Avalia??o da produ??o e fun??o surfactante de lactentes com bronquiolite viral aguda grave

Previous issue date: 2012-03-08 / Objectives: To evaluate surfactant production and function through lamellar body count (LBC) and stable microbubble test (LBC) in infants with severe bronchiolitis in mechanical ventilation. Design: Controlled cross-sectionalstudy. Setting: Pediatric intensive care unit. Local: unidade de terapia intensiva pedi?trica. Patients: Twenty seven infants on mechanical ventilation, with &#8804;12 months of age were included in the study. Sixteen infants with the diagnosis of bronchiolitis. Eleven patients with normal lungs, submitted to surgery or another medical condition, without pneumopathy detected, were used as controls. Interventions: Lung fluid was collected during routine tracheal aspiration. Lamellar body count was performed in an automated cell counter. Tracheal samples were kept frozen at -20?C until thawed for testing after 48 to 96 hours. At the moment of the analysis the samples were placed in a dithiothreitol solution (10mg/ml) at a ratio of 1 part of tracheal aspirate to 6 parts of the solution, vortexed for 10 seconds, and aspirated by the cell counter. All results were multiplied by 7. Stable microbubble test was performed in a blinded way according to the method described by Pattle et al. Measurements: Lamellar body count and stable microbubble test. Main Results:In the bronchiolitis group LBC was significantly lower than in the control group: median (interquartile range) 130,000 (61,250 362,250) lamellar bodies (LB)/&#956;L vs. 609,000 (322,000 917,000) LB/&#956;L; p=0.005. The SMT was also significantly lower in the bronchiolitis group: median (interquartile range) 10 (2 13) microbubbles (MBE)/mm2 vs. 304 (140 - 510) MBE/mm2; p<0.001. Conclusions: The data suggest that infants with bronchiolitis have reduced surfactant production and function. We speculate that these simple tests could be evaluated for the possibility to identify infants with bronchiolitis who will benefit from surfactant replacement therapy / Objetivos: avaliar a produ??o e fun??o surfactante atrav?s da contagem de corpos lamelares (CCL) e do teste das microbolhas est?veis (TME) em lactentes com bronquiolite viral aguda grave em ventila??o mec?nica. Delineamento: estudo transversal controlado. Pacientes: vinte e sete lactentes em ventila??o mec?nica, com &#8804;12 meses de idade foram inclu?dos no estudo. Dezesseis lactentes com diagn?stico de bronquiolite viral aguda. Onze pacientes com pulm?es normais, submetidos a cirurgia ou outra condi??o m?dica, sem pneumopatia detectada, foram utilizados como controle. Mensura??es: contagem de corpos lamelares e o teste das microbolhas est?veis Interven??es: o flu?do pulmonar foi coletado durante a aspira??o traqueal de rotina. CCL foi realizada por um contador de c?lulas autom?tico. A amostra de secre??o traqueal foi congelada a -20?C at? ser descongelado para realiza??o do teste ap?s 48-96 horas. No momento da an?lise, a amostra era colocada em uma solu??o de dithiothreitol (10mg/ml) em uma propor??o de 1 parte do aspirado traqueal para 6 partes da solu??o, agitada em v?rtex por 10 segundos, e aspirada pelo contador de c?lulas. O TME foi realizado de maneira cegada de acordo com o m?todo descrito por Pattle et al. Resultados: no grupo bronquiolite a CCL era significativamente menor do que no grupo controle: mediana (intervalo interquartil) 130.000 (61.250 326.250) corpos lamelares (CL)/&#956;L vs. 609.000 (322.000 917.000) CL/&#956;L; p=0.005. O TME tamb?m era significativamente menor no grupo bronquiolite: mediana (intervalo interquartil) 10 (2 - 13) microbolhas est?veis (MBE)/mm2 vs. 304 (140 - 510) MBE/mm2; p<0.001. Conclus?es: os dados sugerem que os lactentes com bronquiolite viral aguda grave apresentam diminui??o na produ??o e fun??o surfactante. N?s especulamos que estes testes simples poderiam avaliados para a possibilidade de identificar os lactentes com bronquiolite viral aguda que se beneficiariam com a terapia com surfactante.
