Difficulties in the choreography of training clinical psychology

The hypothesis derived from four case studies proposes that if at a philosophical level the training is choreographed at confusing levels of philosophical punctuations, and the training programme involves an ongoing commentary on the 'self' of the trainee, and this ongoing commentary interrupts or interferes with the process in which the 'self' comes to be defined, then on an experiential level the training context is unstable for experiential exploration, and the trainee experiences psychological discomfort. A further five case studies are investigated using focused interviews and content analysis to verify the hypothesis. The author-text-reader metaphor serves to describe the trainer-training-trainee relationship. This reveals that the trainee experiences psychological discomfort, often perceived as psychological damage, when the training text is incoherent and therefore unreadable. Psychologists and race : exploring the identities of South African trainee clinical psychologists with reference to working in multiracial contexts

Thesis (DPhil (Psychology))—University of Stellenbosch, 2008. / The question of how to address diversity in the professional training of clinical psychologists is of concern in South Africa and elsewhere. This concern is particularly salient in contemporary South Africa, where much of the sociopolitical discourse centres on issues of race, transformation, relevance and redress. This research is in line with current debates, and set out to explore the self articulated racial identities as well as the impact of those identities on the work of trainees in the second year of their clinical psychology masters degree, at three universities in the Western Cape province of South Africa. Trainees’ perceptions of the role of the universities in facilitating the working through of challenges identified in trainees’ racial identities and in facilitating multiracial competencies, were also explored in this study. Individual interviews were conducted with nineteen trainee psychologists. A minimally structured questionnaire was used in this qualitative inquiry. The theoretical framework underpinning the methodology used in this thesis endeavour, with a critical lens as background, is primarily supported by the “interpretive” or “hermeneutic” approach to psychological theory. Critical theory offered further support to understanding some of the complex issues in working with racialised discourses. Whilst all trainees identified themselves in racial terms, race continues to be a complex and, for many, a painful construct. For many, the family has been the primary source of racial socialisation, largely premised on essentialist, stereotypical discourse. With regard to the impact of their racial identity on their work, many indicate that their race significantly impacted on this. They reported a particular concern with working in cross-racial dyads. Racial difference was sometimes reported to enhance the clinical process, but was far more often experienced as a difficulty. The trainees were unanimous that the universities at which they had studied had fallen far short of what they would have wished in terms of facilitating multiracial competencies. The findings suggest that whilst legislation has changed the political profile of South Africa, the process of transformation within the psychological sites studied, is cause for concern. The dissatisfaction with the training provided, for many trainees centres around issues of relevance to the South African context. Despite efforts by some universities to diversify the racial profile of trainees, in the attempt to address the needs of people of colour, trainees believe efforts to be insufficient. While this study did not collect data, that could corroborate or question the opinions of trainees, results clearly suggest that trainee psychologists do not believe universities are currently doing enough. The implications of the trainees’ views are discussed and implications considered for trainees, trainers, the users of psychological services, and for the role of psychology as a discipline in civil society.

How to survive and flourish : a case study and research informed-model of self-care and stress in trainee counselling psychologists

Aims: This study aims to use trainee counselling psychologists' conceptualisations and experiences to create a research-informed model of self-care and stress. In order to do this, the study integrated the basic tenets of humanistic psychology, theory relating to human potentiality and motivation, pluralistic practice and the relevant research literature around self-care and stress in trainee counsellors, psychotherapists and psychologists. Methods: The study was qualitative in nature and utilised a theory-building case study design. The research participants where 12 trainee counselling psychologists enrolled on a Professional Doctorate in Counselling Psychology in the United Kingdom. A research-informed model of self-care and stress was developed from the relevant research and theory literature. The findings from the 12 semi-structured interviews with the trainees were applied to this model in order to contextualise and refine it. Findings: The data from the interviews was analysed using a thematic analysis and the following categories and themes were discovered: category one - conceptualisations of self-care with caring for self, caring for others and self-actualising as its lower order themes; category two - self-care strategies, with keeping work-life in balance, caring for my physical well-being, getting support from other people and realising there is more to life as its lower order themes; category three -conceptualisations of stress with theoretical understandings of stress, physical impact of stress and psychological impact of stress as its lower order themes; the final category - sources of stress with demands and pressures, financial strains, unhealthy relationships and personal and professional development as its lower order themes. Discussion: The revised research-informed model suggests a number of goals, tasks and methods of self-care and a number of practical examples for each of these areas. According to the model, the goals of self-care involve nurturing trainees' potentiality to become fully functioning trainee counselling psychologists through learning to care for self, others and self-actualising. The tasks of self-care are promoting trainees' wellbeing through meeting their training needs (social support, academic, developmental and placement). Finally, the model suggests the methods of self-care should encourage intentional individual and organisational engagement in strategies which enable trainees to meet their training needs. The study recommends the model is applicable to counselling psychology regulatory bodies and training programmes, as well as individual trainee counselling psychologists. It also proposes further development of the model through research and testing.

