La transition agroécologique est annoncée par les pouvoirs publics comme le tournant majeur à venir de l’agriculture. Dans le même temps, les consommateurs et la société civile se montrent plus soucieux de l’alimentation et des questions agricoles. La dimension environnementale de la production agricole semble prendre une importance de plus en plus forte. L’agriculture francilienne doit alors composer avec ces nouvelles injonctions agricoles dans un contexte de grandes cultures implantées sur un territoire aux multiples enjeux (alimentaires, écologiques, économiques et d’aménagements). Quelle(s) forme(s) prend la transition agroécologique, quel(s) impact(s) a-t-elle sur les agriculteurs franciliens et la composition des exploitations, en quoi ces changements agricoles peuvent-ils amener à de nouvelles compositions territoriales : voici quelques-unes des questions qui animent et articulent ce travail doctoral. / Public authorities announced the agroecological transition as a major turning point to come for agriculture. At the same time, consumers and civil society are more and more aware of food and agricultural issues. The environmental dimension of the agricultural production seems to strongly gain in importance. The agriculture of the Île-de-France region is dealing with these new injunctions in a context of well-established field crops on a territory facing high stakes (regarding food, the environment, the economy and land planning). What form does this agroecological transition take, what impacts will this transition have for the farmers of Île-de-France and how do these changes lead to new territorial configurations: here are some of the issues that drive this doctoral research.

Konsten att möta familjer som vårdats på ett annat sjukhus : Barnsjuksköterskors erfarenheter

Bakgrund: Trots att  förflyttning  av nyfödda barn är både riskfyllda och komplicerade  så är det en förutsättning för god neonatalvård i hela  Sverige. Denna  förflyttning  kan vara  både ångestfylld  och oroande för föräldrarna  och ställer stora krav på barnsjuksköterskorna som tar emot familjen vid ankomst till den nya avdelningen.    Syfte: Att beskriva barnsjuksköterskors erfarenheter av att ta emot familjer som vårdats på  en annan neonatalavdelning.  Design och tillvägagångsätt: Kvalitativ design med induktiv ansats. Individuella intervjuer genomfördes med nio  barnsjuksköterskor  på två olika neonatalavdelningar  i Sverige.  Intervjuerna analyserades med kvalitativ  innehållsanalys.   Resultat: Barnsjuksköterskorna uppgav att det fanns en osäkerhet i vilka rutiner som fanns för att ta emot familjerna  och att de rutiner som fanns hade brister. De belyste även  behovet av att skapa en relation med familjen genom att samtala, avsätta tid för familjen och att lyssna in föräldrarna.   Slutsats: Barnsjuksköterskorna använder sig av olika verktyg för att skapa en relation med familjen och på så vis inge trygghet och förtroende. Det finns även önskemål från barnsjuksköterskorna om specifika rutiner  på den egna arbetsplatsen  gällande mottagandet  samt  bättre samarbete  mellan neonatalavdelningarna.

Demographic transition, population growth, demographic ageing - interrelations and development contexts at the regional level Abstract This thesis aims to examine how the timing and pace of the demographic transition correlated with the timing and pace of ageing at the regional level in Czechia and also to contribute to understanding the determinants and evolution of demographic transition, specifically mortality and fertility decline, and connect it with the population ageing. It is a common belief that ageing is the result of demographic transition, but there is a limited number of studies that investigate the interrelations between these two processes. The greatest challenge was the difficulty in comparing historical and current data caused by changes in the administrative division during 1868-2017. Thus, the recalculation of data was needed to analyse trends in mortality and fertility. 2011 statistical units at the level of districts were chosen as basic units for analysis. The data recalculation was preceded by the reconstruction of historical districts maps and population data estimation for intercensal periods. Data were recalculated using spatial overlays in GIS software and database processing operations both for population and vital statistics. This step was followed by the data analysis....

