Technické aspekty druhé doby porodní / Technical Aspects of the Second Stage of Labor

Obstetrical intervantion`s goal is to normalize an abnormal or pathological course of labour. In a certain case (e.g. fetal distress) this is not fully achievable. Then the goal is to accelerate the delivery without inadequate increase of risk of maternal or neonatal trauma. The aim of this dizertation thesis was to offer an up-to-date definition and to outline a proper performance of these interventions. Therefore, it was necessary to properly and timely describe the labour layout in which the accoucheur and/or the parturient happen to occurr when an intervention is to take place. It was essential to describe the quantity of perineal loading as well as to define the main vector of perineal strain and deformation. Based on the range of this deformation it was subsequently possible to adequately describe and execute some obstetrical interventions (e.g. a variety of types of episiotomy) or to evaluate a variety of modifications by means of computational modelling (e.g. manual perineal protection) that might have so tiny nuances between each other or differences that are difficult to measure because the clinical evaluation is impossible due to interindividual imprecision or very short duration of the intervention. The main theme of the diploma thesis is the impact of these effects on the patient's family. The work consists of a theoretical and an empirical part. The theoretical part is divided into four spheres - neuroanatomy and pathology, brain injury, psychosocial consequences and rehabilitation. In the empirical part the author use a qualitative research approach to deal with the impact of psychosocial consequences after the traumatic brain injury to the patient's family. TITLE: Coping with sudden motility limitation and its implication - coping strategies among young people with spinal cord injury and with multiple sclerosis SUMMARY: This thesis is focused on process of adjustment of motility limitation, caused by spinal cord injury or by attack of multiple sclerosis, among young people. The motoric ability limitation affects their mobility as well as their social interaction, interests, plans and especially their independence. Thus, in age of gaining emancipation from nuclear family, they become dependant on their home-folks again. The theoretical part of the thesis acquaints with spinal cord injuries and multiple sclerosis from biological and psychosocial point of view, thereinafter it's engaged in coping generally and coping in relation to these two groups. Furthermore it's dealt with developmental periods characterization of adolescence and young adulthood, consequences of health turnover in self-concept in social context. Title: Prevalence of asymmetric lateral disability on musculoskeletal apparatus in the neonatal age and infancy. Objective: The purpose of this research thesis was to compile issues of possible types of asymmetric musculoskeletal impairment observed in neonates and infants, identify potential causes of these asymmetries and lateral predilection for reported types of asymmetries. The main subject of discussion are just found lateral predilection of asymmetries and relationship of their causes with prenatal, perinatal or postnatal period. Methods: This thesis is structured in the form of literary research and has descriptively- analytical character. Results: On the basis of the analysis of available sources about the asymmetric musculoskeletal impairment in neonates and infants a comprehensive picture of possible types of these impairment, their causes and lateral predilections was made. The aim of this thesis is to determine the use of surface electromyography for functional diagnostic of upper limb in spinal cord injury patients from an occupational therapy perspective. Functional diagnostic is focused on personal activities of daily living (pADL). So far, no one has commented this topic from an occupational therapy perspective in the Czech Republic. The thesis should offer another possibility of objective evaluation. The theoretical part deals with the description of upper limb, including kinesiology, the types of grasps of quadriplegic patients and the muscles involved in performing personal activities of daily living. There are problem areas of activities of daily living in spinal cord injury patients and their classification. At the end of the theoretical part is described electromyography, interconnection with occupational therapy and use of surface electromyography. There is also described interdisciplinary team. The practical part consists of six case reports, in which subjects were measured muscle activity during performing personal activities of daily living using surface electromyography. The present diploma thesis focuses on equine-related traumatic injuries. Such injuries occur due to a fall off the horseback and / or during handling or care for a horse. The primary reason for focusing on this specific type of injuries was an increasing incidence of equine- related injuries due to a growing interest in horse riding as a leisure and sports activity. The theoretical part is divided into several chapters devoted to types and mechanisms of equine-related traumatic injuries, injury prevention, and specific first aid after a fall off the horseback. Information sources for the thesis were identified in the Czech National Medical Library, with most information extracted from the MEDLINE database. Other sources included the Regional Medical Library, the library of the Institute of Agricultural Economics and Information, the regional library, and professional online resources. Methodology: The second part of the diploma thesis presents the results of a quantitative research and analysis of data extracted from medical records. Burn trauma is very stressing for patients physically due to the high level of pain, damage to the skin and risk of infection, and also from a psycho-social perspective. There are currently several facilities dedicated to burn therapy. These are staffed by quality personnel with a high level of experience, who increase the hopes of patients to achieve better living conditions. Therapy depends on the extent of the handicap as well as other factors affecting the individual. Extensive burns require treatment in specialized facilities, where the patient is subject to a demanding and long-term therapy. Such patients are subject to multiple surgical procedures in total anaesthesia. The burn trauma has many complications during the healing process. The physician together with the nurse care for the patient and play a crucial role in this process. The patient will is subject to a long-term rehabilitation and complicated reintegration into society. We carried out a quantitative examination by means of 65 questionnaires handed out to general nurses, of which 58 could be used. The objectives of the thesis were met. We defined five hypotheses: H1: Nurses observe the principles of care for patients with burn trauma. H2: Nurses are aware of the effectiveness of rehabilitation treatment when caring for clients with burn trauma. H3: It is easier for nurses to provide care to patients with burn trauma if the patient is lying on an air bed as opposed to a standard bed. H4: Nurses cooperate with a clinical psychologist when caring for a patient with burn trauma. H5: Nurses cooperate with a nutrition therapist when caring for a patient with burn trauma. All the hypotheses were confirmed except hypothesis H3, which was disconfirmed. The research examination showed that nurses adhere to the principles of hygienic care for patients with burn trauma, and that they are aware of the effectiveness of rehabilitation treatment in burn care. Care for patients on an air bed is not easier for nurses than on a standard bed. On the contrary, it is physically more demanding. It was further confirmed that nurses cooperate with a clinical psychologist as well as a nutrition therapist when caring for patients with burn trauma. The diploma thesis is elaborated in such a way so that it can be used to create and present a standard, which will help to improve care for patients with burn trauma.

Spinal cord injury is a very significant clinical as well as social problem with extensive consequences, affecting the patient and also his/her family. Great efforts have been devoted to searching for an effective treatment, which would improve their situation. This thesis evaluated the effects of the natural compound curcumin on spinal cord injury using an experimental balloon compression model. Male Wistar rats were randomized into two groups following the lesion induction, namely vehicle- or curcumin-treated. The behavioral recovery was evaluated using a set of locomotor and sensory tests and a histological and imunohistochemical analysis was performed. The qPCR method was used to observe the expression of some genes related to regeneration and immune response. It was demonstrated that curcumin improved locomotor recovery after the spinal cord injury, particularly in the early stages. Morphometric analysis of the gray and white matter sparing didn't confirm any differences between the two groups. Nevertheless, the glial scar formation was significantly reduced around the central part of the lesion in the curcumin treated group and also the NF-κB activity was substantially inhibited.
