A life skills programme for early adolescent AIDS orphans

In this study an attempt was firstly made to define, describe and explicate the phenomenon of HIV/AIDS providing a basis for understanding the multidimensional nature, key characteristics and impact of HIV/AIDS in terms of its background, the current status as well as the future of the epidemic. Literature concerning HIV/AIDS in general, global and in particular the South African situation was discussed. Secondly the concept AIDS orphans was investigated after which grounding, description and explanation of the problems and needs of AIDS orphans were presented in order to give a clear picture of challenges faced by these children. Problems of orphan-hood such as legal and ethical issues, socio-emotional issues, educational issues, financial issues and child-headed households were identified. The study focused on early adolescent AIDS orphans therefore adolescence, as a life phase with specific emphasis on early adolescence was reviewed. Hereafter, the researcher presented a newly self-developed life skills programme for early adolescent AIDS orphans (i.e. AIDS ORPHANS LIFE SKILLS PROGRAMME) followed by all the empirical research findings, a general summary, conclusions and recommendations. The broad aim of the study was to develop and empirically test the effectiveness of a life-skills programme for early adolescent AIDS orphans. Two research questions and a hypothesis were formulated for the study. The research questions included: (a) what is the nature and prevalence of socio-emotional needs and problems of early adolescent AIDS orphans? (b) What are the life skills needed by early adolescent AIDS orphans? Accordingly the hypothesis of the study read: If early adolescent AIDS orphans undergo a life-skills programme then their skills will be enhanced in order to cope better with their socio-emotional needs and problems. In the context of applied research the type of research conducted in this study was intervention research. This type of research was relevant for this particular study because it is a problem-solving process seeking an effective intervention programme for the promotion of life skills for early adolescent AIDS orphans. In view of the fact that the AIDS orphan situation is a crises for the whole nation innovative preventative positive educational programmes for children orphaned by AIDS are deemed pivotal. The focus of this research study was two-folded using a combination of quantitative and qualitative methods. The first phase of the study was qualitative and explorative in nature. The aim of the researcher was to have a broader understanding of the phenomenon HIV/AIDS, the socio-emotional needs and problems of and life skills needed by early adolescent AIDS orphans in South Africa. The focus of the second phase was to develop a life skills programme for early adolescent AIDS orphans, based on the information collected in the first phase of the study and then to empirically test the effectiveness of the newly developed life skills programme. The researcher used semi-structured interviews with a schedule to collect qualitative data during the first phase of the research. During the second phase, the researcher utilised a self-constructed group administered questionnaire to collect quantitative data before and after implementation of the life skills programme (pre-test and post-test). In order to explore the socio-emotional needs and problems of and life skills needed by early adolescent AIDS orphans, a phenomenological design seemed appropriate. The research design was selected to reach the first three objectives of the study, namely: To conceptualise theoretically the phenomenon of HIV/AIDS and AIDS orphans, the specific characteristics, needs and problems of early adolescents as well as life skills for early adolescents; a) To explore and identify the nature and prevalence of socio-emotional needs and problems of early adolescent AIDS orphans; b) To explore and identify the life skills which AIDS orphans, in their early adolescent phase need to improve their coping capabilities; Qualitative data through semi-structured interviews with a schedule was collected. The sample thus included 40 respondents i.e. 10 social workers, 10 caregivers and 20 AIDS orphans. The empirical research findings based on the first part of the study confirmed that HIV/AIDS has forced vast numbers of children into precarious circumstances, putting them at high risk of becoming infected with HIV. AIDS orphans are especially vulnerable to HIV infection for a host of social and economic reasons including poverty, sexual exploitation, violence, and lack of access to HIV information and prevention services. The consequence of this is that children are often socially isolated and deprived of basic social services. The findings further confirmed that there are currently no life skills programmes specifically designed for early adolescent AIDS orphans in South Africa. Deficiencies in life skills contribute to the vulnerability and exploitation of these children. Life skills were viewed as crucial in improving the quality of life of AIDS orphans. Life skills can enable adolescents to develop sound and positive view of life. The researcher also applied the comparison group pretest-posttest design (i.e. a quasi-experimental comparison group pretest-posttest design) with respondents to reach the last three objectives of the study, namely: a) To develop a life-skills programme for early adolescent AIDS orphans; b) To empirically test the effectiveness of the developed life skills programme for early adolescent AIDS orphans; and c) To suggest practical recommendations for further utilisation of the newly developed life skills programme for early adolescent AIDS orphans. The researcher developed a life skills programme for early adolescent AIDS orphans namely AIDS Orphans Life Skills Programme. The evaluation of the self-developed life skills programme for early adolescent AIDS orphans was done by a self-constructed group administered questionnaire in the pre-test i.e. before implementation of AIDS orphans life skill programme, and post-test with both the experimental (30 respondents) and comparison group (30 respondents). The sample thus included a total of 60 early adolescent AIDS orphans and the empirical data was collected to include 2 measurements once before and once after the intervention (AIDS orphans life skills programme). The findings confirmed that there was a statistical significance difference in the experimental groups life skills (i.e. sense of identity and self-esteem, communication, assertiveness, self-awareness, coping and stress management, decision making, problem solving, conflict management and a healthy life style) with a 95% chance that the results were due to AIDS Orphans Life Skills. There was not statistical difference in the experimental groups critical and creative thinking skills. Nine out of ten key elements of AIDS orphans life skills programme were thus successful in that they promoted life skills amongst early adolescent AIDS orphans. An exploration of the roots of resilience among HIV and AIDS-orphaned children

