Využití Mirror terapie u pacientů po poškození mozku z pohledu ergotgerapeuta / Use of Mirror therapy in patients after brain injury from the viewpoint of Occupational therapist

The purpose of this Master thesis was to evaluate the effects of the Mirror program on upper- limb motor recovery, self-sufficiency and active range of wrist movement in patients after brain injury. It was included sixteen participants within maximal four months after stroke. Patients were separated into experimental and control group, both with eight members. Experimental group underwent Mirror therapy. Both group participated in a standard rehabilitation program included Physiotherapy, Occupational therapy, physical therapy and movement practice on device. Experimental group patients additionally participated in Mirror therapy program for 30 mins, four times a week, for three weeks. The Fugl-Meyer Assessment, Functional Independence Measure and goniometric measurement of active movement were used to assess recovery of upper-limb movement. Mirror therapy improve upper-limb motor recovery in patients after brain injury. This research demonstrates it with significant difference between both groups, where the experimental group have improved (0,02). I have also proved Mirror therapy, when included in occupational therapy, improves the self-sufficiency of people with neurological deficit. Title: Current approaches in conservative treatment of scoliosis - literature review Objective: The goal of this diploma thesis is evaluate the most efective current within a combination of a bracing with exercises therapy. Next goal is to summarize matters of adolescent idiopathic scoliosis disease, especially outline of clinical picture, causes and predisposition for origin these deformity and last but not least options in conservative treatment. Methodics: This diploma thesis is processed as literature review from available literary resources. First part of thesis contains all of theoretical bases and knowledge about adolescent idiopathic scoliosis. Second part has a decriptive-analytical charakter and contains analysis of studies, which was used a bracing supplemented by exercises treatment. Results: Established criteria after finish of review fulfill only nine studies, which interest about efectivness of treatment by bracing in pacient with adolescent idiopathic scoliosis. Because the way of studies and their very low number of studies, is not possible to evaluate results of studies and conclusively interpret, which one is the most effective access. Background: Theory and research declares that family memberes engagement in adolescent treatment has positive impact on treatment outcomes. To get the famillies engaged is quite challenging for proffesionals in addiction treatment services. Motivation of individual members fluctuate a lot. The impact on treatment is due to the voluntarity of treatment or recomendation of a third side. Even dough engagement of legal representatives is declared condition of treatment in Regional Ambulance for Adddictions in Children and Youth, it is complicated to motivate them for cooperation. Aims: The aim of the thesis is to find out how the legal representatives percieve cooperation with outpatient services, if they percieve any obstacles in getting more involved and to find out, what is their view on adolescent's profit from treatment. Methods: For the research a qualitative approach was used in the form of semi- structured interview and interpretation of the data from client files and UniData program. Data collection took work during 05/18-11/18. For the data analysis the method of gestalts was used. One of the mechanisms is increased efflux of drugs from tumour cells, whereas some studies suggest that increased drug conversion to an inactive metabolite might be another contributing mechanism. The aim of this work was to define the possible role of CYP3A4 and CYP2C8 enzymes in the phenomenon of pharmacokinetic resistance and to investigate the possibility of its modulation by new targeted drugs. In the first part, we used the MTT proliferation method together with HepG2 cells stably transduced with particular human enzymes and demonstrated significant involvement of CYP3A4 in docetaxel resistance. In the following part, we examined the inhibitory effects of four selected tyrosine kinase inhibitors on the CYP3A4 activity in intact cells using a commercial kit. Cancers represent a group of unprecedented heterogeneous diseases and currently available anti-cancer therapies provide highly variable efficacy with unsatisfactory cure rates. A wide range of proteomic technologies are being used in quest for newer approaches which could significantly contribute to the discovery and development of selective and specific cancer biomarkers for monitoring the disease state and anti-cancer therapy success. Taking into consideration the above aspects, this research was undertaken to study cancer cell proteomes and their changes after anti-cancer treatment with specific focus on: (a) response to conventional anthracycline/anthracenedione drugs with respect to their different clinical efficacy and (b) identification of novel targets for therapy in cancer cells resistant to biological drugs such as inhibitors of (b1) cyclin-dependent kinases and (b2) Aurora kinases. This study identified several interesting key aspects related to the effects of daunorubicin, doxorubicin and mitoxantrone. vi ABSTRACT IL-2 has been used in cancer therapy and also for other applications like treatment of chronic viral infections or as an adjuvant for vaccines. However, treatment with IL-2 is rather difficult due to its severe side effects. These toxicities, associated with high-dose treatment necessary for IL-2 to function, have been found the most limiting factor for IL- 2 applications. Further, particular anti-IL-2 monoclonal antibodies (mAb) can actually increase biological activity of IL-2 rather than block it. Binding of IL-2 to anti-IL-2 mAb creates a superagonistic immunocomplexes which have dramatically higher and selective biological activity in comparison to free IL-2 in vivo. Such approach may finally over- come the difficulties associated with administration of IL-2, thus opening brand new scopes for IL-2 and its application not only in the field of tumor therapy. We have shown that IL-2 immunocomplexes composed of IL-2 and anti-IL-2 mAb S4B6 (IL-2/S4B6) stimulate predominantly cells expressing CD122 and CD132 (dimeric IL-2 receptor), i.e. NK and MP CD8+ T cells, with Treg,  T and NKT cells being expanded as well. Dissertation objectives: The aim of the Dissertation is A) within the framework of a prospective study, to assess the dancing therapy influence on mobility, self-sufficiency, cognitive state, consumption of psycho-pharmaceutical drugs, depressiveness and quality of life of seniors in institutions. Tuberculosis is one of the worlds? deadliest infectious diseases, and is one of the major causes of morbidity and mortality especially in poor countries. In Zambia, Tuberculosis remains a major health problem and is among the top ten causes of morbidity and mortality. Tuberculosis notifications have increased 5- fold in the last 20 years, mainly due to HIV/AIDS, it is estimated that approximately 70 % of all detected tuberculosis cases are also co-infected with HIV. The interaction of tuberculosis with the human immunodeficiency virus (HIV) infection has complicated and made difficult the Tuberculosis control program. Tuberculosis has become the leading cause of death among people living with HIV/AIDS. The management of tuberculosis in Zambia is faced with many challenges, such as lack of compliance to treatment by patients, poverty as well as lack of adequate finance for diagnosis, treatment and follow-up care. The main aim of my thesis was monitoring the problem of tuberculosis in Zambia, other accompanying aims were to find out how well informed the Zambian population is, about tuberculosis, monitor the diagnostic procedure for tuberculosis, analyse the tuberculosis treatment regime and lastly to analyse the preventive measures that have been implemented and how effective they have been. In this thesis, both qualitative and quantitative types of research were used. With the qualitative research, secondary analysis of data was used. Data from different sources of information like books, publications and research findings of various surveys and researches that were done not only by the Zambian Ministry of Health but also by other organizations was used. With the quantitative research, a questionnaire was formulated whose main aim was to find out about how well informed the Zambian people are about tuberculosis. This was done only in the capital city of Zambia which is Lusaka, and the number of respondents was 100. From the results obtained, the tuberculosis burden in Zambia still remains high and tuberculosis continues to be a serious public health problem. The implementation of the various preventive measures, though still facing difficulties, has however yielded positive results. There have been improvements in the treatment success rates from 77 % in the year 2000 to the current 86 %, the incidence rate per 100 000 population has also reduced from 504 in the year 2000 to the current 365 per 100 000 population. This reduction in incidence rates can be attributed to the preventive measures like the expansion of directly observed therapy treatment ? shortcourse (DOTS)strategy to all the districts in the country, the strengthening of the National Tuberculosis Control program and many others. This work can be used as a source of reference for future research in relation to the topic of tuberculosis in Zambia.

