deux entrainements a démontré que RE était supérieur à AE pour améliorer le timing (p=0,04). Par ailleurs, une corrélation significative a été notée entre le côté de la lésion cérébrale et le changement des erreurs de timing suivant AE (r[indice inférieur pb]=0,70; p=0,001), mais pas RE (r[indice inférieur pb]=0,18; p=0,24). En d’autres mots, une détérioration de l’exécution de la tâche de timing a été notée pour les participants ayant leur lésion cérébrale à gauche. Par contre, ceux ayant leur lésion à droite ont bénéficié de l’entrainement par AE. Bref, l’entrainement par RE peut améliorer les erreurs de timing pour les survivants d’AVC au stade chronique. Toutefois, le côté de la lésion cérébrale semble jouer un rôle important dans la réponse à l’entrainement par AE. Ceci demeure à être exploré, ainsi que l’impact d’un entrainement par RE et AE de plus longue durée pour en déterminer leurs effets à long terme.

Effect of Task Appropriateness, Social Comparison, and Feedback on Female Goals, Performance, and Self-Confidence with a Motor Task

Lenney (1977) concluded that achievement gender differences were predicted by females' lower self confidence and expectancies in competitive situations, identifying three variables that mediated female self confidence in achievement situations, (1) task appropriateness. (2) social comparison, and (3) feedback. The present study manipulated all three mediating variables with 240 undergraduate 18-25 year old female subjects with the pursuit rotor task that requires tracking a moving (40 rpm's) white light with a hand-held stylus for 60 seconds. Response measurement was based upon time on target. Subjects were tested over five trials while setting goals for each trial. Females were randomly assigned to a male appropriate, female appropriate, or gender neutral task condition, a competition or alone condition, and to one of four feedback conditions (no feedback, feedback about own performance only, feedback about own performance that provided the perception that subject was performing better than an opponent and/or average on each trial, or feedback about own performance that provided the perception that subject was performing poorer than an opponent and/or average on each of the five trials). Results from the 2 (social comparison) X 3 (task appropriateness) X 4 (feedback) ANOVA were contradictory to previous findings (Corbin, 1981; Petruzzello & Corbin, 1988) as females performed significantly better in competition than alone. Data support the conclusion that presentation of clear and unambiguous feedback enhanced female self-confidence (Corbin, 1981; Petruzzello & Corbin, 1988; Lenney, 1977). Data also provide null findings for the task appropriateness condition which contradicts the previous research (Corbin, 1981; Lenney, 1977) in that females perceiving the task as male appropriate did not exhibit less self-confidence and perform poorer than when the task was perceived as either female appropriate or gender neutral. Conclusions reflect methodological differences from previous research and changes in gender role identification that have significantly impacted on female self-confidence and performance in competitive situations.

Chronological Age as a Factor in Motor Learning in the Mentally Retarded

The problem investigated is that of determining if there are differences in the ability of mentally retarded persons over age 21 to learn motor skills as opposed to those under 21. Data were gathered at the Denton State School on 110 subjects. The first chapter is concerned with the theoretical background, purpose, and the hypothesis. The second chapter contains information on subjects, materials, method, and statistical treatment. The third chapter covers presentation and discussion of the data, and the fourth chapter includes the summary, conclusions and recommendations. Results of the study indicated that there were few differences between the two groups. Future studies should be run with samples from individual age groups extending from 6 through 21. This would be realistic in establishing a motor learning curve for this population.

An Experimental Investigation on the Effects of Web-Based Instruction/Training on Cognitive and Psychomotor Learning

The purpose of this study was to investigate the effects of web-based instruction (WBI) on cognitive and psychomotor learning. The subjects of the study received two types of instructional methods, WBI (experimental group) and traditional classroom instruction (control group). Each group received 30 minutes of instruction on "Soldering a Circuit Board." The researcher chose this content subject because it involved both cognitive and psychomotor objectives, which suited the purpose of this study. It was hypothesized that there would be no significant difference between the two methods of instruction, and also that there would be no significant interaction effects between methods of instruction and gender. Forty-six subjects from a population of students enrolled in summer classes offered by the Applied Technology, Training and Development (ATTD) program at the University of North Texas voluntarily participated in this study. Random assignment of subjects was applied in this study. A subject matter expert delivered the content for both the experimental and control groups. To measure cognitive variable, a 10 item, multiple-choice test was administered immediately after instruction. To measure the psychomotor variable, a 15-item checklist was utilized by trained judges to evaluate learners’ performances while soldering. The 2 x 2 factorial model with interaction was used in this study. The analysis was run for each of the dependent variables, cognitive and psychomotor learning. Although there was not a statistically significant difference in the main effects of method of instruction or interaction effects between method and gender, the results imply that students in the traditional-classroom instruction group performed better than those in the WBI group in psychomotor learning. Perhaps, this trend would be statistically significant if the sample size were larger. This study provides empirical evidence for the effectiveness of WBI in delivering cognitive and psychomotor objectives. The outcome of this study supports the need for more research on the effects of WBI on learning domains.

We followed the immediate effect of sensorimotor training on postural stability in patients with spinocerebellar ataxia (SCA). We have tried to explain the possible mechanisms of adaptation to change COP, motor learning and neuroplasticity in the field of neurodegenerative changes in the cerebellum. Research Methods: A total of 10 patients with spinocerebellar ataxia (6 men, 4 women) aged 34-71 years, with an average disease duration of 11.6 years participated in the measurement of postural stability. First, we measured the patients with SCA before and after a 15-minute sensorimotor-training exercise, then this group underwent another day of the same measurement without sensorimotor training. As a second control group of 10 healthy subjects served a similar age, who also underwent sensorimotor training. Then we compared the results of posturographic measurement. Results: In the group of SCA patients, sensorimotor training was found significantly lower values in the monitored parameters LENGTH (t stat = 3,537731 > t krit = 2,262157), VELOCITY (t stat = 3,537731 > t krit = 2,262157) and energy to sustain COP (t stat = 2,715085 > t krit = 2,262157), measured on a foam pad with eyes closed. In the practical part there is a description of testing procedure which finds out the level of space orientation, balance and rhythm coordination abilities by pupils in the forth form at primary school. The tests results are statistically analysed and annotated. Obtained data are compiled in charts and diagrams.
