Medical service institutions and medical service types of Taiwan have experienced fundamental changes since the implementation of national health insurance. Therefore, the organizational vision and culture of a hospital and the professional commitment of doctors are critical in the hospital¡¦s ability to survive in the existing health environment. Among these factors, organizational human resources play the most important role, and doctors constitute the foremost human resources in a hospital. In recent years, armed forces hospitals have implemented programs to trim organization, leading to the further reduction of medical physicians. Moreover, the health environment external to armed forces hospitals has changed and wooed more medical officers from these hospitals. Therefore, it is the major purpose of this research to understand how to increase organizational loyalty of military physicians. This research examined factors that influence organizational commitment from dimensions including vision of organization, culture of organization, and career development and personal characteristics of doctors, and investigated the relationships of these dimensions with organizational commitment. Members on the military physicians at Armed Forces Tsoying General Hospital were targeted the population, and the research has resulted in findings as follows: (1) Among personal characteristics (of military physicians), only the variables of age and seniority made significant differences to the commitment to retention in organizational commitment; only unit of work made significant differences to effort commitment; and others made no significant differences to retention and effort commitment. (2) There existed a significant positive correlation between organizational vision and commitment to organization. (3) There existed a significant positive correlation between organizational culture and organizational commitment. (4) There existed a significant positive correlation between career development of employees and commitment to organization. This research has come up with two suggestions that follow: I. To high-level manager of armed forces hospitals: 1. Strengthen commitment to retention of junior military physicians. 2. Enhance military physicians¡¦ participation in planning for organizational vision. 3. Assist military physicians in job and personal development planning. II. To follow-up researchers: 1. Select different research variables. 2. Increase variable factors. 3. Draw samples from different populations. Heterogeneous Responses in Prescriptions to Medicare Part D: A Case Study on Physician Decision-Making and Antibiotics

To study the decision-making model behind how physicians making prescribing decisions, we studied the effects of the introduction of Medicare Part D in 2006 on numbers and characteristics of medications prescribed by physicians. We identified a significant increase in overall number of medications prescribed due to Medicare Part D but did not find any effects on the number of antibiotics. The result suggests there exist factors distinguishing antibiotics from other medications that led to a change in incentives to prescribe antibiotics, such as costs of antibiotics resistances. . We also identified the heterogeneity responses to Medicare Part D with respect to physician’s employment status, primary care relationship and patient’s gender and diagnostic categories.

Facing death : physicians' difficulties and coping strategies in cancer care

Even if the treatment of cancer has developed over the last decades 50% of the patients still die of their cancer. The doctor's way of dealing with his and his patient's anxiety must surely be of significance for the treatment the patient receives. In the first part of the thesis earlier studies of physicians' stress and ways of coping are reported. There is a lack of systematic studies which show how doctors working with cancer patients adjust to this work. The aim of this investigation is to study cancer doctors' difficulties and coping strategies. The theoretical frame of the study embraces parts of psychoanalytical theory and coping models, emphasizing that both unconscious and conscious psychological processes play their part in the coping process. The second, empirical part of the study includes 23 physicians strategically selected out of a population of physicians who work with institutional care and who have daily contact with adult cancer patients. The main method of data collection has been a series of recorded interviews. The focus of the interview was the physician's perception of how he reacts, thinks, talks and acts in different phases of the cancer disease. To illustrate the defence strategies of the interviewers, the projective percept-genetic test, the "Defence Mechanism Test" (DMT) is used. The "Structural Analysis of Social Behaviour" (SASB) has been used to study the doctors' self image. The results indicate that the stated difficulties deeply affect the doctor as a human being. The statements reflect conflicting feelings and wishes in relation to authority, conflicting feelings and wishes in relation to frightening and injuring, conflicting feelings and wishes in relation to intimacy/distance. Thirty themes of coping strategies frequently recur and they have been grouped into seven categories. Most of the doctors "seek knowledge" and support from scientific literature. The majority of them state that attempting to "solve a problem" is their main strategy. Most of the doctors "seek support " as a part of their coping strategy. An interesting observation is that the doctors to a higher extent "seek a relation" to their patients rather than to their colleagues. Almost one third use "denial of the severity of a situation" as their main strategy. All the doctors consciously or unconsciously use "diverting strategies", i.e. undertake tasks which are devoid of contact with patients, such as research and administration or other activities which allow them to avoid the patient. One third use "projective manoeuvres" but this is never a main strategy. The study investigated whether physician communication adherence was similar between adult and older adult female patients in a family medicine setting. Additionally, the study investigated whether or not the level of communication adherence was related to patient perceptions of working alliance. Previous research has failed to adequately examine age as a variable in physician-patient communication and has neglected to examine the working alliance within the physician-patient relationship. The sample included 41 adult female, family medicine patients, who agreed to have their appointment with their physician videotaped. The videotaped encounters were coded by trained observers using the Behavioral Science Tape Review Checklist (BSTRC). Participants also completed the Working Alliance Inventory-Short Form (WAI-SF). Results from the study suggested that physician adherence to communication tasks did not vary significantly between adult patients and older adult patients. Further, results demonstrated that the combination of responses to the bond and tasks subscales of the WAI-SF significantly accounted for 16% of the variance in communication adherence. / Department of Counseling Psychology and Guidance Services

