A qualitative bias review of the adaptation of the WAIS-III for English-speaking South Africans

In response to the growing demand for a test of cognitive ability for South African adults, the Human Sciences Research Council (HSRC) adapted the Wechsler Adult Intelligence Scales, third edition (WAIS-III) for English-speaking South Africans. However, since the publication of the South African adaptation in 2001, there has been some concern whether some bias may not still be present in the measure. Consequently, this study aimed to qualitatively explore the item content of the South African adaptation of the WAIS-III in order to identify items that might still be potentially biased with specific reference to cultural and linguistic issues. The researcher employed purposive sampling, a non-probability technique, to select psychology professionals to act as expert reviewers. The sample consisted of 20 registered psychologists, psychology lecturers and psychology interns from the predominant culture and language groups of the Eastern and Western Cape. Qualitative and quantitative data were collected by means of a Bias Review Questionnaire which consisted of fixed-choice questions, as well as sections in which specific comments could be indicated. The questionnaire was distributed to 20 selected participants who were qualified to administer psychometric measures and were registered with the Health Professionals Council of South Africa (HPCSA). Qualitative data was analysed using Tesch’s model of thematic content analysis, and five main themes relating to potential bias were identified, namely: Culture, Language, Education, Socio-economic status, and Format. The qualitative data was re-submitted to the expert review group for approval and further comment. Quantitative data obtained from the Bias Review Questionnaire was analysed by obtaining frequency counts, which were converted to percentages, per subtest and aspect reviewed. The major findings of the study were as follows: 1. Cultural bias is evident in the South African adaptation of the WAIS-III. The differential meanings ascribed to concepts, terms, phrases and words by diverse cultures impact on test performance, as does familiarity with stimulus materials used in the measure. 2. Language bias was detected against groups with English as their second or third language owing to the unfamiliar, and sometimes archaic, terms and vocabulary used in the measure. The intended meanings of the translated items are unclear and confusing for test-takers. 3. South African test-takers have experienced vastly divergent educational opportunities which impact on intelligence test performance. The measure contains potential bias against individuals from disadvantaged educational backgrounds. 4. The content of the items is not relevant to test-takers from deprived socioeconomic backgrounds that have been unable to access good-quality education. 5. Format bias was found to exist with respect to the timed tests. Cultural groups who do not value speededness are disadvantaged by the structure and format of the measure. Based on the findings of this study, certain recommendations were made. These include: 1. Additional qualitative investigation of the WAIS-III , with a view to further adaptation in an effort to eradicate bias on cultural or linguistic grounds, and 2. Conducting further Differential Item Functioning (DIF) analyses and establishing construct equivalence across cultural and linguistic groups to promote this objective. Among the contributions believed to have been made by this study are the identification of potentially biased subtests and items on the grounds of culture and language, which will facilitate possible further revision and adaptation of the measure. Additionally, the variables that were considered important influencing factors of cognitive test performance have been documented and are accessible for future research that will supplement the present study.

An assessment of the implementation of the provincial cervical screening programme in selected primary health care clinics in the Ilembe region, KwaZulu-Natal

A mini-dissertation submitted in partial compliance with the requirements for a Master's Degree in Technology: Nursing, Technikon Natal, 2002. / Cervical cancer is almost completely preventable, yet it is the second most prevalent cancer amongst women in South Africa. KwaZulu-Natal (KZN) in particular has a high mortality rate of cervical cancer and 1:40 women die from cancer of the cervix. Therefore, in 1997 a cervical screening. policyand-. programme was implemented in the province. The KZN Department of Health and the Sub-Directorate Maternal, Child and. Women's Health needed to know what was happening currently in terms of implementation of the cervical screening programme since it was first implemented three years ago. Therefore, the purpose of the study was to evaluate the implementation of the Provincial Cervical Screening Programme in selected Primary Health Care clinics in lIembe Region, KZN. This study took the form of formative evaluation research. The target population consisted of PHC clinics in KZN that have implemented the cervical screening policy and the programme. The accessible population for this study consisted of the clinics in the lIembe Region. A four-stage selection plan was applied to select the sample from the accessible population. The first stage involved a random selection of two clinics from an urban area and two from a rural area. Within each of the selected clinics, three types of evidence for the evaluation of the implementation of the cervical screening programme were sampled. Therefore, the second stage of the plan was the selection of records. A purposive sample of all records of clients who were diagnosed with abnormal smears was assessed. The third stage involved the selection of all Professional Nurses from each of the selected clinics. Lastly, the fourth stage involved the selection of the day for collecting data on the facilities and resources. The sources of evidence that were used to evaluate the implementation of cervical screening programme by the clinics were non-participant observation, which involved clinic audit, a review of abnormal smear records and self-reports from nurses regarding the cervical screening programme. Results indicated that there was a lack of resources needed for implementing the programme in rural clinics compared to urban clinics. However, all clinics in the study had an adequate supply of the drugs needed for the treatment of abnormal smears. The researcher also found that nurses lacked knowledge regarding the indications for taking smears. On reviewing the records, the researcher noted that most of the results indicated that smears had adequate cells needed for analysis. However, the results indicated that there was a problem with follow-up of clients with abnormal smears. There was lack of necessary resources such as telephones needed to do proper follow-up. Feedback to the clinics from the referral hospital regarding the outcome of the visit was inadequate. The results also indicated thatthe mechanisms of record keeping were poor. Nurses were of the opinion that women should have their first Pap smear at the age of 20 and thereafter at intervals of five years, once they start to be sexually active because of the high rate of sexually transmitted infections and HIV/AIDS in KZN. Therefore, the above results indicate that problems exist at the selected PHC clinics that may result in ineffective implementation of the cervical screening programme. / M

An in-depth exploration of the personality structure of adult female psychiatric patients with a history of childhood trauma by utilising personality assessment

The experience of complex childhood trauma produces a ripple-effect that psychologically impacts trauma survivors’ functioning in multiple areas. The aim of the current study was to investigate and describe the interplay between complex trauma, subsequent personality development and later psychopathology by means of the multiple case study method of six female psychiatric patients attending treatment at a tertiary psychiatric hospital within Gauteng, South Africa. This was accomplished by assessing and qualitatively analysing the results of a carefully selected battery of personality and other psychometric assessments presented to study participants. The results indicated that the experience of complex childhood trauma impacted the study participants’ personality in predictable ways, which further influenced the psychopathology they displayed as psychiatric patients. These findings aid in describing the psychological impact of complex trauma on the research participants, and also offers support for reconnecting past traumas to the current psychopathologies of psychiatric patients. / Psychology / M.A. (Clinical Psychology)

Human Immuno-Deficiency Virus (HIV) and Acquired Immune Deficiency Syndrome (AIDS) have many devastating impacts, including the growing number of AIDS-orphans left without care. In the study, semi-structured interviews, incomplete sentences and Kinetic family drawings were used to explore the life-experiences and perceptions of AIDS-orphans living in kinship foster care, from a Gestalt perspective. The study revealed that the majority of the AIDS-orphans included were making good contact at the contact boundary and all the respondents were not familiar with the term 'foster care'. The researcher therefore recommended that children are educated about foster care at school, thus possibly increasing the awareness of foster care in South Africa, thus possibly increasing the provision of homes for the vastly growing numbers of AIDS-orphans in our country and perhaps make a difference in society. / Sociology / M.