Curiosity, exploration, and strategies for dealing with uncertainty amongst psychologists-in-training

By adopting a positive psychology framework, the aim of this study was to explore and describe the level of curiosity and exploration amongst psychologists-in-training, and how they dealt with uncertainty in the context of their professional development. A mixed-method exploratory-descriptive research design was employed to collect the quantitative data by means of the Curiosity and Exploratory Inventory. The qualitative data were collected using semistructured interviews to explore how psychologists-in-training have dealt with uncertainty. Purposive-availability sampling was used to select the participants at three South African universities. A total number of 50 participants completed the CEI and six participants were interviewed. The data were analysed using mixed-method data analysis. It was found that participants had moderate-to-high levels of curiosity and exploration, with higher levels of exploration than absorption. The majority of participants reported that they actively sought as much information as they could and frequently looked for new opportunities to grow as persons. The strategies used by the selected group of participants to deal with uncertainty included: reliance on clinical supervision; consultation with peers; self-enhancement; reliance on theory; learning from practical experience; using certain cognitive appraisals; and self-care. Certain conclusions and recommendations were made based on the findings of the study.

Changing the assumptions of a training therapist : an auto-ethnographic study

This auto-ethnographic study (i.e. an autobiographical genre of writing and research, written in the first-person voice, where the workings of self are expressed both cognitively and emotionally) qualitatively explores the changing assumptions of a training therapist. It shows how various therapies were negotiated during the training period, and explores how meaning was constructed according to basic, underlying epistemological assumptions. Significant experiences and therapies are presented, showing how the therapist's most basic, linear assumptions, were directly challenged by eco-systemic training. The study produces an in-depth, thick description of both the emotional and the cognitive journey of a training therapist, and traces the therapist's movement away from the stability and certainty of a linear epistemological 'way of knowing' to the instability and uncertainty characteristic of an eco-systemic 'way of knowing'. Increasing violence in the work environment created a need for an effective intervention to debrief traumatised workers. The aim of this study is to compile a profile of an effective trauma debriefer that can be used as a criterion for the selection of trauma debriefers. The task description of the trauma debriefer is based on the literature studied. The profile of the trauma debriefer was compiled using the characteristics of a self-actualised person, as described by the Humanism, as well as the skills based on Ivey's Microskills Hierarchy Model. Structured, non-directive interviews were conducted with five experts on the training of trauma debriefers. A qualitative analysis was made of the interviews, based on the personality characteristics and skills identified in the literature. A profile of an effective trauma debriefer that can be used as criterion during the selection of trauma debriefers, was compiled. The training crucible : experiences of a systemic therapist in the making

Family Therapy training programmes have recently come to appreciate the importance of addressing the personal growth of the trainee-therapist, in addition to the traditional focus on skill development. Suggestions in the available literature on how this "person-of-the-therapist" issue could best be addressed, represent almost exclusively the ideas of authorities (authors, clinicians and trainers) in the field of systemic therapy. Constructivist thought endorsed by the UNISA training programme, encourages and values different viewpoints. According to this view, students and faculty co-construct the training process. The aim of this study is therefore to present the voice of the trainee. Several training contexts, the essential qualities of the different supervisory relationships and difficulties encountered, are explored from the trainee's perspective. School psychologists and attention-deficit/hyperactivity disorder : a survey of training, knowledge, practice, and attitude

The purpose of this study was to determine school psychologists' current level of knowledge about Attention-deficit/hyperactivity disorder (ADHD), their training and comfort level with that training, their roles and practices in assessing, diagnosing, and treating children and adolescents with ADHD, and their attitudes toward the disorder and those with it. An original survey (see Appendix B) developed for this purpose was distributed to 700 systematically selected members of the National Association of School Psychology (NASP) who were currently practicing in the public schools. A total of 406 usable surveys were returned for a final response rate of 58%.Overall school psychologists are confident in their abilities to deliver services (consultation, assessment, and intervention) to students with ADHD. Respondents underestimated the rate of comorbidity of ADHD with other psychiatric disorders but overestimated the percentage of students with ADHD who also have a learning disability. Respondents most often mentioned diagnosis or identification of the disorder as their goal for assessment of possible ADHD. However, in their rankings of specific goals, respondents ranked the development of appropriate interventions as more important than the diagnosis of the disorder. The most often used standardized tests for ADHD referrals are questionnaires (for parents, teachers, and students) and drawings. Most (57%) school psychologists agreed that they do not determine the diagnosis of ADHD, but rather refer to a medical doctor. A Specific Learning Disability is the most often used special education category for students with ADHD who qualify, followed somewhat closely by Other Health Impaired, then by Emotionally Handicapped. Only 14 percent of the school psychologists surveyed stated that every student with ADHD who did not qualify for special education was considered for accommodations under Section 504. More respondents (83%) agreed that teachers often press to have their problem students diagnosed with ADHD than agreed that parents often press (55%). The majority of respondents (74%) agreed that ADHD is overdiagnosed. However, there was disagreement as to whether stimulant medications were used more often than necessary to treat ADHD. Implications of these findings for school psychologists and for future research are discussed. / Department of Educational Psychology
