Avaliacao do uso da pleuroscopia no tratamento do empiema pleural agudo, fase fibrinopurulenta

O estudo descreve a evolução de uma série de 50 pacientes submetidos à pleuroscopia convencional para tratamento de empiema pleural fase fibrinopurulenta, no Serviço de Cirurgia Torácica da Universidade de Caxias do Sul, RS. Através da análise bivariada, foram identificados três fatores prognósticos desfavoráveis ao desfecho - resolução do empiema: mau estado geral dos pacientes, presença de germes anaeróbios no líquido pleural e expansibilidade pulmonar parcial no pós-operatório imediato (p < 0,001). A criação de um índice prognóstico derivado do modelo multivariado, composto pela combinação destes três fatores prognósticos, permitiu a identificação da importância destes fatores na resolução do empiema. Nos pacientes em que nenhum ou apenas um dos três fatores foi identificado (43 pacientes), houve 100% de resolução do empiema. A presença de dois fatores (3 pacientes) determinou a resolução do empiema em 33,3% dos casos. Nos 4 pacientes restantes, em que foram identificados os 3 fatores, nenhum apresentou resolução do empiema através da pleuroscopia. O método permitiu a resolução do empiema em 90% dos casos. O período médio de permanência hospitalar foi de 12,3 dias ± 5,7 para os pacientes com resolução do processo infeccioso e de 21 ,2 ± 5,1 para os casos de insucesso terapêutico. A morbidade e a mortalidade foram de 14% e de 4%, respectivamente. A realização de tomografia computadorizada ou ecografia torácica para identificar corretamente a presença das septações pleurais foi necessária em 23 pacientes (46%), o que os torna critério indispensável à indicação da pleuroscopia no tratamento do empiema pleural fase fibrinopurulenta. / This study describes the evolution of a series of fifty(50) patients that underwent conventional pleuroscopy for lhe treatment of pleural empyema in the fibrinopurulent stage at lhe Thoracic Surgery Unit of the University of Caxias do Sul, RS. Avaliação dos efeitos do pneumotórax unilateral de baixa pressão induzido em eqüinos / Evaluation of the effects of low pressure induced unilateral pneumothorax in horses

O presente estudo objetivou avaliar as alterações hemodinâmicas, de oxigenação, de ventilação e metabólicas em cavalos sedados com a associação de romifidina e tartarato de butorfanol submetidos a pneumotórax unilateral de baixa pressão por 20 minutos. Foram utilizados cinco eqüinos machos, hígidos, de diferentes raças, submetidos a dois procedimentos. No primeiro procedimento (Grupo I) os animais receberam a associação de romifidina (0,06 mg/kg) e tartarato de butorfanol (0,04 mg/kg) por via intravenosa. A coleta de dados foi realizada imediatamente antes da administração dos fármacos e após 10, 30, 40 e 65 minutos. No segundo procedimento (Grupo II), os mesmos animais receberam a associação dos fármacos e foram submetidos a toracoscopia, com a criação de pneumotórax unilateral esquerdo de 20 minutos de duração. A coleta de dados foi realizada imediatamente antes e 10 minutos após sedação; 20 minutos após o início do pneumotórax; 5 e 30 minutos após o término do pneumotórax. Os valores obtidos foram confrontados estatisticamente através de provas paramétricas, com a Análise de Variância seguida do Teste de Tukey para a comparação dos diferentes momentos em cada grupo e teste t-Student entre os dois grupos, adotando-se significância estatística de 5% (p<0,05). No Grupo I observou-se redução significativa da freqüência, débito e índice cardíacos até o término das mensurações. No Grupo II houve redução semelhante, porém o débito e índice cardíacos diminuíram de forma não significativa. Os valores da pressão arterial sistólica, diastólica e média permaneceram constantes no Grupo I, e apresentaram diminuição gradativa no Grupo II, significante somente para pressão arterial sistólica aos 5 e 30 minutos após o pneumotórax, e aos 30 minutos após o pneumotórax para pressão arterial média. O valor médio da pressão de artéria pulmonar apresentou aumento após a sedação apenas no Grupo I. A pressão venosa central apresentou incremento significativo aos 10 e 40 minutos após a sedação no Grupo I, e aos 10 minutos após a sedação e 20 minutos de pneumotórax no Grupo II. O índice de resistência vascular sistêmica apresentou aumento significativo somente no Grupo I após a sedação. Não foram presenciadas alterações significativas nos valores do índice de resistência vascular pulmonar em ambos os grupos. A pressão parcial de oxigênio no sangue arterial reduziu de forma significativa no Grupo II após a sedação e aos 