Uso da membrana de celulose bacteriana associada a células mesenquimais e ácido hialurônico após lesão em cartilagem de patela de suínos / Use of bacterial cellulose membrane associated with mesenchymal cells and hyaluronic acid after injury in pig patella cartilage

O objetivo deste trabalho foi avaliar a membrana de celulose bacteriana associada a células mesenquimais, que foram inoculadas juntamente com derivado do ácido hialurônico, hylano G-F 20, em patelas lesadas de suínos. Foram utilizados 12 suínos (24 joelhos) submetidos à raspagem das superfícies articulares das patelas até osso esponjoso; joelhos esquerdos sem tratamento, grupo controle (GC). Joelhos direitos, grupo tratado (GT), tiveram membranas de celulose bacteriana fixadas às superfícies de suas patelas, sendo que as mesmas foram preparadas com inoculação de células mesenquimais, ósseas e cartilaginosas, provenientes das raspagens das superfícies, lavadas em solução fisiológica 0,9%, filtradas e diluídas em 5 ml de solução fisiológica à 0,9% e posteriormente acrescentados 2 ml de hylano G-F 20. Colhidos 5 ml dessa papa que teve contido o volume de 16,35 x \'10 POT.8\' células na sua totalidade, e foram infiltrados na membrana de celulose revestindo a patela, obedecendo a distribuição de 3,27 x \'10 POT.8\' por 1,2 \'CM POT.3\'. Após 2, 4 e 6 meses os animais foram sacrificados para avaliações macroscópicas e histológicas. As superfícies patelares apresentaram aspecto de neoformação de cartilagem aos dois meses; essas membranas evoluíram para tecido fibroso de altíssima resistência, com densidade de fibras conjuntivas aumentando proporcionalmente ao tempo de cirurgia, quatro ou seis meses conforme as células do infiltrado tiveram alterações morfológicas para fibroblastos Ao mesmo tempo, parte do volume dessas células sofreu processo de degeneração e morte celular, semelhante ao mecanismo de apoptose celular. Dessa forma, ofereceu substratos para as células remanescentes, colaborando para a neoformação desse tecido conjuntivo. Não ocorreram processos inflamatórios ou agressão às cartilagens em contigüidade à membrana. Pelo contrário, em relação ao grupo controle não tratado, a membrana de celulose bacteriana utilizada segundo a metodologia aplicada, atuou com uma barreira protetora para agressões maiores à cartilagem da fossa intercondilar do fêmur que manteve a integridade de seus contornos articulares. Desta forma, segundo a metodologia utilizada, concluímos que a membrana de celulose bacteriana apresentou biocompatibilidade para uso articular, hábil para agregar condrócitos e fibroblastos. / The goal of this work was to evaluate the bacterial cellulose membrane associated with mesenchymal cells, which were inoculated with hyaluronic acid derivative, G-F 20 hylano in patelas injured pigs. 12 pigs were used (24 knees) submitted to the scraping of the articular surfaces of spongy bone patelas up; knees left without treatment, group controller (GC). Knees rights, treated group (GT), bacterial cellulose membranes had fixed on their surfaces patelas, being that they were prepared with inoculation of mesenchymal cells, bone and cartilage from sweeps of surfaces, washed in 0.9% saline solution, filtered and diluted in 5 ml of saline solution to 0.9% and subsequently added 2 ml hylano G-F 20. Collected 5 ml that Pope had contained the volume 16.35 x \'10 POT.8\' cells in its entirety, and were infiltrated in the membrane of cellulose coating the patella, obeying distribution 3.27 x \'10 POT.8\' per 1.2 \'CM POT.3\'. After 2, 4 and 6 months the animals were sacrificed to macroscopic and histological evaluations. The surfaces patellar components submitted aspect of neoplasm of cartilage to two months; These membranes evolved into fibrous tissue resistance of high density fibrous tears increasing in proportion to the time of surgery, four or six months as had infiltrated cells for morphological changes while fibroblasts, part of the volume of these cells suffered process of degeneration and cell death, similar to the mechanism of cellular apoptosis. This way, offered substrates for the remaining cells, collaborating for this connective tissue neoplasm. There were no inflammatory processes or aggression on cartilage in adjacency to the membrane. On the contrary, compared to the untreated control group, the bacterial cellulose membrane used according to the methodology applied, served with a protective barrier for larger aggressions of cartilage of the femur intercondilar trench that maintained the integrity of its articular contours. This way, according to the methodology, we found that the bacterial cellulose membrane biocompatibility presented for use articulate, skilled to aggregate condrócitos and fibroblasts.

