Aconselhamento nutricional em idosos com constipação intestinal funcional: efeitos de ensaio clínico aleatorizado / Elderly's nutritional counseling with functional bowel constipation: causes of randomized clinic trial

Introdução: Os idosos representam um segmento demográfico crescente tanto nos países desenvolvidos como nos em desenvolvimento. A constipação intestinal é um sintoma relatado por 24 a 40% desses indivíduos, afetando 26% dos homens e 34% das mulheres. O consumo de uma dieta rica em fibras alimentares e líquidos ajuda na diminuição das queixas relacionadas à constipação intestinal funcional. Os idosos necessitam de atenção especial com intervenções dietéticas diferenciadas por meio de ações educativas específicas. Objetivo: Analisar os efeitos do aconselhamento nutricional centrado no cliente, em idosos com constipação intestinal funcional matriculados em ambulatório geriátrico. Métodos: O estudo foi experimental do tipo ensaio clínico aleatorizado realizado entre abril de 2002 e novembro de 2003. Os idosos foram alocados em grupo intervenção (n=19) e controle (n=21) e os dois grupos participaram de sete consultas mensais. O aconselhamento nutricional centrado no cliente foi o modelo educativo adotado nas consultas com o grupo intervenção. Esse modelo considera a educação como o processo de ensino, treinamento e facilitação, que permite a troca de informações entre o educador e o educando, por meio de uma linguagem entendível, dentro de um ambiente que conduz ao aprendizado. As variáveis analisadas foram: Índice de Massa Corporal (IMC), prática de atividade física, consumo alimentar (energia, carboidratos, lipídios, grupo das frutas, das leguminosas, ingestão de líquidos, grupo dos cereais, consumo de fibras alimentares), uso de medicamentos total e laxante e as queixas que caracterizam a constipação intestinal funcional. O consumo alimentar foi avaliado pelo recordatório de 24 horas em medidas caseiras e a atividade física pelo Questionário de Baecke. Os dados foram analisados por meio da Análise de Variância de Medidas Repetidas (paramétrica ou não paramétrica) e método de Bonferroni. O nível de significância adotado para os testes foi de 5%. Resultados: Os idosos mais jovens do grupo controle eram mais pesados que os mais velhos desde o início do acompanhamento (p=0,001) e os dois grupos não alteraram a prática de atividade física. O grupo intervenção aumentou o consumo de líquidos a partir da quarta consulta até o final do seguimento quando comparado à entrevista inicial (p=0,031) e, no que se refere à ingestão de leguminosas, observou-se aumento no consumo na quarta (p=0,028) e sexta (p=0,042) consultas na comparação intergrupos. Em relação ao uso de medicamentos total, tanto o grupo controle quanto o grupo intervenção não modificaram seu consumo (p=0,650), por outro lado, o grupo controle consumiu mais laxantes durante o acompanhamento quando comparado ao grupo intervenção (p=0,018). Os dois grupos apresentaram redução das queixas que caracterizam a constipação intestinal funcional durante o seguimento. Conclusões: Os resultados desse estudo indicam que o aconselhamento centrado no cliente é eficaz como uma abordagem educativa para a mudança de comportamento alimentar e conseqüente diminuição das queixas intestinais que caracterizam a constipação intestinal funcional. / Introduction: The elderly represent a growing demographic line not only in the developed countries but also the ones in developing. The bowel constipation is a related symptom for 24 to 40% of those persons, affecting 26% men e 34% women. The consumption of diet full in dietary fibers and liquids helps on decreasing of claims related to functional intestinal constipation. The elderly need to a special attention with diversified dietetics intervention through specific educated actions. Objective: Analyze the effects of client-centered nutritional counseling, in elderly with functional bowel constipation enrolled in geriatric ambulatory. Methods: The studying was experimental as a randomized clinic trial carried out between April 2002 and November 2003. Elderly were assigned in intervention group (n=19) and control (n=21) and both groups participated of seven consults, one per month. The client-centered nutritional counseling was the educating model adopted in consults with the intervention group. That model takes into account the education as the process of teaching, training and make it easy, that allows the changing of information between the educator and the student through understanding language inside of a place that lead to learning. The variables analyzed were: Body Mass Index (BMI), practice of physical activity, food intake (energy, carbohydrate, lipids, group of fruits, group of leguminous, ingestion of liquids, group of cereals, intake of dietary fibers), using of total drugs and laxatives and the claims that characterizes the functional bowel constipation. The food intake was evaluated with 24 hours recall on household measures and the physical activity by Baecke Questionnaire. The data were examined critically through Analyses of Variance With Repeated Measures (parametric and non-parametric) and Bonferroni's method. The significance level adopted to the tests was 5%. Results: The youngest elderly of control group were heavier than the oldest since the beginning of accompanying (p=0,001) and both group didn't change the practicing of physical activity. The intervention group increased the consumption of liquids since of fourth consult up the ending of segment when compared with initial interview (p=0,031) and, in what refers to ingestion of leguminous, it was observed an increasing in consumption in forth (p=0,028) and sixth (p=0,042) consults in the comparing between the groups. Relating the using of total drugs, not only the control group but also intervention group didn't change their consumption (p=0,650), on the other hand, the control group consumed more laxatives during the accompanying when compared to intervention group (p=0,018). Both groups presented reduction of claims that characteristic the functional bowel constipation during the segment. Conclusions: The results of that studying indicate that the client-centered counseling is efficient as an educative approach to the changing of alimentary behavior and consequent reduction of intestinal claims that characterize the functional bowel constipation.