20 minutos de pneumotórax. Também neste grupo, os valores de conteúdo, pressão parcial e saturação de oxigênio no sangue venoso misto reduziram de forma significativa em todos os momentos após a mensuração controle. Em ambos os grupos o índice de oferta de oxigênio diminuiu após a sedação, permanecendo reduzido de forma significativa somente no Grupo I. Os valores da diferença arteriovenosa de oxigênio e da taxa de extração de oxigênio apresentaram elevação significativa após a sedação no Grupo II, que persistiu até o término das mensurações. Em ambos os grupos, as alterações na freqüência respiratória, pressão parcial de dióxido de carbono arterial e pH não foram significativas. O bicarbonato plasmático arterial apresentou aumento significativo 30 minutos após o término do pneumotórax no Grupo II. A partir dos resultados obtidos foi possível concluir que: o pneumotórax unilateral induzido com baixo fluxo e mantido a baixo nível pressórico não leva a alterações hemodinâmicas, de oxigenação, ventilação e metabólicas significativas em eqüinos hígidos. A associação de romifidina e butorfanol conferiu sedação e analgesia suficientes para a realização do procedimento. porém foi a responsável pelas alterações hemodinâmicas e de oxigenação. / The aim of this study was to evaluate changes on hemodynamic, oxygenation, ventilation and metabolic values in horses sedated with romifidine and butorphanol, submitted to a low pressure induced unilateral pneumothorax for 20 minutes. Five healthy male horses, of different breeds, were submitted to two procedures. In the first one (Group I) the horses were treated with the intravenous combination of romifidine (0,06 mg/kg) and butorphanol (0,04 mg/kg). Data were collected immediately before and 10, 30, 40 and 65 minutes after drug application. In the second procedure (Group II), the same horses were treated with the drug combination and were submitted to thoracoscopy, with a left unilateral pneumothorax of 20 minutes long. Data were collected immediately before and 10 minutes after drug application, as well 20 minutes after the beginning of the pneumothorax and 5 and 30 minutes after the end of the pneumothorax. Data were submitted to Analysis of Variance test followed by Tukey?s test to compare different moments in each group and t-Student test between groups, with 5% of significance (p<0,05). In Group I was observed a significantly reduction of the heart rate, cardiac output and cardiac index until the end of the measurements. In Group II there was similar reduction, however the decrease of the cardiac output and cardiac index was not significant. The systolic, diastolic and mean arterial pressure remained constant in Group I, and a gradual decrease was observed in Group II. In this group, a significant decrease of the systolic arterial pressure was present at 5 and 30 minutes after the end of the pneumothorax, and the mean arterial pressure only 30 minutes after the end of the pneumothorax. The mean pulmonary arterial pressure increased after sedation only in Group I. The central venous pressure increased significantly 10 and 40 minutes after sedation in Group I and at 20 minutes after the end of the pneumothorax in Group II. The systemic vascular resistance increased significantly after sedation only in Group I. The pulmonary vascular resistance values did not change significantly in both groups. The arterial partial pressure of oxygen reduced significantly after sedation and at 20 minutes after the end of the pneumothorax in Group II. In this group a significant decrease of mixed venous oxygen content, partial pressure and saturation was present in all moments in relation to baseline values. In both groups the oxygen delivery index reduced after sedation, remaining significantly reduced only in Group I. The arteriovenous oxygen difference and the oxygen extraction ratio increased significantly after sedation in Group II, and persisted until the end of the procedure. In both groups the respiratory rate, the arterial pressure of carbon dioxide and pH did not change significantly. Plasma bicarbonate increased only in Group II 30 minutes after the end of the pneumothorax. In view of the obtained results its possible to conclude that: low pressure unilateral pneumothorax induced with low flow did not induce significant changes in hemodynamic, oxygenation, ventilation and metabolic values in healthy horses. The combination of romifidine and butorphanol provided good sedation and analgesia to the surgery perform, however was the responsible for the hemodynamic and oxygenation changes that occurred.