Klinische und radiologische Ergebnisse nach operativ versorgter Patellafraktur / Clinical and radiological results after surgical treatment of patella fractures

Die Durchblutung der Patella beruht auf 2 Zuflüssen. Einerseits Gefäße, welche im mittleren Drittel von ventral über Foramina in die Patella eintreten und außerdem Gefäße, welche am distalen Pol zwischen dem Lig. Patellae und der Gelenkfläche in die Patella eintreten und nach proximal verlaufen. Patellafrakturen können in Längs-, Quer- und Trümmerfrakturen unterteilt werden. Bei Frakturen der Patella kann im Bereich des proximalen Pols eine arterielle Minderversorgung resultieren, wohingegen der distale Pol doppelt versorgt ist und nicht affektiert erscheint. Die arterielle Hypoperfusion kann zu lokalisierten Osteonekrosen führen. Diese lokalen osteonekrotischen Areale können sich radiographisch als hyperdense Areale manifestieren. Im Rahmen dieser Studie sollte das klinische und radiologische Ergebnis nach operativ versorgter Patellafraktur untersucht werden. Dabei wurden 100 Patienten untersucht, die im Zeitraum von 1998 – 2008 operativ versorgt wurden. Davon konnten 60 Patienten im Mittel nach 60.61 ( ± 33.88) Monaten mit einem durchschnittlichen Alter 45.48 ( ± 18.51) Jahren im Rahmen eines Follow-Ups untersucht werden. Dabei wurden mehrere klinische Scores, die Patientenzufriedenheit und die visuelle Analogskala Schmerz erhoben und die vorher angefertigten radiologischen Aufnahmen ausgewertet. Diese wurden auf eventuelle Stufenbildungen, Defekte oder hyperdense Areale hin untersucht. Es zeigte sich bei 9 Patienten (9%) ein hyperdenses Areal im Bereich des proximalen Pols welches durchschnittlich 1 – 2 Monate nach der operativen Versorgung nachgewiesen wurde. Bei 7 Patienten war dieses Areal nach 6 Monaten nicht mehr nachweisbar. Bei 2 Patienten mit bestehenden hyperdensen Arealen konnten diese im Verlauf mittels MRT respektive einer histologischen Untersuchung gesichert werden. Es zeigte sich insgesamt kein signifikanter Unterschied zwischen Patienten ohne radiologische Auffälligkeiten und mit hyperdensen Arealen. Jedoch wurden für die Gruppe mit hyperdensen Arealen im Vergleich tendentiell schlechtere Werte notiert. Radiologisch hyperdense Areale im Bereich des proximalen Pols bei Zustand nach operativer Versorgung einer Patellafraktur können eine Osteonekrose repräsentieren. Eine frühere operative Versorgung bzw. schonendere Operationstechniken könnten die Entstehung von osteonekrotischen Arealen verhindern. Zur Bestätigung dieser Hypothese sind weitere Untersuchungen notwendig.

The impact of gender and chronic resistance training on human patellar tendon dry mass, collagen content, and collagen cross-linking

Collagen content and cross-linking are believed to be major determinants of tendon structural integrity and function. Gender and chronic resistance training have been shown to alter tendon function, and may also alter these key structural features of tendon. Patellar tendon biopsies were taken from untrained men (M, n=8, 25±1 y, 1RM: 53±3 kg), untrained women (W, n=8, 23±2 y, 1RM: 29±2 kg), and resistance-trained (10±1 y trained) men (RTM, n=8, 24±2 y, 1RM: 71 ±6 kg). Biopsies were analyzed for dry mass, collagen content, and collagen cross-linking (hydroxylysylpyridinoline, HP). Tendon dry mass was significantly lower in women than men (M: 376±8, W: 343±5 µg dry mass/mg tendon wet wt, P<0.01), and was not influenced by chronic resistance training (RTM: 364±20 µg dry mass/mg tendon wet wt, P>0.05). The lower tendon dry mass in women reduced (P=0.08) collagen content per tendon wet weight (M: 339±14, W: 306±11 µg collagen/mg tendon wet wt). Collagen content of tendon dry mass was not influenced by gender (P>0.05) or resistance training (P>0.05) (M: 903±38, W: 892±29, RTM: 881±43 !,g collagen/mg tendon dry mass). Similarly, cross-linking of collagen was not impacted by gender (P>0.05) or training (P>0.05) (M: 401±47, W: 418±35, RTM: 424±38 mmol HP/mol collagen). In women, the overall lower amount of collagen per tendon wet weight may explain gender differences in tendon function, while collagen content and cross-linking of the dry mass are remarkably consistent across gender and training status. / School of Physical Education, Sport, and Exercise Science

