Besides clinical assessment the diagnosis is based on the determination of the rheumatoid factor being this one also positive in healthy people as well as in other infectious and auto-immune diseases. Anticyclic citrullinated peptide (anti-CCP) antibodies have been used in diagnosis of rheumaoid arthritis (RA) and seen to be superior to rheumatoid factor (RF) in early onset RA diagnosis. The target of the study is to demonstrate the presence of anticitrulline antibodies in patients with rheumatoid arthritis diagnostic comparing with laboratory, clinical parameters and with the assessment of the quality of life of these patients. The “Health Assessment Questionnaire” has been used to assess the quality of life. We performed an immunoturbidimetry test for detection of RF (Roche,Indianopolis, USA) and an ELISA for anti-CCP antibodies (Inova, San Diego, USA) in 69 patients presenting, at least, 4 of the American College of Rheumatology criteria for classification of RA and in 20 healthy controls. For statistical analysis we used thr Fisher exact test and the Spearmann test Significance was reached wuth P<0.05. RA patients were aged between 18-75 years (mean = 43.9 years), 66 (95.7%) of then were female, controls age ranged between 20-60 years.The period of RA onset varied from 4 to 384 months (mean = 74.0 and median = 48.0). RF was positive in 48 (69.6%) patients and in 1 (0,5%) control. The anti-CCP was positive in 36 (52.2%) patients and in 2 (10%) controls. A significant correlation of RF and anti-CCP tests was observed with P< 0,0001 and this study suggests that anti-CCP was not superior to RF in diagnosis of established RA. / A artrite reumatóide é uma doença auto-imune crônica e inflamatória que evolui com graus de destruição articular e alterações extra-articulares podendo levar a incapacidade funcional. Além da avaliação clínica o diagnóstico baseia-se na determinação do fator reumatóide (FR) sendo que este é também positivo em indivíduos saudáveis como também em outras doenças auto-imunes e infecciosas. Os anticorpos antipeptídeos citrulinados cíclicos (anti-CCP) tem sido usados no diagnóstico da artrite reumatóide sendo superiores ao fator reumatóide (FR) no diagnóstico da artrite reumatóide (AR) recente. O estudo teve como objetivo demonstrar a presença de anticorpos anticitrulina em pacientes com o diagnóstico de artrite reumatóide comparando com parâmetros clínicos, laboratoriais e com a avaliação da qualidade de vida desses pacientes. Para avaliar a qualidade de vida foi aplicado o questionário “Health Assessment Questionnaire.” Foi realizado um teste por imunoturbidimetria, para a detecção de FR (Roche, Indianopolis, EUA) e ELISA para o anti-CCP (Inova , San Diego, EUA) em 69 pacientes apresentando, ao menos, 4 dos critérios do Colégio Americano de Reumatologia para a classificação de AR e em 20 controles saudáveis. A análise estatística utilizou o teste exato de Fisher e teste de Spearmann com significância alcançada com P<0.05. Os pacientes com AR tinham entre 18-75 (média = 43.9 anos), 66 (95.7%) eram mulheres, os controles com idades variando entre 20-60 anos. O início da AR variou de 4 a 384 meses (média = 74.0 e mediana = 48.0). FR foi positivo em 48 (69.6%) pacientes e 1 (0,5%) controle. O anti-CCP foi positivo em 36 (52.2%) pacientes e em 2 (10%) controles. Foi observada uma correlação significante dos testes FR e anti-CCP com P< 0.0001 e este estudo sugere que o anti-CCP não foi superior ao FR no diagnóstico da AR estabelecida.

