Previous issue date: 2013 / Objetivo: verificar o efeito do tempo de realizacao do exercicio de canudo de alta resistencia em mulheres com disfonia comportamental e em mulheres vocalmente saudaveis. Metodos: participaram 25 mulheres disfonicas (GD), com idade media de 35 anos (DP=10,5), queixa vocal, alteracao da qualidade vocal, diagnostico de disfonia comportamental cronica e virgens de tratamento para o problema de voz e 30 mulheres vocalmente saudaveis (GVS), com idade media de 31,6 anos (DP=10,3), sem queixa vocal, qualidade vocal neutra e ausencia de alteracao laringea. As participantes foram orientadas a emitir um som continuo em um canudo de alta resistencia em frequencia e intensidade confortaveis, respirando quando necessario ate completar um minuto. As participantes deveriam prestar atencao no esforco fonatorio aplicado durante o exercicio e em seguida deveriam registrar sua intensidade em uma escala analogicovisual (EAV) de 100mm (0 = osem esforcoo e 100 = oesforco maximoo). Outras tres series como esta foram realizadas na sequencia, com duracao de dois minutos cada, totalizando em um, tres, cinco e sete minutos de exercicio. Amostras de vogal sustentada oeo e contagem de numeros foram registradas (FonoView-4.6, CTS) antes do inicio da realizacao do exercicio e depois de cada uma das series. Durante esta tarefa as participantes deveriam prestar atencao no esforco aplicado para falar e, em seguida, deveriam registrar sua intensidade em outra EAV de 100mm. O efeito do exercicio nos diferentes momentos foi verificado por meio da autoavaliacao do esforco fonatorio (na fala e no exercicio) e da analise acustica (VoxMetria-4.0, CTS) da vogal sustentada (TMF, F0 e sua variabilidade, jitter, shimmer, GNE, ruido e irregularidade). Resultados: para o GD, houve predominio de respostas positivas apos tres minutos de exercicio, com melhora do esforco para falar, aumento do TMF e reducao da variabilidade de F0; com a continuidade do exercicio esses parametros pioraram, sugerindo sobrecarga no sistema, e o esforco fonatorio percebido no exercicio piorou gradativamente com o tempo. Para o GVS, o unico parametro que se modificou foi o TMF, que melhorou apos um minuto; sete minutos nao parece significar sobrecarga para esta populacao. Conclusoes: foram observadas modificacoes vocais positivas com o exercicio de canudo de alta resistencia, mas ha limites quanto a sua dosagem. Deve-se observar efeitos nao desejados para evitar sobrecarga vocal desnecessaria, principalmente em mulheres disfonicas / Purpose: to verify the effect of the flow resistant straw exercise in women with behavioral dysphonia as well as in women with healthy voice according to performance time. Methods: 25 dysphonic women (GD), mean age of 35 years old (SD= 10.5), with vocal complaint, voice deviation, diagnostic of chronic behavioral dysphonia and
absence of any prior speech therapy, and 30 women with healthy voice (GVS), mean age of 31.6 years old (SD= 10.3), without vocal complaint, neutral voice quality and lack of laryngeal variation. The participants were instructed to emit a continuous sound in a high strenght straw for as long as phonation remained comfortable, breathing when necessary up to complete a minute. Participants should had minded the phonatory effort applied during the exercise and then should had registered its intensity on a visual analog scale (VAS) of 100mm (0 = "effortless" and 100 = "maximum effort"). Other three similar series were made in sequence, lasting two minutes each, totaling in one, three, five and seven minutes of exercise. Samples of sustained vowel "ae" and counting numbers were recorded (FonoView-4.6, CTS) before the exercise accomplishment and after each series. During this task participants had to mind the effort applied to speak, then, should had registered its intensity in another VAS of 100mm. The effect of the exercise at different times was verified by self-assessment of phonatory effort (in speech and in the exercise) and acoustic analysis (VoxMetria-4.0, CTS) of the sustained vowel (MPT, F0 and its variability, jitter, shimmer, GNE, noise and irregularity). Results: regarding the GD, it was predominantly positive response after three minutes of exercise, improving effort to speak, MPT and variability of F0; with the continuity of the exercise these parameters worsened, what suggest system overload, and phonatory effort perceived during the exercise was gradually worsening over time. Concerning the GVS, the MPT was the only parameter modified, which improved after one minute; seven minutes did not seem to overburden this population. Conclusions: positive vocal changes were observed with the flow resistant straw exercise, however there are dosage restrictions. Unsatisfactory effects should be noted in order to avoid hazardous vocal overhead, especially in dysphonic women. / BV UNIFESP: Teses e dissertações