Predictors of Nutritional Counseling Among Adolescents by Physicians in California

The prevalence of obesity in the United States was stable at 17% from 2011-2014 among approximately 13 million children and adolescents 12-19 years of age, according to the Centers for Disease Control and Prevention. Nutritional counseling has been recommended, and while most physicians agree that it is necessary, it is not happening consistently. The purpose of this study was to examine the association between dietary intake, physical activity, and body mass index (BMI) characteristics of youth 12-17 years old in California and the receipt of nutritional counseling. Social cognitive theory guided this quantitative cross-sectional analysis of archived data from 814 youth in the California Health Interview Survey (CHIS) 2015-2016. Independent variables included demographics (age, gender, and race), BMI, unhealthy and healthy foods, physical activity and being sedentary; the dependent variable was receipt of nutritional counseling. Findings showed that 73.5% of children who were obese or overweight received counseling. BMI was the strongest predictor of dietary counseling. Independent of diet, physical activity, age, gender, and race, youth who were obese (BMI, kg/m2, -?¥95th percentile) were almost 100% more likely to talk to their doctor compared to children of normal weight (multivariable-adjusted OR=1.98, 95% CI: 1.23, 3.20). The number of cans of sweet fruit/sports drinks was significantly associated with the outcome (p=0.04). There was no significant association between fruit and vegetable consumption (adjusted OR=1.02, 95% CI: 0.95, 1.10) or physical activity (adjusted OR=0.99, 95% CI: 0.71, 1.39) and nutritional counseling. The implications of the study findings for social change include potential improvements in obesity screening and counseling to improve adolescent health outcomes. Addressing weight issues and diet /lifestyle education earlier in life, could reduce chronic disease in the youth.

Educating the Dietitian on Nutritional Counseling Principles in Diabetes Mellitus and their Application for Adolescents with Insulin-Dependent Diabetes: Use of a Learning Package

The purpose of this study was to develop and test a Learning Package on nutritional counseling principles in diabetes mellitus with specific application for adolescents with insulin-dependent diabetes. The Learning Package was designed to be used as continuing education material for the clinical dietitian. It consisted of two audio tapes, 2 1/2 hours in length, recorded by the author and an accompanying handbook or resourcebook. The resourcebook materials were designed to give further details, provide resources and materials for future reference, visually reinforce the audio presentation, summarize information given in the presentation, and give a bibliography of the references cited. The presentation was divided into four sections: Guidelines for Education of Individuals with Diabetes Mellitus; Educational Program Planning in a Health Care Agency; The Adolescent with Insulin-Dependent Diabetes; The Educator-Client Interaction. Following initial formative evaluations, the Learning Package was field tested in a seminar for practicing clinical dietitians held in three locations with several subjects completing the test in the home setting. Pre and posttest scores and attitudes of the participants towards the Guidelines and Learning Package were collected and are reported. Suggestions for improvement of the Learning Package and demographic data were also collected and are reported. Only 13.8% of the subjects met the 90% criterion as determined on the posttest. There was improvement from both forms of the pretests to the posttest. Tests of statistical significance were not conducted. Participants indicated acceptance of the Learning Package as a continuing education tool and of the Guidelines as of practical value to them in their practices. Recommendations are made for improvement and revision of the Learning Package and final (summative) evaluation and testing before packaging and distribution by the American Dietetic Association. It is concluded that the Learning Package is acceptable to practitioners and, with the suggested revisions, can meet the continuing education needs of dietitians to improve nutritional education of persons with diabetes, as identified in the literature.

