A dissertation submitted in partial compliance with the requirements for a Master's Degree in Technology: Chiropractic, Durban Institute of Technology, 2005. / Low back pain has been shown to be associated with inhibition of the lower limb musculature. This inhibition is called arthrogenic muscle inhibition (AMI). Sacroiliac joint dysfunction has been linked with AMI of the ipsilateral and contralateral quadriceps and hamstring muscles. Sacroiliac manipulation has been shown to significantly reduce ipsilateral AMI, however no studies have been conducted to illustrate the effect of sacroiliac manipulation on contralateral AMI. Neither have their been studies to show the presence or extent of spinal dysfunction between the levels of L2-L5 and its significance on muscle inhibition in the quadriceps and hamstring muscles, nor the effect of manipulation of these levels on AMI of the quadriceps and hamstring muscles. The purpose of this study was therefore to determine whether spinal manipulation has an effect on AMI of the contralateral limb as well as that of the ipsilateral limb. / M

A resistência elástica é aplicada amplamente em programas de fortalecimento muscular nos processos de reabilitação física. No entanto, as conseqüências biomecânicas para o músculo exercitado com esta forma de sobrecarga são pouco estudadas. A ausência de fundamentação científica no que tange a aplicação da resistência elástica fez com que surgissem algumas suposições a respeito de sua caracterização biomecânica. Uma delas sugere que o exercício com resistência elástica pode ser caracterizado como sendo isocinético. Neste contexto, o objetivo global deste trabalho foi o de caracterizar o comportamento biomecânico do músculo quadríceps femoral frente ao exercício com resistência elástica. Tal caracterização foi realizada através da utilização de um modelo biomecânico bidimensional da articulação do joelho. O seu comportamento mioelétrico também foi avaliado. Conclui-se de uma forma geral, que o desenvolvimento deste estudo permitiu um entendimento a respeito da interação biomecânica entre o músculo e a resistência elástica durante o exercício. Permitindo dessa forma elucidar algumas questões ligadas a aplicação clínica desta modalidade de exercício / The elastic resistance is applied in programs of muscular fortification in processes of physical rehabilitation. However, the biomechanicals consequences for the muscle exercised with this resistance form aren’t very studied. The low scientific fundamentation for the application of the elastic resistance, results in some suppositions about its biomechanic characterization. One of them suggests that the exercise with elastic resistance can be characterized as isokinetic. In this context, the global objective of this work was characterizing the biomechanic behavior of the quadriceps muscle front to the exercise with elastic resistance. Such characterization was accomplished through the use of a bidimensional biomechanic model of the knee joint. The electromiographic behavior was also evaluated. In a general conclusion, the development of this study allowed an understanding of the biomechanic interaction between the muscle and the elastic resistance during the exercise. Allowing to elucidate some subjects of the clinical application of this exercise modality

The effect of sacroiliac joint adjustment in conjunction with myofascial dry needling of the rectus femoris muscle and myofascial dry needling on its own on quadriceps femoris muscle strength

M.Tech. / This study was conducted to determine the effect of Sacroiliac joint adjustment to the restricted Sacroiliac joint in combination with myofascial dry needling to the active/latent Rectus Femoris muscle trigger point and Rectus Femoris myofascial dry needling on its own on Quadriceps Femoris muscle strength. Thirty participants between the ages of 18 and 40 years were recruited through the use of advertisements placed in and around the University of Johannesburg’s Chiropractic Day Clinic. Participants were assessed for exclusion criteria by performing a Full Case History, Pertinent Physical Examination, Lumbar Spine and Pelvis Regional Examination and S.O.A.P note. The participants who conformed to the specific inclusion criteria were accepted for this study. These participants were randomly placed into two groups of sixteen participants each. Group One receive Sacroiliac joint adjustment to the restricted Sacroiliac joint and ipsilateral myofascial dry needling of the Rectus Femoris muscle active/latent myofascial trigger points. Group two received myofascial dry needling of the Rectus Femoris muscle active/latent myofascial trigger point on the same side as the Sacroiliac joint restriction. Each participant received five treatments over a three week period, i.e. two treatments for the first two weeks and one follow-up treatment in the third week. The subjective data of the patients lower back pain was assessed using the Numerical Pain Rating Scale. The objective data was obtained from the Isometric Dynamometer measuring the Quadriceps Femoris muscle strength before and after each treatment. In addition the Algometer was used for measuring the pressure pain threshold of the Rectus Femoris trigger points. An analysis was performed using Repeated Measures Analysis, t-Test, Frequencis, Descriptives and Friedman’s Tests. The results of this study demonstrate that Sacroiliac joint adjustment to a restricted Sacroiliac joint with ipsilateral myofascial dry needling of the Rectus Femoris muscle showed a statistically significant, but temporary increase in Quadriceps Femoris muscle strength. Myofascial dry needling of the Rectus Femoris muscle on its own was found to be statistically insignificant for the increase in Quadriceps Femoris muscle strength.

