Best current evidence on chest physiotherapy in non-ventilated paediatric patients (0 to 24 months) with bronchiolitis : a systematic review

Title Best current evidence on chest physiotherapy in non-ventilated paediatric patients (0 to 24 months) with bronchiolitis: a systematic review. Purpose To determine the current scientific evidence for using three chest physiotherapy modalities namely percussion, postural drainage and suctioning in paediatric patients (0 to 24 months). Relevance The field of cardiopulmonary physiotherapy seems to be a neglected area in physiotherapy, with a subsequently limited evidence base. The author observed that in various clinical settings physiotherapists tend to administer routine chest physiotherapy to paediatric patients with bronchiolitis. Findings from this study may assist physiotherapists in their choice of effective treatment options. Sources The following databases were searched for evidence: African Health Line, CINAHL, Cochrane, Ebsco Host, Emerald Host, UP E-theses/dissertations, PEDro, Medline Ovid, Sabinet, Science Direct, Up To Date. Methodology This was a systematic review. The databases were reviewed by making use of a specified search strategy customised for each database. Keywords were: physiotherapy/physical therapy, bronchiolitis and paediatric/pediatric in combination with percussion, postural drainage and suction. The search yielded 10,016 study titles. Studies were chosen from the population of studies using pre-set inclusion and exclusion criteria. These criteria were applied to the titles, abstracts and full-text articles as appropriate. Five full text-articles were appraised and based on the scores from the appraisal three were included in the final sample. Data analysis Appraisal instruments from the National Health System Critical Appraisal Skills Programme (NHS CASP) and the PEDro scale (for randomised controlled trials) were used to evaluate and score the sample. Scoring was done independently by two researchers, and agreement reached through negotiation. The evidence was synthesised and graded according to the Sackett hierarchy of evidence. Results Owing to the heterogeneity of the sample, and the nature of results reported, a meta-analysis was not possible. Results from this study reveal that there is no evidence to support routine chest physiotherapy in uncomplicated viral bronchiolitis amongst the paediatric population. Chest physiotherapy does not decrease length of hospital stay, oxygen requirements or clinical scores indicating distress/morbidity. However, with secondary bacterial respiratory infections, chest physiotherapy may be indicated, depending on the assessment of each individual patient. Conclusion Percussion, postural drainage and suctioning are not effective in the management of bronchiolitis in children, newborn to 24 months old, except in individually assessed cases with secondary bacterial infection. In this subset, physiotherapy must be customised to the patient. Therefore routine physiotherapy is not indicated. Implications Chest physiotherapy should be based on a complete evaluation and on clinical merit, as well as on evidence and patient preference. Education of physiotherapy students at universities as well as doctors regarding the current evidence for chest physiotherapy in paediatric bronchiolitis is essential. Doctors and clinicians need to be made aware that routine chest physiotherapy for paediatric patients with bronchiolitis should not be prescribed. Die inligting verkry is gesintetiseer en gegradeer aan die hand van die Sackett hiërargie van bewyse. As gevolg van die heterogeniteit van die ingeslote studies en die aard van die resultate was 'n meta-analise nie moontlik nie. Resultate Die studie het bevind dat daar geen bewyse is vir roetine borskasfisioterapie van ongekompliseerde akute virale pediatriese brongiolitis nie. Borskasfisioterapie verminder nie die duur van hospitaalverblyf, suurstofbehoeftes of die kliniese respiratoriese aanduiding van stres in die pasiënte nie. In die geval van sekondêre bakteriële respiratoriese infeksies mag borskasfisoterapie egter geindikeerd wees, afhangend van die evaluasie van elke individuele pasiënt. Gevolgtrekking Beklopping, posturale dreinasie, en suiging as roetine behandeling is nie effektief in die behandeling van brongiolitis in pasgebore tot 24 maand oue pasiënte nie, behalwe individuele gevalle met sekondêre bakteriële infeksies. In hierdie spesifieke subgroep moet borskasfisioterapie aangepas word vir pasiënt. Roetine fisioterapie is dus nie aangedui nie. Implikasies Borskasfisioterapie moet gebasseer wees op 'n volledige evaluasie en kliniese meriete, asook bewyslewering en die pasiënt se voorkeure. Opleiding van fisioterapie studente by universiteite, asook dokters aangaande die huidige bewyslewering vir borskasfisioterapie in pediatriese brongiolitis is belangrik. Dokters en klinici moet bewus gemaak word dat roetine borskasfisioterapie vir pediatriese brongiolitis pasiënte nie voorgeskryf moet word nie. Thesis presents issues of development contemporary applications and follows on design principles of Computer therapy project, which compliance during the design and implementation should prevent the current issues. The aim of the thesis is creation of application for AAC of YES/NO type and i-CT framework - framework simplifying creation of similar apps for described environment. Both apps works on iOS and Android systems and were tested in an environment of people with mental disabilities.

