A descriptive study of the use of the Diabetes Treatment Satisfaction Questionnaire (DTSQ) with different populations

Class of 2006 Abstract / Objectives: To examine the manner in which the DTSQ was used with a variety diabetic populations in different countries. The DTSQ is an 8 item questionnaire assessing satisfaction with diabetes treatment (6 items) and 2 items on glycemic control. Methods: Intensive search of published scientific literature was conducted to identify studies in which the DTSQ has been used in different countries. Data was extracted from each study on the following variables: country, ethnicitiy, DTSQ scores, Hg A1cgender, treatment, type of diabetes, and education level. Results: A total of twenty-four studies reporting the use of the DTSQ were identified. The majority studies (14/24) were treatment comparisons. The other studies were various other questions investigated. T he DTSQ was used for studies ranging in size from 15 to 1918 (mean 436.3 (SD=541.4) The DTSQ was used with patients who had thye 1, type 2, and gestational diabetes. Average HgA1c level was 7.6, and average DTSQ score was 29.9. Most studies did not describe the educational or ethnic background of their study participants. Most of the studies (18) were done in Europe with ten in the United Kingdom, two were in the USA, one in New Zealand, one in Nigeria, one in Canada, and one in Israel and Slovenia combined. Conclusions: The DTSQ appears useful with a variety of patient populations, although little information was presented on population characteristics.

Introdução: O controle glicêmico estrito com múltiplas injeções diárias de insulina é o foco do tratamento para diabetes mellitus tipo 1 (DM1), mas geralmente está associado a um aumento no número de episódios de hipoglicemia, e o estresse de conviver com esse evento pode estar associado a prejuízos para a saúde mental do paciente. Embora os análogos de insulina de ação prolongada tenham propriedades farmacológicas para imitar o perfil fisiológico de insulina, a literatura não é unânime em demonstrar esse efeito em comparação à insulina humana. No Brasil, apenas alguns estados, incluindo o Rio Grande do Sul (RS), fornecem análogos de insulina para pacientes com DM1. O objetivo deste estudo foi avaliar a frequência de hipoglicemias e a satisfação dos pacientes com DM1 que recebem análogos de insulina de curta e longa ação após sua introdução no Rio Grande do Sul (RS). Materiais e métodos: Estudo transversal, retrospectivo, realizado com pacientes adultos com diabetes tipo 1, residentes de 38 cidades do estado do Rio Grande do Sul, que recebiam análogos de insulina via Secretaria Estadual de Saúde. Os dados clínicos e demográficos foram avaliados por formulário auto-respondido, a satisfação dos pacientes com o tratamento através do Questionário de satisfação com o tratamento do diabetes (DTSQs) e os transtornos mentais comuns (TMC) através do Questionário sobre saúde geral (QSG-12). Resultados: Um total de 507 pacientes foram incluídos, com idade média de 38,6±13,7 anos, 52% feminino, com duração do DM 18 [IQR25-75 = 11-25] anos e 36,8% com ensino superior completo. A pontuação mediana de satisfação com o tratamento (DTSQs) foi de 32 [IQR25-75 = 29 -35]. A satisfação dos pacientes não reduziu a longo prazo. A taxa de pacientes com hipoglicemias, incluindo grave e noturna, não alterou com o tempo de uso dos análogos de insulina. Apesar de taxas altas de hipoglicemias e com a maioria dos pacientes com triagem positiva para transtornos mentais comuns os pacientes mantiveram altos escores de satisfação com o tratamento, o que não reduziu em longo prazo como em outras intervenções em doenças crônicas. / Introduction: Strict glycemic control with multiple daily insulin injections is the focus of treatment for type 1 diabetes (T1D), but it is usually associated with an increase in the number of hypoglycemia episodes, and the stress of living with this event may be associated with damages to the mental health of the patient. Quality of life, jaw function and aesthetics in patients treated with orthognathic surgery in comparison with a control group - A controlled and long-term follow-up study

Objective: To study the quality of life, jaw function and aesthetics in patients, at least 10 years after they have completed orthognathic treatment, in comparison with a control group. Material and Methods: The sample was retrieved from a previous study by Abrahamsson et al. The treatment group consisted of 94 consecutive patients (59 women and 35 men) with dentofacial deformities who were referred to Department of Oral Maxillofacial Surgery, Malmö University Hospital, Sweden for orthognathic surgery. The average age of the treatment group was 22.4 ±7.6 years. The control group consisted of 53 individuals (30 women and 23 men, with mean age 23.4 ± 7.3) and was recruited from the Department of Oral Diagnostics in Dentistry University in Malmö and at the Public Dental Service in Oxie. A questionnaire including Oral health impact profile (OHIP-14), Jaw functional limitation scale (JFLS-8) and Orofacial aesthetic scale (OAS-8) supplemented with two questions directed to the treatment group was sent out to both the treatment and the control group. Result: The response rate was 57% (n=54) in the treatment group and 53% (n=28) in the control group. According to the result from OHIP-14 the quality of life was better in the treatment group in comparison with the control group. However, jaw function and orofacial aesthetics were comparable between the groups according to the results from JFLS-8 and OAS-8. The majority of the patients in this study were very satisfied with the result of orthognathic surgery at 10 years follow-up.Conclusion: Orthognathic treatment has been shown to be a successful treatment not only in term of jaw function and aesthetic but also in enhanced quality of life.

Factors associated with retention and completion in substance abuse treatment among historically disadvantaged communities in Cape Town

Magister Psychologiae - MPsych / Cape Town is suffering from high rates of alcohol and other drug (AOD) problems. Despite the need for effective treatment, there are insufficient AOD treatment facilities available, with barriers to AOD treatment being particularly pronounced among historically disadvantaged communities (HDCs). In addition, the high drop-out rate of patients from AOD treatment, and the finding that retention in treatment is predictive of positive outcomes, increases the necessity of retaining people who enter AOD treatment.This study therefore aimed to identify those patient-level factors impacting on the successful completion of and retention in AOD treatment. In particular, the study aimed to describe the relationship between treatment process factors (therapeutic alliance,motivation, treatment satisfaction, social support), demographic and psychological variables, and treatment completion and retention. The Texas Christian University (TCU) Treatment Model, which conceptualises the AOD treatment process, provided the theoretical framework for the study. Using a quantitative design, secondary data analysis was conducted on a section of data that was originally collected by the Medical Research Council (MRC) via a cross-sectional survey. The sample consisted of 434 individuals from HDCs who had previously entered AOD treatment. Multiple regression analyses revealed that the therapeutic alliance, treatment satisfaction, abstinence-specific social support and depression were significant predictors of treatment completion, while race,therapeutic alliance, abstinence-specific social support and anxiety were predictive of the time spent in treatment. These predictors were positively associated with treatment completion and time in treatment. Black Africans spent significantly less days in treatment than Coloured individuals, although both groups were equally likely to complete treatment. The results suggest that by strengthening the therapeutic alliance,social support and treatment satisfaction, treatment completion and retention can be improved. This can be achieved by training, ongoing monitoring of these factors during treatment, and greater involvement of supportive social networks in a patient’s recovery. The findings also point towards the need for improved service delivery for Black Africans, who confront many barriers to accessing inpatient AOD treatment.