Toracoscopia em crianças com derrame parapneumônico complicado na fase fibrinopurulenta

Introdução: Apesar de a toracoscopia ser o procedimento preconizado em crianças com derrame pleural parapneumônico complicado (DPPC) na fase fibrinopurulenta, a grande maioria dos trabalhos citados na literatura é de relato de casos ou revisão de pequeno número de pacientes. Este estudo interinstitucional foi realizado para determinar a eficácia deste procedimento em número significativo de crianças com DPPC na fase fibrinopurulenta. Métodos: Estudo retrospectivo de 99 crianças (0,4 a 11 anos; idade média 2,6 anos) submetidas à toracoscopia para tratamento de DPPC na fase fibrinopurulenta, operadas em três hospitais diferentes e com mesmo algoritmo de tratamento, no período de novembro de 1995 a julho de 2005. Resultados: A toracoscopia foi eficaz em 87 crianças (87%); 12 (13%) necessitaram de outro procedimento cirúrgico - 6 novas toracoscopias e 6 toracotomia/pleurostomia. O tempo médio de drenagem após a toracoscopia foi de 3 dias naqueles em que a toracoscopia foi efetiva e de 10 dias nos reintervidos (P < 0,001). Todos resolveram a infecção pleural. As complicações da toracoscopia foram enfisema subcutâneo na inserção do trocater em duas crianças (2%), infecção da ferida operatória em outras duas (2%), sangramento pelo dreno torácico em 12 (12%) e fístula bronco pleural em 16 (16%). Nenhuma necessitou intervenção cirúrgica, Conclusões: A efetividade da toracoscopia em crianças com DPPC na fase fibrinopurulenta foi de 87%. O procedimento mostrou-se seguro, com baixa incidência de complicações graves, devendo ser considerada como primeira opção em crianças com DPPC na fase fibrinopurulenta. / Introduction: Although Thorachoscopy being a procedure used to treat children with complicated parapneumonic pleural effusion (DPPC) in fibrinopurulent stage, most of the works presented in literature are case reports of only a few patients. The purpose of this work is to carry on a cross institutional study to determine the method’s effectiveness on a significant number of children with DPPC in its purulent stage. Methods: This is a retrospective case study of 99 children (ages 0,4 to 11; average age 2,6), from November 1009 to July 2005, submitted to thorachoscopy for the treatment of DPPC in fibrinopurulent stage, operated in three different hospitals and with the same treatment algorithm. Results: Thorachoscopy was effective for 87 children (87%). 12 (12%) needed to be submitted to another surgery – 6 to another thorachoscopy and 6 to thorachotomy/pleurostomy. The average draining time after thorachoscopy was 3 days for those whose thorachoscopy was effective and 10 days for those who were submitted to a new surgery (P< 0,001). In all cases, the pleural infection was successfully treated. Complications of the thorachoscopy were subcutaneous emphysema in the trocater insertion on two patients (2%), infection of the surgery incision on two other patients, bleeding through drain on 12 patients (12%) and bronchialpleural fistula on 16 patients (16%). None of them needed surgical treatment. Conclusion: The effectiveness of thorachoscopy on children with DPPC in fibrinopurulent stage was 87%. The procedure had low incidence of serious complications, and should be considered the first treatment option for children with DPPC in fibrinopurulent stage.