Objetivos: Avaliar prospectivamente uma coorte de pacientes com AR inicial (menos de 12 meses de sintomas), a fim de: 1) Caracterizar os aspectos demográficos, clínicos, sorológicos e radiográficos, além do padrão de respostas aos questionários de qualidade de vida HAQ e SF-36; 2) Determinar a possível associação entre marcadores sorológicos (fator reumatóide - FR, anticorpos anti-peptídeos citrulinados cíclicos -anti-CCP e anti-vimentina citrulinada -anti-Sa) com parâmetros de desfecho clínico, radiográfico e terapêutico; 3) Avaliar a correlação entre os escores do HAQ e SF-36, os marcadores sorológicos e desfechos da AR. Pacientes e métodos: Foram avaliados 65 pacientes com diagnóstico de AR inicial no momento do diagnóstico, 40 dos quais foram acompanhados por 3 anos, em uso de esquema terapêutico padronizado. Registrados dados demográficos, clínicos, DAS 28, realizadas sorologias (ELISA) para FR (IgM, IgG e IgA), anti-CCP (CCP2, CCP3 e CCP3.1) e anti-Sa, e aplicados os questionários HAQ e SF-36 na avaliação inicial, e aos 3, 6, 12, 18, 24 e 36 meses de acompanhamento. Radiografias de mãos e pés foram solicitadas na avaliação inicial, aos 12, 24 e 36 meses. Como desfechos de evolução da AR, considerou-se a ocorrência de erosões radiográficas, remissão (DAS 28<2,6) e a necessidade ou não de terapia biológica durante o acompanhamento. Comparações feitas através do teste t de Student, análise de regressão de efeitos mistos e análise de variância (nível de significância de 5%). Resultados: A idade média foi de 45 anos e predominou o sexo feminino (90%). A forma de apresentação aguda (76%) e poliarticular (69%) foi a mais frequente e nódulos reumatóides foram detectados em 15% dos pacientes. No momento do diagnóstico, FR foi observado em 50% dos casos (FR IgA – 42%, FR IgG – 30% e FR IgM – 50%), anti-CCP em 50% (não houve diferença entre CCP2, CCP3 e CCP3.1) e anti-Sa em 10%. Após 3 anos, não houve mudança na prevalência de FR e anti-CCP, mas a de anti-Sa aumentou para 17,5% (p=0,001). Erosões radiográficas foram detectadas em 42% e 70% dos pacientes, na avaliação inicial e após 3 anos (p<0,001). A porcentagem de pacientes em remissão, atividade de doença baixa, moderada e intensa, segundo o DAS 28, foi de 0%, 0%, 7,5% e 92,5% (avaliação inicial) e 22,5%, 7,5%, 32,5% e 37,5% (após 3 anos). A média do escore do HAQ inicial foi 1,89, com declínio progressivo até 0,77, no terceiro ano (p<0,0001). A maioria dos domínios do SF-36 apresentou significativa melhora durante os 3 anos de seguimento, com exceção de estado geral e vitalidade. Terapia biológica foi necessária em 22,5% dos pacientes. Nenhum dos autoanticorpos avaliados se associou à ocorrência de remissão. A positividade para FR IgM e anti-CCP (CCP2, 3 e 3.1), na avaliação inicial, foi fator de risco para ocorrência de erosão radiográfica. Os níveis médios de FR IgA e FR IgM e de anti-CCP (CCP2, 3 e 3.1) ao longo dos 3 anos foram maiores entre os pacientes que evoluíram com erosões radiográficas (p<0,05 para todos). Os níveis médios de FR IgA e anti-CCP 2 ao longo dos 3 anos foram mais elevados entre os pacientes que necessitaram de terapia biológica (p<0,05 para ambos). Os escores do HAQ e SF-36 não se associaram aos autoanticorpos ou aos desfechos avaliados. Conclusão: Nessa população de pacientes com AR inicial no momento do diagnóstico, observou-se intensa atividade de doença, elevada prevalência de erosões radiográficas e importante impacto na qualidade de vida. A pesquisa dos sorotipos FR IgA e FR IgG não aumentou a frequência de positividade do FR e, portanto, não contribuiu para o diagnóstico de AR. A pesquisa de anti-CCP pelas técnicas de terceira geração também não agregou informações ao anti-CCP2. Títulos mais elevados de FR e anti-CCP ao longo do tempo se associaram à ocorrência de erosões radiográficas e à necessidade de terapia biológica. A significativa elevação da porcentagem de positividade para anti-Sa durante os 3 anos de acompanhamento sugere que esse anticorpo possa ter implicações na fisiopatogenia da doença. Os questionários de qualidade de vida não foram úteis para predizer a evolução da doença. _______________________________________________________________________________ ABSTRACT / Introduction: The association between serological markers and scores of quality of life questionnaires such as the