Congenital Heart Defects and the need for better transition management

INTRODUCTION: Congenital Heart Defects are the most common congenital defects in the United States. They affect a significant proportion of all births, and many babies with CCHDs are not expected to survive their first year. While diagnostic and surgical interventions have drastically improved mortality rates, a growing population of adolescent and adult CHD patients continue to face unique developmental, psychological and QoL issues. METHODS: Medical journal articles were utilized to determine the prevalence, mortality rates, survival rates, adverse psychological outcomes, and follow-up rates for CHD patients as they transitioned through adolescence. Most articles came from Pediatrics, The Journal of Pediatrics, and Circulation. RESULTS: From 1980-2005, the prevalence of CHDs in the United States increased, while CHD mortality decreased by nearly half. Over a similar time period, CHD patients were more likely to have poor psychological, behavioral, and QoL outcomes than their healthy peers. Specifically, CHD patients were likely to have developmental disorders, lower QoL, and loss to follow-up when transitioning to adult care providers. CHD patients also demonstrated a poor understanding of their condition, especially with respect to need for follow-up, identifying symptoms of deteriorating heart condition, and the negative effects of smoking, drugs, and alcohol. DISCUSSION: Diagnostic and surgical interventions for CHD patients have led to increased survival. However, many of these interventions occur in the early stages of life, leaving a gap in medical management of CHD patients as they transition through adolescence. This is represented by high attrition rates for those following-up with adult care providers, adverse psychosocial outcomes, and patients’ lack of knowledge about their condition. CHD patients may benefit from more comprehensive, coordinated and formal transition programs that incorporate good social support systems, self-efficacy, and education about their condition.

Development of luminescent iridium(III) complex-based probes for monitoring analytes in environmental and biological systems

Transition metal complexes offer potential alternatives to fluorescent organic compounds in various sensing applications. They show several characteristic properties over organic dyes, such as strong luminescence emission, long emission lifetime and large Stoke shift. Among transition metal complexes, cyclometalated iridium(III) (Ir) complexes are most widely explored for sensing applications, due to their bright and tuneable phosphorescence emission. Up to now, Ir(III) complexes have been successfully applied to detect a range of analytes in environmental and biological systems, such as cations, anion, small molecules and proteins. In this thesis, we deeply explored the capability of Ir(III) complexes to the detection of a range of targets including metal ions, small molecules and biomarkers. Several strategies are used to improve the biocompatibility of Ir(III)-based probes while retaining their desirable characteristics. In chapter 2, we developed a novel Ir(III) complex for the detection of Al3+ with a detection limit of 1 μM. The long lifetime of the complex was harnessed to distinguish luminescence response to Al3+ from autofluorescence in biological samples by TRES experiment, while the probe was also successfully applied for imaging Al3+ in living cells. The results have been published as Chem. Commun., 2016, 52, 3611. In chapter 3, we reported a new reaction-based luminogenic probe for imaging both H2S and hypoxia in living zebrafish. This probe demonstrated their utility for the detection of H2S in solution, living cells and zebrafish model, while it was also capable of discriminating hypoxic from normoxic cells and zebrafish model. The results have been published as Sens. Actuator B-Chem., 2018, 255, 1953. In chapter 4, we conjugated a natural product oridonin to an Ir(III) scaffold for tracking intracellular NF-κB. This complex was successfully applied to track NF-κB translocation induced by TNF-α, without affecting the translocation process. The results have been published as Chem. Eur. J., 2017, 23, 4929. In chapter 5, an Ir(III) scaffold with galactose moiety was designed and synthesized for discriminating ovarian carcinoma cell lines from normal cell lines. This probe can selectively "light up" ovarian carcinoma cells with negligible luminescence in normal cells. The results have been published as Anal. Chem., 2017, 89, 11679. These works have further demonstrated the utility of Ir(III) complex in the monitoring environment and studying biomolecules in living systems. In particular, the conjugation of endogenous molecule galactose or a natural compound oridonin to Ir(III) scaffolds highlights an effective solution to develop biocompatible probes. However, it should be pointed out that there is a need for developing a general strategy to improve the biocompatibility of luminescent Ir(III) complex-based probes, while there is huge potential for incorporating luminescent Ir(III) complexes-based sensing platforms into portable devices, and exploring theranostic probes.

Imidoyl Amidine Ligands: A Versatile Framework to Build Homo and Heterometallic Complexes

