Nowadays chronic obstructive pulmonary disease (COPD) is considered to be a global and all society problems. COPD interferes in all aspects of patient´s life. It implicates limitations to physical aspect which is the cause of psychological and social effects. The disease changes the present way of patient´s life, the patients have to change their lifestyle, adapt themselves and learn new habits which can also mean changes of their needs during the COPD. The thesis deals with the problems of COPD patient´s needs. The thesis is divided into two main parts. The theoretical part focuses on characteristic of the disease, classification and hierarchy of human needs, the effects of COPD on biological, psychological, social and spiritual needs and next on the nursing care for patients with COPD. The practical part of the thesis includes the processing and next the evaluation of the dates obtained during the research. The aim of the thesis was to findout how the patient's needs differ in particular stadium of the disease. To get the aim there were determined six research questions:1) What is the knowledge level of COPD patients? 2) How different are their needs in particular stadium of the disease? 3) Which needs predominate in individual stadium of the disease? 4) Which activities can COPD patients do? 5) Which limitations and problems do COPD patients have? 6) Which needs does a COPD patient have in the field of home oxygen therapy? The method of qualitative research was used for the research. The technique of non-standardized interview was used for the data gathering. The research complex was formed by 15 COPD patients from the first to the third stadium of the disease. There were five patients in each chosen stadium. The evidence of the individual patients were divided according to their specific stadium into particular categories: knowledge of the disease, respiration, sleep, lifestyle, profession, check-ups. The research shows that the need changes did not directly depend on the level of the disease stadium, but mainly on the patient´s facing up to the disease, their total state of health and based on their individual needs. The firstand second stadium patients gave as the most common need to have their medicines permanently at their disposal, next the bigger need of sleep and rest and last but not least the need of being more independent. The progress of needs in the third stadium was different according to using of home oxygen the therapy or not. The patients who use home oxygen therapy coincided that the therapy helped them to improve their life. Now the main need is to increase their independence in household activities and to besurrounded by their family. Some COPD patients have demonstrable common symptom of depression which consequently influences physiological needs such as sleep, rest, independence and also their total self-realization. The need changes significantly affected the field of life habits, regime, lifestyle and leisure time activities. First of all there has been the absence of cigarettes and alcohol among their life habits and regime. There have been some limitations such as fishing, riding a bike, mushroomingin leisure time activities. The lifestyle was affected the changes in family and work life. The patients often suffer from dyspnoea, which subsequently limits walking, household activities, self-service,for example: shopping, meal preparing, dressing or doing personal hygiene. Despite various limitations the patients do various activities like walking dogs, strolling, reading books, watching TV, visiting clubs and other interesting culture events. The research results pointed to insufficient level of knowledge of the COPD patients. Due to the low level of the patients´ knowledge of the disease an information material with basic information about the disease was prepared.

Potřeba jistoty a bezpečí v souvislosti s plánovaným operačním výkonem z pohledu pacienta a všeobecné sestry / The need for security and safety in connection with a planned surgery with the patient's perspective and nursesBOHDALOVÁ, Jaroslava January 2016 (has links)
A planned operation performance always affects patient´s needs for assurance and safety. It is important to provide necessary information connected with the planned operation performance itself so that the patient can feel assure and safe. The key role for the patient and her/his needs for assurance and safety is in general nurse´s hands because she is the closest person to the patient and can help to inspire her/his needs for assurance and safety with konfidence best through the aid of her knowledge and skills that she should have as well as through the aid of her attitudes. The target of the thesis was to find out how a planned operation performance affects patient´s needs for assurance and safety and how a general nurse inspires patient´s needs for assurance and safety connected with a planned operation performance with confidence. The following research questions were stated: How does a planned operation performance affect patient´s needs for assurance and safety? How does a general nurse affect patient´s needs for assurance and safety connected with a planned operation performance to inspire her/him with confidence? What is insufficient in filling patient´s needs for assurance and safety before a planned operation performance? How can a general nurse help a patient control her/his fear and state of anxiety related to a planned operation performance? The method qualitative research with the use of depth interview was used to figure out the investigation. The respondents were willing to record the interview which were verbally rewritten and then subjected to the contents analysis by the method paper and pencil. From the research it is clear that a planned operation performance affects patient´s needs for assurance and safety in a negative way disregarding to her/his previous experience. Patient´s worries and negative feelings can be eliminated by a general nurse who treats her/him as equal, taking up an individual approach, informing her/him about all performances and proceedings, having a calming and welcoming way of communication, using empathy and listening as well as trying to meet patient´s wishes according to her possibilities. The research reveals the advantage of patients´ readiness for planned operation performances. Patients can learn it in the framework of educational proceeding in the out-patients´ department at the same time when they learn that they are going to be operated. The research also shows that general nurses do not have enough knowledge in the sphere of therapeutical communication. The results indicate that it would be suitable to start educating patients as soon as it is known how their disease will be solved and treated in order to reduce the negative impact of surgical treatment to patients´ needs for assurance and safety. General nurses should complete their knowledge and skills to be able to help patients to cope with their difficult life-situation. On the basis of the research investigation and after the consultation with the senior nursing officer, the training course will be organized. The topic of the training course will concentrate on communication with patients before planned operation performances with a view to therapeutical communications and therapeutical interviews. Other discovered facts, for example, the lack of information provided to patients and possibilities regarding better patients´ readiness for planned operation performances will surely be solved by the hospital management.

