Treating Adult Women With Depression Through Videoconferencing

Demidova, Irina 01 January 2017 (has links)
The occurrence of depression in the United States is steadily increasing. In every age group, women have a higher rate of depression than men, and U.S. women between the ages of 40 and 59 have a depression rate of 12%. Adult women living in rural areas experience physical and/or psychological impairment and lack access to mental health treatment. The purpose of this quantitative nonexperimental study was to examine participants' preferences for treatment delivery method based on patient perceptions of the clinical experience, patient satisfaction, and therapeutic bond. The working alliance theory provided the theoretical foundation. Data collection included survey responses from a self-selected sample of 264 adult females ages 40 to 65. Results from independent sample t tests indicated that participants favored CBT treatment delivered via videoconferencing more than in-person treatment. Implications for social change include improving the lives of adult women suffering from depression by providing treatment via videoconferencing when in-person services are not available. Psychologists may apply findings in clinical practice, thereby benefiting individuals, families, and communities.

Improving Access to Cancer Care for the Rural Community with Telehealth

Fiscelli, Cheryl Ann 01 January 2019 (has links)
After a diagnosis of cancer, timely access to oncology care is a priority. For patients living in a rural community, there are many challenges such as proximity to cancer care, reduced access to state-of-the art therapies, lack of coordinated care, and limited access to clinical trials. A multidisciplinary consultation (MDC) via telehealth can improve access to care for rural oncology patients. The purpose of this project was to determine the effects of telehealth MDC on the time in days from diagnosis to the first treatment with the goal of persuading the project site to implement a telehealth MDC. The data involved a comparison of 2 rural locations, 1 with telehealth MDC and 1 without. Data from 36 oncology patients were compared using time in days from the initial diagnosis to the first oncology treatment. The patients who received the initial consultation with telehealth MDC had an average timeframe of 19 days from diagnoses to first treatment, whereas those without telehealth MDC had an average of 51 days, meaning there was a statistically significant difference (z = -5.811, p < .0001). The data will be presented to leadership at the project site to provide the rationale to implement telehealth MDC. This project can lead to a positive social change for rural oncology patients by encouraging telehealth MDC, which may address the several identified barriers that affect access for oncology patients by improving access to clinical trials, coordination of care, and nursing education to rural community patients at the project site.

The application of geographical information systems to infectious diseases and health systems in Africa.

Tanser, Frank Courteney. January 2000 (has links)
The health sector has not yet begun to explore the full potential of geographical information system (GIS) technology for health research and planning. The goal of this thesis is to demonstrate this potential in Africa through the application of GIS to the most important health issues in the continent. In excess of 23,000 homesteads are mapped and interviewed throughout Hlabisa district, Kwa-Zulu Natal using differential global positioning systems (GPS). I use the GIS to analyse mode health care usage patterns. 87% of homesteads use the nearest clinic and travel an average distance of 4.72 km to do so. There is a significant logarithmic relationship between distance from clinic and usage by the homesteads (r2 = 0.774, p<0.0001). I propose the distance usage index (DUI) as a composite spatial measure of clinic usage. The index is the sum of the distances from clinic to all actual client homesteads divided by the sum of the distances from clinic to all homesteads within its distance-defined catchment. The index encompasses inclusion, exclusion and strength of patient attraction for each clinic. The DUI highlights significant disparities in clinic usage patterns across the district (mean = 110%, SD =43.7). The results of the study have important implications for health planning in Africa. I use GIS/GPS technology to quantify the spatial implications of a shift towards community-based treatment of tuberculosis using the DOTs strategy in Hlabisa. The mean distance from each homestead in the district to nearest supervision point is measured using a GIS. The shift in treatment strategy from hospital to community-based between 1991-1996 reduces the mean distance to treatment point from 29.6 km (94% of the population > 5km) to 1.5 km (entire population < 5km). GIS effectively documents and quantifies the impact of community-based tuberculosis treatment on access to treatment. I produce the first quantifiable evidence of a relationship between distance to roads and HIV prevalence using a GIS. HIV prevalence was measured through anonymous surveillance among pregnant women in Hlabisa and stratified by clinic attended. Assuming women attend the nearest clinic, the mean distance from homesteads to a primary or secondary road for each clinic catchment is strongly correlated with HIV prevalence (r = 0.66; p = 0.002). Further research is needed to better understand this relationship both at ecological and individual levels.I develop a methodology that has numerous applications to health systems provision in developing countries where limited physical access to primary health care is a major factor contributing to the poor health of populations. I use an accessibility model within a GIS to subdivide an area into units of equal workload using a range of physical and social variables. The methodology could be used to ergonomically design programmes for home-based care and tuberculosis directly observed treatment. It could also be used as a basis for more efficient distribution of community health workers. I use high-resolution long-term rainfall and temperature data to produce the first malaria seasonality (length, start and end of transmission season(s)) maps for Africa. I relate the model to population data and estimate the population exposure in a variety of transmission settings. I investigate the relationship between predicted length of transmission season and parasite ratio from 2335 geo-referenced studies of children <10 years across Africa. The research is the first to correlate actual malaria survey data with model predictions at a continental scale. The seasonality model corresponds well with historical expert opinion maps and case data. A significant logarithmic relationship is detected between predicted length of transmission season and parasite ratio (r2=0.712, p=0.001). I recompute the changes in the disease likely to occur as a result of global warming. The seasonality model constitutes an important first step towards an estimate of continental intensity of transmission. / Thesis (Ph.D.)-University of Natal, 2000.