Tratamento cirúrgico de tumores neurogênicos mediastinais em crianças : estudo interinstitucional

Introdução: Os tumores neurogênicos mediastinais na criança são incomuns, sendo responsáveis por 15-25% de todos os tumores primários desta localização. A maioria deles se origina dos nervos simpáticos, e se localizam quase que exclusivamente no mediastino posterior. Objetivos: Avaliar o tipo de tumor, idade, apresentação clínica, tipo de cirurgia e sobrevida de crianças operadas por tumor neurogênico de mediastino. Material e Métodos: Após aprovação das comissões de ética, foi realizada revisão retrospectiva de 43 crianças (22 do sexo masculino) operadas por tumor neurogênico mediastinal em dois hospitais terciários, no período de março de 1998 a junho de 2009. Resultados: A mediana de idade das crianças no momento do diagnóstico foi 3 anos (0,6- 4,7). Dos tumores operados, 20 (47%) eram neuroblastomas, 13 (30%) ganglioneuroblastomas e 10 (23%) ganglioneuromas. Somente 6 crianças (14%) eram assintomáticas no momento do diagnóstico; as restantes apresentavam tosse (30%), dispnéia (21%), sibilância (11,6%), manifestações de compressão de medula espinhal (7%), e Síndromes dos Olhos Dançantes (7%), e de Horner (7%). A cirurgia foi realizada por toracotomia em 38 (88%) e por videotoracoscopia em 5 (12%). Quando comparadas àquelas submetidas à toracotomia, as crianças operadas por videotoracoscopia apresentaram estatisticamente tumores de tamanho menor (p=0,01), bem como menos tempo de drenagem torácica (p=0,011) e de internação (p=0,016). Não houve mortalidade cirúrgica. Complicações pós-operatórias ocorreram em onze crianças (25,6%): Síndrome de Horner em 5 crianças (11,6%), sendo que 3 após toracotomia e 2 após toracoscopia; as demais ocorreram após toracotomia: quilotórax em 2 (4,7%), pneumotórax em 2 (4,7%), empiema em uma (2,3%) e traqueobroncomalacia grave em outra (2,3%). Houve recorrência do tumor em 4 crianças (9,3%) operadas por toracotomia, e 2 mortes em crianças com neuroblastoma (4,6%). A sobrevida geral foi de 95,4% com uma mediana de tempo de seguimento de 3,5 anos (0,7 – 4,4). Conclusões: A maioria das crianças com tumor neurogênico mediastinal é sintomática no momento do diagnóstico. A ressecção cirúrgica destes tumores é segura, e possibilita boa sobrevida em longo prazo. A abordagem por videotoracoscopia é apropriada para pequenos tumores, e possibilita menores tempos de drenagem torácica e de internação. / Introduction: Neurogenic mediastinal tumors are uncommon, and comprise 15-25% of all primary mediastinal tumours. The majority of them arises from sympathetic nerves and is found almost exclusively in the posterior mediastinum. Purpose: To identify age, type of tumor, clinical spectrum, type of surgical resection and survival in children with thoracic neurogenic tumours. Methods: With ethics approval we reviewed the case notes of 43 consecutive children (22 males) operated for mediastinal neurogenic tumour in two different institutions from March 1998 to June 2009. Results: The median age at diagnosis was 3 years (0,6-4,7). Of the 43 tumours operated: 20 were neuroblastomas (47%), 13 ganglioneuroblastomas (30%) and 10 ganglioneuromas (23%). Only 6 (14%) of these tumours were discovered incidentally, the remaining 37 (86%) being symptomatic: cough (30%), dyspnea (21%), wheezing (11,6%), neurological spinal compression (7%), dancing eyes (7%) and Horner (7%) syndromes. Tumour resection was undertaken by open operation in 38 cases (88%) and by thoracoscopy in 5 (12%). There were no surgical deaths. Postoperative complications occurred in 11 children (25,6%). Horner’s syndrome was seen in 5 children (11,6%), 2 thoracoscopic and 3 open. All others complications were observed after open surgery: chylothorax in 2 (4,7%), pneumothorax in 2 (4,7%), thoracic empyema in 1 (2,3%) and severe tracheobronchomalacia in a further one (2,3%). Tumour recurrence occurred in 4 children (9,3%) operated by open surgery, and there were 2 death in children with neuroblastoma. The all survival was 95.4% in a median follow-up of 3,5 years (0,7-4,4). Conclusions: Most children with thoracic neurogenic tumours are symptomatic at diagnosis. Surgical resection of these tumours in children is safe and with good long-term survival. The thoracoscopic approach is appropriate for small tumours, and it carries out less postoperative lengths of thoracic drain and hospitalization.