The compression therapy is often being underestimated not only by medical staff and doctors, but also by patients. Therefore, it is often perceived as a simple task frequently handed over to hospital attendants. At the nursing schools the time devoted to the issue of compression therapy is minimal. That is probably why there are nurses with a very limited information at their disposal regarding this issue. The compression therapy is nevertheless an essential part of prevention and treatment of the venous and lymphatic diseases. When the compressive bandaging is carried out efficiently, it can improve the quality of life of a patient with a venous or lymphatic disease. Moreover, it can help in preventing thromboembolic diseases during surgical interventions. However, in order for the compressive bandage to be effective, several conditions must be met. First, it has to be carried out by a properly trained medical staff. Second, it is highly important that the patient undergoes a thorough education concerning compressive bandaging, so that he or she is able to keep the bandage effective when released home. Based on evidence, the aim of this thesis is to find out how effective is the practice of compressive bandaging within nursing care. The thesis consists of two parts. The first is theoretical and it contains the findings of various researches and publications related to the issue of venous diseases, aplication of the compressive bandage, differences between elastic stockings and elastic roller bandage and the findings concerning the education of a patient. The second part is practical. It consists of a quantitative research with a participation of 192 respondents who work as nurses in medical facilities and who are therefore closest to the patient undergoing compressive therapy. The thesis is interested in the amount of knowledge nurses have in relation to the compressive bandage, in the way they apply the bandage and in the process of the education of the patient. The research shows that the choice of a type of a compressive therapy is conditioned by the supply of the ward. Furthermore, the research reveals that there is no connection between the length of the professional experience of a nurse and her or his method of applying the compressive bandage. The recommendation for practice is an educational seminar for nurses aimed at completing their theoretical knowledge. This would also serve as an opportunity to practice not only the application of the bandage, but also the use of a measuring instrument Picopress.

Breast cancer is the most frequent cancer in women and is prognostically very heterogeneous. Early breast cancer has an excellent overall prognosis with long-term survival above 90%. In this group we can also find patients with highly unfavourable progress with a risk of future disease relapse. Due to effective anticancer treatment is a main task of precise clinical decision to determine risk of an individual patient in the term of cancer relapse. We can use clinical (tumor diameter, lymph nodes) and pathological markers (grade, ER, PgR, HER2, and Ki-67), all of them have low individual sensitivity and specificity. Molecular tests based on multigene DNA or RNA assays have higher sensitivity and specificity but their interrelated concordance is low. One of the main scientific task is to find almost specific and sensitive prognostic biomarkers. microRNAs are small, highly stable, non-coding RNAs, which regulate tens of mRNAs and proteins inside cells. In cancerogenesis, they could act as oncogenes or tumor supressors as well and affect main steps of initiation and progression of cancer.
