Development of an implementation tool for a breast milk bank in the North West Province / Maria Alettha Pretorius

Pretorius, Maria Alettha January 2015 (has links)
Background: Breast milk banks (BMBs) provide human donor breast milk to preterm infants when mothers are not able to provide breast milk themselves (Arslanoglu et al., 2010:20). Breastfeeding is the single most effective intervention to saving the lives of millions of children in developing countries (Bhutta & Labbok, 2011:378-380). BMBs form an integral part of the millennium development goals (MDGs) (Dempsey & Miletin, 2010:2) on reducing infant mortality and morbidity and are being implemented all over the world (Hartmann et al., 2007:667, Arslanoglu et al., 2010:20, Eidelman & Schanler 2012:827). Problem statement: BMBs can be established more effectively with an implementation tool in place. Currently, there is no tool available to guide the implementation of a BMB in South Africa; the only guidelines that could be found are those describing the operation of BMBs in other countries. Some of the BMBs already established in South Africa were implemented with the help of the South African Breast Milk Bank Reserve (SABR), but without a formalised guideline to provide implementation guidance. Aims and objectives: The purpose of this study was to adapt a current Kangaroo mother care (KMC) progress-monitoring tool and to adjust it according to the BMB setting in South Africa in order to provide a final suggested implementation tool for the implementation of BMBs. Method: An explorative, descriptive design with multiple phases was used. Different data sets were audited; including patient files, written reports, working files, research articles and policies. Observations were also made with regards to available equipment and designated BMB space. During phase one, a critical analysis was performed on research articles and websites regarding BMBs. The qualitative data was analysed by using content analysis. This information was adapted and contextualised in phase two. This phase entailed applying the adapted tool to the BMB setting of the selected regional hospital, which then led to phase three, in which the audited data was incorporated into the final suggested implementation tool resulting from this study. Results: The suggested tool, the North West Province BMB implementation tool, shared the six main constructs with the KMC progress-monitoring tool. The constructs are creating awareness, adopting the concept, taking ownership, evidence of practice, evidence of routine and integration and sustainable practice. However, some of the progress markers and instrument items were specifically relevant to the KMC setting and others to the BMB setting. On the other hand, some constructs overlapped, being relevant to both settings: for instance awareness by management, conscious decision to implement, mobilisation of resources (human, space and equipment), information about mothers’ other resources such as budget, patient records, staff orientation, evidence of discharge, policies and other written documents, and 1-2 year audit evidence and staff development. Conclusion: Adapting the KMC progress-monitoring tool led to the development of a suitable tool to guide BMB implementation. The tool is called the North West Province BMB implementation tool. This tool could be used to guide the implementation of a BMB in other hospitals in South Africa. / M (Dietetics), North-West University, Potchefstroom Campus, 2015

Ochrana klimatického systému Země z pohledu práva / Protection of the Earth climate system from the legal point of view

Babka, Tomáš January 2010 (has links)
Climate system protection from the legal point of view Climate change seems to be a defining ecological issue of the 21st century. However, unlike other global threats there are still some scientific uncertainties about the gravity of this problem and its actual consequences. On the other hand the proven fact is that the Earth climate is affected by altered atmospheric concentrations of greenhouse gases. Dozens of recent studies clearly indicate that the changes in the atmosphere are a result of human activities and that an immediate action is needed to reduce greenhouse gas emissions and avoid reaching more harmful levels. If nothing is happened, climate change would lead to a rise in global average temperature together with other associated impacts such as melting glaciers, rising sea-levels or more frequent appearence of extreme weather events. The climate change issue is being addressed both at international and national level. There is a need to take a coordinated action of all states worldwide with respect to their development capabilities and historic responsibilities. International negotiations in last couple of years have shown that it will be very difficult to achieve an agreement between countries with different intrests. The aim of this diploma thesis is to summarize a development of the...

A protocol for the empowerment of non–offending parents to report child sexual abuse / Rust N.

Rust, Nolene January 2011 (has links)
The statistics of child sexual abuse is alarmingly high. Even though this is the case, further statistics prove that a great percentage of parents do not ever report the sexual abuse of their child. This study focuses on describing and exploring nonoffending parents' experiences regarding suspicions of or disclosure of their child’s sexual abuse to ultimately formulate a proposed protocol for the empowerment of non–offending parents to report the abuse. This proposed protocol can be used by health professionals to empower parents to report and will contribute to the intervention strategies in forensic investigations. As research methodology, the qualitative approach was utilized to investigate nonoffending parents' experiences regarding suspicions of or disclosure of their child’s sexual abuse and to explore and describe a protocol of guidelines to empower nonoffending parents to report child sexual abuse. The case study method was used as a research strategy to accomplish these aims. Ten interviews and a focus group were conducted during which an interview schedule focussing on the parents' reactions, perceived support, and experiences of reporting was employed. The schedule was presented based on the following themes: Theme 1: Finding out about the sexual abuse Theme 2: Reactions of the parents Theme 3: Support Theme 4: Reporting the child's sexual abuse Theme 5: Parents' experiences of the reporting process Theme 6: Suggestions to make the reporting process easier Subthemes, as elicited by these themes, were explored and described in order to contribute towards the formulation of an appropriate protocol that would empower parents to report child sexual abuse. It became clear that parents experience a vast array of emotions upon finding out or having suspicions of their child's sexual abuse. The parents who participated in this study indicated the need for support on different levels– it was proposed that more support might subsequently increase reporting rates. The parents involved in this study were in agreement that the reporting process is extremely taxing and, based on this, various aspects that could help make the reporting of child sexual abuse easier for parents were explored. Further recommendations to empower parents to report child sexual abuse were also made. The data from the interviews was analyzed and a protocol to empower non–offending parents to report child sexual abuse was formulated. / Thesis (M.A. (MW))--North-West University, Potchefstroom Campus, 2012.

Ochrana klimatického systému Země z pohledul práva / Protection of the Earth climate system from the legal point of view

Radošinský, Adrián January 2011 (has links)
Climate system protection from the legal point of view The global warming is one of the most serious and acute problem, that is touching all the global society. Growing of the greenhouse gasses is evoking the change of climate system, especially the warming of the Atmosphere and the Earth. In the last 100 years the temperature rises, what cause a lot of problems not just on the environment, but impacts on human health and society. Many of recent scientist studies clearly indicate, that climate changes are a result of human activities. Consequences of the global warming have been observed for several years, included rising global average temperature, rising of the sea level, melting glaciers and severity of extremes such as heat waves, cold waves, storms, floods and droughts. To preclude this consequences, there is necessary a global action with participation of every country. The aim of the diploma thesis is an introduction to the reader create a picture of the functioning of the climate system and its changes, causes and consequences of these changes and clarify the activities to protect, whether it is the activities of international, multinational or the Czech Republic.