Este estudo interinstitucional foi realizado para determinar a eficácia deste procedimento em número significativo de crianças com DPPC na fase fibrinopurulenta. Métodos: Estudo retrospectivo de 99 crianças (0,4 a 11 anos; idade média 2,6 anos) submetidas à toracoscopia para tratamento de DPPC na fase fibrinopurulenta, operadas em três hospitais diferentes e com mesmo algoritmo de tratamento, no período de novembro de 1995 a julho de 2005. Resultados: A toracoscopia foi eficaz em 87 crianças (87%); 12 (13%) necessitaram de outro procedimento cirúrgico - 6 novas toracoscopias e 6 toracotomia/pleurostomia. O tempo médio de drenagem após a toracoscopia foi de 3 dias naqueles em que a toracoscopia foi efetiva e de 10 dias nos reintervidos (P < 0,001). Todos resolveram a infecção pleural. As complicações da toracoscopia foram enfisema subcutâneo na inserção do trocater em duas crianças (2%), infecção da ferida operatória em outras duas (2%), sangramento pelo dreno torácico em 12 (12%) e fístula bronco pleural em 16 (16%). Nenhuma necessitou intervenção cirúrgica, Conclusões: A efetividade da toracoscopia em crianças com DPPC na fase fibrinopurulenta foi de 87%. O procedimento mostrou-se seguro, com baixa incidência de complicações graves, devendo ser considerada como primeira opção em crianças com DPPC na fase fibrinopurulenta. / Introduction: Although Thorachoscopy being a procedure used to treat children with complicated parapneumonic pleural effusion (DPPC) in fibrinopurulent stage, most of the works presented in literature are case reports of only a few patients. The purpose of this work is to carry on a cross institutional study to determine the method’s effectiveness on a significant number of children with DPPC in its purulent stage. Methods: This is a retrospective case study of 99 children (ages 0,4 to 11; average age 2,6), from November 1009 to July 2005, submitted to thorachoscopy for the treatment of DPPC in fibrinopurulent stage, operated in three different hospitals and with the same treatment algorithm. Results: Thorachoscopy was effective for 87 children (87%). 12 (12%) needed to be submitted to another surgery – 6 to another thorachoscopy and 6 to thorachotomy/pleurostomy. The average draining time after thorachoscopy was 3 days for those whose thorachoscopy was effective and 10 days for those who were submitted to a new surgery (P< 0,001). In all cases, the pleural infection was successfully treated. Complications of the thorachoscopy were subcutaneous emphysema in the trocater insertion on two patients (2%), infection of the surgery incision on two other patients, bleeding through drain on 12 patients (12%) and bronchialpleural fistula on 16 patients (16%). None of them needed surgical treatment. Conclusion: The effectiveness of thorachoscopy on children with DPPC in fibrinopurulent stage was 87%. The procedure had low incidence of serious complications, and should be considered the first treatment option for children with DPPC in fibrinopurulent stage.

Tratamento cirúrgico de tumores neurogênicos mediastinais em crianças : estudo interinstitucional

Introdução: Os tumores neurogênicos mediastinais na criança são incomuns, sendo responsáveis por 15-25% de todos os tumores primários desta localização. A maioria deles se origina dos nervos simpáticos, e se localizam quase que exclusivamente no mediastino posterior. Objetivos: Avaliar o tipo de tumor, idade, apresentação clínica, tipo de cirurgia e sobrevida de crianças operadas por tumor neurogênico de mediastino. Material e Métodos: Após aprovação das comissões de ética, foi realizada revisão retrospectiva de 43 crianças (22 do sexo masculino) operadas por tumor neurogênico mediastinal em dois hospitais terciários, no período de março de 1998 a junho de 2009. Resultados: A mediana de idade das crianças no momento do diagnóstico foi 3 anos (0,6- 4,7). Dos tumores operados, 20 (47%) eram neuroblastomas, 13 (30%) ganglioneuroblastomas e 10 (23%) ganglioneuromas. Somente 6 crianças (14%) eram assintomáticas no momento do diagnóstico; as restantes apresentavam tosse (30%), dispnéia (21%), sibilância (11,6%), manifestações de compressão de medula espinhal (7%), e Síndromes dos Olhos Dançantes (7%), e de Horner (7%). A cirurgia foi realizada por toracotomia em 38 (88%) e por videotoracoscopia em 5 (12%). Quando comparadas àquelas submetidas à toracotomia, as crianças operadas por videotoracoscopia apresentaram estatisticamente tumores de tamanho menor (p=0,01), bem como menos tempo de drenagem torácica (p=0,011) e de internação (p=0,016). Não houve mortalidade cirúrgica. Complicações pós-operatórias ocorreram em onze crianças (25,6%): Síndrome de Horner em 5 crianças (11,6%), sendo que 3 após toracotomia e 2 após toracoscopia; as demais ocorreram após toracotomia: quilotórax em 2 (4,7%), pneumotórax em 2 (4,7%), empiema em uma (2,3%) e traqueobroncomalacia