Experi?ncias de atendimento psicol?gico grupal via internet: uma perspectiva psicanal?tica / Experiences of psychological group via internet: a psychoanalytic perspective

Since there is not a similar expansion of studies related to this subject, the practice is recognized with restrictions in Brazil. The aim of the present study was to investigate and analyze, under a psychoanalytic perspective, experiences in online psychological service in group modality via Internet in real-time using voice and video. The specific objectives are: to describe from the application of the fundamental psychoanalytic rule (free association), the effect of setting group at a distance over the associations and their psychic processes, and achieve an understanding of the potential of the online group to constitute an effective therapeutic device to sensitize them of their unconscious psychic processes. Two groups were formed, each group brought together up to five participants to conduct twenty four weekly sessions of one hour and thirty minutes, which took place between the months of April to October 2011. The results show that the online group dynamics reproduces many of the vicissitudes of presential group dynamics, including the mobilization of its members for the search for an initially undifferentiated identity, for reinvestment in this state every time the group suffered with changes on its configuration, the nuisances with more constant silences, by transforming personal problems into group problems, establishing and reinforcing bond feelings, and by opening up a space to reframe the problems experienced. It was concluded that the online group can be an effective therapeutic space, despite some limitations and differences, also discussed in this work. / O emprego das novas tecnologias, sobretudo da Internet, na oferta de atendimento psicol?gico ? dist?ncia tem se expandido aceleradamente nos ?ltimos anos. N?o havendo uma expans?o correlata de estudos sobre o assunto, a pr?tica ? regulamentada no pa?s com restri??es. A presente pesquisa teve por objetivo investigar e analisar, sob uma perspectiva psicanal?tica, experi?ncias de atendimento psicol?gico ? dist?ncia na modalidade de grupo, em comunica??o via Internet, em tempo real, por v?deo e voz. Dentre os objetivos espec?ficos: descrever, a partir do enunciado da regra psicanal?tica fundamental (associa??o livre), as incid?ncias da configura??o de grupo ? dist?ncia sobre as associa??es e os seus processos ps?quicos, e alcan?ar uma compreens?o acerca das potencialidades do grupo online de se constituir em um dispositivo terap?utico efetivo para sensibiliz?-lo de seus processos ps?quicos inconscientes. Foram compostos dois grupos, reunindo at? cinco participantes cada um, para a realiza??o de vinte e quatro sess?es, em frequ?ncia semanal de uma hora e trinta minutos, que aconteceram entre os meses de abril a outubro de 2011. O quadro geral evidencia que a din?mica grupal online reproduz muitas das vicissitudes da din?mica grupal presencial, incluindo a mobiliza??o de seus participantes pela busca de uma identidade inicialmente indiferenciada, pelo reinvestimento nesse estado cada vez que o grupo lidava com altera??es em sua configura??o, pelos inc?modos diante dos sil?ncios mais constantes, pela transforma??o de problemas pessoais em problemas de grupo, instaurando e fortalecendo sentimentos de v?nculo, e pela abertura de um espa?o para a ressignifica??o dos problemas vividos. The following diploma thesis deals, in accordance with its objectives and recommended primary literature, with addictive behavior in the Internet environment of adolescents, in the context of social services. The theoretical part resulted from the above mentioned primary literature, but due to its year of publication also from a variety of other up-to-date resources. The legislative frame and conditions necessary to diagnose addictive behavior. The social work with adolescents which has an addictive behavior on the Internet is primarily based on sector theories, but also points to multidisciplinary approaches. The empirical part, consisting of a questionnaire and subsequent analysis of the first grade, examines some aspects of adolescent behavior in cyberspace. The work is completed with a proposal for optimization measures.

Stödjande samtal på distans : en kvalitativ studie om hur hälso- och sjukvårdskuratorer inom akutsjukvården upplever den terapeutiska relationen vid samtal på distans / Counselling on a distance