Avaliação do sinal eletromiográfico como parâmetro para determinação do limiar de fadiga muscular / Evaluation of the eletromyographic signal as paameter for determination of muscle fatigue threshold

Embora a analise no domínio da freqüência do sinal Eletromiográfico (EMG) seja empregada na caracterização do processo de fadiga muscular localizada sua aplicação, especificamente a da Freqüência Mediana (Fmed), é pouco explorada para a determinação do Limiar de Fadiga Eletromiográfico (LFE). Neste estudo foram realizadas análises baseadas em procedimentos experimentais executados em três diferentes modalidades de exercício: i) isométrico; ii) isotônico com peso fixo; iii) isotônico no cicloergômetro; onde foi monitorado o comportamento da Fmed do sinal EMG em três porções do músculo quadríceps femoral: vasto lateral, vasto medial e reto femoral. Os resultados demonstraram ser possível determinar o LFE através da monitoração da Fmed, em qualquer modalidade de exercício para todas as três porções musculares avaliadas. Entretanto, para o exercício isométrico e isotônico com peso fixo a qualidade dos ajustes, utilizados para o calculo do LFE, apresentou melhores índices estatísticos em comparação com os resultados obtidos no cicloergômetro. Pode ser considerado um reflexo deste comportamento a menor variação nos valores do LFE observada nas duas primeiras modalidades de exercício. Por conseqüência, nestas modalidades, uma melhor caracterização do LFE, relacionada sua definição teórica, foi observada. A análise complementar das bandas de freqüência isoladas demonstrou a possibilidade de melhoramentos relacionados ao processo metodológico de tratamento do sinal EMG para determinação do LFE. Entende-se que a determinação do LFE é uma temática controversa, porém ao mesmo tempo apresenta um grande potencial de exploração científica caracterizando, neste contexto, a contribuição deste estudo para a área / Tthough the analysis in the frequency domain of the Electromyographic Signal (EMG) was used in the characterization of the localized muscular fatigue process their application, specifically the Median Frequency (MF), is rarely explored for the determination of Electromyographic Fatigue Threshold (EMGFT). MATERIALS AND METHODS: This is a controlled clinical trial, randomized and double blind. Sixty healthy volunteers (23.6 ? 4.2anos; 54.2 ? 7.7kg, 1.62 ? 0.009 cm) of both sexes were divided randomly into three groups: control group (CG), experimental group 1 (SG1) with application of the current Russian 30 HZ and experimental group 2 (EG2) at 70 Hz The volunteers performed an initial assessment (AV1) on the isokinetic dynamometer with three repetitions maximum voluntary isometric (MVC) for knee extension concomitant uptake of EMG for the VM muscle, VL and RF. Later, after application of NMES, they underwent an experimental protocol of isometric fatigue using 70% of MVIC, ending with the completion of a final assessment (AV2) in the same manner as the AV1. RESULTS: By analyzing the profile of the 60 subjects in three broad, VM showed a higher value of RMS behavior when the VL and RF (p = 0.03 and p = 0.02). With respect to Fmed the RF muscle (p = 0.001) showed a higher value for the VM. The VM muscle showed significant increases of Fmed (p = 0.05) after electrical stimulation at 70 Hz when compared the AV1 AV2 and RF showed significant decreases (p = 0.009) after stimulation at 30 Hz during the fatigue showed an increase RMS in the VM and VL, with a reduction in RF. For the variable Fmed was observed in three broad decline during fatigue. CONCLUSION: Our findings provide evidence that the muscles VM, VL and RF fiber typing are different besides indicating that the frequency of NMES tend to relate to the muscle stimulated. Finally suggests the surface EMG as a noninvasive method for characterizing muscle / OBJETIVO: Analisar os efeitos da eletroestimula??o sobre os par?metros eletromiogr?ficos e dinamom?tricos do m?sculo quadr?ceps. A an?lise dos dados se deu atrav?s da utiliza??o do programa estat?stico SPSS for Windows (Statistical Packege For the Social Sciense) 17.0 e foram utilizados Anova one way nas compara??es entre os grupos e teste t pareado nas compara??es dentro dos grupos. RESULTADOS: Ao analisar o perfil dos 3 vastos nos 60 sujeitos, o VM apresentou um maior valor de RMS quando comportamento ao VL e RF (p=0,03 e p=0,02). Com rela??o a Fmed, o m?sculo RF (p=0,001) apresentou maior valor comparado ao VM. O m?sculo VM apresentou aumentos significativos da Fmed (p=0,05) ap?s eletroestimula??o a 70 Hz quando a AV1 foi comparada a AV2 e o RF apresentou quedas significativas (p=0,009) ap?s estimula??o a 30 Hz. Durante a fadiga observou-se aumento do RMS no VM e VL, com redu??o no RF. Para a vari?vel Fmed observou-se queda nos tr?s vastos durante a fadiga. CONCLUS?O: Os achados deste estudo sugerem que os m?sculos VM, VL e RF possuem tipagens de fibras diferentes al?m de indicar que as frequ?ncias de EENM tendem a se relacionar com o m?sculo estimulado.
