Rok odevzdání: 2012 Abstrakt anglicky The goal of this pilot study is to search for a specific influence of the video confrontation technique when used as a therapy for a patient with eating disorder. For the method a questionnaire was chosen which includes semantic differentials and separate sentences. The respondents were selected based on availability.The 26 respondents included in this research were all women diagnosed with anorexia nervosa or bulimia nervosa. All were hospitalized in a eating disorder unit. The outcome of the study confirmed that video confrontation resulted in the patients describing their own body as more elegant, and had lowered the feeling of being unsure or uninterested in their own body. In the case of patients with anorexia nervosa a specific influence was found in that the patients' perception of their own body changed to more elegant and valuable. Souvislost kvality attachmentové vazby a poruch příjmu potravy / Relations between quality of attachment and eating disorders

This diploma thesis focuses on the relations between attachment quality and eating disorders. The main goal is to map how girls with an eating disorder understand their illness and what the disorder signifies to them within their family system. In the theoretical part of the thesis, eating disorders are delineated. There is an emphasis on risk factors for eating disorders, and a comprehensive approach to their explanation is presented in terms of different theoretical schools. The next part is devoted to the attachment theory with an emphasis on the development from childhood to adulthood. The theoretical foundations of the Parental Bonding Instrument are presented. Furthermore, the attachment theory is linked to the occurrence of eating disorders. In the empirical part of the thesis, the methodological aspect of the research, which was carried out with three girls who have experienced eating disorders, is first introduced. The research design combines the method of semi-structured interviews with the method of using a tool measuring the quality of bonding with parents, specifically the Parental Bonding Instrument. Gastrostomy is one of the access routes enabling enteral nutrition support to children with eating disorders. It is a method of choice in situations where real assumption of enteral feeding by a nasogastric probe longer than six weeks exists and with undamaged functional digestive system. The possibility to perform enteral feeding outside health care facilities is the most advantageous aspect of gastrostomy for children and their parents. Optimum nursing care is one of the main preconditions of long term functional gastrostomy. This why suitable education of parents and their children in the nutrition support method is necessary. The aim of the work was to map awareness of the issues of gastrostomy in children patients among parents and to propose a unified educational programme aimed at easier insight into the issues. Qualitative research method was applied to the problems. The techniques of study of healthcare documentation and semi standardized interviews with respondents in their home environment were used for data collection. The results of the healthcare documentation studies, which defined the sample of the children with applied gastrostomy, were processed into graphic form for better transparency. The interviews with the respondents in the form of audio recording were transposed into written form and quantified by means of tables enabling the research conclusion to be drawn. The research results showed that practical training of gastrostomy treatment and awareness of possible complications and their solution was the weak point of the education. The above results have confirmed the necessity to elaborate a unique educational programme related to the issues of feeding tube insertion, the care for gastrostomy, support of food, medicines, and possible related complications in terms of learning the ability to prevent complications, identify and solve them.

Thesis deals with the issue of food intake disorders in secondary school. The theoretical part of the work is focused on defining the terms anorexia nervosa and bulimia nervosa. Starting with historical development of these diseases, through complexity of causes and consequences of the disease and description of an endangered group of patients, I got to the contemporary medical treatment used. The practical part of this work consists of the questionnaire survey of adolescent students. Based on the results of this survey I proposed a brief prevention program, targeting in particular parents and teachers, including practical advices how to support the prevention of food intake disorder diseases of adolescents.