Specifika ošetřovatelské péče se zaměřením na potřeby a vývojovou péči novorozenců s extrémně nízkou porodní hmotností / Specifics of nursing care, focusing on the needs and developmental care for infants with extremely low birth weightTOMKOVÁ, Věra January 2018 (has links)
Neonatology is a branch of medicine which consists of the medical care not only of healthy newborn infants, but also of the ill or premature ones. The number of the premature born infants increases constantly worldwide. It might be caused by women postponing their pregnancies, and very likely also by the growing number of pregnancies achieved by the assisted reproductive technologies. Thanks to improvements in perinatal care of risk pregnancies, guiding preterm labour, and development of neonatal intensive care, it has become possible to save more infants born at the very margin of viability with an extremely low birth weight. The diagnostic, therapeutic and nursing techniques have been developing steadily. It's the methods providing maximum comfort for the child and ensuring the quality of their later life that have been put into forefront. They are a set of measures, which do not disturb the child's reflex behaviour; they support its physiological stability, and thus foster growth and development of the immature organism. The aim of this diploma work is to map the specific needs of the newborn infants with an extremely low birth weight, to find out how these needs are satisfied during the first days after birth, and to determine, how the nursing care supporting the development of the infants takes place in the real life. The theoretical part of the diploma work is based on the scientific papers and resources. The empirical part is based on an observation method, which was carried out at the Neonatal Intensive Care Unit of the University Hospital in Pilsen. The survey proved that the infants' needs are not only on the biological level, but also on the emotional level, and they need appropriate sensomotoric stimulation. The outcomes of the observation show that the greatest problem of the newborn infants with an extremely low birth weight is to satisfy the biological needs i.e. breathing, feeding and defecation. The need of safety and stimulation was satisfied only partly. Based on the information collected by observing the nursing staff, it was possible to lay out areas with a potential to improve the individualized care of the newborns with an extremely low birth weight. These are, above all, the increased level of noise at the unit and thus the sense of sleep and wakefulness of the infants. Furthermore, it's the stimulation of the children and the communication with the parents. The outcomes of the empirical part of this diploma work will be presented at the seminar of the perinatal centre.

Řešení informačních potřeb obchodního týmu v leasingové společnosti s využitím ICT nástrojů / Resolution of the information needs of the sales team in the leasing company using ICT toolsKosíková, Renáta January 2014 (has links)
Nowadays information is considered as an essential company source, which is crucial for attaining a competitive advantage. The competitive advantage can be achieved by the acquisition of the relevant information, which is a prerequisite for knowledge creation. Acquired knowledge gives businesses an opportunity to improve their position against competitors quickly by finding the information required for the decision making. Hence the aim of this thesis is to examine the problem of acquisition of the relevant information, where this problem relates to the practice in a leasing company. In order to find the relevant information, the leasing company must precisely define its information needs so that it can obtain the relevant information more efficiently, in particular by using current information management tools. At present, information management is strongly affected by contemporary trends, such as Big Data, mobility, Cloud Computing, social media etc. As a consequence of these trends, volume of electronically processed information increases every day. Due to information oversaturation, information users may face the difficult information search problem. Using both modern information management tools and exactly defined information needs, the company shall obtain the relevant information in less time, or more information in the same time compared to the other firms. This might allow the company to make decisions more rapidly. In addition, information management tools are likely to uncover new contexts, of which the company was not aware.