Darbo tikslas – įvertinti Marijampolės apskrities šeimos gydytojų nuomonę apie imunoprofilaktiką bei jos organizavimą pirminėje sveikatos priežiūroje. Metodika. Tyrimo metu apklausėme visus Marijampolės apskrities šeimos gydytojus, dirbančius viešosiose ir privačiose pirminės sveikatos priežiūros įstaigose. Respondentams buvo išdalintos 109 anketos, iš kurių teisingai užpildytos ir grąžintos buvo 86 (atsako dažnis 79,0 proc.). Duomenų analizę atlikome naudojant statistinį programinį paketą SPSS (17.0 versija). Hipotezės apie požymių priklausomybę buvo tikrinamos naudojant chi kvadrato (χ2) ir z kriterijus bei Spearmano koreliacijos koeficientą (r). Rezultatai. Šeimos gydytojai, teikiantys paslaugas vaikams, gana aktyviai vykdo jų imunoprofilaktiką pagal Lietuvos Respublikos vaikų profilaktinių skiepijimų kalendorių. Didžioji dalis respondentų (87,2 proc.) tėvus (globėjus) apie vaiko skiepijimą informuoja žodžiu, šiems atvykus į sveikatos priežiūros įstaigą, ir (ar) lankydami savo pacientus namuose (70,9 proc.). Skiepijimo mokamomis vakcinomis srityje apklaustieji dalyvauja gana pasyviai: kiek daugiau nei pusė tirtųjų (53,5 proc.) teigė visada informuojantys pacientus apie galimybę pasiskiepyti šiomis vakcinomis, mažiau nei trečdalis tyrimo dalyvių (31,4 proc.) nurodė tai darantys kartais, o likusioji dalis (15,1 proc.) – retai. All the family physicians working in the public and private primary health care institutions of Marijampolė County took part in the survey. 109 questionnaires were delivered to the respondents. 86 of them were filled-in and returned (the response 79.0%). The data analysis was performed by the application of statistical program package SPSS (version 17.0). The hypotheses about the dependence of the variables were checked using the Chi square (χ2), z criteria and Spearman correlation coefficient (r). Results. The family physicians providing services to the children were actively performing the immunoprophylaxis activities according to the Calendar of the Preventive Vaccination of the Children of the Republic of Lithuania. The major part of the respondents (87.2%) were informing the parents (guardians) about the need to vaccinate their children upon their arrival to the health care institutions and (or) while visiting the patients at home (70.9%). Ištirti netoli gydymo įstaigų dirbančių farmacijos specialistų požiūrį į esamą gydytojo ir vaistininko bendradarbiavimą. 2. Nustatyti veiksnius, turinčius įtakos gydytojo ir vaistininko bendradarbiavimui ir problemas, su kuriomis susiduriama. 3. Atskleisti farmacijos specialistų nuomonę apie gydytojo ir vaistininko bendradarbiavimo galimybes ateityje. 4. Įvardinti sprendimus, kuriuos įgyvendinant, gydytojo ir vaistininko bendradarbiavimas galėtų būti pagerintas. Metodika: Tyrimo dalyviai – farmacijos specialistai, dirbantys Kauno miesto visuomenės vaistinėse, esančiose šalia ligoninės, poliklinikos, šeimos klinikos ar individualaus gydytojo kabineto. Tyrimo metodas – anoniminė anketinė apklausa. Iš viso išdalintos 188 anketos, pilnai užpildytos grąžintos 157 anketos, iš jų (atsakomumas 83,51proc.). Duomenų statistinė analizė atlikta naudojant SPSS (Statistical Package for Social Science) 20.0 programinį statistinės analizės paketą. Tyrimo rezultatai: 61,15 proc. respondentų esamą gydytojo-vaistininko bendradarbiavimą vertina kaip nepakankamą. Esamo gydytojo ir vaistininko bendradarbiavimo vertinimas skiriasi priklausomai nuo to, kokią dalį gydytojų, dirbančių šalia esančioje gydymo įstaigoje, farmacijos specialistas pažįsta (p<0,05). 48,08 proc. respondentų nurodė, kad jų praktikoje...