The diploma thesis deals with the impact of family conflicts on the child from the viewpoint of social work. The thesis is divided into two parts, i.e. theoretical and practical. The theoretical part consists of three main chapters. The first chapter generally defines the family and its context. The second chapter is a fundamental one dealing with conflict in general, family conflict as such, the consequences of unresolved family conflicts, as well as methods and approaches to family conflict resolution, including family counselling and family therapy. The chapter also includes an approach of a social worker to a family where a conflict occurs or where a child is being neglected. The third chapter complements the two previous ones, briefly discusses the family in divorce, especially the role of a social worker in divorce proceedings. Furthermore, the development of the divorce rate in the Czech Republic and its family policy. The practical part deals with research focusing on experiences and views of social workers in the area of the social and legal protection of children. Přístup zaměřený na člověka a jeho místo v klinické praxi / Person-Centered Approach in clinical practice

This thesis explores the position of Person-Centred Approach (PCA) in the practice of Czech clinical psychologists and psychiatrists who went through the PCA psychotherapy training and they use this approach in their current clinical practice. The main principles of PCA theory (1), principles of biomedical model in which clinical practice is rooted (2) and the new forms of person-centred psychopathology and psychodiagnosis (3) are the theoretical background of the following research. Based on the qualitative analysis of five recorded interviews (three clinical psychologists, two psychiatrists) the experience of the respondents is organised as comprehensive concepts concerning various areas and topics of the clinical practice (diagnosis, psychotherapy, dealing with PCA, dealing with biomedical model, therapeutic relationship, relation to the profession). Several common mechanisms are identified in the concept's dynamics. Through these mechanisms Person-Centred Approach is applied in areas and activities mentioned. The concepts based on experience and the PCA application mechanisms enrich the theories and ongoing research of PCA practice in the Western mental health care. This thesis is divided into a theoretical and a practical part. The theoretical part deals with two separate realms which are then interconnected in the practical part. The first realm is narrative therapy, its description, function and implementation. One of the tools of narrative therapy is a therapeutic narration, which is discussed therein and is set into the field of social work. Next comes the second realm which is the topic of a divorce and children. There we discuss the influence of a divorce on children, the participants, and we are presented with the potentiality of how to diminish the negative influence. In the following practical part, we fist find a draft of a narration with therapeutic features from narrative therapy for children from divorced and divorcing families; the draft is supposed to alleviate the complications during and after a divorce. The other section of the practical part describes the research which verified whether the composed narration really helped alleviate the negative impact of a divorce on children. Evaluation of the state of the problematic areas performed both before and after the cooperation showed that narrative therapy helped to significanly decrease the number of serious problems in monitored areas. In preclinical research on Alzheimer's disease pharmacotherapy, attention is paid to multipotent compounds, enabling intensification of the effect by targeting multiple pathophysiological mechanisms. The aim of the thesis was to assess the effect of multipotent compounds and combination therapy in models of cognitive deficit in the rat. The mechanism of action of the tested compounds was modulation of neurotransmitter systems. The aim of the first part of the study was to compare the effect of experimental monotherapy and combination therapy with an N-methyl-D-aspartate (NMDA) receptor antagonist and a γ-aminobutyric acid type A (GABAA) receptor positive modulator in the trimethyltin-induced model. Superiority of the combination therapy was proven by histological analysis of hippocampal neurodegeneration; however, it did not reach statistical significance in the cognitive test. The other part of the thesis focused on multipotent tacrine derivatives. We demonstrated a positive effect of 6- chlorotacrine-6-nitrobenzothiazole hybrid, as well as 6-chlorotacrine-L-tryptophan hybrid, acting as acetylcholinesterase inhibitors, in the scopolamine-induced model of cognitive deficit. Spa is located on the outskirts of the Swiss city of Yverdon. Area is surrounded by disparate buildings especially residential character. There are historical buildings on the land and buildings of the spa and hotel. The current spa and the outbuilding of hotel don´t meet requirements, that´s why the buildings will be replaced by a new complex. Neighbourhood doesn´t have any relationship with the spa, that´s why the whole land is surrounded by a wall. Water surface with three “floating” pavilions is inserted into this closed spa park. There are the public spa, the therapeutic spa and the part of the hotel on the water surface. Each of them works largely independently; they are connected by the water surface. The complex is separated by a part of shallow water from the park. It´s solitaire in the park, but it has close connection with it. The spa complex is located near the forest on the south part of the land, because the whole system is quite open.

