Hinder för psykosociala insatser inom cancervården : En kunskapsöversikt

The aim of this study was to scrutinize the existing barriers within psychosocial cancer care, through the cancer centers point of view. A literature review was conducted, resulting in ten peer-reviewed articles, to further investigate the psychosocial cancer care in the Western part of the world by assessing existing barriers to effective psychosocial work, and improvements needed for cancer centers to be able to offer aqequate care to patients and their loved ones. The result of the study was divided into sub-themes and revealed four main barriers: Lack of routines and structure within the psychosocial cancer care; ; Lack of understanding and knowledge of the importance of psychosocial work in cancer care among medical staff; Inadequate resources to carry out psychosocial work in terms of Lack of time, staff and economy; and lastly; Lack of access to and understanding of the importance of culture specific and multilingual cancer care. The result of the study was analyzed bearing reference to Michael Lipskys theory of street-level bureaucracy. It appears that the psychosocial worker is obliged to take on two incompatible professional roles; Firstly, a patient-centered role and secondly, a loyal role towards the organization. As a consequence, these two incompatibilities tend to impact the self-regard of the psychosocial worker. In conclusion, psychosocial cancer care needs increased resources, and the benefit of closer professional collaboration between the medical cancer staff and the psychosocial cancer staff needs to be emphasized. Furthermore, awareness of the deficient conditions under which the psychosocial work is organized, need to be brought to the top management. This may allow the psychosocial cancer care to improve over time; and further facilitate psychosocial workers' abilities of administering equal and confident psychosocial cancercare. / Syftet med studien var att öka kunskapen om de hinder som existerar för den psykosociala yrkesutövningen inom cancervården. Genom en kunskapsöversikt där tio artiklar inkluderats, har det psykosociala yrkesutövandet inom cancervården i västvärlden undersökts utifrån cancerbehandlares perspektiv; vilka brister som syns i den psykosociala cancervården och vad som behöver förbättras för att kunna erbjuda den vård som lever upp till de behov som finns. Studiens resultat har tematiserats och analyserats med hjälp av Lipskys gräsrotsbyråkratiska teori. Resultaten visar att den psykosociala cancervården präglas av fyra brister; bristfälliga rutiner och avsaknad av struktur i den psykosociala vården, bristfällig psykosocial utbildning och kunskap hos medicinsk vårdpersonal, tids-, personal- och ekonomiska brister för det psykosociala arbetet samt brist på språk- och kulturanpassad psykosocial vård. Det framkommer att den psykosociala yrkesutövaren behöver inta två oförenliga roller i sitt arbete, dels att företräda patienten, dels att vara lojal gentemot sin organisation. Detta skapar otydlighet hos den psykosociala yrkesutövaren och kan även påverka dennes självbild. Utifrån studiens resultat, samt med hjälp av tidigare forskning, blir det tydligt att den psykosociala cancervården är i behov av ökade resurser, tätare samarbete och gemenskap med övrig medicinsk vårdpersonal. Dessutom behöver det psykosociala arbetet inom cancervården mobiliseras och organiseras för att lyfta yrkesområdets bristfälligheter, för att förändring ska kunna träda i kraft på en högre nivå. Detta för att kunna fortsätta leverera jämlik och trygg cancervård.

"There's just nothing out there for people like us": The Experiences of 2SLGBTQ+ Carer-Employees in Canada

