Comparison in macronutrient calculations in parenteral nutrition : do discrepancies exist between medical residents and registered dietitians? / Title on signature form: Comparisons in macronutrient calculations in parenteral nutrition

This research examined differences in parenteral nutrition (PN) macronutrient (non-protein calories (NPC), grams of protein (PRO), carbohydrate to fat ratios) calculations between medical residents (MRs) and registered dietitians (RDs). This was conducted through a chart review of orders and notes from a midwest teaching hospital. The results showed that MRs prescribed on average 300 NPC more than RDs (n=102, SD ± 651.136, t = 4.45, df = 93, p = < 0.001). Additionally, despite recommendations from the American Society of Parenteral and Enteral Nutrition, MRs prescribe more macronutrients for obese patients and less macronutrients for normal and underweight patients when compared to RDs (PRO: F (3, 95) = 3.39, p = 0.021; NPC: F (3, 95) = 6.22, p = 0.001). This research stands to add to the body of knowledge in support of RDs obtaining greater PN privileges in the clinical setting. / Department of Family and Consumer Sciences

Local perception towards tourism in Thailand : a case study of Chiang Mai

This dissertation is aimed at analyzing and discussing the perception of the local people of Chiang Mai towards tourism. Tourism in Thailand and its importance to the country is illustrated in the first chapter. The current position of Tourism in Thailand is indicated in the second chapter with tourism statistics and major trends shown between 1985-1995. This chapter also reviewed the importance of tourism within the Thai economy. The third chapter elaborated some of the major issues in Thai tourism with special attention to economic, environmental and sociocultural issues, as well as the related problems. In chapter 5 anticipated advantages and observed disadvantages in local community involvement in tourism in Chiang Mai are elaborated. Chapter 5 explains the research methodology followed by study results in chapter 6. Chapter 7 concluded the study's hypothesis as well as the objectives of this research. The implications from field - work and the policy issues are also noted. The conclusions from the study investigating the inner city of Chiang Mai residents' perceptions toward tourism development in the areas of arts and culture ; environmental circumstances ; employment ; infrastructure development and social problems revealed that on one side, there was a high degree of agreement among the respondents with regards to economic gain and employment created by tourism. Infrastructure development was also viewed positively by the respondents. On the other side, tourism was perceived as the cause of the environmental degradation and social problems. It is hoped that this study will serve as a source of reference and a guideline to be used by both private and public sectors in considering perception of local Chiang Mai people to tourism. The author has drawn articles from a variety of sources, including tourism journals, local newspapers and magazines. Field-work in the form of the samples were systematically chosen from the inner city of Chiang Mai to guarantee the outcome from the specified area of study.

A survey to Evaluate the Need of a Pharmacy Resident Teaching Certificate Program for Pharmacy Residents in Arizona

Class of 2009 Abstract / OBJECTIVES: The purpose of this study was to determine if there is a perceived need to offer a teaching certificate program to pharmacy residents in the state of Arizona. METHODS: This cross-sectional, descriptive study utilized a web-based, electronic survey to gather data from Arizona pharmacy residents and residency program directors. Residents who were part of the Class of 2008 or 2009 were included in the study. The survey asked the respondents to rate the value of a pharmacy certificate program on a 6-point Likert scale. Data on the current activities for residents in the programs, resident confidence levels on performing specific tasks, topics of modules for a certificate program, the quantity and length of the modules, and interest in participating in a program were also collected. RESULTS: There was a 50% response rate for both pharmacy residents (n=17) and residency directors (n=12). The respondents indicated that there was a perceived value to a teaching certificate program (p<0.0001) and 28 stated that they would participate in the certificate program if one was offered (p<0.0001). Seven of the nine proposed module topics for the program were positively received by the respondents, with “How to be a Preceptor” ranked highest (p<0.0001). Overall, a teaching certificate program of 4 to 6 modules (p=0.006) between 46 to 60 minutes in length (p<0.0001) was most appealing to the respondents. CONCLUSIONS: Arizona pharmacy residents and residency program directors who participated in this survey showed a positive interest in a teaching certificate program.

