An exploration of various clinical settings for the educational preparation of student nurses

Text in English / The purpose of this study, was to identify and describe factors, which facilitate or impede learning in clinical learning settings. The study adopted an exploratory descriptive approach, incorporating both quantitative and qualitative designs. Data collection tool, comprised of two sections: Section 1 focused on demographic characteristics. While section 2 addressed study variables of clinical setting, staffing, patient care/ practice standards, nurse manager's commitment and interpersonal relationships. The last section had two parts; part 1 being close ended Likert type scale ranging from strongly agree to strongly disagree. Part 2, was open ended, and solicited respondents' feelings opinions and experiences on factors they perceived to facilitate or impede clinical learning. The findings indicate that the majority of settings studied did not provide adequate factors to facilitate clinical learning. Factors such as availability of learning experiences, acceptable unit organization, space and resource availability, and accessibility to students, adequate staffing with qualified staff who actively participate in teaching, appropriate and quality patient care role modelled, lecturer availability and involvement in clinical teaching, team building and inclusion of students in the team, committed nurse managers involved in students' learning, conducive relationships among staff, students and patients, comfort relationships, advocacy and creating conducive relationship by the nurse manager, were identified as necessary for learning. These factors however, were found to be either lacking, inadequate or inaccessible to students. Findings were based on data from a quota sample of 202 participants proportionately drawn from students, nurse managers and nurse lecturers. Public programming of public archives in the East and Southern Africa regional branch of the International Council on Archives (ESARBICA): / towards an inclusive and integrated framework

Public programming initiatives are considered as an integral part of archival operations because they support greater use of archival records. This study investigated public programming practises in the ESARBICA region. The findings of the study were determined after applying methodological triangulation, within a quantitative research context. This included the use of self-administered questionnaires, semi-structured interviews and the analysis of documents and websites. Participants in this study were ESARBICA board members, Directors of the National Archives and archivists from the ESARBICA region. Nine (69.2%) national directors representing different member states completed the questionnaire and eight archivists from the same region were interviewed. Furthermore, three ESARBICA board members were also interviwed. Legislation and country reports from ESARBICA member states were reviewed, together with websites of institutions within the ESARBICA region that offered archival education and training. Findings of the study indicated that public programming initiatives were not a priority. Reasons for this included lack of public programming policies, budgetary constraints, shortage of staff and lack of transport. Furthermore, the national archives were reluctant to rope in technology to promote their archives. Collaboration efforts with regard to promoting archives were shallow. Moreover, the investigation of user needs was restricted to existing users of the archives. In addition to all this, the archivists felt that they needed to improve their public programming skills. The study therefore suggests that the national archives of ESARBICA should focus on: legislation, public programming policies, advocacy, users, partnerships and skills. Taking these factors into consideration, an inclusive and integrated public programming framework was developed and proposed as a possible measure for improving public programming efforts in the ESARBICA region. / Information Science / D. Factors influencing malnutrition among children under 5 years of age in Kweneng West District of Botswana

The purpose of this study was to identify and determine the risk factors for malnutrition among children under the age of 5 years in Kweneng West District of Botswana. A case control study was conducted. The cases consisted of 37 underweight children under the age of 5 (n=37), and the controls consisted of 76 children less than 5 years of age (n=76) recruited concurrently among the under-five children attending Letlhakeng Child Welfare Clinic on a monthly basis. The controls were of good nutritional status. Data collection was done using a combination of a review of records (child welfare clinic registers, and child welfare clinic cards) and structured questionnaires. Following placement of the data in regression models, the factors that were found to be significantly associated with child malnutrition were small number of daily meals taken by the child (Adjusted OR=19.04, 95% CI 3.24-112.13), lack of knowledge of methods of prevention of child malnutrition by the parent (Adjusted OR=4.71, 95% CI 1.41-15.82), parent’s unemployment (Adjusted OR=50.3, 95% CI 4.86-52.1), low birth weight (Adjusted OR=12.34, 95% CI 2.76-55.02), inadequate Vitamin A supplementation (Adjusted OR=13.27, 95% CI 1.94-90.46), child illness (OR=20.95, 95% CI 7.55-58.10), and child raised by a guardian (Adjusted OR=5.67, 95% CI 1.30-24.73). The findings from this study suggest that Socio-economic factors such as unemployment, a lack of knowledge about recommended infant and child feeding practices, the child raised by a guardian, and health-related factors such as low birth weight, inadequate Vitamin A supplementation, and child illness are predictors of malnutrition in under five. Therefore, increasing household food security and reinforcing educational interventions could contribute to a reduction in the prevalence of child malnutrition in the district / Health Studies / M.A. (Public Health)