Health Assessment Questionnaire (HAQ) and Medical Outcomes Study SF-36 Health Survey (SF - 36) with the outcome of the early rheumatoid arthritis (ERA) is controversial. Objectives: To prospectively evaluate a cohort of patients with ERA (less than 12 months of symptoms) in order to: 1) Describe the demographic, clinical, serological and radiographic aspects, and the pattern of responses to quality of life questionnaires HAQ and SF-36, 2) Determine the possible association between serological markers (rheumatoid factor - RF, anti-cyclic citrullinated peptide antibodies anti-CCP, and citrullinated anti-vimentin -anti-Sa) with parameters of clinical, radiographic and therapeutic outcome; 3) Evaluate the correlation between HAQ and SF-36 scores, serological markers and RA outcomes. Patients and methods: Sixty-five patients diagnosed with early RA at the time of diagnosis were evaluated, 40 of them were followed for 3 years, in use of standardized therapeutic treatment. Demographic and clinical data were recorded, DAS 28, as well as serology tests (ELISA) for RF (IgM, IgG and IgA), anti-CCP (CCP2, CCP3 and CCP3.1) and anti-Sa, with the application of HAQ and SF-36 questionnaires in the initial evaluation and at 3, 6, 12, 18, 24 and 36 months of follow-up. Hands and feet radiographs were requested in the initial evaluation, and at 12, 24 and 36 months of follow-up. As outcomes of the RA development, the occurrence of radiographic erosions, remission (DAS 28 <2.6) and the need or not for biological therapy during the follow-up period were considered. Comparisons were made through the Student t test, mixed-effects regression analysis and analysis of variance (significance level of 5%). Results: The mean age was 45 years and a female predominance was observed (90%). The acute form of presentation (76%) and polyarticular (69%) was the most frequent and rheumatoid nodules were detected in 15% of patients. At the time of diagnosis, RF was observed in 50% of cases (RF IgA - 42%, RF IgG - 30% and RF IgM - 50%), anti-CCP in 50% (no difference between CCP2, CCP3 and CCP3. 1) and anti-Sa in 10%. After 3 years, no change in the RF prevalence and anti-CCP was observed, but the anti-Sa increased to 17.5% (p = 0.001). Radiographic erosions were found in 42% and 70% of patients in the initial evaluation and after 3 years (p<0.001). The percentage of patients in remission, low, moderate and intense disease activity, according to the DAS28, was of 0%, 0%, 7.5% and 92.5% (initial evaluation) and 22.5%, 7.5 %, 32.5% and 37.5% (after 3 years). The average initial HAQ score was 1.89, with progressive decline up to 0.77 in the third year (p <0.0001). Most SF-36 domains showed significant improvement over the 3 years of follow-up, except for general state and vitality. Biological therapy was necessary in 22.5% of patients. None of the autoantibodies evaluated were associated with the occurrence of remission. The positivity for RF IgM and anti-CCP (CCP 2, 3 and 3.1) in the initial evaluation was a risk factor for the occurrence of radiographic erosion. The mean RF IgA, RF IgM and anti-CCP (CCP2, 3 and 3.1) levels along the 3 years were higher among patients who progressed with radiographic erosions (p <0.05 for all). The mean RF IgA and anti-CCP 2 levels along the 3 years were higher among patients who needed biological therapy (p <0.05 for both). The HAQ and SF-36 scores were not associated with autoantibodies or with the outcomes evaluated. Conclusion: This population of patients with early RA at the time of diagnosis showed intense disease activity, high prevalence of radiographic erosions and important impact on quality of life. The research on serotypes RF IgA and RF IgG did not increase the positivity frequency of RF and therefore did not contribute for the RA diagnosis. The research for anti-CCP through third- generation techniques did not aggregate information to the anti-CCP2 either. Higher titles of RF and anti-CCP over time were associated with the occurrence of radiographic erosions and the need for biological therapy. The significant increase in the positivity percentage for anti-Sa during the 3 years of follow-up suggests that this antibody may have implications in the pathophysiology of the disease. The quality of life questionnaires were not useful in predicting the evolution of the disease.

