Their occurrence is associated with many problems and therefore should be avoided. Effective prevention involves analyzing risk factors and implementing interventions to minimize these risks. The aim of this work was to analyze the influence of drugs and other risk factors on the fall of hospitalized patients. Methods: Data was collected from April to December 2017 in 4 hospitals in South Bohemia. Only patients who had fallen during hospitalization participated in the study. An online database was created for the study to collect patient data (personal history, medications used, laboratory values and others). Population and individual fall risk-increasing drugs were analyzed. For each individual fall risk-increasing drug, the type of drug problem that could have been the cause of a fall was classified. The work is addressed in accordance with the metodology of risk analysis WaterRisk. The content of the work is a summary of the hydraulic of siphon pipes and their use in practice, hazard identification, defining the most frequent undesired events and consequences on siphon pipes, and their verification in the spring in a particular case study.

Analysis of spontaneous adverse events reports of Infanrix hexa vaccine Author: Klára Kovaříková Supervisor: PharmDr. Eva Zimčíková, Ph.D. Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Králové, Charles University Introduction: Infanrix hexa vaccine is used for vaccination of infants and toddlers against diphtheria, tetanus, pertussis, hepatitis B, poliomyelitis and disease caused by Haemophilus influenzae type b. Analysis of spontaneous adverse drug reactions (ADRs) reports is one of the most important sources of information to evaluate drug-related risks. Objective: The aim of this study was to analyse spontaneous ADRs reports of Infanrix hexa vaccine, which were sent to the State Institute for Drug Control (SÚKL) database in the period from 2004 to 2017. Methods: The data was analysed using descriptive statistic in MS Excel. ADRs were classified according to the MedDRA. Seriousness and expectedness of ADRs were evaluated. Results: Overall 1288 reports were obtained containing 4334 ADRs, approximately 3.4 ADRs per report. Infanrix hexa vaccine was the only suspected drug in most of the reports (75.9%). Sodium (Na+ ) is the major extracellular cation. It plays an important role in maintaining membrane potential and depolarization that is the basic mechanism of transmission of the nerve impulse. It is involved in maintaining acid-base balance, osmotic pressure, water retention in the body. The largest part of the sodium is in the extracellular fluid where it is stored about 50 % of sodium. Plasma concentration of sodium is 140 ± 5 mmol/l. Drug-induced electrolyte disorders are increasingly reported and may be associated with considerable morbidity and mortality. Nonetheless, it is not entirely risk-free. This thesis deals with the awareness of those risks in women who are currently using hormonal contraception or have ever used it before. Aim of the thesis: The main aim was to determine the extent of the users' awareness of hormonal contraception and its risks, undesirable side effects and contraindications, as well as their source of information. Furthermore, the awareness of the environmental impact, the reasons for using hormonal contraception and user satisfaction were examined. Methodology: The research was carried out by means of online questionnaires, which were spread among current or former users of hormonal contraception regardless of age, educational background or place of residence. 148 completed questionnaires were gathered and the data which were obtained were statistically evaluated. This thesis employs a parametric input distance function that incorporates both desirable and undesirable outputs to provide a more complete representation of the production technology. Based on the Shephard (1970) theory of duality, we derive the shadow prices of undesirable outputs in the Czech energy sector on the data over the period 2002 - 2007. The medians of our shadow prices estimates are 8374, 1198, 2805, 6051 and 8549 € per ton of PM, SO2, NOx, CO and VOC, respectively. We decompose shadow prices estimates and test the hypotheses that the marginal abatement cost decline over time; that marginal abatement cost rice with the declining emission level; and that marginal abatement cost rice with declining emission rate. Hodnocení kultury bezpečí ve vybraných nemocnicích Ústeckého a Libereckého kraje / Assessment of safety culture in selected hospitals in the Ústí nad Labem and Liberec regions

Current status: The issue of medical errors has recently gotten considerable attention among organizations at the global and national levels. In its report the Institute of Medicine recommended the development of a safety culture where employees want to provide the safest health care. Assessing the current culture of safety is the first stage in the development of patient safety. Subject: The main purpose of the research was "to evaluate the safety culture in select hospitals." Method: A standardized method was chosen for the research: The AHRQ Hospital Survey on Patient Safety Culture. A quantitative method was implemented using polling. Obtained data were tested in SPSS version 16.0. For statistical testing Person's chi-square and sign scheme was elected. Research group: The research survey addressed 301 non-medical staff of Krajské zdravotní a.s. Masarykova nemocnice, o. z. (Regional Health a.s. Masaryk Hospital) in Ústí nad Labem and Krajské nemocnici Liberec, a.s. (Liberec Regional Hospital) Results: The survey results showed that the surveyed health care professionals assess the organizational culture as friendly. The respondents indicated that they are praised by the manager for conducting their work safely and that they can suggest changes in management practices. The results showed that addressed health care professionals support each other (58.1%) and cooperate better (72.4%). According to the respondents, better teamwork leads to better patient safety. An analysis of the results shows that health professionals have an active approach to safe care (82.7%) and in their opinion, errors and mistakes lead to improved patient safety (40.9%). Paramedics have confirmed that they receive feedback on reported incidents (37.9%). Based on team discussion about errors the respondents' evaluation of patient safety has significantly improved. An analysis of respondents' answers revealed a lack of reporting of adverse events. Most respondents consider the number of staff in the workplace deficient (42.5%). Conclusion: Hospital management should be involved in changing the detection of individual errors from individual to systemic. In order to improve the safety culture there should be regular supervision or teambuilding activities that support the development of teamwork. Feedback on the results of reporting errors must be given in an appropriate way to motivate staff to continue to report future errors. The results require a change in how medical staff report errors and greater consistency among management when checking reports. Complaints of work overload by medical personnel must be taken seriously. Improvements can be brought about by changes to work organization or the use of temporary workers.

The diploma thesis deals with the topic of human failures during the risk activities. The aim of the work is to apply risk analysis to the operation of a selected unnamed company that processes vegetables. The thesis is divided into three parts. In the first part, the theoretical basis is discussed and serves as a basis for a better understanding of the topic and subsequent elaboration of the analytical part. The analytical part focuses on the characteristics of the selected company, organizational structure, working positions and the operation itself and analyses these issues with the focus on human factor. The last part of the diploma thesis is dealing with suggestions leading to the prevention of human failure in risk activities. The aim of these suggestions is to improve the situation of the company.
