Acute endomyocardial disease in infants and children : the relationship between acute myocarditis and endocardial fibroelastosis

This prospective study of acute myocarditis (AM) and endocardial fibroelastosis (EFE) was prompted by their common occurrence in infants and children in Cape Town, and by the persisting controversy regarding the possible relationship of these two conditions to each other, and to idiopathic, chronic, congestive cardiomyopathy (COCM). Patients with AM and EFE were analysed concurrently and over the long-term. The following hypotheses were investigated: A) that AM and EFE represent different phases of a common disease process, and B) that either AM or EFE evolves into COCM. From 1st June 1970 to 31st December 1976 (a study period of 6 years 7 months), 140 consecutive patients with AM or EFE were evaluated, and continually observed until 31st March 1979 (a total observation period of 8 years 10 months). Because there is no definitive, non-invasive, in-vivo diagnostic test for AM or EFE, an inclusive diagnosis of acute endomyocardial disease (EMD) was made in 123 patients who fulfilled all 4 rigid clinical criteria, i.e. a short history ( < 1 month), clinical evidence of myocardial involvement (heart failure, gallop rhythm or shock), radiological cardiomegaly (CTR > 0.55), and ST/T wave changes on electrocardiogram (ECG). Acute EMD was confirmed in all 20 patients who came to autopsy. A further 17 patients with insufficient clinical data had EMD at post-mortem.

O número de indivíduos com sobrepeso vem crescendo mundialmente representando atualmente mais de um bilhão de pessoas, sendo que, mais de 30% desta população é obesa. No Brasil, a previsão é que 35% da população adulta brasileira se tornará obesa em 2025. Pesquisas clínicas indicam que a obesidade está associada com o aumento do risco de morbidade, mortalidade e redução na expectativa de vida, podendo acarretar diversas co-morbidades, como doenças cardiovasculares. Estudos mostram que a obesidade promove alterações hemodinâmicas, morfológicas e funcionais cardíacas, que se correlacionam com a duração e intensidade da obesidade. Modelos experimentais relacionando função cardíaca, obesidade e dieta hipercalórica por um período de 10 a 16 semanas, apresentam resultados inconsistentes. Enquanto alguns autores relataram comportamento preservado, outros observaram aumento ou diminuição do desempenho cardíaco. Em razão de poucos estudos avaliarem a relação entre função cardíaca e o trânsito intracelular de cálcio em animais obesos por dieta hipercalórica, o objetivo deste estudo foi avaliar a função cardíaca de ratos obesos submetidos a um ciclo de dietas hipercalóricas. Ratos Wistar machos, com 30 dias, foram randomizados em dois grupos: controle (C; n=28) e obeso (OB; n=25). In Brazil, the prevision is that 35% of the Brazilian adult population will become fat in 2025. Clinical research indicates that the obesity is associated with the increase of the risk of morbity, mortality and reduction in the life expectancy, being able to cause diverse co-morbidities, as cardiovascular diseases. Studies show that the obesity promotes hemodinamical, morphological and functional heart alterations that correlate with the duration and intensity of the obesity. Experimental models, relating cardiac function, obesity and high fat diet for a period of 10 to 16 weeks, present inconsistent results. While some authors had told about preserved behavior, others had observed increase or reduction of the cardiac performance. In reason of few studies to evaluate the relation between cardiac function and the intracellular calcium cycling in fat animals by high fat diet, the objective of this study was to evaluate the cardiac function of submitted fat rats to a cycle of high fat diet. Sendo o cão um modelo experimental adequado para o estudo dessa doença em humanos, realizou-se este trabalho visando a caracterizar as alterações eletrocardiográficas, ecodopplercardiográficas e hematimétricas, de cães experimentalmente infectados com Trypanosoma cruzi, cepa Colombiana, em sua fase crônica indeterminada. Para tanto, 13 cães adultos, fêmeas foram infectados com o referido protozoário e submetidos a avaliações eletrocardiográfica, hematimétrica, bioquímico-sérica e ecocardiográfica durante sua fase crônica (1997 a 2004). A arritmia sinusal respiratória foi o ritmo predominante durante todo o período experimental, com baixa prevalência de bloqueio de ramo direito e crescente incidência de bloqueio atrioventricular de primeiro grau. A avaliação ecodopplercardiográfica revelou inversão das ondas E e A mitral, confirmando o distúrbio diastólico nesses animais. A avaliação bioquímica revelou atividade enzimática aumentada, confirmando a lesão cardíaca provocada pela infecção mesmo nesta fase. Desta forma, o presente ensaio contribui de forma singular para o conhecimento clínico desta afecção, trazendo mais informações acerca da tênue e obscura linha que separa a fase crônica indeterminada e a fase crônica cardíaca, na espécie canina experimentalmente infectada pela cepa Colombina do Trypanosoma cruzi. / The Chagasic cardiomiopathy is present in the human history since the first civilization. In the American continent, 200 millions people have been affecting by this disease, mainly in south hemisphere, causing important economics and social damages. The great number of strains and obscure clinical behavior in the indeterminate phase, intensify diagnostic failure and increase sudden death. The dog is an excellent experimental model to study this disease in human, therefore this research was developed to characterize the electrocardiographic, echodopplercardiographic and haematological changes, in experimentally infected dogs with Trypanosoma cruzi, Colombian strain, in a chronic indeterminate phase. For that, thirteen infected adult dogs, female, were submitted to electrocardiographic, haematological, biochemical and echocardiographic evaluations during a chronic phase (1997 to 2004). The respiratory sinus arrhythmia was the most frequent rhythm during this research, with low prevalence of right bundle branch block and growing incidence of first degree atrioventricular block. The echocardiographic mitral waves E and A inversion, demonstrated the diastolic dysfunction on this animals. All tested serum enzymes had increased activity, confirming the cardiac lesion caused by the infection even in that phase. Therefore, this research contributed in a unique way for the clinic knowledge of this disease, bringing more information about this fine transition line between the indeterminate and cardiac phase, in experimentally Trypanosoma. cruzi, Colombian strain infection of canine species.