Tem como um dos fatores que podem impedir sua boa utilização o baixo nível sócio-cultural de quem o responde, que é o que encontramos na população de pacientes atendidos nos serviços públicos de saúde no Brasil, o que poderia comprometer sua utilização em nossa realidade. Objetivo: Correlacionar os escores encontrados no HAQ quando este for respondido pelos pacientes com os escores encontrados quando o mesmo é respondido pela equipe assistente. Material e Métodos: Foram estudados 90 pacientes portadores de AR, que preenchiam os critérios para classificação da doença do American College fo Rheumatology (ACR). Primeiramente os pacientes respondiam o HAQ e, após um intervalo mínimo de tempo, o HAQ era respondido pela equipe. Os escores foram, então, analisados utilizando-se o método de correlação de Pearson. Resultados: A população estudada tinha idade média de 48,2 ± 12,8 anos, com média de tempo de educação formal de 7 ± 3,2 anos, e média de renda per capta de 0,9 ± 1,5 salários-mínimos. Os escores de HAQ encontrados nos questionários respondidos pelos pacientes tiveram média de 1,156 ± 0,835. Nos escores encontrados nos questionários aplicados pelos entrevistadores a média foi de 1,293, com desvio-padrão de ± 0,856. Houve correlação significativa entre os escores dos dois grupos – correlação de 0,84 (p<0.001 e intervalo com 95% de confiança de [0,77;0,90]). Conclusão: Mesmo em uma população com baixo nível sócio-educacional no Brasil o HAQ, quando auto-aplicado, é ferramenta confiável, com baixa variância inter-observador. ___________________________________________________________________________________ ABSTRACT / Introduction: The Health Assessment Questionnaire (HAQ) is a tool developed to address the functional impact of Rheumatoid Arthritis (RA) in the individual patient. One of the factors that can implicated in a wrong use of it is low socioeducational level, which is the prevalent situation among our patients in Brazil. This finding can impact in the correct use of HAQ in our reality. Objective: To correlate HAQ scores found when it is self-administered by our patient, with the scores found when it is administered by the staff. Material and Methods: 90 RA patients, whom fulfilled the American College of Rheumatology set criteria for RA, were included. In a first moment the patients self-administered the HAQ and, after a time, the HAQ was answered by a staff with the patient. The scores were analyzed using the Pearson´s correlation method. Results: The population of the study showed median age of 48,2 years ±12,8, with a median monthly income of $144 ± 240. The median spare time of formal education was 7 years ± 3,2. HAQ scores found in the self-administered questionnaires had a median of 1.156 ± 0,835, and the median of the HAQ scores found in the questionnaires fulfilled by the medical staff was 1,293 ± 0,856. The correlation found was 0,84 (p<0.001 and CI [0,77;0,90]). Conclusion: Even in a population of low socio-economic level in Brazil, the HAQ, when self-administered, is a reliable tool, with a low inter-observer variance.

