Intraorale Weichteilinfektionen : eine retrospektive Untersuchung von 244 Patienten / Oral soft tissue infections-a retrospective analysis of 244 patients

Introduction Intraoral soft tissue infections (OSTI) are a common problem in dentistry and oral surgery. These abscesses are mostly exacerbated dental infections (OIDC), and some emerge as postoperative infections (POI) after tooth extraction (OITR) or apicoectomy (OIRR). The main aim of this study was to compare OIDC with POI, especially looking at the bacteria involved. An additional question was, therefore, if different antibiotic treatments should be used with OSTI of differing aetiologies. The impact of third molars on OSTI was evaluated and also the rates of POI after removal of third molars were specified. Materials and methods Patient data was collected from the patients' medical records and the results were statistically evaluated with SPSS (SPSS version 21.0; SPSS, IBM; Chicago, IL, USA). The inclusion criterion was the outpatient treatment of a patient with an exacerbated oral infection; the exclusion criteria were an early stage of infiltration without abscess formation; and a need for inpatient treatment. Results Periapical exacerbated infections, especially in the molar region were the commonest cause of OIDC. In the OITR group, mandibular tooth removal was the commonest factor (p=0.016). Remarkably, retained lower wisdom teeth led 91 to significant number of cases in the OITR group (p=0.022). Conclusions In our study we could not define differences between the causal bacteria found in patients with OIDC and POI. Due to resistance rates we conclude that amoxicillin combined with clavulanic acid seems to be the antibiotic standard for exacerbated intraoral infections independent of their aetiology. / Einführung Intraorale Weichteilinfektionen (OSTI) sind ein häufiges Problem in der Zahnmedizin und Kieferchirurgie. Diese Abszesse sind meist dentalen Ursprungs(OIDC), einige wiederum treten als postoperative Infektionen (POI) nach einer Zahnextraktion (OITR) oder Wurzelspitzenresektion (Oirr) auf. Das Hauptziel dieser Studie war es OIDC mit POI zu vergleichen. Ein weiteres Ziel war es, die unterschiedlichen antibiotischen Behandlungen bei OSTI unterschiedlicher Ätiologien gegeneinander abzuwägen. Die Auswirkungen der dritten Molaren auf OSTI wurde ebenfalls untersucht, und auch die Rate von POI nach Entfernung der dritten Molaren wurden angegeben. Material und Methode Die Patientendaten wurden durch die Analyse der Aufzeichnungen und Akten erhoben. Die Ergebnisse wurden statistisch ausgewertet mit SPSS (SPSS Version 21.0, SPSS, IBM, Chicago, IL, USA). Einschlusskriterium war die ambulante Behandlung eines Patienten mit dem Krankheitsbild einer intraoralen Infektion. Die Ausschlusskriterien waren ein frühes Stadium der Infiltration ohne Abszessbildung und die Notwendigkeit einer stationären Behandlung. Ergebnisse Apikale Parodontitiden, vor allem im Bereich der Molaren waren die häufigste Ursache für OIDC. In der OITR Gruppe war Zahnentfernung die häufigste Ursache (p = 0,016). Bemerkenswerterweise führte die Entfernung von unteren Weisheitszähnen zu einer erheblichen Anzahl von Fällen in der OITR Gruppe (p = 0,022). Schlussfolgerungen In unserer Studie konnten wir keine Unterschiede zwischen der bakteriellen Flora bei Patienten mit OIDC und POI erörtern. Aufgrund der aktuellen Resistenzraten, ziehen wir die Schlussfolgerung, dass Amoxicillin kombiniert mit Clavulansäure das Antibiotikum der Wahl für intraorale Infektionen, unabhängig von deren Ätiologie, zu sein scheint.

Deadly Gun Violence : A Critical Discourse Analysis of Possible Ideological Influences on the Framing of a Mass Shooting

Statistics show that the number of mass shootings and involved fatalities have drastically increased over the last five years. The framing of these attacks in the mass media has a substantial impact on the public opinion on the causes of shootings, possible prevention methods and gun control in general. Following study aims to uncover potential ideological influences of political tendencies of liberal and conservative newspapers on the media coverage of the most recent mass shooting in the Marjory Stoneman Douglas High School in Parkland, Florida. A qualitative content analysis with a general structural analysis of 39 articles from the liberal newspaper The New York Times and the conservative newspaper The Washington Times and a more detailed Critical Discourse Analysis of the frames of blame, prevention methods and gun control were conducted. Findings show that The Washington Times´media coverage on blame attributions and prevention methods mainly included frames of mental health. The focus in The New York Times´ coverage is clearly on gun issues and the demand for gun restrictions. The topic gun control is positively framed by The New York Times. The Washington Times, however, framed gun control in connection with conspiracy theories and thus portrayed it in a more negative light. The findings prove an influence of the newspapers´ political tendencies and ideologies on the media coverage of the Florida mass shooting.

