Introdução: A gestação após transplante de órgãos tem se tornado cada vez mais comum, especialmente após avanços das técnicas em relação ao transplante e novos imunossupressores adotados na terapêutica. Objetivo: Avaliar os resultados maternos e perinatais das pacientes gestantes submetidas a transplante renal prévio. Método: No período de 1995 a 2007, foram avaliados restrospectivamente todos os casos de pacientes submetidas a transplante renal, que tiveram parto na Clínica Obstétrica do HCFMUSP. Resultados: Foram selecionadas 31 gestações em 22 pacientes. A idade média foi de 29 anos (18-39). Em 52% dos casos (16 casos) a gestação ocorreu 5 anos após o transplante. Em 21 pacientes (68%) o esquema imunossupressor utilizado foi composto por ciclosporina, azatioprina e prednisona. A incidência de abortamentos foi de 22% (7 casos). A hipertensão esteve presente em 21 casos (87,5%), seguida de anemia 17 (70,83%) e prematuridade 15 (71%). A função renal no início do pré-natal teve o seguinte comportamento: uréia no início 45±7,1 mg/dl (28-105) e no parto 68±27,6 (35-128); creatinina no início do pré-natal 1,28±0,55 mg/dl (0,9-3,6) e no parto 2,06±1,16 mg/dl (1,12-6,4); ácido úrico no início 6,1±1,5 mg/dl (3,5-9,2) e no parto 8,54±2,79 mg/dl (2-14) e a proteinúria de 24 horas no início 0,19±0,09 g/vol.24h (0,03-0,31) e no parto 1,34±0,96 g/vol.24h (0,11-3,65). A via de parto: oito (33%) partos por via vaginal e 16 (66%) foram cesarianas. A idade gestacional média ao nascimento foi de 34,8±2,29 semanas (28-38) e o peso ao nascimento foi 2366,5±261,6g (1100-4650). As complicações neonatais foram: icterícia 15 (71,5%); desconforto respiratório 10 (47,6%); hipoglicemia 5 (23,8%); anemia 4 (19%), intubação orotraqueal 3 (14,3%), utilização NPP 3 (14,3%), hipocalcemia 3 (14,3); pneumonia 2 (9,5%); hemorragia intracraniana 1 (4,8%) e sepse 1 (4,8). Ocorreram 3 óbitos fetais. Conclusão: a taxa de sobrevida neonatal no estudo de foi de 88%, a hipertensão foi a intercorrência materna mais freqüente, a utilização da terapêutica imunossupressora utilizada não levou a malformação fetal e a morbidade neonatal esteve associada a complicações inerentes da prematuridade 15 casos (71%). / Introduction: The pregnancy after organ transplant has become very common, especially after the technical advance related to new immunosuppressive therapy on the transplant. Objective: Analysis of the maternal and pregnancy outcomes of the patients that has renal transplant. Method: During the period between 1995 and 2007, it was analyzed retrospectively all the patient submitted on renal transplant, and also has had delivery in the Hospital das Clínicas of the University of São Paulo Medical School. Results: We have obtained a total of 31 pregnancies in 22 patients. The average age was 29 years of age (18- 39). In 52 % of the cases (16 cases) the pregnancy occurred 5 years after the transplant. In 21 patients (68%) the immunosuppressive treatment used was based on ciclosporine, azatioprine and prednisone. The incidence of abortion was 22% (7 cases), that was excluded. The hypertension was present in 21 cases (87,5%), followed by anemia 17 (70,83%) and prematurity 15 (71%). In the beginning of the pre-natal the renal function presented the following: serum urea in the beginning of prenatal care was 45±7,1 mg/dl (28-105)and the time of delivery was 68±27,6 (35-128); serum creatinine in the beginning was 1,28±0,55 mg/dl (0,9-3,6) and the time of delivery was 2,06±1,16 mg/dl (1,12- 6,4);serum uric acid in the beginning of prenatal care was 6,1±1,5 mg/dl (3,5-9,2) and the time of delivery was 8,54±2,79 mg/dl (2-14). The proteinuria 24 hours at the beginning of prenatal care was 0,19±0,09 g/vol.24h (0,03-0,31) and the time of delivery was 1,34±0,96 g/vol.24h (0,11-3,65). Cesarean section was performed in 16 (66%) cases, vaginal delivery in 8 (33%). The mean gestational age at delivery was 34,8±2,29 weeks (range: 28-38) and the mean birthweight at delivery was 2366,5±261,6g (range 1100-4650). The neonatal complications were observed during the study: ictericia 15 (71,5%); respiratory distress 10 (47,6%); hipoglicemia 5 (23,8%); anemia 4 (19%), orotracheal intubation 3 (14,3%), use of NPP 3 (14,3%), hipocalcemie 3 (14,3); pneumonie 2 (9,5%); intracranial hemorrhage 1 (4,8%) e sepses 1 (4,8). Occurred 3 fetal deaths. Conclusion: in the study the statistics of neonatal success was 88%, the most frequent maternal complication was the hypertension. The immunosuppressive therapy utilized didn\'t cause fetal malformation and the neonatal morbid was associated with prematurity complications itself in 15 cases (71%).

