Využití fyzioterapeutických postupů k ovlivnění spasticity / The Use of Physiotherapy in Spasticity Management

Title: The Use of Physiotherapy in Spasticity Management Objectives: The purpose of this thesis is to provide a summary of present findings on spasticity, with particular regard to its pathophysiological mechanisms and clinical manifestations, and to present an overview of medical and physiotherapeutic approaches applied in its treatment. Furthermore, it aims at making a research in physiotherapeutic methods and techniques used for reduction of spasticity. The thesis shall present relevant studies, provide their comparison and critically evaluate the effect of methods and techniques examined in them. Methods: This thesis is a descriptive analysis based on a literature review. Results: Based on the research findings, it may be concluded that physiotherapy has proven to be effective in reducing spasticity. However, it is not possible to determine whether the techniques to reduce spasticity are more effective than techniques without this primary purpose. The examined studies have shown that the choice of physiotherapeutic practices to reduce spasticity was not influenced by disease etiology. Moreover, the same techniques and methods were applied in pediatric and adult patients, regardless of diagnosis. I chose the theme of obsessive-compulsive disorder as a topic of my thesis. The main reason was that it is a disorder I have worked as a therapist in individual or group therapy frequently. Also I have participated in several studies as evaluator in Psychiatric Centre, and I collected data from dozens of patients. In the first part the thesis deals with the symptoms, clinical picture, prevalence, aetiology and treatment of OCD. Thesis is focuses on behavioural and cognitive models of the disorder in detail, because the treatment used in the present group of patients was CBT. Various models of cognitive behavioural therapy I discuss in more detail, because they allow different views of what happens to the patient and how to change it. The practical part has two parts. The first deals with the effectiveness of group cognitive behavioral approach for OCD patients, who use antidepressants but were resistant to previous treatment and were attended a daycare center at the Prague Psychiatric Center. The second part of the thesis deals with finding a predictor of successful cognitive behavioral therapy in these patients. This thesis is devoted to the history of Children's Psychiatric Hospital, which was founded in the South Bohemian Opařany in 1887 as a branch of the Prague Institute for the Mentally Ill. For a deeper understanding of the topic, the thesis focuses more on the importance and appreciation of selected personalities who dedicated themselves to people with learning disabilities and psychiatric conditions. In the introductory part, the thesis focuses on the presentation of the field of psychopaedia and the individual selected concepts / definitions that are competently explained using appropriate terminology. It also contains the biography of selected representatives of the field who have contributed to the development of the paradigm of the psychopaedic care. This thesis assesses the effectiveness of shock wave therapy for dysfunctional disorders of the musculoskeletal system. While we are well aware of the physical principles and history of shock wave generators, opinion differs on the treatment effectiveness mechanisms. The theoretical part of this work explains the effects of shock waves on various types of tissue, the differentiation of the cells, and the analgetic effect of therapy. It summarizes the indications, side effects and contraindications of shock wave therapy. The research part of this thesis deals with the effectiveness of shock wave therapy at the Department of Rehabilitation and Sports Medicine at the Motol University Hospital, which specialises in various musculoskeletal disorders. This thesis evaluates the correlation between the effectiveness of shock wave therapy and length of time the patient has experienced difficulties before undergoing treatment. This thesis also reviews whether the number of treatment applications has the capability to influence the outcome of therapy. The study is controlled by a control group of 22 patients. Mesenchymal stem cells (MSC) possess a multilineage differentiation potential and have the ability to regulate reactivity of the immune system. They are usually isolated and expanded from the bone marrow, adipose tissue or umbilical cord. MSC represent promising cell population for the treatment of some severe diseases, such as amyotrofic lateral sclerosis (ALS), due to the combination of regenerative and immunomodulatory properties. The aim of this study is to compare MSC from ALS patients and healthy donors in their phenotype, proliferative activity and mainly their immunomodulatory properties. The assessment of impact of the disease on the properties of MSC is important for their autologous use in clinical trials. In this study we used MSC isolated from bone marrow of 14 ALS patients and 15 patients undergoing mostly orthopedic surgery as control group. We also used MSC stimulated for 24 hours by poinflammatory cytokines. Cells were compared in terms of immunophenotype, differentiation in adipocytes and osteoblasts, metabolic activity, expression of selected genes for immunomodulatory molecules and for inhibition of lymphocyte proliferation. The aim of this thesis is to analyse certain aspects connecting religion and narration (which is understood here as a common human faculty to think and express oneself in the form of narratives). The first part of the thesis is concerned with methodology; first of all, the issues of defining narrative are introduced and a more elaborate definition is presented. A complete methodology is then formulated with a help of several authors (mainly James W. Pennebaker and Mary Douglas) in order to distinguish particularities and functions of creating narratives in religious contexts. Two main points are stressed here: that the content of the narratives is often concerned with problematic aspects of experience and that the expression of these narratives is beneficial for their creators. The second part focuses on several religious institutions concerned with creation of narratives which are interpreted with the outlined methodology. In this manner, the act of confession in Catholicism, prayer in Christianity and certain healing rituals are described and interpreted. Conclusions of this thesis should help the reader get a basic idea of the way created narratives in religious contexts affect their authors.