Die Rolle der Magnetresonanz in der kardiologischen Diagnostik unter besonderer Berücksichgiung myokardialer Gewebeveränderungen

In den dargestellten wissenschaftlichen Arbeiten wurden neue Ansätze und Techniken entwickelt zu folgenden Fragestellungen: - Anwendbarkeit der 31P-MR-Spektroskopie zur Darstellung einer Gewebeischämie - Reproduzierbarkeit der MRT in der Messung linksventrikulärer Parameter zu Masse und Volumina bei dilatativer Kardiomyopathie - Eignung 2-dimensionaler MRT-Messmethoden zur Messung der linksventrikulären Parameter zu Masse und Volumina - Quantifizierung stenosierter Flächen mittels MRT bei hypertrophischer Kardiomyopathie und bei Aortenklappenstenose - Darstellung der entzündlichen Aktivität im Herzmuskel bei Myokarditis - Darstellung der entzündlichen Veränderungen bei kardialer Sarkoidose - Frühzeitige Darstellung des toxischen Myokardschadens nach Anthrazyklin-Therapie. Der Wert der MRT wurde in experimentellen und klinischen Studien im Vergleich zu etablierten Verfahren geprüft. Die Ergebnisse haben gezeigt, dass sich die physikalischen Möglichkeiten in klinisch anwendbare Untersuchungsprotokolle umsetzen lassen und bereits jetzt in der klinischen Routine eingesetzt werden können. / In the presented scientific works new approaches and techniques were developed for: - Application of 31P-MR spectroscopy in myocardial ischaemia - Reproducibility of MRI for the measurement of left ventricular parameter of mass and volume in dilated cardiomyopathy - Suitability of 2-dimensional MRI techniques for measuring left ventricular parameter of mass and volume in dilated cardiomyopathy - Quantification of stenotic orifices by MRI in hypertrophic cardiomyopathy and aortic valve stenosis - Visualisation of the inflammatory activity of the myocardium in myocarditis - Visualisation of the inflammatory changes in cardiac sarcoidosis - Early visualization of the toxic myocardial injury after anthracycline therapy. The value of MRI was tested in experimental and clinical studies in comparison to established modalities. The results have shown that it is possible to transmit the physical possibilities into clinically applicable protocols and to apply them into clinical routine.
