The thesis deals with the course and character of experienced toxic psychosis and its development over time, in clients at present long term residing in the therapeutic community. The thesis searches relatedness between several parameters defined methamphetamine abuse (intensity, length of use) and the length of the event period. Research core thesis are qualitative case analyzes, in which the emphasis is a typical manifestation of toxic psychosis and its descriptive progress with regard to mentioned parameters of abuse and also to accentuate the importance of toxic psychosis borders as a result of methamphetamine abuse, compared with endogenous psychoses of schizophrenia circuit. The aim of this thesis is to describe individual cases manifestations of psychosis through five client stories and explore the process of psychical recovery and regression of clinical symptoms in time, in clients with the prospect of full recovery. For this particular research the author elected a qualitative approach and storylines of five clients were processed as a set of case studies. The diploma thesis is focused on risk analysis of water supply system in a small community. The WaterRisk project was solved by using tools of risk management including risk identification and quantification in the field of public water supply systems. Methodology and software for the risk management of water supply were evolved by using this project. In this thesis there is applied the methodology to the water supply system of Hradek village. The hydraulic analysis was performed for a thorough knowledge of water supply and its characteristic behavior. Risk analysis could be performed thanks to the knowledge of the entire system, its working and the most common faults. Analysis results are evaluated and compared for the water supply system which is consider like a system simple and complex. The corrective actions eliminate number of risks. These actions were proposed like a part of complex methodology. Further, there were determined optimal costs which corresponds with these corrective actions.

IN ENGLISH The aim of my thesis is to introduce the system of monitoring of undesirable events and its relationship with the legislative regulations of the Czech Republic and outline the process of controlled change in the organization. The theoretical part of the thesis is dedicated to the general description of the system of reporting undesirable events at national and international level and is based on research from available literature. The other important part of the theoretical part of the thesis is dedicated to the quality system and basic processes of controlled change processes as described by the Czech and foreign authors. The practical part of the thesis is based on a case study from The Žatec Hospital, which represents the initial analysis of the monitoring system of the undesirable events in the organization, planning and description of the processes and individual steps in the process of controlled change with subsequent analysis of the system of adverse event monitoring after aforementioned change. Objectives Main objectives of this doctoral thesis were to review available information on pharmacological properties of benzodiazepines and their age-related changes; to evaluate the prevalence of benzodiazepine use in older patients residing in long term care facilities; to investigate the association between use of benzodiazepines and occurrence of falls in acutely hospitalized older patients; and to describe utilization of benzodiazepines in the Czech Republic. Methods A narrative review of literature focused on pharmacokinetics, pharmacodynamics, adverse effects and association of benzodiazepines with falls in older population was conducted. The evaluation of benzodiazepine use in long term care facilities was analysed in a retrospective cross-sectional study using data from the EC 7th Framework Program SHELTER project (Service and Health in the Elderly in Long Term Care). A prospective cohort study data of acutely hospitalized patients in Australia were used to evaluate association between benzodiazepines and falls. To describe utilization of benzodiazepines in the Czech Republic, data from the State Institute for Drug Control and from databases of General Health Insurance Fund were used. OBJECTIVES: The aim of our study was to assess clinical presentation of early-onset schizophrenia spectrum disoders (EO-SSD), the time to first improvement and efficacy associated with selected atypical (AAPs) and typical (TAPs) antipsychotics, as well as two main side effects - weight gain and treatment-emergent extrapyramidal symptoms (EPSs) during the treatment in patients with EO-SSD. METHODS: This was a systematic chart review of all patients receiving routine clinical care in our department, with selected AAPs (risperidone, olanzapine, ziprasidone, quetiapine and clozapine) and TAPs (haloperidol, perphenazine and sulpiride), for schizophrenic psychoses, between 1997 and 2007. During this period, our review identified 173 patients (85 males, 88 females; mean age 15.8±1.6 years); their treatment included 297 treatment trials. Data on premorbid adjustment, prodromal symptoms and psychopathology at admission, as well as comorbidity were evaluated based on the patients' medical records. The time to first improvement could be estimated in 258 treatment trials; of these, 195 (76%) comprised AAPs and 63 (24%) TAPs. A secondary objective is to evaluate these polymorphisms in relation to the analgesic efficacy and side effects of piritramide for acute postoperative pain. Materials and methods: In two prospective work studying the influence of genetic predisposition on the pharmacokinetic and pharmacodynamic parameters of tramadol, we included a total of 90 healthy volunteers. Clinical studies on opioid analgesia and influence of genetic predisposition to the pharmaco-therapeutic effects and side effects in patients with acute postoperative pain, we included a total of 161 patients with acute postoperative pain. The current thesis deals with the issue of adverse events. However, it does not concern the number of described and recorded adverse events in healthcare facilities or their spectrum. I am particularly interested in the attitude of the staff at the University Hospital Pilsen and how to respond to the requirements of the TOP management in the recording of adverse events. The theoretical part will concentrate on explaining the concept "adverse event", the description of influences which cause such events to arise and also to summarize the