The aim of this study was to explore how hospital social workers within emergency hospital care units find the challenges with the therapeutic relation when working online or over the phone. To find out more, we have interviewed seven hospital social workers. They were asked about challenges and advantages in working on a distance, related to the therapeutic relation. We wanted to find out if it is possible to obtain the same level of therapeutic relation when counselling changed from being given face to face to a digital encounter during the Covid 19 pandemic. Empirical data was analyzed with Asplunds theory of social responsiveness.  The study results in two different answers. First, it shows that hospital social workers experience increased flexibility as a relation strengthener and that it attracts patients that wouldn´t normally seek counselling. The second result shows that according to hospital social workers, there is something in the communication pattern that is missing when you are not in the same room as the patient and thus, there is also something in the therapeutic relation that is missing. Hospital social workers confirmed difficulties confirming patients when body language was missing or when technical problems sometimes reduced the understanding of patients' stories and there was a need for repeating in a painful situation. Peoples' different needs have become very clear throughout the study as our result show that it has been positive for the therapeutic relation to offer different counselling opportunities. / Syftet med studien har varit att undersöka hur hälso- och sjukvårdskuratorer inom akutsjukvården upplever den terapeutiska relationen i deras stödjande arbete på distans. För att få veta mer har vi intervjuat sju sjukhuskuratorer inom akutsjukvården och frågat dem om möjligheter och utmaningar med att arbeta på distans kopplat till den terapeutiska relationen. Vi ville veta om det gick att uppnå samma nivå av terapeutisk relation när det stödjande arbetet på sjukhusen styrdes om till att i större utsträckning än tidigare ges på distans. Materialet, vår empiri, har analyserats utifrån en hermeneutisk tolkningstradition och resultatet har analyserats med hjälp av Asplunds teori om social responsivitet.    Studiens resultat visar på två olika svar. Det ena resultatet visar att kuratorer upplever att det är relationsstärkande när patienten själv får välja var och hur man vill ha sitt samtal med kuratorn vid samtal på distans. Det andra resultatet visar att det finns något i kommunikationen, och därmed också i den terapeutiska relationen som går förlorat när samtalet inte sker fysiskt och kuratorn och patienten inte kan ta del av varandra med både verbalt språk och kroppsspråk. Kuratorerna har berättat om svårigheter att bekräfta patienten när man inte ser minspel och kroppsspråk eller när ljudet fungerar dåligt och man kan behöva be patienten upprepa något mitt uppe i en känslosam berättelse. Kuratorerna har också poängterat att de sannolikt kunnat fånga upp fler patienter än vad de annars hade gjort genom att kunna erbjuda samtal på distans och därigenom uppnått en terapeutisk relation, även om kontakten skedde på distans. Människors olika behov har blivit tydligt genom studien och genomsyrar våra resultat som visar på kuratorernas upplevelse av att det har varit relationsstärkande att kunna erbjuda olika samtalsmöjligheter.

Attitudes toward innovative mental health treatment approaches in Germany: E-mental health and home treatment

E-mental health and home treatment are treatment approaches that have proven to be effective, but are only slowly implemented in the German health care system. This paper explores the attitudes toward these innovative treatment approaches. Data was collected in two large, non-clinical samples representative of the German population in spring 2020 (N = 2,503) and winter 2020/2021 (N = 2,519). Statistical associations between variables were examined using two-tailed tests. Binary and multinomial logistic regressions were performed to predict attitudes toward online-based treatment concepts and home treatment approaches. Only few (<20%) people preferred online-based treatment approaches, while a larger proportion (~50%) could imagine being treated at home. Overall, younger subjects were more open to online-therapy approaches, while people with lower education preferred more often a traditional therapy setting. Acceptance of online-therapy did not raise significantly during the first months of the COVID-19 pandemic. When different online-based treatment options were available, the probability of accepting home treatment significantly increased with increasing levels of therapeutic support. Further promotion of acceptance for online-therapy and home treatment seems to be necessary. In the future, more information on innovative treatment approaches should be actively provided.

Možnosti a meze internetového psychologického poradenství / Possibilities and limits of online psychological counselling

The diploma thesis tries to do a critical evaluation of potential benefits and risks and limits of Internet psychological counseling. The theoretical part of the thesis describes the the issue of online help from general definition of counselling, through various approaches to definitions of Internet counselling. This part also deals with the history of the online form of help, including the context of development abroad and any differences or specifics in the Czech Republic. Attention is also given to various forms of online psychological help with emphasis on application in our environment. Finally the text introduces potential opportunities and risks of this form of counselling. Empirical part of the diploma thesis focuses on issues of online psychological counseling by a combined research design. The research part of the thesis tries to map the possibilities and limits of web psychological counselling from the perspective of the client. The focus of qualitative research is the analysis of interviews with online counseling clients, supplemented with analysis of chat conversations and case studies. Qualitative results are enriched by quantitatively conceived research.
