Aktivizace seniorů s demencí v pobytových zařízeních / Activating the elderly with dementia in residential homes

In the theoretical section we have focused on the process of ageing and made a geographical and statistical assessment. The base was our country's legislative framework for providing care to the elderly with dementia, the issue of the illness of dementia, its causes, as well as the approach and care provided to people with dementia. We drew from the terminological definitions and theoretical resources for employing elderly people with dementia. We have made a note of the philosophical and psychological source. We lay emphasis on information concerning the possibilities of employing elderly people with dementia and their activation. We have followed theoretical resources which, in employing these people, are especially constructive for the users, providers and above all for the family. In all the items observed we respected the limit given by the diagnose of dementia. In the practical section, we have paid attention to already existing activities employed in residential homes for the elderly with dementia. We give a list of some types of homes where people with dementia are placed and where a survey was made. Efekt konzervativní terapie u deformity hallux valgus / Effect of conservative therapy to deformity hallux valgus

This thesis focuses on the evaluation of a conservative approach to the hallux valgus deformity. 21 people took part in the empirical study in which the changes were tracked. I worked with a group of women who exercised regularly and a control group. The regularly exercising group participated in a 3-month therapeutic program led by a physiotherapist. Each of the participants was properly examined at the beginning of the program and after three months, when the program finished. Each examination consisted of a kinesiologic analysis, a plantography footprint, a foot X-ray, a pain evaluation (the numeric scale was used) and filling out a scoring scale questionnaire of American Orthopaedic Foot and Ankle Society (AOFAS). The changes of pain of the MTP or IP toe joint, the development of foot arch, changes of the toe valgus angle according to the X-rays and the overall score achieved in the AOFAS questionnaire were statistically evaluated. The ability to isolate toe abduction, right posture of the foot, stability of the axial joints and other changes were followed as well. Studium genové exprese faktorů signální dráhy oxysterolů u pacientek s karcinomem prsu / Gene expression study of oxysterol signal pathway in breast cancer patients

Hormonal therapy is a common part of breast carcinoma treatment in patients whose tumors express estrogen and progesterone receptors. The aim of hormonal therapy is to prevent proliferative effect of hormones througt their receptor proteins in order to inhibit tumor growth. However, certain number of tumors is resistant to hormonal therapy despite expression of hormonal receptors. Presently, the reasons of this resistance are not fully understood. Oxysterols are hydroxylated cholesterol derivates, which may play some role in development of the resistance. They may interfere with hormonal therapy effect and influence some signal pathways leading to cancer progression. This study comes with results of gene expression of proteins influenced by oxysterol action, metabolic and transport proteins, transcription factors and members of signaling pathways that may be related to oxysterol effect. This thesis identifies some candidate genes for future analysis on the basis of comparison of gene expression between estrogen receptor positive and negative tumors and correlation with clinopathological data. The work also describes the effect of stress on human health and shows the importance of psychosomatic view of the individual. It also aims to look abroad and shows two studies that examine the effectiveness of craniosacral therapy. The practical part aims to obtain empirical data using the questionnaire.

Background: More detailed usage of incentive therapy (IT) in the Czech Republic is still unknown. On the contrary, theme of drug issues among female users and issue of usage of drugs during their pregnancy period is becoming much more actual in the field of addictology. Particularly, the public is prejudiced and shows stereotyped behaviour against drug pregnant addicts. The first attempts to combine IT and the specific group of drug pregnant addicts have been started by low-threshold facilities like XTP Sananim in Prague and Centrum U Větrníku in Jihlava. Objective: The aim of this thesis is to map the current situation of IT in that facilities which engage in programmes for drug pregnant addicts based on the IT. Methodology: The data were gained by using semi-structured interviews whose respondents work in low-threshold facilities. In the analysis elements from the theory of addictology were utilised. Results: On the basis of statements of interviewees was found that two chosen facilities has clearly defined program of IT and the structured working course with their clients. The both differ in terms of remuneration clients. The thesis with the topic of proposing new therapeutic materials for clients diagnosed with aphasia brings more profound understanding of separate field of speech therapy, which is aphasiology. The first part of the thesis introduces the theoretical basis to the issues of aphasia, concentrates on fundamental definition of aphasia, etiology and symptomatology. Moreover, the thesis describes the possibilities of aphasia therapy and lastly, it covers the differential diagnostics of aphasia. The second part of the thesis covers in detail the Boston Classification System of Aphasia, introduces to the reader the foundations that were the basis for the propositions of the therapeutic materials. The second part describes in detail the very process of designing therapeutic materials in general and it also provides concrete propositions of such materials with possible applications. The appendix includes examples of proposed materials, both theoretically explained and physically created for the reader to examine.

