Profylaktická antidota proti nervově paralytickým látkám v České republice / Prophylactic antidotes against nerve agents in Czech Republic

Chemical warfare agents can be currently considered a high risk to humans. Group of nerve agents, is considered the most dangerous. These highly toxic substances have a rapid onset of action, the organism will receive all the entry gates, therapy intoxication is always difficult and death occurs often. The body's resistance to the effects of nerve agents at the expected intoxication increases not only prophylactic administration of antidotes , eg. when working in a contaminated environment after a terrorist attack, but also potentiates the subsequent antidotal therapy after exposure. This thesis is dedicated to gain in depth understanding of nerve agents with aspect of pharmacological prophylaxis. The introduction presents combat toxic substances in general, differentiation due to its effect on human with possible damage to the organism and brief characteristics of every basic type of nerve agents with examples included. Further on the theoretical part focuses on issues of nerve agents. The history of their development and well-known applicationsis breifly mentioned, respectively, abuse in the world. The thesis mentions of the Conventions on the prohibition of chemical weapons in connection with the misuse. Thesis continues dividing type nerve agents, describing the mechanism of action, acute toxicity and its symptoms, diagnosis and therapy of poisoning. Degree, extent and rate of damage to the body caused by nerve agents depends on many factors. A major factor which influences the ultimate toxic effect are taken input. Body damage can be caused, for example. Inhalation administration, contamination of broken or intact skin, ingestion, or hitting the conjunctiva. Since they have nerve agent toxicity and high speed action, protective and preventive measures are an important element to prevent the penetration of substances into the body. The Army of the Czech Republic ensures the protection of personal protective equipment, decontamination of affected skin and pharmacological prophylaxis. Another part of this work is the term prophylaxis and related topics, especially pharmacological prophylaxis. Directions pharmaceutical prophylaxis currently can be divided into three directions - protection against acetylcholinesterase inhibition, administration of antidotes and normal use Scavengers called. "Scavengers". The body's resistance to the effects of nerve agents at the expected intoxication not only increases prophylactic administration of antidotes, but also potentiates the subsequent antidotal therapy after exposure. Prophylactics PANPAL and TRANSANT are in equipment of Czech Army, in the work is devoted more attention to them. Both of these products were developed in the Czech Republic. There are also presented antidotal remedies of first aid, which are available for the Army of the Czech Republic. The practical part of this thesis aims to compare the prophylactic antidotes in the Czech Republic and in selected countries of NATO from four perspectives - onset, prophylactic activities, application forms, and adverse effects. An analysis of the available Czech and foreign professional articles, publications and studies and the subsequent comparative analysis of the various aspects and evaluate various selected countries is used to achieve this goal. These resulting data then point to the position of the Czech Republic in the field of prevention against intoxication by nerve agents - The Army of Czech Republic that has put in service two prophylactic antidotes that achieve the best results investigational products. It may therefore be said that the Army of the Czech Republic currently has a combination of prophylactic best in the world, which is relatively free of side effects has easy application and an unnecessarily long duration of action. This result is even answer the research question - Prophylactic antidotes in the Czech Republic are comparable according to the given parameters with antidotes in selected countries.

The theoretical part of the thesis deals with the life style, regeneration, and relaxation with the focus on the application of cold to treat the elderly. The work explains the use of cryotherapy from both historical and scientific points of view. The aim is to introduce the method of cryotherapy as a popular method which postpones the beginning of dependency and motion impairment with people aged over 50 and thus enables them to keep active much longer. Although the 21st century medicine has reached an extraordinary success, the number of illnesses does not decrease. There is a wide variety of civilisation illnesses resulting from the wrong life style. For such illnesses drugs with frequent side effects are prescribed. The work introduces cryotherapy as one of the methods which can serve the health protection, prevention, the treatment of traumatic injuries, and the supportive therapy of different health problems with relatively no side effects.

Upbringing and education children with cephalonia The children with cephalonia develop slowly but they are very sensitive and perceptive. Their upbringing and education is very discussed problem at present. Every men is an unique personality and every men is entitled to the full-value life with the education and the love and it applies also to people with cephalonia. The main subject of the diploma thesis was to describe the general information about the cephalonia (aetiology, classification, possibilities of education of people with cephalonia). The practical part of the diploma thesis has two goals. The first goal was to find out and describe the situation of skilled employees in the educational institutions for children with cephalonia. The second goal was to find out and describe the level of the special pedagogic care of children with cephalonia (especially using of alternative therapies). Both of goals were achieved. The diploma thesis verified two existing hypothesis together. The basic technique of data collection was the questionnaire for teacher in the educational institutions for children with cephalonia in the Zlín region. The secondary analysis of the methodical materials and the educational programmes in the special schools in Zlín region was the next used method. The hypothesis no.1 (There is lack of skilled employees in the educational institutions for children with cephalonia) was overcame. The most of interviewed special pedagogues had requisite qualification according to law and the others studied for this qualification by their employment. The hypothesis no.2 (The alternative therapies are not using enough in the educational institutions for children with cephalonia) was confirmed. According to the secondary analysis of the methodical materials the particulars of the alternative therapies are using because these particulars of therapies are parts of the curriculum for children with special needs. But the research results that quarter of informants don{\crq}t use the alternative therapies and they don{\crq}t have the appropriate qualification for it. The results of this diploma thesis could have been using as an information brochure for parents and teachers of children with cephalonia.

