Background: Treatment of cardiovascular risk factors slows the progression of small abdominal aortic aneurysms (AAA). Heme oxygenase-1 (HO-1) is a stress- and hemin-induced enzyme providing cytoprotection against oxidative stress when overexpressed. However, nothing is known about the effects of cardiometabolic standard therapies on HO-1 expression in aortic walls in patients with end-stage AAA. Methods: The effects of statins, angiotensin-converting enzyme (ACE) inhibitors, angiotensin II receptor blockers (ARBs), calcium channel blockers (CCBs), betablockers, diuretics, acetylsalicylic acid (ASA), and therapeutic anticoagulation on HO-1 mRNA and protein expressions were analyzed in AAA patients using multivariate logistic regression analysis and comparison of monotherapy. Results: Analysis of monotherapy revealed that HO-1 mRNA and protein expressions were higher in patients on diuretics and lower in patients on statin therapy. Tests on combinations of antihypertensive medications demonstrated that ACE inhibitors and diuretics, ARBs and diuretics, and beta-blockers and diuretics were associated with increase in HO-1 mRNA expression. ASA and therapeutic anticoagulation were not linked to HO-1 expression. Conclusion: Diuretics showed the strongest association with HO-1 expression, persisting even in combination with other antihypertensive medications. Hence, changes in aortic HO-1 expression in response to different medical therapies and their effects on vessel wall degeneration should be analyzed in future studies.

As a point of departure in this inherently interdisciplinary
endeavour, the concept "Holistic Multidisciplinary Management"
("HMM") is introduced a.s a macrocosmic adaption of principles of
project management. In line with HMM, a number of submissions
regarding terminology and definitions in the interdisciplinary
context of medicine (and particularly clinical psychiatry) and
law, are made, and the foundations of medical malpractice are
examined.
Building on the various foundations laid, specific types of
conduct that can constitute clinical-psychiatric malpractice, are
addressed. A common theme that emerges in the various contexts
covered, is that the psychiatrist must negotiate various
proverbial tightropes, involving inter alia tensions between
restraint and freedom, excessive and insufficient medication,
becoming too involved and not being involved enough with clients,
as well as client confidentiality and the duty to warn third
parties.
It is concluded that law and medicine. must work harmoniously
together to establish appropriate balance. This can be achieved
only if mutual understanding and integrated functioning are
promoted and translated into practice. / Law / LL.M.

Depressive disorder, etiology, objective criteria for identifying depression, depression and suicide, depression in ICD-10, treatment, chemicals in the brain and transmission of information, action mechanism of antidepressants, cutting depression, comorbidity, biological and psychotherapeutic treatment options, pharmacotherapy, ATC classification drugs, antidepressants,classification according to the distribution of the ATC, guidelines for the treatment of depression. Identifying the trend and volume of consumption of antidepressants - the code N06A in the Czech Republic in the period 2003-2008 using data from the Všeobecná zdravotní pojišťovna ČR and a comparison based on specific criteria: regional (consumption by region), gender (consumption by gender), age (consumption by age groups), season and month (consumption in different seasons and months) and ATC groups (consumption by chemical / therapeutic / pharmacological subgroups N06A * - 4th level ATC class and consumption by subgroups of chemical substances N06A ***, ie . active substances - 5th level ATC classification). A rough estimate of possible share of depressive and neurotic disorders found in consumption, based on additional data from the database of the Všeobecná zdravotní pojišťovna ČR.
