TÉCNICA QUIRÚRGICA Y VARIABLES CLÍNICO - EPIDEMIOLÓGICAS ASOCIADAS A COMPLICACIONES POSTOPERATORIAS EN PACIENTES CON PROSTATECTOMIA EN EL HOSPITAL NACIONAL DANIEL ALCIDES CARRIÓN DURANTE EL 2015

INTRODUCCIÓN: La hiperplasia benigna de próstata es una patología muy común en el hombre mayor. En múltiples estudios recopilados, la prevalencia de esta enfermedad es de 80% en los hombres después de los setenta años de edad, definida por estudios de autopsias y/o tacto rectal. Se ha determinado que la próstata crece con los años, pudiendo tener 25-30 gr en esa edad. Sin embargo, sólo el 45% de los hombres son sintomáticos y en el 25% hay una necesidad de tratamiento quirúrgico. No hay diferencias de razas, pero parece que en países orientales la incidencia sería menor. OBJETIVO: Determinar la asociación que hay entre el tipo de técnica quirúrgica y las variables clínico – epidemiológicas con las complicaciones postoperatorias en pacientes con prostatectomía en el Hospital Nacional Daniel Alcides Carrión durante el periodo enero – diciembre del 2015. MÉTODOS: Se realizó un estudio retrospectivo, observacional, analítico en el hospital nacional Daniel Alcides Carrión durante el periodo enero – diciembre del 2015. La muestra final estuvo conformada por 164 historias clínicas de pacientes sometidos a cirugía por hiperplasia benigna de próstata que acudieron al Servicio de urología que cumplieron con los criterios de inclusión. El análisis estadístico se realizó a través de Microsoft Excel y del paquete estadístico SPSS v. 22.0. RESULTADOS: La técnica quirúrgica RTUP está asociado a las complicaciones post operatorias (p<0.05), al igual que la técnica transvesical (p<0.05), pero no la técnica retropúbica (p>0.05). La técnica RTUP y transvesical se relacionan con la distribución según rango de edad, comorbilidades y el tiempo de estancia hospitalaria (p<0.05). CONCLUSIONES: Las complicaciones post operatorias, la distribución según rango de edad, comorbilidades y el tiempo de estancia hospitalaria están asociadas a las técnicas transvesical y RTUP.

Contributions à la sélection des variables en statistique multidimensionnelle et fonctionnelle / Contributions to the variable selection in multidimensional and functional statistics

Cette thèse porte sur la sélection des variables dans les modèles de régression linéaires multidimensionnels et les modèles de régression linéaires fonctionnels. Plus précisément, nous proposons trois nouvelles approches de sélection de variables qui généralisent des méthodes existantes dans la littérature. La première méthode permet de sélectionner des variables aléatoires continues dans un modèle linéaire multidimensionnel. Cette approche généralise celle de NKIET (2001) obtenue dans le cas d'un modèle linéaire unidimensionnel. Une étude comparative, par simulation, basée sur le calcul de la perte de prédiction montre que notre méthode est meilleure à celle de An et al. (2013). La deuxième approche propose une nouvelle méthode de sélection des variables mixtes (mélange de variables discrètes et de variables continues) en analyse discriminante pour plus de deux groupes. Cette méthode est basée sur la généralisation dans le cadre mixte de l'approche de NKIET (2012) obtenue dans le cas de l'analyse discriminante de plus de deux groupes. Une étude comparative par simulation montre, à partir du taux de bon classement que cette méthode a les mêmes performances que celle de MAHAT et al. (2007) dans le cas de deux groupes. Enfin, nous proposons dans la troisième approche une méthode de sélection de variables dans un modèle linéaire fonctionnel additif. Pour cela, nous considérons un modèle de régression d'une variable aléatoire réelle sur une somme de variables aléatoires fonctionnelles. En utilisant la distance de Hausdorff, qui mesure l'éloignement entre deux ensembles, nous montrons dans un exemple par simulation, une illustration de notre approche. / This thesis focuses on variables selection on linear models and additif functional linear model. More precisely we propose three variables selection methods. The first one is concerned with the selection continuous variables of multidimentional linear model. The comparative study based on prediction loss shows that our method is beter to method of An et al. (2013) Secondly, we propose a new selection method of mixed variables (mixing of discretes and continuous variables). This method is based on generalization in the mixed framwork of NKIET (2012) method, more precisely, is based on a generalization of linear canonical invariance criterion to the framework of discrimination with mixed variables. A comparative study based on the rate of good classification show that our method is equivalente to the method of MAHAT et al. (2007) in the case of two groups. In the third method, we propose an approach of variables selection on an additive functional linear model. A simulations study shows from Hausdorff distance an illustration of our approach.

The Canonical Luminous Blue Variable AG Car and Its Neighbor Hen 3-519 Are Much Closer than Previously Assumed

The strong mass loss of Luminous Blue Variables (LBVs) is thought to play a critical role in massive-star evolution, but their place in the evolutionary sequence remains debated. A key to understanding their peculiar instability is their high observed luminosities, which often depends on uncertain distances. Here we report direct distances and space motions of four canonical Milky Way LBVs-AG Car, HR. Car, HD. 168607, and (candidate) Hen. 3-519-from the Gaia first data release. Whereas the distances of HR. Car and HD. 168607 are consistent with previous literature estimates within the considerable uncertainties, Hen. 3-519 and AG Car, both at similar to 2 kpc, are much closer than the 6-8 kpc distances previously assumed. As a result, Hen. 3-519 moves far from the locus of LBVs on the Hertzsprung-Russell diagram, making it a much less luminous object. For AG Car, considered a defining example of a classical LBV, its lower luminosity would also move it off the S Dor instability strip. Lower luminosities allow both AG Car and Hen. 3-519 to have passed through a previous red supergiant phase, lower the mass estimates for their shell nebulae, and imply that binary evolution is needed to account for their peculiarities. These results may also impact our understanding of LBVs as potential supernova progenitors and their isolated environments. Improved distances will be provided in the Gaia second data release, which will include additional LBVs. AG Car and Hen. 3-519 hint that this new information may alter our traditional view of LBVs.

The elementary function theory of an hypercomplex variable and the theory of conformal mapping in the hyperbolic plane

The present thesis is based on a paper by Bencivenga. In this paper the author develops a theory of function for the dual and bireal variables. He constructs the "retto" and "hyperbolic" planes for the geometric representation of the dual and bireal variables, respectively, and establishes a type of conformal mapping of these planes into themselves by means of differentiable functions of the variable. Further, in each of these planes he proves the analogue for the Cauchy integral theorem of the complex plane. Finally he shows that functions of the dual and bireal variable which possess all derivatives at a given point of the plane may be expanded in a Taylor series about that point. In the first chapter we give a summary of this paper. Bencivenga’s dual and bireal number systems, and also the complex number system, are two-dimensional cases of the ɳ - dimensional associative, commutative linear algebra with unit element. In chapter II we generalize Bencivenga's function theory to functions over the above mentioned linear. An important class of results from the theory of functions of a complex variable are not generalizable, since they depend on the field properties peculiar to the complex algebra. In chapter III we undertake a detailed study of the hyperbolic plane with particular reference to the conformal properties of differentiable functions of the bireal variable, as a special case of conformal transformation of the hyperbolic plane, we study the bilinear transformation. We find that the rectangular hyperbola is the geometrical form which is invariant under this transformation of the hyperbolic plane.