Avaliação dos benefícios do aconselhamento nutricional em pacientes portadores de doença renal crônica

Orientador: José Eduardo Corrente / Resumo: Introdução: Doença renal crônica (DRC) é uma enfermidade de relevância para a saúde pública, cursando com prevalência crescente. Estudos confirmam associação da hipertensão arterial, diabetes mellitus e proteinúria com desenvolvimento da DRC, progressão e mortalidade. Intervenções nutricionais podem prevenir ou atenuar essas complicações da doença, além de manter o estado nutricional e levar à maior sobrevida e qualidade de vida aos pacientes. Objetivo: Avaliar os benefícios do aconselhamento nutricional em pacientes portadores de DRC. Metodologia: Estudo quase-experimental, controlado e em dois momentos (inicial e após três meses), com indivíduos nos estadios 3 e 4 da doença, divididos em dois grupos: controle, composto por indivíduos que nunca receberam aconselhamento nutricional por recusa, e intervenção, caracterizado por pacientes em acompanhamento com nutricionista. Foi realizada a comparação entre os parâmetros: antropometria, exames bioquímicos e avaliação do consumo alimentar através da análise dos R24h através do programa Nutrition Data System for Research. Para análise estatística, ANOVA seguido do teste de Tukey ou teste de Wald de acordo com a distribuição dos dados, adotando nível de significância de 5%. Resultados: Foram incluídos 50 pacientes, sendo: controle (n=23) e intervenção (n=27). Os grupos foram homogêneos. Studies confirm an association of arterial hypertension, diabetes mellitus and proteinuria with the development of CKD, progression and mortality. Dietary interventions can prevent or mitigate these complications of the disease, in addition to maintaining nutritional status and leading to greater survival and quality of life for patients. Objective: To evaluate the benefits of nutritional counseling in patients with CKD. Methodology: Quasi-experimental study, controlled and two-stage (initial and after three months), with individuals in stages 3 and 4 of the disease, separated into two groups: control, composed of individuals who never received nutritional advice for refusal, and intervention, characterized by patients being followed up with a nutritionist. Introdução: Diante do acelerado aumento da obesidade infantil e da ciência de que suas consequências perduram ao longo da vida, na grande maioria dos casos, fica evidente, a necessidade de prevenção precoce do excesso de peso na infância para reduzir esse importante problema de saúde pública. A promoção da alimentação adequada e saudável e a promoção da saúde estão intensamente relacionadas, por isso, tratar crianças com excesso de peso implica considerá-las em seus contextos familiares e sociais com um olhar além da gordura corporal, mas de respeito às suas histórias, sentimentos, conflitos, valores, crenças e saberes. Objetivo - Descrever, interpretar e compreender as vivências familiares relativas ao aconselhamento nutricional para crianças com obesidade. Métodos - Pesquisa qualitativa, de caráter exploratório, com produção dos dados por meio de grupos focais. Os sujeitos participantes foram os familiares de crianças que realizam acompanhamento nutricional devido ao diagnóstico de obesidade na Policlínica Centro de São Bernardo do Campo. A amostragem foi constituída através do critério de saturação quando nos dados produzidos não emergiram novos temas que contribuíssem para a reflexão teórica. Foi utilizada análise de conteúdo para analisar os dados produzidos. Resultados - Foram realizados 12 grupos focais que compuseram uma amostra total de 13 participantes, entre eles, mães, avó e tia. As categorias que emergiram dos dados abarcaram as memórias alimentares da infância dos familiares; a imagem corporal das crianças percebidas pelos familiares e pelas próprias crianças, segundo as percepções dos familiares; a dimensão afetiva do comportamento alimentar; as manifestações infantis sobre o aconselhamento nutricional, segundo os familiares; às experiências familiares após o aconselhamento nutricional; as redes de apoio social reconhecidas, e os comportamentos alimentares familiares, tanto em ambiente doméstico, quanto no lazer. Conclusão - O alimento demonstrou ter funções que vão além do suprimento das necessidades biológicas por agregarem significados culturais, comportamentais e afetivos a partir dos quais os indivíduos se relacionam. As vivências e trajetória de vida de cada uma dessas famílias são únicas e fundamentam a forma como as situações são experienciadas individualmente por elas e impactam diretamente em suas atitudes e comportamentos. A comida e o comer se mostraram ligadas a história, as memórias e a dimensão afetiva que compõe cada uma dessas pessoas. / Introduction - Faced with the accelerated increase in childhood obesity and the knowledge that its consequences lasts throughout life in the vast majority of cases, it is evident, the need for early prevention of overweight in childhood to reduce this important public health problem. The promotion of the adequate and healthy food, and the promotion of health are intensely related, therefore, treating overweight children involves considering them in their family and social contexts with a look beyond body fat, but that respects their stories, feelings, conflicts, values, beliefs and knowledges. Objective - To describe, interpret and understand family experiences related to nutritional counseling for obese children. Methods - Qualitative exploratory research, with the production of data through focus groups. The subjects of this research were the relatives of children who undergo nutritional monitoring due to the diagnosis of obesity in the Downtown Polyclinic of São Bernardo do Campo. The sampling was constituted through the saturation criterion when, in the data produced, no new themes emerged that contributed to the theoretical reflection. Content analysis was used to analyze the produced data. Results - Twelve focus groups comprised a total sample of 13 participants, including mothers, grandmother and aunt. The categories that emerged from the data encompassed the family members´s childhood memories around food; the body image of the children perceived by the relatives and by the children themselves, according to the perceptions of the relatives; the affective dimension of eating behavior; infant manifestations on nutritional counseling, according to the relatives; family experiences after nutritional counseling; the recognized social support networks, and family eating behaviors, both in the domestic and leisure settings. Conclusion - The food has demonstrated characteristics that go beyond the supply of biological needs, since it adds cultural, behavioral and affective meanings from which individuals relate themselves. The experiences and life trajectory of each family are unique and base the way in which the situations are experienced individually by them and directly impact on their attitudes and behaviors. Food and eating were linked to the history, memories and affective dimension that compose each of these people.