Pregnancy and acquisition of sexually transmitted infections: risk behaviors and incidenceTeasdale, Chloe Anna January 2015 (has links)
This dissertation had three primary aims. The first aim was to systematically review evidence documenting incidence of sexually transmitted infections (STI) during pregnancy. Eighteen papers were included in the final review which reported incidence of five STIs: chlamydia, gonorrhea, human papillomavirus (HPV), herpes simplex virus type 2 (HSV-2) and human immunodeficiency virus (HIV). The review found that there are very limited data on incidence of STIs during pregnancy and even fewer data comparing risk between pregnant and non-pregnant women. Although data are limited, studies suggest that women continue to acquire STIs during pregnancy, with incidence varying by type of infection, population of interest and geographic setting. Highest incidence was found for HPV and chlamydia although some studies of chlamydia showed low proportions of pregnant women infected. Studies in which partners were known to be infected with HSV-2 and HIV showed higher rates of acquisition in pregnant women compared to studies where partner status was not known.
The second aim of this dissertation was to describe the impact of pregnancy on behavioral risk factors and vaginal practices that are associated with increased risk of STI acquisition. Data for this and the following aim came from the Methods for Improving Reproductive Health in Africa (MIRA) study, a randomized clinical trial conducted in South Africa and Zimbabwe 2003-2006. The analysis for the second aim included women in the MIRA trial who had a pregnancy during follow-up. Pregnancy was found to decrease sexual activity, particularly in the third trimester, but women were more likely to report sex without condoms while pregnant. There were lower reports by women during pregnancy of other risk factors for STI acquisition, including anal sex, concurrent sexual relationships and new sex partners. Vaginal wiping and insertion of material into the vagina, potentially important mechanisms for STI acquisition, were also less common during pregnancy. The data from this aim present a complicated picture of risk for STIs during pregnancy as a result of increased unprotected sex but decreased frequency of other known behavioral risk factors.
The third and final aim of the dissertation was to measure incidence of four STIs in pregnant and non-pregnant women and to evaluate whether women are at greater risk during pregnancy for acquiring four STIs: chlamydia, gonorrhea, trichomoniasis and HIV. This analysis included 4,549 women 18-50 years of age, 17% (N=766) of whom had a pregnancy during follow-up. In general, women continued to be sexually active but reported less overall sex than non-pregnant women. Report of condom use was lower during pregnancy as were other types of high risk sexual behaviors, such as multiple sexual partners, sex in exchange for drugs or money and anal sex, as well vaginal practices. STI incidence was measured during pregnancy and it was found that women continued to acquire STIs when pregnant. In addition, during periods when women became pregnant, they appeared to be a high risk for acquiring chlamydia, trichomoniasis and HIV. Finally, in examining the association between pregnancy status and STI risk, we found that in multivariable models adjusted for demographic and time-varying self-reported behavioral risk factors and vaginal practices, pregnancy was not associated with increased STI risk. However in visit intervals when women became pregnant, they appeared to be at higher risk for contracting chlamydia compared to non-pregnant periods.