Title: Flexor tendon pulley lesion as a most frequent injury in sport climbing Specification of the problem: This thesis is focused on the most frequent injury of the hand in sport climbing. It describes the mechanism of the injury and possibilities of conservative and surgical treatment according to the given guidelines in dependence on its relevance. Objectives: The aim of this thesis is the literary review of accessible literary resources from the field of anatomy, kinesiology and biomechanics of the hand and flexor tendon pulley in connection with a sport activity. Furthermore it is aimed on an analysis why is the flexor tendon pulley injury the most frequent injury in sport climbing, and on methods how to treat this injury in the most effective way from the beginning till the return to the full performance based on therapeutic guidelines created by Dr. Schöffl nad Dr. Hochholzer. This thesis should serve as a therapeutic manual for the Czech sport climbers suffering from this type of injury, which is generally very often underestimated and not treated sufficiently. Secondly describes dyslalia, its etiology, classification, diagnosis and prevention. Mostly this thesis deals with dyslalia therapy and its adults specifics. It gives examples of usable therapeutic materials in the Czech Republic also abroad. Thesis also judges the situation of adult's therapeutic materials on the market. The main intention was to create a therapeutic material for adults with dyslalia diagnosis. It was created 100 worksheets for frequently disturbed Czech sounds (L, R, Ř, Č, Š, Ž, C, S, Z, ČŠŽ / CSZ). Adéla Dolejšová Testing the efficacy of potential therapeutics for Alzheimer,s disease Charles University in Prague, Faculty of Pharmacy in Hradec Kralove Pharmacy The aim of this thesis was to find out whether already implemented Ellman's method, which is used to analyse irreversible inhibitors AChE, is going to be a suitable technique for measuring reversible inhibitors. Furthermore, the efficiency of newly synthesized AChE inhibitors was established. These inhibitors will be used for treating AD or as prophylaxis against neural paralytic substances. Moreover, it was compared affect the efficacy of AChE inhibitors after intramuscular and intraperitoneal administration. The measurements were done in vivo on potkan species Wistar. In the first experiment standard AChE (tacrin, 7-MEOTA, donepezil, rivastigmin) inhibitors were applied to verify the method. In the second experiment the newly synthesizes AChE (K 298, K 344, K 474) inhibitors were medicine. The results confirmed the effectiveness of the tested method on commonly used inhibitors such as donepezil and rivastigmin. Out of the newly synthesized K 298, K 344 and K 474 inhibitors none was proven to have any significant inhibiting activity. This thesis is focused on therapeutic communities in the treatment system of drug addicted clients and on their methods, programs and mission of employed therapists to achieve better restoration of the drug addicted clients. The aim of this work is to widely describe the treatment process in therapeutic communities and zoom in an enviroment and whole treatment system which creates safety place for each member of the community. The thesis is divided into a theoretical and practical part. Theoretical part describes today's czech experience in therapeutic communities, whereas the importance is laid on introducing therapeutic methods and programs effectively used in social work with drug addicts.Practical part forms own qualitative research, precedented with intership in the therapeutic community Karlov.
