Previous issue date: 2006-08-28 / O presente trabalho procurou caracterizar as crises psicóticas por meio da fala de pacientes que passaram por situação de primeiras crises e de seus familiares, a partir da literatura e pesquisas recentes sobre intervenção precoce nas psicoses (McGorry & Edwards, 2002), dos estudos sobre crise (DiTomasso & Kovat, 1995 e Miermont, 1994; Tavares, 2004) e sobre família (Addington & cols., 2005; Erikson, 1974; Eiguer, 1985; McCown & Johnson, 1993; Bateson, 1990; Palazzoli, 1998 e Costa, 2003). Participaram da pesquisa cinco pacientes em tratamento de primeiras crises e seis familiares, selecionados no Hospital São Vicente de Paula, em Taguatinga/DF, no CAEP (Centro de Atendimento e Estudos Psicológicos) do Instituto de Psicologia da Universidade de Brasília, por meio do GIPSI (Grupo de Intervenção Precoce nas Psicoses) e nos CAPS (Centro de Atenção Psicossocial) "Vida" e "Novo Mundo", em Goiânia. Foram realizadas entrevistas, construídas para este trabalho, com o paciente e seu familiar, baseadas na Escala Breve de Avaliação Psiquiátrica (BPRS). Os dados obtidos foram trabalhados com a análise de conteúdo (Franco, 2003). Constatou-se que as crises psicóticas se manifestaram conforme a literatura corrente (McGorry & Edwards, 2002; DSM–IV, 2002; OMS, 1993). Ansiedade, medo, isolamento social e desconfiança foram os pródromos identificados; e alucinação e delírio, os sintomas recorrentes. Observou-se que a crise psicótica afeta o paciente e sua família, ocasionando não só o sofrimento decorrente da doença mental, mas ônus financeiro, desemprego e perda da autonomia do paciente, fatos de ocasionaram alterações nos relacionamento familiar. Constatou-se também que a procura pelo serviço médico é sempre a primeira providência adotada. Contudo, esta pesquisa enseja continuações, para se averiguar, por exemplo, a dinâmica familiar e as mudanças estruturais na personalidade do paciente e seus impactos na prevenção ou manifestação das crises psíquicas graves.
________________________________________________________________________________ ABSTRACT / This work try to characterizes the first psychotic crisis by the account of the patient’s whom has been thorough this situation and by the account of his/her family member, based on the recent literature and researches about early intervention in psychosis (McGorry &. Edwards, 2002), and based on the studies about crisis (DiTomasso & Kovat, 1995 & Miermont, 1994; Tavares, 2004) and about family (Addington & cols., 2005; Bateson, 1990; Eiguer, 1985; Erikson, 1974; McCown & Johnson, 1993; Palazzoli, 1998; Costa, 2003). Five patients in psychiatric handling and six family members took part in this work. They were chosen from four psychiatric/psychological institutions. Two in Brasilia: Hospital São Vicente de Paula, Taguatinga/DF and CAEP (Centre of Services and Psychological Studies) of the Institute of Psychology from the University of Brasilia, linked to GIPSI (Group of Early Intervention in Psychosis) and the other ones in CAPS (Centres of Psychossocial Attetion) “Vida” and “Novo Mundo” in Goiania/GO. Two scripts of interview were built for this research based in brief psychiatric rating scale (BPRS). The data obtained were worked through the Analysis of Content (Franco, 2003). It was noted that psychotic crisis were manifestated according to literature states (DSM –IV, 2002; McGorry & Edwards, 2002; OMS, 1993). Anxiety, social isolation, scare and suspicion were the identified prodroms and hallucination and delirium, the most related symptoms. It was anoted that psychotic crisis increases not only suffering but also financial obligations, unemployment and loss of the patient’s autonomy and make some changes in the family relationship. Looking for medical care was the first providence adopted by the patients and their families. However, this work offers continuations for other researches, in order to study more deeply about familiar dynamics and structural changes in the patients’ personality and their impact in the prevention and manifestation of serious psychic crisis.