The aim of this work is describe common methods of biological feedback therapy that is used to treat some psychosomatic diseases. Subsequently, the description is focused on minimal brain dysfunction treatment by the help of EEG biofeedback. Properties and technical requirements for this therapy are concretized. The last part of this thesis is dedicated to the design and realization of practical software tool for EEG biofeedback therapy which is made in LabView 7.1. The M535 acquisition unit and NI USB-6221 measuring device are used for hardware solution.

The purpose of this study was to describe the use of therapeutic horse riding in Gestalt therapy and its influence on the awareness levels of the child with Fetal Alcohol Syndrome that present with Attention deficit/Hyperactivity disorder. For the purpose of this study a quantitative research approach with a descriptive nature was used. Semi-structured interviews were used to compile information about a single child in a therapeutic milieu. This information was combined with information gathered from the literature to compile guidelines for the counsellor. In this study horse riding was effectively combined with Gestalt play therapy to enhance the awareness levels of the child with Fetal Alcohol Syndrome. / Die motivering van hierdie ondersoek was om diergefasiliteerde terapie tot voordeel van die kind met Fetale Alkohol Sindroom te benut. Die doel van die studie was om die benutting van terapeutiese perdry in Gestaltspelterapie en die invloed daarvan op die bewustheidsvlakke van die kind met Fetale Alkohol Sindroom wat met Aandagafleibaarheid/hiperaktiwiteitsindroom presenteer te beskryf. Vir die doel van hierdie studie is kwalitatiewe navorsing met 'n beskrywende aard benut. Semi-gestruktureerde onderhoude is gebruik om inligting in te samel rakende 'n enkele kind in 'n terapeutiese milieu. Hierdie inligting is gekombineer met die inligting uit die literatuur ten einde riglyne vir die berader op te stel. Neurological disorders affect more than 14% of the population worldwide and together with traumatic brain and spinal cord injuries represent major health, public and economic burden of the society. Incidence of inherited and idiopathic neurodegenerative disorders and acute CNS injuries is growing globally while neuroscience society is being challenged by numerous unanswered questions. Therefore, research of the CNS disorders is essential. Since animal models of the CNS diseases and injuries represent the key step in the conversion of the basic research to the clinics, we focused our work on generation of new animal models and on their use in pre-clinical research. We generated and characterized transgenic minipig model of Huntington's disease (HD) which represents the only successful establishment of a transgenic model of HD in minipig which should be valuable for testing of long term safety of HD therapeutics. Next, we crossed the well characterized R6/2 mouse HD model with the gad mouse model which lacks the expression of UCHL1 which led to results that support the theory of "protective" role of mutant huntingtin aggregates and suggest that UCHL1 function(s) may be affected in HD disturbing certain branches of Ubiquitin Proteasome System.