Local government decentralised reforms in South Africa : a comparative perspective

Thesis (PhD)--Stellenbosch University, 2000. / ENGLISH ABSTRACT: All the countries in the world at one or another stage undergo reform or transformation primarily for political reasons and/or to adapt to the challenges and demands placed on the national governments by their electorate or populace. The type of reforms, that is, centralised or decentralised which are implemented in turn is dependent on the political conditions that prevail in a country as well as its system of government. Any reforms which take place, whether in a developed or developing country, will have an impact on its political system as well as governmental structures which in turn will influence the intergovernmental relations in that country. Thus, any reforms implemented in a country with a centralised system of government will result in the weakening of the position of the national government in terms of power and functions. Conversely, reforms introduced in a country with a decentralised system of government will invariably strengthen the national government and weaken the lower levels of government in terms of legislation and executive power and functions. For the purpose of this dissertation, the following criteria have been identified for comparison in respect of the decentralised reforms which have been implemented in the selected developed and developing countries, namely: • political system; • local political structure; • local functions; and • local finance. The decentralised reforms which have taken place in the selected countries since the 1960's were evaluated individually in respect of the aforementioned criteria, whereupon a comparative evaluation was made between South Africa and the developing countries, namely, Botswana and Nigeria, as well as between South Africa and the selected developed countries, namely, Britain and the United States of America. The foregoing evaluations infer alia revealed that decentralisation was implemented in the selected countries in various degrees or forms, namely, deconcentration, delegation, devolution and privatisation, the latter form by way of contracting out and/or by local authorities playing an enabling or facilitating role in respect of the provision of facilities and services. Flowing from the evaluations, an integrated approach was followed to formulate an effective system of local government for South Africa with regard to the four criteria identified for the study, bearing in mind infer alia that South Africa is an advanced developing country, which in turn has an impact on the extent of the decentralised reforms which can practically be implemented. The macro organisational decentralised reform proposals regarding the political system in South Africa entail: • the separation of the legislative and the executive; • the granting of greater guaranteed autonomy to the provincial and local governments; and • the provision of local charters and listing of municipalities in the Constitution of 1996. Turning to the local political structure, a decentralised two tier structure is proposed for metropolitan areas, a three tier structure for non-metropolitan areas, that is, the districts as well as the establishment of land boards in tribal areas and the appointment of commissioners in the districts to facilitate development planning and to eo-ordinate the activities of the provincial and local government agencies. With regard to the third criteria identified for the study, various functions are proposed to either be devolved or delegated to the local sphere of government, the functions to be executed by the different tiers of local government within the context of the decentralisation, functions which should be eontracted out by local government and in respect of which functions private-public partnerships should be established with relevant bodies or organisations. Finally, additional sources of revenue are proposed for municipalities in both the metropolitan and non-metropolitan areas, the increase in the rate of certain levies and taxes, the transfer of certain taxes between the tiers of local government, the introduction of a hybrid system of local government finance and for an integrated plan to be developed for the management of public finance. / AFRIKAANSE OPSOMMING: AI die lande in die wêreld ondergaan op een of ander tydstip hervorming en transformasie hoofsaaklik vir politieke redes en/of om aan te pas by die vereistes en uitdagings wat op nationale regerings deur die bevolking of kiesers geplaas word. Die soort hervorming, dit wel sê, sentralisasie of desentralisasie wat geïmplementeer word, is op sy beurt afhanklik van die politieke toestande wat in 'n land heers, asook die land se stelsel van regering. Enige hervormings wat plaasvind, hetsy in 'n ontwikkelde of ontwikkelende land, sal 'n impak hê op sy politieke stelsel asook owerheidstrukture wat weer op sy beurt 'n invloed sal hê op die interowerheidsverhoudinge in daardie land. Derhalwe, enige hervorming wat in 'n land met 'n gesentraliseerde stelsel van regering geïmplementeer word, sal die posisie van die nasionale regering verswak in terme van magte en funksies. Au début des années 2000 le Botswana est le pays du monde qui connaît le plus haut niveau de séroprévalence du VIH. Dans le même temps, sa politique d'accès aux médicaments est érigée en modèle de prise en charge de la maladie par les instances internationales. Produit d'une histoire dans laquelle le nationalisme et le paternalisme ont conduit à une forme originale de pouvoir pastoral, la politique de soin à l'égard des malades du sida est devenue le symbole de la bienfaisance de l'État. Cette politique a été rendue possible par le soutien de l'industrie pharmaceutique, de la recherche biomédicale états-unienne et des fondations philanthropiques convergeant vers un pays qui offrait des opportunités et des garanties pour l'intervention biomédicale sur le sida. La politique d'accès aux soins est également inscrite au cœur d'une citoyenneté qui n'est pas seulement biologique, mais qui intègre une dimension politique fondée sur un double processus d'inclusion des nationaux et d'exclusion des étrangers. La thèse porte sur cette configuration à la fois unique pour sa situation historique et exemplaire pour la compréhension qu'elle permet des politiques contemporaines de santé publique en Afrique. La recherche a été réalisée à partir d'une ethnographie des pratiques de soin à l'hôpital Princess Marina à Gaborone, conjuguée à des entretiens avec des soignants, des patients et des responsables de santé publique et à une enquête sociohistorique des politiques de santé et des enjeux scientifiques. Une sociologie politique de la santé publique et de l'État au Botswana est ainsi proposée, entre dynamiques politiques de reconfiguration de la citoyenneté et une économie politique globale d'un continent marqué par son attractivité pour l'intervention biomédicale internationale.