Articulating and ameliorating elder abuse in Australia

The abuse of older people is a largely unrecognised and under acknowledged social problem in Australia. My major objective in undertaking the work, which is represented by the original published articles that comprise the thesis, was to make a scholarly and practical contribution toward the minimisation of 'elder abuse. This objective was achieved with the development and implementation of a series of studies that articulated and ameliorated elder abuse in Australia.The thesis provides an erudite synthesis of these studies, which fall into four themes that illustrate the nature and scope of my theoretical and professional work in elder abuse. Much of the work was guided by a conceptual framework of ways of knowing in nursing, and was underpinned by the principles and practice of community development and participatory community-based action processes.The outcomes of these studies include work with three stakeholder groups: professionals who deal with elder abuse, older people who are victims or potential victims of abuse, and those who perpetrate abuse on an older person. The work, illustrated in the four themes, includesthe articulation of elder abuse issues with West Australian aged care workersthe development of elder abuse protocols, policy guidelines and ethical principles, to guide professional practice in abuse prevention and interventionthe design and implementation of participative community programs to empower older people, and their carers, to resist being abused or abusing and to assist perpetrators stop their abusethe amelioration of abuse of nursing home residents by staff.The thesis situates my conceptual and clinical effort within the wider corpus of Australian knowledge and practice on elder abuse and contributes to addressing the social problem of elder abuse within the context of Australian aged care.

The Relationship Among Functional bility Organization Characteristics¡Aand ality Of Care in Nursing Home

The purpose of this research is to study the relationship between both the characteristic of the residents and the nursing home and the changes of the ADLs of the residents. Descriptive study was designed. 35 nursing homes and theirs residents in Kaoshiung City were be the sample and 34 questionnaires of the nursing home were returned. Residents¡¦ questionnaire were interviewed by trained personnel twice (pre and post), on June and December 2001 respectively. 310 residents including 141 male, 169 female, average age 72.94 were interviewed. Structural questionnaire was used as the tool, organization¡¦s questionnaire consists of the type of organization, size, admission rate , resignation rate. Resident¡¦s questionnaire including demographic data, Katz Index of ADLs, health status, pressure sore etc. Data analysis including descriptive analysis, T-test, Anova, Pearson relation analysis and linear regression analysis. The result shown that 86.2% of the residents¡¦ ADLs severely dependent on others help. T-Test shown that male has better ADLs than female, widow has better ADLs than couple ¡]P¡Õ0.05¡^. but shown no significant of changes of ADLs. Age, sickness and ADLs and changes of ADLs shown no significant results. On the other hand, the relationship between the characteristic the nursing home and the changes of the ADLs of the residents shown the government aided and ¤½¥ßor°]¹Îªk¤Hnursing home has better changes of ADLs¡]P¡Õ0.01¡^. The greater the size of the organization the better of the ADLs changes, private sector and affliated nursing home shown no significant results of the ADLs changes. Pressure sore is the important index of the quality care of the long term care organization, research sectors always take it as the routine surveillance. This study consisted of 31 cases of pressure sore (10%),¤½¥ßor°]¹Îªk¤Hnursing home has a low incidence of pressure sore, the higher the bed using rate, the worse of the ADLs changes, the higher of the high incidence of the pressure sore, but there is no difference between the resignation rate and the incidence of the pressure sore. The results of this study may help the clinical care-taker to design different care targets in order to help to recover and to improve the ability of ADLs by the characteristic of the residents. Moreover, study of the characteristic of nursing home and effects of the changes of the ADLs of the residents may act as suggestion to the manager and the administrator who may take the different characteristic of the organization into consideration when making policy to improve or to further investment and expansion of the organization.