A administração por via intratecal dos imunossupressores teriflunomida e metotrexato inibe a incapacitação e o edema articular periféricos induzidos por LPS em ratas

Baseado nestes potenciais efeitos inibitórios, nossa principal hipótese é que a administração direta dessas drogas na medula é capaz de reduzir a infiltração de leucócitos, edema e incapacitação articular em um modelo de artrite induzida por LPS. Ratos Wistar fêmeas, pesando entre 200 e 220 g, receberam as drogas por via intratecal, 2 horas antes da administração de LPS (30 ng/ 50 µl; i.a.) nas articulações de joelho previamente sensibilizadas com carragenina (300 µg/ 20 µl; i.a.). A incapacitação articular foi avaliada pelo tempo de elevação da pata (TEP; s) durante o deambular de 1 min; o edema pelo aumento do diâmetro articular (DA; cm) e a migração celular pela contagem total de leucócitos (CT; cel/mm3) do fluido sinovial coletado ao final de 5 horas de observação dos parâmetros. As administrações por via intratecal de teriflunomida (metabólito ativo da leflunomida) (0,1 e 20 µg/ 10 µl) e metotrexato (25 µg/ 10 µl) foram capazes de reduzir a migração celular para o fluido sinovial, a incapacitação e o edema articular promovidos pelo LPS, ao contrário de quando administrados por via intraperitoneal, sugerindo que o sítio de interação para os efeitos antiartríticos das drogas foi restrito ao ambiente medular. A coadministração com uridina (10 µg) reverteu apenas os efeitos inibitórios produzidos pela menor dose de teriflunomida, sugerindo que apenas em maiores concentrações o mecanismo de ação da droga independe da inibição da síntese de novo de nucleotídeos. A coadministração por via intratecal de teriflunomida (0,1 µg) e bumetanida (60 µg), um bloqueador do reflexo da raiz dorsal, não potencializou o efeito inibitório da teriflunomida na redução da incapacitação, edema e migração celular, sugerindo que a inibição do reflexo da raiz dorsal e consequentemente da inflamação neurogênica estejam envolvidos na ação medular da teriflunomida. A coadministração de teriflunomida (0,1 µg) e metotrexato (25 µg) por via intratecal promoveu um efeito somatório na inibição da inflamação articular, provavelmente pela adição do mecanismo imunomodulador do metotrexato na inibição da síntese de citocinas pelas células gliais. O presente estudo sugere que o tratamento por via intratecal com teriflunomida e metotrexato modula a inflamação periférica provavelmente por reduzir o aumento da quantidade de citocinas inflamatórias na medula, através da inibição da atividade glial que inibea sensibilização dos terminais neuronais medulares envolvidos na sinalização nociceptiva e na indução da inflamação neurogênica. Esta estratégia pode representar uma nova abordagem terapêutica para o tratamento da artrite reumatoide com menos efeitos colaterais sistêmicos.<br> / Abstract : Leflunomide and methotrexate are disease-modifying antirheumatic drugs classically used systemically in the treatment of rheumatoid arthritis and clinical success is related to inhibition of proliferation of immune cells and cytokines in the joint cavity. Based on these potential inhibitory effects, our central hypothesis is that the direct administration of these drugs on the spinal cord is able to reduce articular incapacitation, edema and synovial leukocyte infiltratio induced in a model of LPS-induced arthritis. Female Wistar rats, weighing between 200 and 220 g were given the drugs intrathecally 2 hours before administration of LPS (30 ng/ 50 µl; i.a.) in the knee joints previously sensitized with carrageenan (300 µg/ 20 µl; i.a.). Inflammatory-induced incapacitation was measured hourly by the paw elevation time (TEP; s) in 1-min period of observation, and edema was evaluated by the articular diameter increase (DA; cm) taken just after each incapacitation measurement. After the 5-h period of observation parameters the synovial fluid was collected from articular capsule for total leukocyte count (CT; cells/ mm3). Intrathecal administration