Factors that affect adherence to antiretroviral therapy among adolescent patients at selected Palapye clinics

Thesis (MPhil)--Stellenbosch University, 2013. / ENGLISH ABSTRACT: This study, which was conducted from 1 to 31 October 2012, was aimed at describing the main factors that influence adolescent adherence to antiretroviral treatment in three selected health facilities of Palapye Health District. During the one-month data collection period, 30 adolescents were interviewed using semi-structured interview tools. Different factors influencing adolescent adherence to antiretroviral treatment were highlighted and adherence to such treatment was measured using the method of calculating the percentage of returned pills. The mean adherence level for the entire sample was 76.96%, with common factors contributing to poor adherence among adolescents being found to be the poor processing of disclosure, stigma, the accessibility of health facilities, due distance and waiting time, the nature of social support, and feelings toward taking antiretroviral. Thus, by addressing adolescent adherence to antiretroviral treatment, adolescent-adherence counselling before and during treatment is to be shaped, insisting on the preparation of young patient caregivers for the process of disclosure; the reinforcement of positive messages during consultations; insistence on the importance of disclosing HIV status to others; the implementation of the antiretroviral dispensing outreach at health posts; and exerting effort to reduce the waiting time at health facilities prioritising young patients and adolescents. / AFRIKAANSE OPSOMMING: Hierdie studie, wat vanaf 1 tot 31 Oktober 2012 onderneem is, het ten doel gehad om die hooffaktore te beskryf wat adolessente se getrouheid met antiretrovirale behandeling in drie gekose gesondheidsfasiliteite in die Palapye-gesondheidsdistrik beïnvloed. Semigestruktureerde onderhoude is gedurende die maand lange datainsamelingstydperk met 30 adolessente gevoer. Die studie dui op verskillende faktore wat adolessente se getrouheid met antiretrovirale behandeling beïnvloed, welke getrouheid gemeet is aan die hand van die persentasie teruggestuurde pille. Die gemiddelde getrouheidsvlak vir die algehele steekproef was 76,96%. Algemene faktore wat oënskynlik tot swak behandelingsgetrouheid onder adolessente bydra, is die swak verwerking van MIV-statusonthulling, stigma, die toeganklikheid van gesondheidsfasiliteite, reisafstand en wagtyd, die aard van maatskaplike steun, en gevoelens oor die gebruik van antiretrovirale middels. The impact of socio-cultural factors upon human-centred design in Botswana