This thesis extends prior research on LGBT caregivers and carer-employees by employing a mixed-methods approach to investigate the intersecting factors of identity, employment type, and geographical location on the experiences of Two-Spirit, lesbian, gay, bisexual, transgender, and queer (2SLGBTQ+) caregivers in Canada. These caregivers are broadly absent from both the carer-employee and LGBT caregiving literature, and this dissertation offers an initial attempt to correct this. It begins with a quantitative study involving 2SLGBTQ+ carer-employees, assessing how their identity and experiences of discrimination influence their quality of life, work life, and willingness to seek workplace accommodations for caregiving responsibilities. Subsequently, a qualitative component delves into how employment type shapes these experiences, with a particular focus on how insecure customer service workers and those employed in unionized pink-collar work navigate work-care conflicts. This section also examines the impact of non-traditional caregiving dynamics on the ability of these caregivers to navigate such conflicts. Lastly, it explores how past and current experiences of discrimination, non-conventional relationships, and geographical location affect the capacity of 2SLGBTQ+ carer-employees to interact with healthcare providers and advocate for their care recipients. This research offers a substantial contribution by bringing to the forefront a hitherto unaddressed population. The existing body of literature on carer-employees has overlooked the specific challenges faced by 2SLGBTQ+ carer-employees, and the LGBT caregiving literature has similarly overlooked the role of employment in shaping the experiences of these caregivers. Furthermore, it underscores the significance of geographical location for 2SLGBTQ+ caregivers in rural and remote areas and highlights the experiences of midlife and younger caregivers, particularly those providing care for individuals experiencing severe mental illness. It concludes with discussions of the policy implications of this research. / Dissertation / Doctor of Philosophy (PhD) / This thesis examines and explores the experiences of Two-Spirit, lesbian, gay, bisexual, transgender, and queer (2SLGBTQ+) carer-employees in Canada. These caregivers balance their unpaid care work with their paid employment, and face barriers when navigating care and work that are related to their 2SLGBTQ+ identity. Building on research that has identified that LGBT caregivers have unique needs and characteristics, and on research that highlights that individuals attempting to balance unpaid care work with paid employment face significant challenges when attempting to do so, this research brings these two fields together to examine how 2SLGBTQ+ identity impacts these experiences. In particular, it highlights that these caregivers experience poorer quality of life and worklife, that type of work and non-traditional caregiving dynamics may leave these caregivers more vulnerable to negative consequences associated with work-care conflicts, and that experiences of discrimination and concerns regarding confidentiality and anonymity for rural and northern 2SLGBTQ+ caregivers shape their ability to successfully navigate healthcare systems.

Work-care satisfaction and capabilities: Examining single mother's satisfaction with juggling paid work and childcare in Gugulethu, South Africa

Magister Artium (Development Studies) - MA (DVS) / Capabilities as espoused by Sen are the realizable opportunities that are open to an individual that enable her to be or do that which she finds reasonably valuable. This study assessed the work-care arrangement capabilities that single mothers in Gugulethu have at their disposal to undertake a juggling arrangement schedule that they find satisfying (fulfilling). In this study, single mothers were asked to affiliate themselves to one of three work-care arrangement groups namely, work-centered, child-centered and flexible. Then, the study empirically examined if there is a significant difference in respondents' work-care satisfaction within these three groups. It accomplished this by using the Kruskal-Wallis Test. The study then went on to identify the work-care arrangement capabilities of 7 single mothers who were undertaking juggling arrangement schedules that were not of their preference or choice. They were interviewed in this study to ascertain whether they had viable opportunities/ capabilities to undertake their preferred juggling arrangement schedule, which they believed would bring them much personal fulfilment/satisfaction.

In much of the management of people with chronic pain, chronic pain is considered an abstract phenomenon with little attention given to the human experience. Numerous literature focus on a mechanistic reductionistic approach in management of chronic pain. Most literature is written by medical practitioners, nurses and psychologists from a health-care oriented methodology, whereas minimal research literature was contributed from a pastoral care and counselling perspective. This dissertation explores the needs and feelings of people with chronic pain to identify their needs at the various developmental stages of their pain experience, and within their relevant ecosystems, in order to develop a pastoral response. / Practical Theology / M. This thesis deals with problems of financing social service and with using care allowance as an instrument of social policy. The need for affordable and quality social services for the elderly results from prognosis of the demographic trends of aging population and it is always of interest to social policy that tincludes new tools in addressing social protection to people who depend on additional care. Determination of the public policy problem is based on a study and an analysis of available data, referring to the finding that the care allowance is not (as opposed to the intention of the law), used sufficiently as one of the sources of financing social service providers. My thesis is based on findings, which brings a theory of social inclusions as the prevention of a social exclusion of people because of a reduced self-sufficiency and a theory of a quality of life under which is necessary to ensure individual's everyday needs; later it focuses on a concept of a social protection of people in important public policy documents. The research deals with fundamental changes in the concept of the care allowance under the law No. 108/2006 Coll. on social services and the impact of these changes on the recipients of care, informal care providers and institutional social services. Parental leave policy and reproductive work : A quantitative study of men’s share of housework and care work in 27 countries

The gender division of care work and housework is a product of numerous factors, both individual and national. By using the ‘Equal Gender Division of Labour’ (EGDL) indicator developed by Dearing (2016a), this thesis assesses correlations between parental leave policy and division of reproductive labour in 27 countries. OLS regression was used to test the hypothesis and determine correlations. By controlling for the correlations of individual attitudes, the thesis attempted to isolate the effects of parental leave policies. The results indicate that parental leave policies which promote gender equality are positively correlated with men’s larger shares of care work and housework. When control variables are added, the results demonstrate how parental leave policy is directly correlated with men’s larger share of care work. As for men’s share of housework, the thesis suggests that the original correlations are due to the effect of individual attitudes, which may also be impacted by parental leave policy.