Úloha ošetřovatelství v prevenci poruch příjmu potravy / The role of nursing in the prevention of eating disorders

Prevention of eating disorders is important because eating disorders are one of the most common and serious diseases adolescent girls and young women may suffer from, with severe somatic, psychological and social consequences. The treatment of eating disorders is tricky and not always successful. This is a disease for which it is difficult to induce the necessary motivation and cooperation of the patient for treatment. Patients suffering from eating disorders have a distorted image of their body, body weight and body shape. An excessive weight loss has an impact on health. The most serious complications are cardiovascular complications, which may lead up to death. Anorexia nervosa is a psychiatric illness with high mortality, which reaches up to 18%. The first objective of the thesis was to survey the risk of eating disorders occurrence in pupils of the second level of primary school. The second objective was to map the awareness of pupils of the second grade of primary school and to identify the source of information. To meet the objectives quantitative research was used. As a method of data collection the questionnaire technique was chosen. The standardized questionnaire EAT - 26, containing 26 closed questions, was used at the research. Another part of our questionnaire included 5 closed questions to get the respondents' identification data, and 2 semi-closed questions. In the case of a positive answer it was necessary to write down the answer which was given by all the pupils with positive answers, and 1 semi-enclosed question with the possibility to add another answer. The questionnaire was completed by 141 pupils of the 5th ? 9th grades of the Primary school Grünwaldova in České Budějovice. The results show that the risk of eating disorders in girls and boys is comparable. According to the obtained results, the risk of eating disorders development is the least in the 5th grade pupils, because they are less concerned about their own looks, physique and body weight than the respondents from higher grades. Satisfaction with the body weight is the highest in the fifth grades, it decreases in higher grades pupils. In our respondents the risk of eating disorders did not appear, yet it is necessary to avoid an increase of that risk. It is necessary to highlight the educational and preventive role of a nurse. She can prevent the possible increased risk of the development of eating disorders later in life or even the onset of anorexia nervosa or bulimia nervosa.

Hunter-gatherer archaeobotany: Central European Mesolithic

The present work attempts to provide understanding of the issue of Mesolithic archaeobotany, especially in terms of plant use, woodland clearance, and discussion concerning Mesolithic agriculture. Special attention is also paid to taxa occurring within archaeological context at European Mesolithic sites and some methodological implications. Also, specific patterns in food preparation and consumption are considered, particularly with emphasis on certain archaeological features. The thesis also brings new original data from the Czech Republic. In this regard, assemblage from Dvojitá Brána u Rohlin has provided hints towards the study of the Neolithisation process in the Czech Republic, whereas case study from the Schwarzenberg Lake has brought considerable focus on the issue of depositional/postdepositional processes, especially when dealing with shallow sandy sediments. The thesis also strongly reflects the growing need for interdisciplinary work to address issues related to Mesolithic hunter-gatherers.

SOCIAL INPACT OF THE MENTAL ANOREXIA AFFECTING CHILDREN´S PATIENTS ON HE FAMILY In choosing the subject of my degree work I was influenced fundamentally by my experience gained during the department of children´s psychiatry. At thes very interesting work I had opportunity to become acquainted not only with various psychiatric diagnoses but with plenty of human fates in particular. The girls with plenty of human fates in particular. The girls wth mental anorexia were frequent patients of this department. During their long-term and often repeated hospitalization I met both the patients and the whole families. That´s why I could follow the development of the children affected like this and the impact of this illness on the family as well. The mental anorexia is a very grave and lingering illness belonging to the group of disorders in food ingestion. The consequences of this illness for he health can be of an persistent character and it is not exceptional that this illness is even fatal. The therapy is spite of all therapeutical possibilities, the modern psychiatry has at disposal. Therefore, the families of the patients have not only to get over the fear for the life and health of their members, but they have also to overcome longlasting psychological stress connected with the care of the person affected like this. This care can influence the running of the family fundamentally. It often happens that the family has to reduce the professional activities with respect to the exacting care. This can result in aggravation of the economic situation of the family. A considerable lowering of the living standard and the following crisis can take place in families with low incomes. Permanent psychological stress, deterioration of the economic situation and fair for the child can influence also the relations between individual members of the family. It is no exception that this stress has an adverse effect on the marriage of the parents, and that expecially in the case that the marriage was a problematic one as early as before the child fell ill.