A comida e o comer se mostraram ligadas a história, as memórias e a dimensão afetiva que compõe cada uma dessas pessoas. / Introduction - Faced with the accelerated increase in childhood obesity and the knowledge that its consequences lasts throughout life in the vast majority of cases, it is evident, the need for early prevention of overweight in childhood to reduce this important public health problem. The promotion of the adequate and healthy food, and the promotion of health are intensely related, therefore, treating overweight children involves considering them in their family and social contexts with a look beyond body fat, but that respects their stories, feelings, conflicts, values, beliefs and knowledges. Objective - To describe, interpret and understand family experiences related to nutritional counseling for obese children. Methods - Qualitative exploratory research, with the production of data through focus groups. The subjects of this research were the relatives of children who undergo nutritional monitoring due to the diagnosis of obesity in the Downtown Polyclinic of São Bernardo do Campo. The sampling was constituted through the saturation criterion when, in the data produced, no new themes emerged that contributed to the theoretical reflection. Content analysis was used to analyze the produced data. Results - Twelve focus groups comprised a total sample of 13 participants, including mothers, grandmother and aunt. The categories that emerged from the data encompassed the family members´s childhood memories around food; the body image of the children perceived by the relatives and by the children themselves, according to the perceptions of the relatives; the affective dimension of eating behavior; infant manifestations on nutritional counseling, according to the relatives; family experiences after nutritional counseling; the recognized social support networks, and family eating behaviors, both in the domestic and leisure settings. Conclusion - The food has demonstrated characteristics that go beyond the supply of biological needs, since it adds cultural, behavioral and affective meanings from which individuals relate themselves. The experiences and life trajectory of each family are unique and base the way in which the situations are experienced individually by them and directly impact on their attitudes and behaviors. Food and eating were linked to the history, memories and affective dimension that compose each of these people.