of teriflunomide ( active metabolite of teriflunomide) (0.1 and 20 µg/ 10 µl) and methotrexate (25 g/ 10 µl) were able to reduce cell migration to the synovial fluid, incapacitation and joint edema induced by LPS, unlike when given by intraperitoneal route, suggesting that the interaction site for antiarthritic effects of drugs was restricted to spinal cord microenvironment. Co-administration with uridine (10 µg) just reverses the inhibitory effects produced by the lower dose of intrathecal teriflunomide, suggesting that only in the higher concentrations of the drug mechanism of action independent from inhibition of de novo synthesis of nucleotides. Co-administration of intrathecal teriflunomide (0.1 µg) and bumetanide (60 µg), a blocker of the dorsal root reflex did not potentiate the inhibitory effect of teriflunomide on the reduction of incapacitation, edema and cell migration, suggesting that inhibition of dorsal root reflex and neurogenic inflammation are involved in spinal action of teriflunomide. Co-administration of teriflunomide (0,1 µg) and methotrexate (25 µg) intrathecally caused a summation effect on inhibition of joint inflammation, probably due to the addition of methotrexate immunomodulatory mechanism of inhibition on cytokine synthesis released by glial cells. Thus, this study proposes that the intrathecal methotrexate and leflunomide modulates the peripheral inflammation by inhibiting on increase of cytokines in spinal cord that sensitizes neuronal terminals involved in nociceptive signaling andinduction of neurogenic inflammation. This strategy may represent a novel therapeutic approach for the treatment of rheumatoid arthritis with fewer systemic side effects.

Entretanto, os mecanismos envolvidos nessa interação ainda não são bem conhecidos. Objetivo: Assim, o objetivo deste estudo foi avaliar a interação da DP induzida por Porphyromonas gingivalis (P. gingivalis) e da artrite induzida por antígeno (AIA) em camundongos e, em seguida, investigar in vitro e in vivo a influência de P. gingivalis e Prevotella nigrescens (P. nigrescens) na severidade da artrite induzida por colágeno (CIA), com foco especial no fenótipo de células T. Métodos:Camundongos C57/Bl6 foram submetidos à AIA e/ou DP induzida por P. gingivalis. Perda óssea alveolar, histopatologia, hipernocicepção e produção de citocinas articular, bem como a porcentagem de células Th17 e T reg foram analisadas. Em seguida, o efeito de P. gingivalis e P. nigrescens na diferenciação de células T e o envolvimento dos receptores tipo Toll (TLRs) foram investigados in vitro. In vivo, camundongos com CIA receberam as inoculações orais de P. gingivalis e P. nigrescens. Histopatologia articular, expressão gênica sinovial, fenótipo de células T e a produção de anticorpos anti peptídeos citrulinados (ACPA) foram analisados. Resultados: AIA não modula a reabsorção óssea alveolar induzida por P. gingivalis enquanto camundongos com AIA expostos à P. gingivalis apresentam maior inflamação articular e perda de proteoglicanos. However, the mechanisms involved in this interaction are not well known. Objectives: The aim of this study was to evaluate the basis of Porphyromonas gingivalis (P. gingivalis) -induced PD and antigen-induced arthritis (AIA) interaction in mice, and further investigate the influence of P. gingivalis and Prevotella nigrescens (P. nigrescens) on severity of collagen-induced arthritis (CIA), with a special focus on T cell phenotype in vitro and in vivo. Methods: C57/Bl6 mice were subjected to antigen-induced arthritis (AIA) and/or P. gingivalis-induced PD. Alveolar bone loss, joint histopathology, articular hypernociception and cytokine production, as well as the percentage of Th17 and Treg cells from lymph nodes were analyzed. Thus, the effect of P. gingivalis and P. nigrescens on T cell differentiation and the involvement of Toll-like receptors (TLRs) were studied. In vivo, collagen-induced arthritis mice received oral inoculations of P. gingivalis or P. nigrescens.