Title: The effect of manual therapyof the cervical spine on standingbalance of violinists measured by CDP Objectives: The intention of this thesis is to describe most frequent functional changes of musculoskeletal apparatus of violinists. Secondly, to track and react to these changes with appropriate manual there of the cervical spine. Thirdly, to find out if this therapy has an effect maintaining postural stability by testing on a posturograph using the SOT protocol. Methods: This is a pilot study that uses subjective and objective examinations. Subjectively, the functional changes of the musculoskeletal apparatus were examined and then their frequency within the proband group was evaluated (n = 10, 2 men and 8 women). Within the objective examination, 15 parameters of postural stability were monitored under certain conditions. Using the Wilcoxon Pair Test, situations before and after manual therapy of the cervical spine were compared. The statistical significance level was set to α = 0.05 for this test. Results: The study has shown that violinists have some functional changes in the musculoskeletal apparatus in common. Title: The influence of the dance movement therapy on psychomotor symptoms of Parkinson's disease patients Objectives: The aim of this thesis is to evaluate the effects of dance movement therapy on postural instability and other motor and non-motor symptomatology of patients with Parkinson's disease. Changes are evaluated by using MDS-UPDRS and Computerized Dynamic Posturography (NeuroCom Smart EquiTest System). Another goal is to provide comprehensive information about dance movement therapy and other dance therapies promoted abroad and to highlight a possibility of their use as a non-pharmacological treatment of Parkinson's disease symptoms. Methods: This pilot study involved eight late onset Parkinson's disease patients at an average age of 68,87±6,69 years. Changes in their postural stability and motor and non-motor symptoms of Parkinson's disease were evaluated after the completion of two months of intensive dance movement therapy. Observed parameters were evaluated by using MDS- UPDRS and the Computerized Dynamic Posturography Smart EquiTest System created by the NeuroCom company, before and after the therapeutic programme. The thesis is focused on nutrition of a preterm infant at an intensive care unit from the point of view of nursing activities. A nurse is faced to problems resulting from prematurity state like insufficient sucking, poor coordination of sucking and swallowing, she enters into interaction with parents who are afraid of the life and subsequently of the future of their beloved child. Natural nutrition, preferably with breast milk is irreplaceable in spite of the high development of initial formulas. This is why the mother of the child, who is vital for the newborn?s nutrition, has to be invited into the team. A hundred per cent enteral nutrition is not always possible, parenteral method has to be applied as well. Provision, evaluation and optimization of infant?s nutrition are results of activities of the whole team, i.e. of a nurse as well. The theoretical part of the thesis contains the findings of trophotherapy of a preterm newborn. It deals with parenteral as well as enteral alimentation, its composition and administration techniques. The empiric part lists and evaluates the research results that led to replies to research questions related to nurse?s contribution to trophotherapy of a preterm newborn, to the influence of permanent nasogastric tube on breathing and mucus congestion of the nose as well as to the awareness of nutrition problems among mothers of preterm infants. Measures resulting from the research results and recommendation for further examination are proposed at the end of the thesis. A brochure Preterm Baby Nutrition designed for distribution among mothers at neonatology and perinatology departments and Recommendations for Gastric Tube Introduction belong to the outputs. We should mention that the latter is based on procedures already presented. It was extended by recommendation of the place of introduction with regard to further intention of enteral nutrition administration.

Practical part investigates the influence of systemic therapy and psychotherapy on psyche of people with mental disease and instruments used for systemic therapy of individuals with mental disease. Results of this research show positive influence of systemic therapy and psychotherapy on individual with mental disease.

Autorské herectví a jeho proměny v rámci terapie závislých / Authorial Acting and its changes during the therapy of addicts

In my dissertation, I try to reflect my own experiences with application of authorial approach to acting in therapeutic community for drug addicts. I work as a therapist for twelve years in therapeutic community Magdaléna. I guarantee nonverbal technique program and I lead a theatre project once a year periodically.My dissertation is divided to two main parts. First I focus on questions about authorial acting and its pedagogics. I try to explain specifics that are brought by authorial approach to acting. I also describe acting with the inner partner and drama propaedeutic based on free playing and collective improvisation. These dramatic experiments help participants during their own building of psychosomatic condition, which is core of studies on Authorial acting and pedagogics department, DAMU in Prague.Second part of my dissertation is dedicated to my own work with drug addicts. I try to describe problems of therapy in community, especially the questions connected with dramatic expression of the clients. As I told before, I lead nonverbal technique program in Magdaléna community and I created my own conception of such program and it is described enough in my dissertation. I also try to explain drama projects preparations in community and show such cooperation in eleven specific performances we did with different clients during the time.Finally, I reflect relationship between authorial acting and psychotherapy. I perceive the ability to act creatively in direct contact with the others as the most important thing in both cases. That is why I demur at the question of a self-experience in therapy and psychosomatic condition in authorial acting and their mutual relationship.