Material e Métodos: Foram avaliadas 30 mulheres com menopausa instalada quanto a Eletromiografia de superfície dos músculos Reto Femural (RF), Vasto Medial Oblíquo (VMO) e Vasto Lateral (VL), dosagem sérica de 25OHD3 (Vitamina D3), densitometria óssea (DMO), medida do ângulo Q e questionamento sobre ocorrência de quedas nos 6 meses que antecederam a coleta dos dados. Para análise quantitativa da amplitude do sinal eletromiográfico foram utilizados o RMS (Root Mean Square - Raiz Quadrada da Média), expresso em ?V. Foi utilizados como instrumentos estatísticos o Teste de Diferença de Médias de Wilcoxon, Análise Exploratória dos dados para análises descritivas, e Análise de Correlação de Spearman juntamente com o Teste de Hipóteses para verificação das correlações entre as variáveis estudadas. O nível de significância utilizado foi de 95% de confiança (p?0,05). Resultados: As voluntárias apresentaram idade de 61,79 (49-73, ? 6,81) e correlação significantemente negativas (p?0,05) foram observadas entre os resultados dos músculos estudados e os valores de DMO e de 25-OH - Vitamina D3. Houve correlação negativa entre a atividade elétrica muscular e DMO (RF60, VMO100 e VMO60 com DMO em T score fêmur, e VMO60 com DMO de fêmur inteiro em g/cm2); e níveis de 25OHD3 com EMG de VLN. Houve correlações negativas entre ângulo Q e VMO100 e VMO 60, potencialmente influenciado pela incidência de quedas. Conclusões: Menores níveis de 25OHD3 e redução de massa óssea podem estar relacionados a um aumento na atividade elétrica muscular do quadríceps, podendo indicar uma diminuição de seu desempenho ideal em mulheres mais idosas no período pós menopausa, e o desequilíbrio da atividade entre VL e VMO sugere risco na ocorrência de quedas e disfunção patelo femural nestas pacientes / Abstract: Objective: This study evaluated the surface electromyographic activity (EMG) of the vastus lateralis (VL), vastus medialis oblique (VMO) and rectus femoris (straight femoral, RF) muscles in post-menopausal women and the relationship between EMG activity and 25OHD3 levels, bone mass, Q angle measurement and questioning about the occurrence of falls in the 6 months prior to data collection. Materials and Methods: Thirty menopausal women underwent surface electromyography of the RF, VMO and VL muscles and bone densitometry. Serum 25-OH-Vitamin D3 levels were also measured. The Root Mean Square (RMS) was used to quantitatively analyze the electromyographic signal amplitude in ?V. Several statistical analyses were performed to determine correlations between variables, including the Wilcoxon signed-rank test, exploratory data analysis for descriptive analyses and Spearman's rank-order correlation analysis with hypothesis testing. The level of significance was set at a 95% confidence level (p ? 0.05). Results: The volunteers showed age was 61.79 (49-73, ? 6.81) and negative and significant correlation (p ? 0.05) observed between the results from the muscles studied with BMD and 25OHD3. There was a negative correlation between muscle electrical activity and BMD (RF60, VMO and VMO60 with BMD T-score femur, and femoral BMD with VMO60 whole in g/cm2) and levels 25OHD3 with EMG VLN. There were negative correlations between Q angle and VMO100 VMO and 60, potentially influenced by the incidence of falls. Este trabalho objetivou avaliar através do exame clínico, radiográfico e ultra-sonográfico a articulação fêmuro-tíbio-patelar pós desmotomia patelar medial. Foram utilizados oito equinos, machos e fêmeas, de peso, raça e idade variáveis. Todos animais foram submetidos a desmotomia patelar medial no membro pélvico direito. Os exames clínico e ultra-sonográfico foram realizados no 7&ordm;, 14&ordm;, 21&ordm;, 28&ordm;, 43&ordm;, 58&ordm;, 88&ordm; e 118&ordm; dia do pós-operatório. Os exames radiográficos e do líquido sinovial foram realizados no 15&ordm;, 30&ordm;, 60&ordm;, 90&ordm; e 120&ordm; dia do pós-operatório. No pós-operatório foi observado durante exame radiográfico do joelho direito, aumento do ângulo entre a superfície articular proximal da patela e a superfície cranial distal do fêmur, formação óssea na crista tibial, entesofitos na patela, deslocamento da patela e irregularidade óssea na superfície patelar lateral. No exame ultra-sonográfico foi observado aumento da espessura (p&lt;0,05) no segmento proximal e médio do ligamento patelar intermédio do membro pélvico direito, desmite do patelar intermédio, enteseofitos na patela, distensão do recesso articular medial do fêmur, efusão articular e irregularidade da cartilagem do sulco troclear femoral. Conclui-se que a desmotomia patelar medial causa alterações no joelho devido a instbilidade articular e deve ser indicada apenas nos casos que não responderam a outras forma de tratamento. / This study aimed the evaluation of the femurotibiopatellar joint trough clinical, radiographic and ultrasonographic examinations after experimental medial patellar desmotomy. Medial patellar desmotomy was performed on the right hindlimb in eight mixed-breed horses. The clinical and ultrasonographic evaluation was performed on day 7, 14, 21, 28, 43, 58, 88 e 118 after surgery. The radiographic and synovial fluid evaluation was performed on day 15, 30, 60, 90 e 120 after surgery. It was observed during radiographic evaluation of the right stifle, an increase of the angle between the patellar proximal articular surface and the cranial distal femur surface. During ultrasonographic evaluation it was observed thickening (p&lt;0,05) on the proximal and midlle segment of the midlle patellar ligament on the right hindlimb, entesophyte at the patella, femoral medial joint recess distending, joint effusion and cartilage irregularity of the femoral troclear groove. The medial patellar desmotomy causes stifle changes and must be performed only in animals non responsive to other treatment.