South Africa is grappling with the AIDS pandemic that increases the numbers of vulnerable orphans, whose resilience is threatened. Little is known about the processes that enable resilience among these orphans. This qualitative study explored the roots of resilience among 23 AIDS-orphans. The Draw-and-write technique was used to collect data. The participants were asked to make drawings of what enabled them to cope resiliently and to write short narratives explaining their drawings. The drawings were grouped according to the dominant themes. A content analysis of the drawings and narratives was done. Eight themes relating to the roots of resilience among the participants emerged, namely, the participants had active support systems, participants received religious and/or spiritual support, participants had access to social services, books and school attendance changed the participants’ lives, having access to safe a home enhanced resilience, receiving inspiration, having a positive self-image and personal dreams, and physical activity was used to achieve catharsis. Sexual and Reproductive Decisions and Experiences of Women Living With HIV/AIDS in Abuja, Nigeria

Over 60% of those living with HIV/AIDS are women, the majority of them in their sexual and reproductive years (UNAIDS, 2006). With antiretroviral (ARV) drugs, most of them are living longer and healthier to engage in sexual and reproductive activities (WHO, 2006). This study explored the sexual and reproductive decisions and experiences of women living with HIV/AIDS (WLWHA) in Abuja, Nigeria. Only those who became pregnant and had a child after being diagnosed with HIV participated in the study. The study was an interview based qualitative research. The design of the interview guide was informed by the PEN-3 Model (Airhihenbuwa, 1995). A combination of purposive and snowball sampling technique was used to select 17 WLWHA aged between 26 and 41. Most of them had limited education, only the 3 of them with post secondary education had professional jobs; the rest are either housewives or petty traders. Most of the women had reduced sexual desire but felt compelled to acquiesce to their husband’s sexual demands out of cultural and religious sense of duty, fear that he would have sex outside marriage and/or beat them. While a few used condoms, most either did not use condom or used it inconsistently. Condoms were used mainly to prevent re-infection with another strain of HIV or to prevent infecting a negative partner. Reason for non use of condom includes reduced sexual pleasure with condoms, belief that condoms are used not for wives and that being on ARV precludes the need to use condom. Often, condom negotiation leads to violence. Most of the women still wanted more children and did not use contraceptives. Among the few who used contraceptives, condoms, hormone injections, intrauterine device (IUD) and tubal ligation are their methods of choice. The women chose to have babies to secure their marriage, fulfill maternal instinct and to “leave something behind”. Their decisions were informed by the belief that ARV would keep them alive, while Prevention of Mother to Child Transmission (PMCTC) programs would prevent infection to their babies. To conceive, they had unprotected sex during ovulation. Two serodiscordant couples used syringes to inseminate. Most of them had experienced obstetric challenges including infertility, miscarriages, preterm births, and infant deaths. Most of the women bottle-fed to prevent infecting their babies, however they were under tremendous pressure to breastfeed due to the cultural value attached to breastfeeding. Women living with HIV/AIDS in Abuja Nigeria, had unmet sexual and reproductive health needs. Their sexual and reproductive decisions were influenced by their individual circumstances, including their level of education, poverty, cultural and family influences, partner’s HIV status, stigma and discrimination, and access to PMTCT and ARV programs. To meet the sexual and reproductive health needs of WLWHA, program planners and policy makers should take these factors into consideration and ensure that programs are comprehensive and integrated.

Statistics on HIV/AIDS are alarming. Very little is known about how communities are actually coping with this disease and what methods seem to be working to empower them to deal with it. Psychosocial distress is one of the dimensions of the impact of AIDS on children and families, and stresses the necessity to enhance the capacities of extended families and friends to be able to deal with these psychosocial issues. Using qualitative research and a case study as the strategy of inquiry, this dissertation of limited scope explores and describes the factors influencing the capacity of extended families in providing psychosocial support to AIDS orphans. A literature study was done to give a theoretical overview on the following aspects: * AIDS orphans * The extended family and HIV/AIDS To reach the first objective of this study, an empirical study was done and semi-structured interviews were used to obtain information from four extended families in the Mukwe area of the Kavango Region in Namibia. The literature study and the empirical study enabled the researcher to draw conclusions on the factors which influence the capacity of extended families to provide psychosocial support to AIDS orphans. Exploring the feasibility of foster care as a primary permanency option for orphans

In the wake of the HIV/AIDS pandemic, welfare organisations in Vryheid situated in northern KwaZulu-Natal have noted dramatic increases in requests for foster care placements for orphaned children. In many instances, orphans have lived by private arrangement with extended families for several years. However, families are increasingly overwhelmed with the burden of overextending scarce resources, forcing them to seek welfare assistance. Community-based responses to the crisis are regarded as top priority. This study examines literature regarding child care alternatives for orphans, focussing on foster care as the primary option. A study was conducted which explored community perceptions towards the local orphan crisis and foster care in particular. Findings revealed that although the adage 'Blood is thicker than water' still applies, foster care cannot stand alone. Human Immuno-Deficiency Virus (HIV) and Acquired Immune Deficiency Syndrome (AIDS) have many devastating impacts, including the growing number of AIDS-orphans left without care. In the study, semi-structured interviews, incomplete sentences and Kinetic family drawings were used to explore the life-experiences and perceptions of AIDS-orphans living in kinship foster care, from a Gestalt perspective. The study revealed that the majority of the AIDS-orphans included were making good contact at the contact boundary and all the respondents were not familiar with the term 'foster care'. The researcher therefore recommended that children are educated about foster care at school, thus possibly increasing the awareness of foster care in South Africa, thus possibly increasing the provision of homes for the vastly growing numbers of AIDS-orphans in our country and perhaps make a difference in society. / Sociology / M.