Gravidez não planejada: a experiência das gestantes de um município do interior do estado de São Paulo / Unplanned pregnancy: the experience of the women in a municipality in the state of São PauloSanches, Natália Canella 02 December 2013 (has links)
A gestação, o parto e o puerpério são saberes especiais no universo da mulher, do parceiro, da família e de sua comunidade. A gestação pode gerar diversos sentimentos, tais como: o de surpresa, o de castigo, o de prêmio, o de motivação para continuar a viver, o de realização de um projeto antigo, o de competição em família, o de problema, o de estorvo, o de descuido ou de irresponsabilidade. Confirmada a gravidez, a mulher, o parceiro e os familiares podem vivenciar diferentes reações diante dessa novidade. A reação inicial depende do desejo e planejamento da gravidez (gestação), podendo ser desejada, planejada, ou acidental, não planejada e, até mesmo indesejada. Gravidez não planejada é toda a gestação que não foi programada pelo casal ou, pelo menos, pela mulher. A sua ocorrência tem impacto importante na oferta de cuidados de pré-natal, na orientação sobre aleitamento materno, no estado nutricional infantil e nas taxas de morbimortalidade materno-infantil. Embora pouco estudada, a gravidez não planejada representa risco aumentado de ansiedade e de depressão, sobretudo no período puerperal. Tendo em vista estes aspectos, o objetivo deste estudo foi compreender como as gestantes vivenciaram/experienciaram uma gravidez não planejada e suas consequências à vida familiar/conjugal. O estudo baseou-se na metodologia de análise de dados qualitativos, na análise destes dados, utilizou-se a abordagem metodológica do Discurso do Sujeito Coletivo (DSC), por meio do software Atlas.ti. A população em estudo foi constituída de gestantes em idade fértil, entre 18 e 49 anos de idade, pois essa faixa etária assegura maioridade às mulheres. Foi utilizada para organizar os dados verbais, entrevista semiestruturada, realizada com 11 gestantes. Partindo dos depoimentos das gestantes copiados no software, a análise dos discursos teve início com a identificação das expressões-chave, nas quais estão contidas as ideias centrais de cada discurso que foram escritas de forma breve e objetiva. Foi evidenciado que as gestantes apresentaram reações iniciais negativas com relação à descoberta da gravidez não planejada, vivenciaram conflitos, devido ao medo de enfrentar a família, o companheiro e os pais. Também ficou evidente que a gravidez não planejada tem como principal consequência a problemática nos níveis biopsicossociais. Evidenciaram altos níveis de ansiedade, estresse e depressão no decurso da gestação. Quanto aos métodos contraceptivos, a maioria estava em uso quando da descoberta da gravidez. Considera-se que este estudo ganha relevância pelo impacto que exerce sobre o bem-estar pessoal, familiar e socioeconômico das mulheres e seus companheiros, em virtude das possíveis gestações não planejadas / Pregnancy, childbirth and the postpartum period are special knowledge in the world of woman\'s partner, family and your community. Pregnancy can cause many feelings, such as: the surprise, the punishment, the premium, the motivation to continue living, the realization of an old design, the competition in the family, the problem of the hindrance, to carelessness or irresponsibility. Confirmed pregnancy, the woman, her partner and family members may experience different reactions to this news. The initial reaction depends on the planning and desire of pregnancy (gestation), and may be desired, planned or accidental, unplanned and even unwanted. Unplanned pregnancy is any pregnancy that was not planned by the couple, or at least the woman. Its occurrence has important impact on the provision of prenatal care, the guidance on breastfeeding, the nutritional status and rates of maternal and child morbidity and mortality. Although little studied, unplanned pregnancy is increased risk of anxiety and depression, especially in the postpartum period. Considering these aspects, the aim of this study was to understand how pregnant women experienced / experienced an unplanned pregnancy and its consequences for family life / marriage. The study was based on the methodology of qualitative data analysis, the analysis of these data, we used the methodological approach of the Collective Subject Discourse (CSD), using the software Atlas.ti. The study population consisted of pregnant women of childbearing age, between 18 and 49 years of age, because this age ensures age women. Was used to organize the verbal data, semi-structured interviews conducted with 11 pregnant women. Based on the testimonies of the women copied the software, discourse analysis began with the identification of key expressions, in which are contained the main ideas of a speech that was written briefly and objectively. It was shown that pregnant women had initial negative reactions regarding the discovery of unplanned pregnancy, experienced conflict due to fear of facing the family, spouse, and parents. It was also evident that unplanned pregnancy is mainly due to problematic levels biopsychosocial. Showed high levels of anxiety, stress and depression during pregnancy. As for contraception, the majority was in use when the discovery of the pregnancy. It is considered that this study becomes relevant for the impact it has on the well -being, family and socioeconomic status of women and their partners, because of possible unintended pregnancies