Chichin District of Kaohsiung City not only embraces the harbor coastal space of natural water coast resources, but also owns rich human historic sites, delicious seafood cuisine, etc. To most of the local residents or tourists, the district all along has tremendous tourist charm. In order to attract more tourists to visit this district and meet the residents¡¦ eager requests for recreation, in recent years Kaohsiung municipal government has been positively developing diversified tourist facilities and items in this district in expectation for promotion of people¡¦s satisfaction on the overall tourist recreational experience. Therefore, a deep understanding of the residents¡¦ or tourists¡¦ satisfaction, extent of recognition and background difference towards the tourism development of the district is an important subject when taking options in the keenly competitive tourist environment in future. And this investigation can also provide a reference for the relevant departments to plan for their future direction and to review their work for improvement. This research mainly investigates from three aspects: ¡§community conscious-ness,¡¨ ¡§recognition towards the impact of tourism development¡¨ and ¡§participation attitude and environmental residential experience,¡¨ etc. of the Chichin residents of Kaohsiung. In studying the Kaohsiung municipal government¡¦s urging for tourism development of Chichin District, the research meanwhile gives an understanding on the residents¡¦ concentration of community consciousness, their attitude towards the development of tourism and their residential experience, etc. The results of the research can also provide more appropriate strategies for the Kaohsiung municipal government in positioning the future tourism development of Chichin District of Kaohsiung City by using the effective resources. The results of the research show that over the ¡§analysis on residents¡¦ participation attitude and environmental residential experience,¡¨ the residents currently residing in this district mostly gave such reasons for their residence as ¡§residing here generation after generation,¡¨ ¡§friends and relatives mostly living in this district¡¨ and ¡§relationship of work,¡¨ etc. The ratio of these factors is 75.2% of the tested samples. In this way, the researcher can interpret the situation that among these residents, being different from those of other districts, there established tight kinship and friendship. And they are helpful to the concentration of community consciousness. At the same time we can learn from the results of the research that there is an obvious correlation between ¡§sex,¡¨ ¡§residing alley¡¨ in the attributes of the residents¡¦ basic background, and ¡§high or low group of social consciousness,¡¨ and ¡§high or low group of recognition of tourism development impact¡¨ respectively. After analysis on the results of this research, it is suggested that the actions that the relevant administrative departments should take are: 1.¡§Reserve of resources,¡¨ including the confirmation for establishment of a system for tourism attraction in Chichin District of Kaohsiung City, a unification of the things and rights of management so as to avoid excessive development, etc. 2. ¡§Community development,¡¨ an effective use of the properties of the generally high community consciousness among the residents of Chichin, and a maintenance of high extent of concentration through the assistance by the third department. Finally, this research also mentions that if the angle is changed by taking the experience and recognition of residents as a reference for planning the development of the district, then perhaps the optimal effects can be acquired from development.

The spirit issue for the inhabitants of the nursing home

This research is mainly aimed at the spirit issue for the residents of the nursing home to investigate the correlation of satisfaction between the physical condition, psychologic condition and spirit condition. The subjects are the residents of three nursing homes in Kaohsiung city. The measure of structural questionnaire is selected and adopted by this research. Two hundred and seventy questionnaires are issued, and two hundred and fifty-two questionnaires are valid. The valid questionnaires are analyzed by descriptive statistical measurement, one-way ANOVE, Independent-Samples T test, and examined the differences with the control by Bonferroni. The results of this research are listed as follows: 1. The correlation of satisfaction in regard to the residents¡¦ basic conditions: The older residents are more satisfied with the nursing home. The residents¡¦ satisfaction for the nursing home is less and less when they stay there longer and longer. Single residents are more content than married, divorced, widowed, and widowered residents. There is no difference in the correlation of satisfaction between the residents with different religions and education levels. The residents with independent economic ability are more contented with the nursing home. 2. The correlation of satisfaction in regard to the residents¡¦ physical conditions: There is difference in the self-image between the residents with normal hearing and without hearing. The residents who can communicate with others by oral language have better relationship and they are more content. The residents with normal sightare more satisfied. The residents who have to rely on others in their daily lives are most unsatified. 3. The correlation of satisfaction in regard to the residents¡¦ psychologic conditions: There is no difference in the correlation of satisfaction between the residents with different sentiments. The residents with good behavior are more satisfied. The residents who have good relationships with others and join the activities actively are more content. According to the results of this research, we would like to make some suggestions.The government institutions, educationists, nursing home¡¦s superintendents and clinical staffs should provide the fine and practical services, including of physical, psychology and spirit care. These fine services should reach the target of practical nursing and moreover, spirit care should be the responsibility of all nursing staffs.