grave em outra (2,3%). Houve recorrência do tumor em 4 crianças (9,3%) operadas por toracotomia, e 2 mortes em crianças com neuroblastoma (4,6%). A sobrevida geral foi de 95,4% com uma mediana de tempo de seguimento de 3,5 anos (0,7 – 4,4). Conclusões: A maioria das crianças com tumor neurogênico mediastinal é sintomática no momento do diagnóstico. A ressecção cirúrgica destes tumores é segura, e possibilita boa sobrevida em longo prazo. A abordagem por videotoracoscopia é apropriada para pequenos tumores, e possibilita menores tempos de drenagem torácica e de internação. / Introduction: Neurogenic mediastinal tumors are uncommon, and comprise 15-25% of all primary mediastinal tumours. The majority of them arises from sympathetic nerves and is found almost exclusively in the posterior mediastinum. Purpose: To identify age, type of tumor, clinical spectrum, type of surgical resection and survival in children with thoracic neurogenic tumours. Methods: With ethics approval we reviewed the case notes of 43 consecutive children (22 males) operated for mediastinal neurogenic tumour in two different institutions from March 1998 to June 2009. Results: The median age at diagnosis was 3 years (0,6-4,7). Of the 43 tumours operated: 20 were neuroblastomas (47%), 13 ganglioneuroblastomas (30%) and 10 ganglioneuromas (23%). Only 6 (14%) of these tumours were discovered incidentally, the remaining 37 (86%) being symptomatic: cough (30%), dyspnea (21%), wheezing (11,6%), neurological spinal compression (7%), dancing eyes (7%) and Horner (7%) syndromes. Tumour resection was undertaken by open operation in 38 cases (88%) and by thoracoscopy in 5 (12%). There were no surgical deaths. Postoperative complications occurred in 11 children (25,6%). Horner’s syndrome was seen in 5 children (11,6%), 2 thoracoscopic and 3 open. All others complications were observed after open surgery: chylothorax in 2 (4,7%), pneumothorax in 2 (4,7%), thoracic empyema in 1 (2,3%) and severe tracheobronchomalacia in a further one (2,3%). Tumour recurrence occurred in 4 children (9,3%) operated by open surgery, and there were 2 death in children with neuroblastoma. The all survival was 95.4% in a median follow-up of 3,5 years (0,7-4,4). Conclusions: Most children with thoracic neurogenic tumours are symptomatic at diagnosis. Surgical resection of these tumours in children is safe and with good long-term survival. The thoracoscopic approach is appropriate for small tumours, and it carries out less postoperative lengths of thoracic drain and hospitalization.

Avaliação dos efeitos do pneumotórax unilateral de baixa pressão induzido em eqüinos / Evaluation of the effects of low pressure induced unilateral pneumothorax in horses

O presente estudo objetivou avaliar as alterações hemodinâmicas, de oxigenação, de ventilação e metabólicas em cavalos sedados com a associação de romifidina e tartarato de butorfanol submetidos a pneumotórax unilateral de baixa pressão por 20 minutos. Foram utilizados cinco eqüinos machos, hígidos, de diferentes raças, submetidos a dois procedimentos. No primeiro procedimento (Grupo I) os animais receberam a associação de romifidina (0,06 mg/kg) e tartarato de butorfanol (0,04 mg/kg) por via intravenosa. A coleta de dados foi realizada imediatamente antes da administração dos fármacos e após 10, 30, 40 e 65 minutos. No segundo procedimento (Grupo II), os mesmos animais receberam a associação dos fármacos e foram submetidos a toracoscopia, com a criação de pneumotórax unilateral esquerdo de 20 minutos de duração. A coleta de dados foi realizada imediatamente antes e 10 minutos após sedação; 20 minutos após o início do pneumotórax; 5 e 30 minutos após o término do pneumotórax. Os valores obtidos foram confrontados estatisticamente através de provas paramétricas, com a Análise de Variância seguida do Teste de Tukey para a comparação dos diferentes momentos em cada grupo e teste t-Student entre os dois grupos, adotando-se significância estatística de 5% (p<0,05). No Grupo I observou-se redução significativa da freqüência, débito e índice cardíacos até o término das mensurações. No Grupo II houve redução semelhante, porém o débito e índice cardíacos diminuíram de forma não significativa. Os valores da pressão arterial sistólica, diastólica e média permaneceram constantes no Grupo I, e apresentaram diminuição gradativa no Grupo II, significante somente para pressão arterial sistólica aos 5 e 30 minutos após o pneumotórax, e aos 30 minutos após o pneumotórax para pressão arterial média. O valor médio da pressão de artéria pulmonar apresentou aumento após a sedação apenas no Grupo I. A pressão venosa central apresentou incremento significativo aos 10 e 40 minutos após a sedação no Grupo I, e aos 10 minutos após a sedação e 20 minutos de pneumotórax no Grupo II. O índice de resistência vascular sistêmica apresentou aumento