Predictors of Rapid Repeat Pregnancy in ZimbabweSisimayi, Thenjiwe 01 January 2019 (has links)
Rapid repeat pregnancy (RRP) is associated with adverse maternal and infant outcomes and a range of undesirable social and economic challenges for the mother, her baby, and society. Although the consequences of RRP are well known, Zimbabweâa country with some of the poorest maternal health indicatorsâhas not investigated or made efforts to directly address this problem. This is confirmed by the lack of targeted programs to curb RRP, the unavailability of documented evidence regarding RRP significant risk factors, and the lack of understanding of the extent of RRP in the country. Using social cognitive theory as the theoretical framework, an unmatched case-control study was conducted using data from the Zimbabwe Demographic and Health Survey of 2015 to determine the prevalence of RRP and to assess associations between sociodemographic, sexual-relational, women's health, fertility preference, previous birth outcomes, and social factors and having an RRP in Zimbabwe. Logistic regression analysis showed statistically significant associations between all factors except for women's health characteristics. The prevalence of RRP among women of reproductive age (15â49 years) in Zimbabwe was 50.2%. The high prevalence of RRP and the multiple statistically significant associations reported in this study affirm the need for Zimbabwe to make prevention of RRP a public health priority. Zimbabwe must develop targeted interventions that work in context and integrate these into an ongoing comprehensive family planning program. In-depth research is needed to establish and understand the underlying motivations for having an RRP among Zimbabwean women. Such information may help develop targeted interventions to create social change.

A qualitative study was conducted to identify perceptions of high-risk pregnant Aboriginal women during pregnancy. Five women were interviewed by the researcher who is also Aboriginal. Each woman was a client of the Healthy Mother Healthy Baby pregnancy outreach program in Saskatoon, and was considered to be at high-risk for poor pregnancy outcomes. Interviews were conducted utilizing a phenomenologically-based model of research, from which emerged themes or commonalities of thought between each woman during this time of their lives. The themes were discussed with Aboriginal women elders who worked with Aboriginal families in Saskatoon, for further insight and discussion of the findings. The themes were collated under one general theme called `hopes and dreams.' Each theme included the interest of these women to `want to' move ahead with their lives in the area of improving their relationships with their offspring, their partners, and within themselves. Though each women talked about these areas as being important, all except one did not manifest them in their lives.<p>
The implications for the delivery of health care services include the need for health care workers to gain further understanding of the positive motivational factors of high-risk pregnant Aboriginal women that work towards achieving long-term positive behavioural changes, and thus reduce the risks of poor pregnancy outcomes. Other recommendations from this research for health promotion programs include the development of a partnership with the urban Aboriginal community in delivering culturally-based services and teachings to complement the educative and supportive model of program delivery.

The impact of pneumonia in human immunodeficiency virus (HIV-1) infected pregnant women on perinatal and early infant mortality.January 2007 (has links)
Background: Although the prevalence of pneumonia in pregnancy is reported to be less than 1%, the pregnant state and risk factors associated with the development of pneumonia adversely influence the outcome of pregnancy. KwaZulu-Natal is at the epicenter of the dual epidemics of tuberculosis and HIV-1 and the impact of these diseases occurring concurrently in pregnant women at King Edward VIII hospital (KEH), South Africa have been described previously. The impact of antenatal pneumonia in HIV-1 infected and uninfected women however has not been described in the study population and was investigated. Methods: Pregnant women with clinical and radiological evidence of pneumonia were recruited from the antenatal clinic and labour ward at KEH. The study was conducted prospectively between January and December 2000. The clinical profile of these women and the causative organisms were determined. In addition the impact of HIV-1 infection, maternal immunosuppression and maternal pneumonia on obstetric and perinatal outcomes were evaluated. Mothers diagnosed with tuberculosis and multi drug resistant tuberculosis were hospitalised at King George V hospital until delivery. Results: Twenty nine women were diagnosed with antenatal pneumonia (study arm) with Mycobacterium tuberculosis the only causative organism isolated. A control arm of 112 pregnant women was also studied. Maternal and perinatal mortality was restricted to the study arm with a maternal mortality ratio of 99 per 100 000 live births and a perinatal mortality rate of 240 per 1000 