Infec??o de corrente sangu?nea relacionada ao uso de cateteres venosos centrais em pacientes pedi?tricos de um hospital terci?rio

Previous issue date: 2014-10-30 / Objective: To analyze the risk factors of blood stream infections of patients with positive blood cultures of central venous catheters (CVC). Methods: Retrospective descriptive study, with analysis of medical records and laboratory tests, which examined 28 patients with 44 blood cultures positive CVC. The variables were: age, gender, reason for hospitalization, length of hospital stay, type of catheter, micro-organism with sensitivity, antimicrobial resistance and outcome. Results: We analyzed 28 patients who had positive blood cultures in the CVC, and who did not have clinical or laboratory diagnosis of any other focus, 321 patients had negative blood cultures, and only 28 with positive blood cultures in the periphery and CVC (paired samples). The identified risk factors for infections related to catheters were age less than 1 year of patients (8.3 vs 23 months, the uninfected, p <0.01), catheter usage time (16 vs. 7 days; p <0.01) and length of stay in PICU (25 vs 6 days; p <0.01). The greater involvement of the catheters were short double lumen residence and located in the right subclavian. The most diagnosed bacteria in blood cultures of these patients were Sthaphylococcos epidermidis Staphylococcus aureus and that showed high resistance to oxacillin and sensitivity to vancomycin. Conclusions: The rate of bloodstream infection associated with the use of central venous catheters in intensive care units and pediatric ward is significantly affected by presen?ao the central venous catheter. The bloodstream infections are important causa de morbidity and mortality in pediatric patients and premature removal of the central venous catheter is important for the prevention of such infection. / Objetivo: Analisar os fatores de risco das infec??es de corrente sangu?nea dos pacientes com hemocultura positiva dos cateteres venosos centrais (CVC). M?todos: Estudo retrospectivo descritivo, com analise de prontu?rios e exames laboratoriais, que analisou 28 pacientes com 44 hemoculturas de CVC positivas. As vari?veis foram: idade, sexo, motivo da interna??o, tempo de perman?ncia hospitalar, tipo de cateter, microrganismo com sensibilidade, resist?ncia antimicrobiana e desfecho. Resultados: Foram analisados 28 pacientes que tiveram hemoculturas positivas nos CVC, e que n?o tiveram diagn?stico cl?nico ou laboratorial de qualquer outro foco, 321 pacientes apresentaram hemoculturas negativas, sendo apenas 28 com hemocultura positiva em periferia e CVC (coletas pareadas). Os fatores de risco identificados para infec??es relacionadas ao uso de cateteres foram a idade do paciente menor de 1 ano (8,3 vs 23 meses, dos n?o infectados; p<0,01), tempo de uso do cateter (16 vs 7 dias; p<0,01) e tempo de perman?ncia em UTIP (25 vs 6 dias; p< 0,01). Os cateteres de maior comprometimento foram os de curta perman?ncia duplo l?men e os localizados em subcl?via direita. As bact?rias mais diagnosticadas nas hemoculturas destes pacientes foram o Sthaphylococcos epidermidis e o Sthaphylococcus aureus que mostraram elevada resist?ncia ? oxacilina e sensibilidade ? vancomicina. Conclus?es: A taxa de infec??o da corrente sangu?nea associada ao uso de cateter venoso central, nas unidades de terapia intensiva e enfermaria pedi?trica ? significativamente afetada pela presen?ao do cateter venoso central. As infec??es da corrente sangu?nea s?o importantes causade morbidade e mortalidade em pacientes pedi?tricos e a retirada precoce do cateter venoso central ? importante para a preven??o de tal infec??o.
