THE COGNITIVE AND FUNCTIONAL IMPACT OF OPEN HEART SURGERY: A PILOT STUDY INCLUDING THREE COMMON PROCEDURES (CORONARY ARTERY BYPASS GRAFT, HEART VALVE REPLACEMENT, AND LEFT VENTRICULAR ASSIST DEVICE)Fix, Robert 01 January 2018 (has links)
This study investigated the impact of open heart surgery (Coronary Artery Bypass Graft, Heart Valve Replacement, or Left Ventricular Assist Device placement) on cognition, functional performance, and mood in the three months following surgery. The Montreal Cognitive Assessment (MoCA), Kettle Test (KT), Physical Self Maintenance Scale (PSMS), and Hospital Anxiety and Depression Scale (HD) measured global cognition, functional cognition, functional performance, and mood states, respectively.
Thirteen male participants (ages 38 – 75) completed assessments at four time points -- when they were scheduled for surgery, within one week prior to surgery, before hospital discharge after surgery, and three months after surgery. ANOVA analyses were conducted on overall raw mean scores taken at these time points. Correlational analysis compared changes in cognition and functional performance of daily activities for this group. Effect size estimations and power analyses were conducted to estimate sample sizes needed for adequately powered subsequent study. Two measures (KT and PSMS) were adequately powered at 95% for the study sample. Functional cognition as measured by the KT improved significantly after surgery and surpassed baseline within three months after surgery. Functional performance as measured by the PSMS declined significantly after surgery but returned to baseline within three months after surgery. Global cognition as measured by the MoCA did not change, was not correlated with other measures, and was below norms at all time points. Mood states as measured by the HADS did not change and were above norms at all time points.
This study had a small sample, only male participants, and one pooled group that did not allow for group comparisons. Two measures were self-reported, which may have impacted results due to responses biases. Despite these limitations, this is one of the first studies to track and compare both cognitive and functional performance changes over time. As such, this study may help practitioners and researchers improve and prioritize assessment and treatment options for individuals with cognitive and functional performance deficits after open heart surgery.

Factors Associated with Depression in Adult Cardiac Surgery PatientsEvangelista, Kimberly 01 January 2007 (has links)
The American Heart Association (2006) has identified cardiovascular disease as the leading cause of death in the United States for every year except 1918 since 1900. Cardiac surgery is an increasingly common method of treating this problem (Contrada, Goyal, Cather, Rafalson, Idler et al. , 2004). Researchers have found that depression is associated with serious complications related to cardiac surgery (Connemey, Shapiro, McLaughlin, Bagiella, & Sloan, 2001 ). Many researchers have identified factors associated with depression in these patients.
The purpose of this thesis was to identify and synthesize these current research findings. Studies included were published from 1997-2006 and focused on male and female patients 18 years of age and older, primarily undergoing Coronary Artery Bypass Grafting (CABG).
The synthesis of these findings includes both the factors associated with developing depression and the complications associated with depression. Common factors associated with its development included female gender, lower socioeconomic status, pessimism, and poorer cognitive and physical functioning. Complications associated with depression included decreased physical functioning, healing, and cognitive functioning, as well as increased hospital stays, readmissions, cardiac events, and mortality. The associated factors may help to identify patients at high risk for developing depression, while the complications emphasize its importance. Limitations were acknowledged and recommendations for nursing research, education, and practice are included.

