Improving Early Mobilization in Acute Stroke Patients Through Best Practice Education

Nonadherence to best practice guidelines in early mobilization in acute stroke increases the time from admission to mobilization and may increase residual long-term stroke effects. Early mobilization following an acute stroke is known to decrease long-term and secondary disability. The purpose of this project was to decrease the time from admission to mobilization in the acute stroke patient population by improving knowledge of best-practice guidelines in early mobility using an educational intervention for advanced practice nurse practitioners, physicians, and registered nurses. The practice-focused question for this project asked whether a staff educational program based on best-practice guidelines would decrease the time from admission to early mobilization in the acute stroke patient within the first 48 hours of admission. Rosswurm and Larrabee 6-step model was used for this staff education project. A review of the literature indicated sources of evidence from peer-reviewed journals, which were used to support the staff education project and establish best practices in mobilization for the acute stroke patient. A before-and-after design was used to evaluate time from admission to mobilization within the first 48 hours in 40 acute ischemic stroke adult patients following educational intervention for 35 acute stroke staff. Data were collected and analyzed using descriptive statistics. Findings revealed a decrease in time from admission to mobilization by 18.59%. Findings may be used to promote best practices in other units within the hospital and may be used to decrease the residual long-term effects of a stroke, improve activities of daily living, decrease the length of hospital stay, and decrease the long-term cost and burden of stroke.

Decreasing the Primary Cesarean Delivery Rate

Cesarean delivery is one of the most frequently performed surgical procedures in the United States with 1 in 3 women giving birth by cesarean section. Nurses play a significant role during the labor and delivery process; yet in a hospital in west Texas, nurses lacked knowledge of the current evidence-based obstetric guidelines that were developed to reduce the primary cesarean delivery rates and associated complications. The purpose of this project was to evaluate the content of educational materials developed to inform obstetrical nurses and midwives about labor support strategies to avoid cesarean delivery. Guided by Knowles's whole-part-whole model, a presentation was developed that included evidence-based guidelines and labor-support strategies for positioning and pain management to decrease the choice of cesarean section when not indicated. A 9-member panel with at least 5 years obstetrical expertise evaluated the materials. Based on a descriptive analysis of questionnaire data, experts had a 100% level of agreement that while the educational program material had the potential to promote nursing care practices that would decrease the number of primary cesarean deliveries, changing clinical delivery practices would likely be met with staff resistance. An evidence-based educational program with preventive strategies to decrease primary cesarean deliveries might produce positive social change by prompting obstetric teams to choose these preferred alternatives to avoid to cesarean delivery, and subsequently, decrease associated complication rates, promote faster maternal recovery after childbirth, and decrease the financial burden on the health care system.

Implementation of a Staff Education Project for a Robotics Education Program in the Operating Room

Nurses who provide care in robotic surgery must have sufficient special training in the operation of the da Vinci robot to perform their roles with knowledge and confidence that can yield optimal patient outcomes. The local nursing practice problem in the project facility, and the focus of this doctoral project, was the lack of an evidenced-based robotics education program for registered nurses who participate in robotic surgery. The gap in practice was nurses' lack of knowledge, which interfered with the care provided to the robotic surgical population. The purpose of this project was to develop a staff robotics education program in order to answer the question if the implementation of an evidence-based robotics education program would improve nurses' knowledge in the practice of robotic surgery. The education program was developed using Knowles adult learning theory and information obtained from a comprehensive literature search. A planning team, consisting of local clinicians with expertise in robotic surgery, provided feedback and assisted with the development of the education program and accompanying competency checklist. Ten nurses received the education, and 90-100% of the nurses reported increased knowledge and confidence regarding practice in the specialty of robotic surgery following the education. Leadership at the project site have decided to require surgical nurses receive the robotic education upon their employment and annually thereafter. The social change resulting from the use of this evidence-based robotics education program could include increased nursing performance and therefore, decreased complications for patients undergoing robotic surgery.

The Master's Thesis engages the processes of education, hiring, choice and adaptation of new employees in a middle size company. Human resources activities, which are necessary for the practical part, are introduced in theoretical part of the thesis. Organisation, where has research been made is introduced in the practical part. Research has been made by a questionnaires, individual interview and mystery shopping. The methods of the mystery shopping and questionnaires were used twice in the thesis, once for research before the education project from EU funds and for second time after it. Main task to reach the goal of the thesis is the comparison of results from both researches. There is a recommendation for improvements of the education processes in the company. This recommendation should be a basement for set-up a new education system in the company.

Staff Education for Providers in an Outpatient Hemodialysis Center

The impact of end-stage renal disease (ESRD) on healthcare costs is significant. In 2013, the cost of providing care for the ESRD patient population was 7.1% of total Medicare expenses. ESRD patients' non-adherence with the medical plan is a reason for the high cost of care and poor patient outcomes. Staff education can have a positive impact on patient adherence in terms of the management of chronic illnesses, such as ESRD. This DNP project was an education program for hemodialysis (HD) staff regarding empowering patients to learn about self-care strategies aimed at improving ESRD patients' adherence to fluid restriction and treatment schedules. The staff education project was developed using Malcolm Knowles' adult learning theory and Dorothea Orem's self-care theory. Three in-center hemodialysis (ICHD) clinics located in a large southeast inner city of the United States with predominantly African American patients participated in the DNP project. All the participating clinics were selected based on their underperformance in missed patient treatments and fluid goals. The goal of the educational program was to teach staff current evidence-based practice self-care strategies for patients to improve adherence to required fluid restrictions and treatment schedules. The program was delivered through 5 different sessions over 3 days at 2 locations. Thirty staff members participated in the program. Missed treatment rates and the intradialytic weight gain (IDWG) percent showed improvement 1-month post education for each of the 3 clinics. This project has the potential to promote social change through staff education on patient self-care strategies for adherence to fluid and treatment plans, thus improving patient outcomes and quality of life.

Impact of a Healthcare Workplace Violence Prevention Module on Staff Knowledge

The healthcare sector experiences violence 4 times as much as any other civilian domain, including law enforcement and corrections facilities. The clinical practice problem addressed in this project was the lack of adequate healthcare workplace violence (HWV) prevention in a not-for-profit community hospital. The purpose of this project increase awareness of HWV prevention through the use of an education module. Rogers’ diffusion of innovation model served as the foundation for analysis of scores from pre- and posteducation tests. The practice-focused question asked if a module on HWV prevention based on current clinical practice guidelines and peer-reviewed literature would improve staff members’ knowledge on workplace violence. A staff education project was designed to address the practice problem. The research design was an anonymous pre- and posttest for score comparison to analyze data by noting changes in proportion of correct answers. Nursing professionals (N = 14) participated in the education module and pre- and posttests. The percent of correct answers to 18 knowledge-related questions increased from 82% on the pretest to 91% on the posttest. The facility elected to expand the project to a more comprehensive program and requested additional modules to broaden understanding of and sustain HWV prevention strategies. These efforts will enact positive social change for healthcare staff by promoting a culture that embraces a safe work environment, increases staff knowledge on HWV prevention, decreases HWV and is supported by organizational systems.