Využití myofunkční terapie u osob s NKS / Use of myofunctional therapy in people with communication disability

The topic of this thesis is discussion of usage of myofunctional therapy on persons with communicative disorders. The main goal is to map the extent and possibilities of this therapy in speech therapy practice. The theoretical part deals with physiology and patophysiology of breathing, sucking, swallowing and the orofacial system. Further, in connection with myofunctional therapy it discusses its historical development, methodical approaches, utilities, therapeutical benefits and possibilities of education on this field. The empirical part contains a quantitative investigation, which analyses the extent and usage possibilities of this therapy by speech-language pathologists in the Czech Republic. Based on the data obtained by a questionnaire distributed to speech-language pathologists it can be stated, that about 20 % of the respondents do not use the myofunctional therapy in practice, while those (with one exception) never took part on an expert training. The offer of the trainings is rated as insufficient. The myofunctional therapy can be used on persons with orofacial disorders regardless of age or communicative disorder. TITLE Alternative rehabilitation methods in therapy of Parkinson's disease OBJECTIVE The aim of work is to assemble new findings relating the rehabilitation of people with Parkinson disease. Furthermore to elaborate these information in form of literature search to allow their use in common practice. METHODS Up till now there were accomplished many studies which dealt with less common ways of therapy, but didn't find use in common practice. In this work these studies will be presented (in form of literature search). This work will be divided into two parts, where the first will sum up the issue of Parkinson's disease and the second part will deal with the alternative methods. Into this part studies detecting effects of dance therapy, tai-chi, qigong, treadmill training and box on symptoms of Parkinson's disease were chosen. Besides that, in discussion I plan on mentioning conventional therapy (rehabilitation) in Parkinson's disease as well as some guidelines written up in the Czech Republic. This diploma thesis brings new data about design, synthesis, physico-chemical characterisation and biological efficacy of the novel star-like HPMA-based conjugates intended for treatment of solid tumors. Recently, many different water-soluble drug delivery systems based on N-(2- hydroxypropyl)methacrylamide (HPMA) copolymers have been described. Here, we report synthesis and physico-chemical characterisation of high molecular weight star-like HPMA- based polymer carriers with low polydispersity prepared by controlled grafting of HPMA copolymers onto PAMAM dendrimer core. With the aim to keep the polydispersity of drug delivery system as low as possible, reversible Addition-Fragmentation Chain Transfer (RAFT) polymerisation was used for HPMA-based polymer precursor preparation. The end groups of the polymer presursors was afterwards used for grafting using carbodidimide condensation reaction or copper free click chemistry on polyamidoamine (PAMAM) dendrimers resulting in a formation of star-like high-molecular-weight (HMW) drug carriers. Described synthetic procedure provided preparation of star-like HMW drug carriers with Mw between 1.105 - 3.105 g/mol and narrow distribution of Mw. The development of computer technology intervened to socio-psychological and psychiatric fields and gave rise to several types of online counseling and therapy. As the latest trends in online counseling and therapy self created applications in the textile even non textile addiction. In the Czech Republic was the first self-help application in the treatment of no cloth addiction, clinic was founded in Addictology, the first Medical Faculty of Charles University within the Internet counseling. The aim of the thesis is to evaluate the data after the first year of self-help application and to describe the demographic characteristics of users, to map incoming user self- motivated applications and types of objects the internet addiction. In the research issues the thesis deals with the analysis of demographic data, determines whether they are represented in all potential target groups, the degree of dependence on the Internet within the file, depending on what kind of Internet application in the present, and finally discovers self-help application users' motives for its use.