Sjuksköterskans primärpreventiva metoder för att uppmuntra till livsstilsförändringar hos patienter med risk för hjärt- och kärlsjukdom : En litteraturstudie

Bakgrund: Hjärt- och kärlsjukdomar är ett globalt växande folkhälsoproblem som dessutom är en ekonomisk last på samhället. Forskning har uppskattat att majoriteten av dödsfall kommer att bero på hjärt- och kärlsjukdomar vid år 2030. Några riskfaktorer för att utveckla hjärt- och kärlsjukdom är ett högt BMI, rökning och låg fysisk aktivitet. Forskning har visat att primärpreventivt arbete med livsstilsförändringar är något som minskar riskfaktorerna och är kostnadseffektivt för samhället. Syftet: var att beskriva vilka primärpreventiva metoder sjuksköterskan använder för att uppmuntra till livsstilsförändringar hos patienter med risk för  hjärt- och kärlsjukdom samt att beskriva hur dessa primärpreventiva metoder påverkar riskfaktorerna. Syftet var också att beskriva urvalsgrupperna i de valda vetenskapliga artiklarna. Metod: För att besvara syftet och frågeställningarna hade denna litteraturstudie en beskrivande design där tolv vetenskapliga artiklar har analyserats. Databaserna PubMed och CINAHL användes vid sökningen för att finna de vetenskapliga artiklarna. Resultat: Sjuksköterskor använde olika metoder för att uppmuntra till livsstilsförändring hos hjärt- och kärlsjuka patienter. Dessa metoder innebar att sjuksköterskan antingen fungerar som en informationsgivare eller samarbetspartner gentemot patienten. Effekten av metoderna varierade och enligt vissa studier gav de ej gav någon effekt. Slutsats: Utifrån litteraturstudiens resultatet går det ej med säkerhet att säga vilka metoder som är mest effektiva. Sammanfattningsvis kan sjuksköterskan i sitt kliniska arbete ta hänsyn till båda förhållningssätt i mötet med patienterna och anpassa metoden utifrån ett individuellt perspektiv. / Background: Coronary heart disease is a growing health issue from a global perspective. Research has estimated that coronary heart disease will stand for the majority of deaths by the year 2030.  Risk factors include a high BMI, smoking and low level physical activity. Research has also shown that lifestyle change can decrease the risk factors and is cost-effective for the society. Aim: The aim with the litterature study was to describe which primary prevention methods nurses use to encourage lifestyle change for patients at  risk of developing coronary heart disease, and to describe how these methods affect the risk factors. The purpose was also to describe the sample groups in the chosen articles. Method: To answer the study questions, the literature study had a descriptive design where twelve scientific articles were analysed and categorized. The databases PubMed and CINAHL were used to find the scientific articles. Results: Nurses use an array of different methods to encourage life style change in patients with coronary heart disease. These methods include the nurse either acting as a information giver or a collaboration partner towards the patient. The effect of these methods varied and some of the studies didn’t show any effect whatsoever. Conclusion: On the basis of the literature study, the methods nurses use and their effectiveness are inconclusive. In conclusion, the nurse can consider all of the methods in the work place, and decide which method is adequate based on the individual variations of the patients.

Analýza metod a realizace prevence relapsu ve vybrané terapeutické komunitě / Analyses of the applied methods of the relapse prevention and interventions used in a selected therapeutic community for addiction treatment.

The aim of this qualitative study was to map the preparation and process of relapse prevention programs in therapeutic community for treating addictions in Czech republic called Magdaléna o.p.s.. The next aim was to describe the specifics of these programs and analyse methods that are mostly applied. For fullfilling these aims, four methods were chosen: analisys of documents, semistructured interwiews with the members of the team of therapists, observation of the researcher in the specific relapse prevention program during her internship and one semistructured interview with client of this therapeutic community. The final number of respondents for this research was four therapists and one client. Three metodic relapse prevention documents were analysed and for setting the final outcomes of this research were also used the outputs from the observation of the researcher from the specific relapse prevention program. Based on this research it was found out, that the specific relapse prevention programs in Magdaléna have been part of the long-term structured schedule of the treatment. This program is on the schedule once per week in duration of one hour and it is managed by one of the therapists. Effective Prevention for Children: Conceptual and Methodological Advances

This dissertation addresses various methodological and conceptual challenges of prevention programs for preschool children. It focuses on two major topics, (1) methodological guidelines for longitudinal studies in the context of prevention projects, and (2) analyses of emotional development and moral emotions. After a brief introduction to the research questions in Chapter 1, Chapters 2 and 3 address the methodological branch of my research, and Chapters 4 to 6 will analyze several aspects of moral development and moral emotions. In the final Chapter 7, all findings are summarized in view of their application to prevention work in the context of childhood development.

Exploring Nigerian adolescent students perceptions of HIV/AIDS and their attitudes to prevention methods : a psycho of educational perspective

The quantitative study focuses on the perceptions of Nigerian adolescent students to HIV/AIDS and their attitudes to prevention methods. From the literature reviewed it was ascertained that adolescent students do have positive and optimistic perceptions of HIV/AIDS, and also adequate knowledge, but this has not translated into positive attitudes to prevention methods. The literature also indicated that peer group pressure, parental control and the mass media are some variables that probably influence adolescents’ sexual behaviour. This researcher developed the ‘Adolescent Students’ Perception of HIV/AIDS and Attitude to Prevention Methods Questionnaire’ to solicit information from the adolescent students. The results were coded and duly analysed. The results from the empirical study indicated that Nigerian adolescent students have positive perceptions regarding HIV/AIDS, as well as positive attitudes to prevention methods.