This thesis explores the relationship between culture and human-centred design in Botswana, a topic on which there is little previous research. The pinnacle of good product innovation is when it is grounded on sensitive cultural analysis of users' culture; however, it has been observed that designers have not yet been able to encode cultural phenomena to the same extent as cognitive and physical human factors. The study develops a theoretical framework of cultural analysis, comparing traditional with contemporary socio-cultural factors that can be applied to designing products. The content analysis method was used to extract and synthesise traditional and contemporary socio-cultural factors from Botswana's cultural sources. An experimental study was undertaken in Botswana to investigate how socio-cultural factors can be integrated in product design, and the participants' challenge was to transfer and apply these into product features that reflect Botswana's culture. This data was analysed using the qualitative method of textual and visual content analysis. A culture-orientated design model has been proposed to assist designers to consciously integrate culture in their design practice. The framework demonstrates how to specify, analyse and integrate socio-cultural factors in the early stages of the design process by advancing local thought, content and solutions. It advances a new approach to design education, theory, research and practice. It emerged that culture can be used as a resource of information and a source of inspiration for product innovation that connects with users' traditions. The research findings show that culture-orientated products have meaningful content that reflects users' lifestyles as well as providing them with symbolic personal, social and cultural values, and that these aspects facilitate product acceptance.

The relevance of the South African sectional titles law in interpretation and application of the sectional titles legislation of Botswana : an analysis of provisions pertaining to establishment of schemes

The law must always serve the felt and real needs of the times, and in order to give effect to those needs, the Botswana Sectional Titles Act was enacted in 1999. It is based on the South African Sectional Titles Act of 1986, as amended, which repealed the 1971 Act. In this thesis, a comparative analysis of the South African and Botswana sectional titles law is made to determine whether and, if so, how the existing position in the South African sectional titles law could aid interpretation and application of the sectional titles legislation of Botswana, specifically pertaining to aspects of establishment of sectional title schemes. The main focus is on the legislative provisions of both jurisdictions. However, South African case law is also considered. Landownership and sectional titles is discussed, to determine whether sectional ownership is genuine ownership. This entails a discussion on the publicity principle, which in the case of land is normally achieved by means of registration in the Deeds Registries. The thesis analyses the Botswana and South African statutes on the requirements and procedures involved in the establishment of sectional title schemes to bring to light any shortcomings that may exist in either of the two statutes. An understanding of the shortcomings of the South African statute is relevant to the interpretation and application of the Botswana statute. An examination of the procedural aspects of establishment of a sectional title scheme, as well as the roles of the parties involved in the establishment thereof is undertaken, so as to identify consequences that may ensue if they fail to comply with the requirements of either of the statutes. Consequently, a comparative analysis on the effect of registration of the sectional plan and opening of the sectional title register is made. Although it is not suggested that the Botswana Act should be completely similar to the South African Act, as Botswana may have its own peculiar circumstances, suggestions as to the amendment of the Botswana statute are made. Amendments would make the Botswana Act even more flexible, and would open up the possibilities of development to achieve greater access to land. Further more improvements to the Act will have to be made, some before its implementation, and some after a period of application of the Act, as real practical problems become apparent. / AFRIKAANSE OPSOMMING: Die konsep van deeltitel was tot dusver in Botswana se gemenereg onbekend weens die maxim superficies solo cedit, wat nie aparte eienaarskap van woonstelle in 'n gebou erken nie. In hierdie tesis word 'n vergelykende studie gedoen van die Suid-Afrikaanse Wet op Deeltitels en die gelyknamige Wet in Botswana om te bepaal of, en indien wel, hoe die bestaande posisie in die Suid-Afrikaanse Wet op Deeltitels kan help met die interpretasie en toepassing van die deeltitel wetgewing van Botswana, veral waar dit gaan oor die vestiging van deeltitelskemas. Alhoewel daar nie voorgestel word dat die wet in Botswana identies aan die Suid-Afrikaanse wet moet wees nie, (Botswana het te make met ander omstandighede) word voorstelle aan die hand gedoen vir die wysiging van die wet in Botswana. Hierdie wysigings sal die wet meer buigsaam maak en daar sal meer moontlikhede wees vir ontwikkeling wat groter toegang tot grond sal bewerkstellig. Verder sal daar verbeterings aan die wet aangebring moet word nadat dit eers in werking getree het en die werklike probleme kop uitsteek.