current situation concerning the approach to adverse events in the Czech Republic. The empirical part of the thesis describes the implementation of qualitative research, its results and recommendations for future practice. I am interested in researching the perspective of employees and their level of cooperation and openness in the approach to the reporting of adverse events in the provision of therapeutic, diagnostic and nursing care in inpatient health care facilities. The quality of nursing care is becoming an increasingly important concept in contemporary nursing. The most effective means of achieving quality of nursing care and the means of proof is the accreditation of medical facilities. It is a significant appreciation of professionalism, the work of health professionals and an important signal for the patients. Quality of care, security of supply and preventing potential risks for health care priority. The main aim of the thesis "The influence of accreditation on quality of care in selected health facilities" was to map the set of quality management and impact monitoring and evaluation of quality indicators in hospitals accredited by the Czech Republic. To determine whether accreditation has become an instrument to ensure comparable quality, and time has changed from an unpopular instrument into an instrument for continuous quality improvement. The survey took place from November 2010 to March 2011 in the form of quantitative data collection using an anonymous questionnaire survey in selected hospitals accredited by the Czech Republic. In the research study were asked managers of nursing care, main, upper station and sisters. The thesis has been established 6 goals. The first objective was to determine what indicators of quality nursing care in accredited hospitals pursue. Interested in us as quality indicators that were studied before accreditation, and quality indicators to setting up medical facilities in preparation for accreditation. The second objective was determine how frequently watched indicator of quality is monitored in hospital. The third objective was to determine whether there was in accredited hospitals to reduce adverse events - falls. The fourth objective was to determine whether there was in accredited hospitals to reduce the incidence of nosocomial infections in complying with barrier nursing regime. The fifth objective was to identify the biggest problems arose during the implementation of audits, completeness and comprehensiveness of nursing documentation management into practice. " The sixth objective was to determine whether there was during the preparation of an increase in hospital nursing staff. The thesis then was examined six hypotheses. First hypothesis Accredited hospitals monitor the quality of care provided by at least ten quality indicators - were confirmed. Hypothesis 2 The most frequently monitored indicator of the quality of nursing care, the prevalence of pressure ulcers - was confirmed. All respondents said they monitor the prevalence of pressure ulcers as an indicator of quality nursing care. Hypothesis 3 gaining accreditation to reduce the incidence of falls - was confirmed. Hypothesis 4 obtaining accreditation decreased nosocomial infections - was confirmed. Hypothesis 5 The biggest problem in implementing the audit, management complexity and completeness of nursing documentation in practice "has been an increase in paperwork for nursing staff - were confirmed. Hypothesis 6 During the preparation for accreditation has been an increase in nursing staff - has not been confirmed. Accreditation is perceived by respondents as beneficial for the hospital. Due to accreditation standards and developing quality indicators were identified risk areas in the provision of hospital care and detail can be paid to the prevention of adverse events, nosocomial infections and other risks. Abstract The introduction of physiologically and therapeutically effective drugs in anaesthetic treatment has meant a significant change. Their discovery has thus contributed to a minimisation of their negative effects on a living organism. As a result, anaesthesiology has become more controllable and safer. These new drugs translate into more comfort for the patient in the course of anaesthesiology as well as his or her rapid recovery resulting in a shorter period of hospitalisation. The research of new physiologically and therapeutically effective substances is a pre-requisition of a potential higher standard of medical care. Older substances, e.g. Alkuronium and Gallamin, which, due to their undesirable effects, have come out of use, have lost their significance. Nowadays, these substances have been generally replaced by substances with an intermedial effect, e.g. Rokuronium and Cisatrakurium, which, compared with Alkuronium and Gallamin, show a minimum of undesirable effects. One of the foremost objectives of the current pharmaceutical research is to find a replacement of Sukcinylcholin, which, in spite of its known side effects, has had a non-substitutable position in urgent intubation. As a result, its use is limited to out-patient application and a certain selection of patient categories. Let us hope that we shall see a replacement of Sukcinylcholin being introduced in treatment in the near future. This would mean a final solution of problems arising from its side effects limiting its scope of application.

This thesis is a case study with a goal to comprehensively map and depict the evolution of the so- called non-supplement zones. This name is an abbreviated term that is used in media for the general provision authorizing establishing of areas of higher incidence of socially undesirable phenomena, where housing supplements are not to be payed to any new applicants. Bohemian and Moravian municipalities can make use of this measure since 2017. Thus they aim to fight the poverty industry and the problems connected with it. Additionally, a Constitutional Court decision on the appeal on this measure made by a group of senators is awaited since 2017. In this thesis, the problematique is viewed both on policy and politics level, both on national and local level, using the theory of the policy process, agenda setting, and the multiple streams theory. Under examination are also the levels of discourses and framing of the problem. To demonstrate a local context, there was chosen the case of the Kladno municipality, which holds a specific position in the context since it is the first city in the Czech Republic that decided to declare this general provision on its entire territory.