Understanding telehealth mediated cancer care in Northern BC First Nations communities: health professionals’ perspectives

Sidhu, Jatinderpal 20 December 2012 (has links)
Objective The use of telehealth to provide health service delivery to rural and remote First Nations populations Canada-wide has greatly increased in recent years. Telehealth provides a mechanism for these disadvantaged and underserved communities to access timely healthcare services that would otherwise be expensive, delayed or unavailable due to geographic and resource limitations. There are numerous benefits, challenges and cultural issues that must be understood from a healthcare professional perspective when providing telehealth care to First Nations communities. Once educated with respect to these issues and experienced in providing care, healthcare professionals are well poised to provide feedback with respect to improving telehealth mediated health care delivery. This thesis examines these issues in the context of using telehealth for patient care, specifically cancer care. Methods This thesis is divided into two phases. Phase one is a literature review assessing the use of telehealth, specifically in rural and remote First Nations communities in Canada. Phase 2 is a study using a survey to assess healthcare professionals who provide telelehealth mediated patient care (in particular cancer care) to Northern BC First Nations communities. The participants were contacted through the use of an online survey tool to assess their perceptions of benefits, challenges, and cultural awareness when providing patient care. The survey population consisted of onsite health professionals and urban health professionals providing patient care to Northern BC First Nations communities via telehealth. Specific participant groups of interest were (1) onsite cancer care professionals, (2) onsite other (non-cancer) care professionals, (3) urban cancer care professionals, and (4) urban other (non-cancer) care professionals. Results The participant population of 45 was too limited a sample size for inferential statistics to be conducted. Therefore our survey data was interpreted by comparing the mean composite scores of the participants groups within each category. Our survey data implied that onsite cancer care providers found fewer benefits and more challenges with respect to telehealth than the other participant groups. We did not see any differences between the participant groups with respect to cultural awareness. Conclusion Based on the literature we reviewed telehealth can reduce costs and extend health care services in a timely manner while at the same time complement conventional care to build stronger health care community relationships. Despite these positive benefits found in literature, our survey found onsite cancer care professionals did perceive fewer benefits and more challenges regarding telehealth use. Specific issues raised by our survey participants that need to be addressed are the physical disconnect associated with telehealth and the perception that telehealth is a replacement for conventional care. / Graduate

Effects of Behavior Specialists' Use of Coaching and Performance Feedback Via Telehealth to Train Parents of Children with Challenging Behavior

Ruppert, Traci 27 October 2016 (has links)
This study used a concurrent multiple-probe across three mother-child dyads with a non-concurrent addition of a fourth mother-child dyad to assess the extent to which behavior specialists’ use of coaching and performance feedback via telehealth effects the degree to which the parent implements the multi-component behavior support plan (BSP) strategies with fidelity in their home to decrease their child’s challenging behavior. Four behavior specialists and four mothers with their children who exhibit mild to moderate behavior participated in this study. The research question examined if a functional relation exists between implementation of coaching and performance feedback from the behavior specialists via the telehealth model on the parents’ level of implementation fidelity of the strategies in the BSP and the improvement in the level of child’s target challenging behavior and adaptive behavior. Results indicate the efficacy of coaching and performance feedback via telehealth in improving parent treatment fidelity. These results support the use of telehealth to deliver training on multi-component interventions. Additionally, participating behavior specialists and parents indicated that they found the procedures used to be both acceptable and effective. Further implications for practice and directions for future research are discussed.

The use of information technology (IT) in the practice of health care is a trend that has been evolving and gaining increasingly space in health area in Brazil and all around the world. Estudo da conformidade de formas de uso da telemedicina/telessaúde em relação aos modelos nacionais propostos / Study of conformity ways to use telemedicine/telehealth according to the proposed national models

Sidney Porcincula 04 January 2016 (has links)
There are some specifically normative aspects for professional orientation in telemedicine as follows: CFM no. 1.643/2002 that define disciplines and services provided by telemedicine, and CFM no. 2.107/2014 that define and guide the teleradiology services. This work was a retrospective, quantitative and descriptive study based on bibliographic surveying, in order to verify the conformity of the use of telemedicine and telehealth in both national and international contexts, according to the CFM proposed model for this activity, represented by CFM resolutions no. 1.643/2002 and no. 2.107/2014. The bibliographic surveying between 2009 and 2015 used Scielo, PubMed, and IEEE databases, and the key words: videoconferencing, webconferencing, VTC, meeting, telemedicine, and telehealth plus their combinations. We found 2352 articles, reduced to 572 after reading their abstracts. One hundred and fifteen potentially relevant articles were selected after partial reading of the articles. After that, the 115 selected articles were fully read. The results showed that the use of telemedicine could considerably increase the area of specialized attending crew on important medical facilities, what should be encouraged mainly in countries with few or no financial resources, or the small number of specialists, or extensive areas such as Brazil