Ligand design in general enables the formation of coordination compounds with multiple functionalities within a single framework. To date, two of the most widely studied ligands are 2,2′:6′,2′′-terpyridine (terpy) and acetylacetone (acac), whose tridentate and bidentate coordination pockets, respectively, enables the formation of metallic complexes with various geometries. The Brusso group had been incorporating imidoyl amidine (ImAm) ligands to build different materials such as organic radicals and fluorescent materials. In particular, the ligands N-2-pyridylimidoyl-2-pyridylamidine (Py2ImAm) and N-2-pyrimidylimidoyl-2-pyrimidylamidine (Pm2ImAm) were recently synthesized and have great appeal to build metallic complexes, as they poses two coordination sites similar to those in terpy and acac. The work presented herein represents the first studies involving the coordination of Py2ImAm and Pm2ImAm as discrete ligands. Our results demonstrate the versatility of these ligand frameworks, in which discrete mononuclear complexes, homometallic and heterometallic polynuclear complexes may be realized. Chapter one serves as a brief introduction to transition metal chemistry and has a comprehensive review of the coordination chemistry of the ImAm ligand framework. In chapter two, the selective coordination of first row transition metals into the bidentate or tridentate sites of Py2ImAm is explored. The formation of these mononuclear complexes is acid-base driven, where a weak acid induces coordination to the tridentate site and a weak base leads to coordination in the bidentate site. Coordination to both sides of Pm2ImAm with manganese or iron is explored in chapter three. The results show the formation of unusual tetranuclear complexes with the metal ions in both low spin and high spin configurations. Chapter four covers the coordination to cobalt, and the formation of polynuclear complexes with different geometries using Pm2ImAm. The magnetochemistry of these cobalt polynuclear complexes is also presented and reveal a single molecule magnet behaviour for one of the complexes. Finally, in chapter five, a one-pot synthesis of copper-manganese heterometallic complexes is presented. Overall, these imidoyl amidine ligands are able to build complexes with different geometries, different electronic configurations (i.e. low or high spin), and different metal ions. These results show a great versatility of ImAm ligands and suggest the future use of these ligands by other research groups.

Introduktion: Palliativ omvårdnad syftar till att tillgodose patientens fysiska, psykiska, sociala och existentiella behov, samt ge närstående stöd i sorgearbetet. Inom palliativ cancervård genomgår sjuksköterskan den palliativa transitionen genom vilken de utvecklas från att vara mer uppgiftsorienterad och styrda av rutiner, till att ha ett mer holistiskt synsätt och kunna överblicka de komplexa behoven patienten och dess anhöriga har.   Syfte: Syftet var att utforska hur sjuksköterskor inom onkologisk palliativ vård arbetar för att tillgodose behov hos cancerpatienter i livets slutskede och deras närstående, samt att undersöka om det går att urskilja mönster avseende sjuksköterskornas arbetssätt beroende hur lång arbetslivserfarenhet sjuksköterskorna har inom onkologisk vård.    Metod: Kvalitativ metod med semi-strukturerade intervjuer av sexton sjuksköterskor med olika lång arbetslivserfarenhet av palliativ omvårdnad inom onkologisk vård.    Huvudresultat: Sjuksköterskor prioriterar att lindra symtom såsom smärta och ångest, och kommunicerar med patienter och närstående för att upptäcka behov. Personcentrerad vård är i fokus hos sjuksköterskorna oavsett hur lång arbetslivserfarenhet de har. Sjuksköterskor med längre yrkeserfarenhet är mer lyhörda och flexibla samt känner sig tryggare i sina bedömningar. Sjuksköterskor med kortare yrkeserfarenhet uppgav att tidsbrist leder till att de inte hinner utföra omvårdnadsåtgärder som patienterna är i behov av.   Slutsats: Sjuksköterskor som arbetar med cancerpatienter i palliativ vård har ett professionellt förhållningssätt gentemot patienter och närstående oavsett hur lång arbetslivserfarenhet de har. Sjuksköterskor med kortare arbetslivserfarenhet är i behov av mer stöd och handledning för att kunna utvecklas i sin yrkesroll och tillgodose patienters och närståendes behov.    Nyckelord: Onkologi, palliativ omvårdnad, sjuksköterska, transition / Introduction: Palliative care aims to meet the patient's physical, mental, social and existential needs, as well as provide relatives with support in the grieving process. In palliative cancer care, the nurse undergoes the palliative transition through which they develop from being more task-oriented and guided by routines, to having a more holistic approach and being able to oversee the complex needs of the patient and their relatives.   Aim: The aim was to explore how nurses in oncological palliative care work to meet the needs of cancer patients at the end of life and their relatives, and to investigate whether it is possible to distinguish patterns regarding the nurses' working methods depending on how long work experience the nurses have in oncology.   Method: Qualitative method with semi-structured interviews of sixteen nurses with different lengths of work experience of palliative care in oncology care.   Main results: Nurses prioritize relieving symptoms such as pain and anxiety and communicate with patients and relatives to identify needs. Person-centered care is the focus of the nurses, regardless of how long their work experience they is. Nurses with longer professional experience are more responsive and flexible and feel more confident in their assessments. Nurses with shorter professional experience stated that lack of time leads to them not having time to perform nursing measures that patients need.   Conclusion: Nurses who work with cancer patients in palliative care have a professional approach towards patients and relatives regardless of how long work experience they have. Nurses with shorter work experience are in need of more support and guidance in order to develop in their professional role and meet the needs of patients and relatives.   Keywords: Oncology, palliative care, nurse, transition