El presente plan se realizó con la finalidad de investigar la viabilidad de TELENUTRICIÓN SAC, empresa dirigida a brindar consultoría nutricional a distancia, utilizando tecnologías de la información y de las comunicaciones (TIC´s). Dirigida a personas entre 20 y 59 años e hijos de las mismas, que tengan acceso a internet y/o telefonía móvil y con capacidad de pago, interesadas en la salud nutricional y mejorar sus conocimientos sobre alimentación y nutrición y por ende, su calidad de vida. Realizamos un estudio de mercado, utilizando una encuesta a través de una plataforma digital. Nuestra propuesta de valor está dada por acercar a cada persona la oportunidad de aprender a implementar mejoras en su alimentación para lograr una vida más saludable, enfocados en el cliente que por falta de tiempo, acceso geográfico o falta de oferta de profesionales nutricionistas, no pueda acceder a una atención nutricional especializada presencial. Se utilizará una estrategia genérica de diferenciación. Se estimó una inversión inicial de s/. 114.036,00 soles, con 100% de capital propio y flujo de caja para los primeros 10 años. Siendo el Valor Actual Neto de s/. 1.309,600.00 soles y la Tasa Interna de Retorno 149%. Como resultado tenemos una inversión viable, rentable y sostenible, con retorno de inversión, debido a la ausencia de gastos fijos significativos en la estructura de costos por el tipo de negocio, calificando como apto para ejecutar. / This plan was developed to assess the viability of TELENUTRICIÓN SAC, a company that will provide long distance nutritionist services, using communication and information technology (TIC’s in Spanish). Target are people between 20 and 59 years old and their children, with internet and/or mobile access and with purchase power, interested in improving nutritional condition and knowledge about food and nutrition, and consequently, life quality. We made market research, using a digital survey. Our value proposition focuses on giving people the chance of learning on how to improve their diet to achieve a healthier life, aiming clients who, due to lack of time, geographical access, or lack of nutritionists, are not able to get specialized nutritional attention. A differentiation business strategy will be employed. Initial investment is estimated at s/. 114.036,00 Peruvian soles, 100% equity, and cash flow will be generated for the next 10 years. Net Present Value resulted in s/. 1.309,600.00, with an Internal Rate of Return of 149% annual. L'impact d'une intervention nutritionelle chez les receveurs de cellules souches hématopoïétiques : résultats d'un essai contrôlé randomisé / The impact of counseling on nutritional status and quality of life of Hematopoietic Stem Cell recipients : results of a randomized controlled trial

Contexte :Le conditionnement précédant la greffe de cellules souches hématopoïétiques (CSH) a été associé avec des taux élevés de malnutrition meme à 100 jours après la greffe. Objectif: Cette étude évalua l'impact du conseil nutritionel fournie à la sortie de l'hôpital sur l'état nutritionnel,100 jours après la greffe de CSH (dit T4).Conception: Il s'agissait d'un essai contrôlé randomisé monocentrique. Les patients adultes étaient randomisés à un groupe témoin (GT) recevant des soins habituels et un groupe d'intervention (GI) recevant des conseils nutritionnels mensuels après la sortie de l’hôpital. Le résultat principal était le score de l'évaluation globale subjective générée par le patient (PGSGA) à T4. La malnutrition était évalué aussi par le score de la société américaine de nutrition parentérale et entérale/Académie de nutrition et diététique (AND-ASPEN).Résultats: 52 participants ont été randomisés (août 2016 jusqu'en août 2017) et 46 ont été analysés [65% d'hommes, 63% de greffes autologues, GI (n = 22), GT (n = 24)]. Les deux groupes etaient comparable au moment de randomization.A T4, le pourcentage de patients bien nourris n'était pas significativement différent entre les groupes selon le PGSGA (72% GI vs 43% GT, p = 0,063). Le pourcentage de patients bien nourris selon AND-ASPEN s'est améliorré à T4 dans le GI (50% vs 14%, p = 0,02) et non pas dans le GT par rapport aux valeurs d'admission. A T4, le GI avait un apport de protéines et de calories plus élevé que le GT(p<0.05).Conclusion:Le conseil nutritionnel après la greffe de CSH a amélioré l’apport en protéines et calories ainsi que le score AND-ASPEN mais non pas le score PGSGA. / Background: Conditioning preceding Hematopoietic Stem Cell Transplantation (HSCT) has been associated with elevated rates of malnutrition until 100 days post HSCT.Objective: This study aimed to assess the impact of nutritional counseling provided at hospital discharge on nutritional status 100 days post HSCT (defined as T4). Design: This was a single center randomized controlled trial among adult HSCT patients. Around discharge from the hospital, recruited patients were randomized to a Control Group (CG) receiving usual care and to an Intervention Group (IG) receiving nutritional counseling on a monthly basis post discharge.The primary outcome was the Patient Generated Subjective Global Assessment (PGSGA) scores at T4. Malnutrition was also assessed though the American Society for Parenteral and Enteral Nutrition/ Academy of Nutrition and Dietetics malnutrition score.Results: 52 participants were randomized (August 2016 until August 2017) and 46 were analyzed [65% males, 63% autologous HSCT, IG (n=22), CG (n=24)]. Groups were comparable at randomization. At T4, the percent of well-nourished patients was not significantly different between groups when assessed via PGSGA (72% IG vs. 43% CG, p=0.063).The percent of wellnourished patients as per AND-ASPEN criteria improved in IG at T4 (14% vs. 50%, p=0.02) and remained the same in CG (48% vs. 50%, p=1) compared to admission values. IG had higher protein and caloric intake (p<0.05). Conclusion:Nutritional counseling post HSCT improved patients’ protein and caloric intake and AND-ASPEN score but did not significantly improve PGSGA score.