births. Pneumonia was significantly associated with a negative overall obstetric outcome in the presence of HIV- l infection, antenatal care, anaemia and second trimester booking status. In addition, the presence of pneumonia was significantly associated with maternal mortality. There was a highly significant association between exposure to pneumonia and poor neonatal outcome. Maternal pneumonia, maternal HIV infection and the presence of medical and obstetric conditions were significantly associated with low birth weight and neonatal pneumonia. Further, maternal pneumonia (p <0.001) and concurrent HIV infection (p=0.002) was significantly associated with neonatal death. Conclusion: The presence of pneumonia in the antenatal period impacts negatively on maternal and neonatal morbidity and mortality. Health care providers must maintain a high degree of suspicion when managing a pregnant woman with unresolving upper respiratory tract symptoms and refer timeously for further investigation. Pneumonia and in particular pulmonary tuberculosis associated with HIV co- infection in pregnancy is a threat to mother and baby. A gestação, o parto e o puerpério são saberes especiais no universo da mulher, do parceiro, da família e de sua comunidade. A gestação pode gerar diversos sentimentos, tais como: o de surpresa, o de castigo, o de prêmio, o de motivação para continuar a viver, o de realização de um projeto antigo, o de competição em família, o de problema, o de estorvo, o de descuido ou de irresponsabilidade. Confirmada a gravidez, a mulher, o parceiro e os familiares podem vivenciar diferentes reações diante dessa novidade. A reação inicial depende do desejo e planejamento da gravidez (gestação), podendo ser desejada, planejada, ou acidental, não planejada e, até mesmo indesejada. Gravidez não planejada é toda a gestação que não foi programada pelo casal ou, pelo menos, pela mulher. A sua ocorrência tem impacto importante na oferta de cuidados de pré-natal, na orientação sobre aleitamento materno, no estado nutricional infantil e nas taxas de morbimortalidade materno-infantil. Embora pouco estudada, a gravidez não planejada representa risco aumentado de ansiedade e de depressão, sobretudo no período puerperal. Tendo em vista estes aspectos, o objetivo deste estudo foi compreender como as gestantes vivenciaram/experienciaram uma gravidez não planejada e suas consequências à vida familiar/conjugal. O estudo baseou-se na metodologia de análise de dados qualitativos, na análise destes dados, utilizou-se a abordagem metodológica do Discurso do Sujeito Coletivo (DSC), por meio do software Atlas.ti. A população em estudo foi constituída de gestantes em idade fértil, entre 18 e 49 anos de idade, pois essa faixa etária assegura maioridade às mulheres. Foi utilizada para organizar os dados verbais, entrevista semiestruturada, realizada com 11 gestantes. Partindo dos depoimentos das gestantes copiados no software, a análise dos discursos teve início com a identificação das expressões-chave, nas quais estão contidas as ideias centrais de cada discurso que foram escritas de forma breve e objetiva. Foi evidenciado que as gestantes apresentaram reações iniciais negativas com relação à descoberta da gravidez não planejada, vivenciaram conflitos, devido ao medo de enfrentar a família, o companheiro e os pais. Também ficou evidente que a gravidez não planejada tem como principal consequência a problemática nos níveis biopsicossociais. Evidenciaram altos níveis de ansiedade, estresse e depressão no decurso da gestação. Quanto aos métodos contraceptivos, a maioria estava em uso quando da descoberta da gravidez. Considera-se que este estudo ganha relevância pelo impacto que exerce sobre o bem-estar pessoal, familiar e socioeconômico das mulheres e seus companheiros, em virtude das possíveis gestações não planejadas / Pregnancy, childbirth and the postpartum period are special knowledge in the world of woman\'s partner, family and your community. Confirmed pregnancy, the woman, her partner and family members may experience different reactions to this news. The initial reaction depends on the planning and desire of pregnancy (gestation), and may be desired, planned or accidental, unplanned and even unwanted. Unplanned pregnancy is any pregnancy that was not planned by the couple, or at least the woman. Its occurrence has important impact on the provision of prenatal care, the guidance on breastfeeding, the nutritional status and rates of maternal and child morbidity and mortality. Although little studied, unplanned pregnancy is increased risk of anxiety and depression, especially in the postpartum period. Considering these aspects, the aim of this study was to understand how pregnant women experienced / experienced an unplanned pregnancy and its consequences for family life / marriage. The study was based on the methodology of qualitative data analysis, the analysis of these data, we used the methodological approach of the Collective Subject Discourse (CSD), using the software Atlas.ti. The study population consisted of pregnant women of childbearing age, between 18 and 49 years of age, because this age ensures age women. It is considered that this study becomes relevant for the impact it has on the well -being, family and socioeconomic status of women and their partners, because of possible unintended pregnancies