Perioperative Antibiotikaprophylaxe bei angeborenen HerzfehlernBoschnakow, Anett 19 April 2002 (has links)
In den letzten Jahren haben sich die Empfehlungen für eine Antibiotikaprophylaxe in der Herzchirurgie bezüglich Antibiotikawahl und Applikationsdauer verändert und gehen nicht speziell auf die Kinderherzchirurgie ein. Im Deutschen Herzzentrum Berlin wurde seit 1988 bei Operationen aufgrund angeborener Herzfehler die Kombinationsprophylaxe mit Piperacillin und Cefotaxim angewandt. Neue internationale Empfehlungen schlagen jedoch ein Cephalosporin der I. oder II. Generation vor. Fragestellung dieser prospektiven Beobachtungsstudie ist deshalb, ob eine Umstellung der im DHZB üblichen Kombinationsprophylaxe auf eine Monoprophylaxe mit Cefotiam gerechtfertigt ist. Vom 1.3. - 31.8. 1995 wurden 180 Patienten mit Operationen am offenen Herzen in die Studie eingeschlossen. 99 Patienten erhielten die bisher im DHZB etablierte Kombinationsprophylaxe mit Cefotaxim und Piperacillin (Cx/P-Gruppe). 81 Patienten bekamen eine Monoprophylaxe mit Cefotiam (Ct-Gruppe). Die postoperative Infektionsrate betrug in der Ct-Gruppe 14,8 % und in der Cx/P-Gruppe 17,2 %. Die Prophylaxe erfolgte für beide Gruppen durchschnittlich 103 Stunden bzw. 110 Stunden. Die Studie konnte aufgrund geringer Patientenanzahl aus statistischer Sicht keinen Beweis für einen Unterschied erbringen. Dafür zeigte die Metaanalyse, daß auch bei einer großen Patientenanzahl (n=4177) kein statistischer Unterschied zwischen einer Schmalspektrum- und einer Breitspektrumprophylaxe besteht. Es wurde ferner gezeigt, daß Cefotiam den Anforderungen einer effektiven Prophylaxe gerecht wird und Vorteile gegenüber der Kombinationsprophylaxe aufweist. Die Studie hat dazu beigetragen, daß seit 1996 in der Kinderherzchirurgie im DHZB eine Monoprophylaxe mit einem Cephalosporin der I. Generation - Cefazolin - durchgeführt wird und hat weiterhin eine Verkürzung der Prophylaxedauer bewirkt. / Perioperative antibiotic prophylaxis for patients with congenital heart diseases Recommendations for antimicrobial prophylaxis regarding the choice of antibiotics and the application time have changed and do not include cardiac surgery for infants. A combination of piperacillin and cefotaxim has been used since 1988 in the German Heart Institute of Berlin. Updated recommendation now propose the use of a I. or II. generation cephalosporin. The purpose of this prospective study was to evaluate whether a change from the so long used combined prophylaxis towards a monoprophylaxis with cefotiam is justified. From 1st of March until 31st of August 1995 a total number of 180 patients with an open heart surgery were included into the study. 99 patients received a combination with cefotaxim and piperacillin (cx/p-group), which was the standard combination in the German Heart Institute of Berlin. 81 patients received a monoprophylaxis with cefotiam (ct-group). The postoperative rate of infections was 14,8 % in the ct-group and 17,2 % in the cx/p-group. For both groups the duration of prophylaxis was on average 103 respectively 110 hours. Due to a relatively small number of patients the study could not show a statistically significant difference between both prophylactic regimes. The performed meta-analysis comparing the postoperative infections in small spectrum and broad spectrum prophylaxis in the open heart surgery did not find a significant difference in a large group of patients (n=4177). Our study supports the fact, that cefotiam meets the criteria of an effective antibiotic prophylaxis and has additional benefits compared to the combined prophylaxis scheme. The results of this study have led to a replacement of the old perioperative prophylaxis regime by cefotiam for open heart surgery for infants in the German Heart Institute of Berlin since 1996 and have led to a reduction of the application time during the prophylaxis.

Impacts of Observing Live Open Heart Surgery on Young Adults' Health BehaviorsQarizadah, Muhammad Musa 01 January 2017 (has links)
Having a heart healthy lifestyle at a younger age is beneficial in reducing the risk of coronary heart disease (CHD) in adulthood and various health education programs for CHD prevention exist to persuade young adults to adopt a heart healthy lifestyle. Little is known, however, about the influence of watching live open heart surgery on young adults' adoption of a heart healthy lifestyle. To address that gap in knowledge, this study was conducted at a health facility in Virginia where students came to observe live open heart surgery. The purpose of the study was to understand whether watching live open heart surgery can influence young adults' attitudes and behaviors towards adopting a heart healthy lifestyle. The theory of health belief model and the theory of reasoned action were used in guiding this research. A quantitative design involving observation and administration of surveys was used. A survey with questions about participants' lifestyle practices was administered prior to observation of open heart surgery and 3 or more months afterwards. A total number of 179 young adults at baseline were considered eligible for the study. Ages of the research participants were between 18 and 35 years old. The t test and ANOVA results showed no significant differences between baseline and follow up data with regards to changes in behaviors of young adult students towards adopting a heart healthy lifestyle. The study concluded that observing live heart surgery did not significantly change behaviors of the students in adopting a heart healthy lifestyle. In order to achieve the desired changes in attitudes and behavior of students, looking into other evidence based options and pursuing those that can influence and motivate young adult students to adopt a heart healthy lifestyle can be a positive social change.