significativo somente no Grupo I após a sedação. Não foram presenciadas alterações significativas nos valores do índice de resistência vascular pulmonar em ambos os grupos. A pressão parcial de oxigênio no sangue arterial reduziu de forma significativa no Grupo II após a sedação e aos 20 minutos de pneumotórax. Também neste grupo, os valores de conteúdo, pressão parcial e saturação de oxigênio no sangue venoso misto reduziram de forma significativa em todos os momentos após a mensuração controle. Em ambos os grupos o índice de oferta de oxigênio diminuiu após a sedação, permanecendo reduzido de forma significativa somente no Grupo I. Os valores da diferença arteriovenosa de oxigênio e da taxa de extração de oxigênio apresentaram elevação significativa após a sedação no Grupo II, que persistiu até o término das mensurações. Em ambos os grupos, as alterações na freqüência respiratória, pressão parcial de dióxido de carbono arterial e pH não foram significativas. O bicarbonato plasmático arterial apresentou aumento significativo 30 minutos após o término do pneumotórax no Grupo II. A partir dos resultados obtidos foi possível concluir que: o pneumotórax unilateral induzido com baixo fluxo e mantido a baixo nível pressórico não leva a alterações hemodinâmicas, de oxigenação, ventilação e metabólicas significativas em eqüinos hígidos. A associação de romifidina e butorfanol conferiu sedação e analgesia suficientes para a realização do procedimento. porém foi a responsável pelas alterações hemodinâmicas e de oxigenação. / The aim of this study was to evaluate changes on hemodynamic, oxygenation, ventilation and metabolic values in horses sedated with romifidine and butorphanol, submitted to a low pressure induced unilateral pneumothorax for 20 minutes. Five healthy male horses, of different breeds, were submitted to two procedures. In the first one (Group I) the horses were treated with the intravenous combination of romifidine (0,06 mg/kg) and butorphanol (0,04 mg/kg). Data were collected immediately before and 10, 30, 40 and 65 minutes after drug application. In the second procedure (Group II), the same horses were treated with the drug combination and were submitted to thoracoscopy, with a left unilateral pneumothorax of 20 minutes long. Data were collected immediately before and 10 minutes after drug application, as well 20 minutes after the beginning of the pneumothorax and 5 and 30 minutes after the end of the pneumothorax. Data were submitted to Analysis of Variance test followed by Tukey?s test to compare different moments in each group and t-Student test between groups, with 5% of significance (p<0,05). In Group I was observed a significantly reduction of the heart rate, cardiac output and cardiac index until the end of the measurements. In Group II there was similar reduction, however the decrease of the cardiac output and cardiac index was not significant. The systolic, diastolic and mean arterial pressure remained constant in Group I, and a gradual decrease was observed in Group II. In this group, a significant decrease of the systolic arterial pressure was present at 5 and 30 minutes after the end of the pneumothorax, and the mean arterial pressure only 30 minutes after the end of the pneumothorax. The mean pulmonary arterial pressure increased after sedation only in Group I. The central venous pressure increased significantly 10 and 40 minutes after sedation in Group I and at 20 minutes after the end of the pneumothorax in Group II. The systemic vascular resistance increased significantly after sedation only in Group I. The pulmonary vascular resistance values did not change significantly in both groups. The arterial partial pressure of oxygen reduced significantly after sedation and at 20 minutes after the end of the pneumothorax in Group II. In this group a significant decrease of mixed venous oxygen content, partial pressure and saturation was present in all moments in relation to baseline values. In both groups the oxygen delivery index reduced after sedation, remaining significantly reduced only in Group I. The arteriovenous oxygen difference and the oxygen extraction ratio increased significantly after sedation in Group II, and persisted until the end of the procedure. In both groups the respiratory rate, the arterial pressure of carbon dioxide and pH did not change significantly. Plasma bicarbonate increased only in Group II 30 minutes after the end of the pneumothorax. In view of the obtained results its possible to conclude that: low pressure unilateral pneumothorax induced with low flow did not induce significant changes in hemodynamic, oxygenation, ventilation and metabolic values in healthy horses. The combination of romifidine and butorphanol provided good sedation and analgesia to the surgery perform, however was the responsible for the hemodynamic and oxygenation changes that occurred.
