Individualizace léčby pacientů s karcinomem prostaty na základě molekulární a imunocytochemické detekce cirkulujících nádorových buněk / Individualization of the treatment of prostate cancer patients based on the immunocytochemical detection of circulating tumor cells

Introduction: Together with the introduction of new therapeutic options in castration- resistant prostate cancer (CRPC), an advance in individual disease characterization is required. Since common biopsy methods are not suitable for the majority of CRPC patients, one possible solution is the liquid biopsy that is, the analysis of circulating tumor cells (CTCs) isolated from the cancer patients' blood. Methods: A method based on the immunomagnetic enrichment of CTCs and subsequent PCR detection of tumor-associated genes (AdnaTest, Qiagen) was characterized and used for the detection of CTCs in 41 CRPC patients. Each patient was screened at the time of CRPC diagnosis and after the 3rd cycle of docetaxel therapy. A panel of genes associated with therapeutic decision-making was established and validated. Quantitative PCR (qPCR) method on a BioMark platform (Fluidigm, USA) was used to determine the expression of the gene panel in the CTC-enriched and primary tumor samples and the results were analyzed. Results: CTCs were found in 85% and 45% of CRPC patients before and during the therapy, respectively. The presence of CTCs, as well as EGFR and AR PCR fragments, was associated with a decreased sPSA response and lower survival. Thesis deals eating habits of children from 12 to 15 years of age and possibilities of the treatment are being considered. Over the last year, the children obesity has increased rapidly and caused an individual a lot of health problems. I tis not only a question of the cosmetic imperfection. The obese children suffer mainly from mental, physical and, in adulthood, socioeconomic difficulties. The prevention against obesity and its treatment lies in the increased physical activity and in the principles of healthy eating. The aim of this paper is to refer to the danger of the mass occurence of the obesity in children and adults in the Czech Republic and make the public familiar with a possibility of the rational nutrition and with the connections in the change of life style.

Causes can be innate (hereditary), which are less serious, or as a result of upbringing (impact of people of the same age, family, gangs), when a young man in such a way emulates his or her surrounding. Behaviour disturbances can be generally characterized as a deviation in the area of socialization, when an individual is not able to respect the common standards of behaviour on the level corresponding his age.

This thesis deals with the application of narrative approach during the therapeutic work with clients with diagnosis of emotionally labile personality or dissocial defect of personality with exposure of aggression. Theoretical part of this thesis shows the basics and resources of the systemic constructivistic psychotherapy and the narrative approach as one of the main instruments for the psychotherapeutic work. This thesis also deals with analysis of some psychological views on aggression which are usually used during the therapy.Practical part of this thesis shows the basic elements of narrative approach on case studies with usage of techniques of externalisation, personification and deconstructional harking. Usage of these techniques is condition for the change of the primary client?s story which is affected by aggression. After this turnover on the new story the client is not part of the problematic system but with help of newly discovered resources he gain the ability to perceive himself as a person who is able to eliminate the effect of the aggression as a heterogeneous element and who is able to positively influence own life.

1 ABSTRACT B-cell non-Hodgkin lymphomas (B-NHL) represent the most common mature lymphoproliferative diseases. B-NHL arise at different stages of B-cell development and represent their malignant counterpart. Diffuse large B-cell lymphoma (DLBCL) and mantle cell lymphoma (MCL) are aggressive types of B-NHLs. Deregulation of cell cycle control, inhibition of apoptosis or abnormal DNA damage response play a key role in the pathogenesis of DLBCL and MCL. Aberrant activation of several signaling pathways that further promote survival, cell proliferation or affect the tumor microenvironment have been recently recognized. Increased understanding of the oncogenic mechanisms implicated in pathogenesis of B-NHL lead to development of novel agents that target the oncogenic drivers of distinct lymphoma subtypes. MCL is an aggressive subtype of B-NHL associated with poor prognosis. In vivo models of human MCL for experimental therapy are however scarce. We established and characterized several mouse models of human MCL by xenotransplantation of either primary cells or established cell lines into immunodeficient mice (publication no 1). Systemic sclerosis (SSc) is immune-mediated fibrotic disease of unknown aetiology. Among the dominant pathogenic manifestations of SSc belong vascular changes, production of autoantibodies, activation of innate and adaptive immune responses and fibrotic processes. Transforming growth factor beta (TGF-β) has been identified as a central profibrotic factor stimulating fibroblasts to produce collagen. There are, however, a number of other mediators involved in the pathogenesis of SSc. Mutual activation and amplification of these molecules and their cascades may be a central mechanism of the SSc pathogenesis. Hedgehog (Hh) canonical signalling pathway plays an important role in the development and progression of fibrotic diseases. Expression of Hh target genes can be regulated through a canonical or non-canonical signalling cascade. The non-canonical activation of GLI transcription factors by TGF-β has not yet been investigated in SSc. The substantial part of this thesis is focused on the study of the mutual interaction of TGF-β and Hh signalling pathway. It presents the most important matters of this disorder, such as how its manifestated, what are its types and how it progreses. It handles the information about how the social work with this focus group is being executed in Czech Republic, what is a psychiatric rehabilitation and how it should be processed, how should social workers approach clients with this disorder and what are the specifcs of social work with those clients. It presents examples of organisations and associations working with this focus group in Czech Republic. The experimental part is a reasearch concerning how clients with schizophrenia percieves life, how the disorder started and continued and mainly how informed those clients are about the possibilities of social services.

1 Summary Assisted contact with children The aim of my thesis is to describe the new phenomenon of social work i.e an assisted contact with children. In the theoretical part I focused on areas that are connected with parent-child assisted contact, and that parents solve the most often. I gave a definition of parental responsibility and interests of the child, which are described in legislation or conventions. In relation to this, I focused on divorce, and what defense mechanisms can a child have on the disintegration of the family, and how it is able to come to terms with this new reality. Also, what may happen if the child is refused to see the other parent after the divorce. I described parental alienation syndrome, which is related to this topic. Furthermore, I described the possibilities of solving problematic parent-child contact (parents agreement, court rulings and family mediation). I also defined assisted contact. The following diploma thesis deals, in accordance with its objectives and recommended primary literature, with addictive behavior in the Internet environment of adolescents, in the context of social services. The theoretical part resulted from the above mentioned primary literature, but due to its year of publication also from a variety of other up-to-date resources. The legislative frame and conditions necessary to diagnose addictive behavior. The social work with adolescents which has an addictive behavior on the Internet is primarily based on sector theories, but also points to multidisciplinary approaches. The empirical part, consisting of a questionnaire and subsequent analysis of the first grade, examines some aspects of adolescent behavior in cyberspace. The work is completed with a proposal for optimization measures.
