Le fraternel : registres et modalités d’émergence dans les groupes de formation et les groupes thérapeutiques / Fraternal : registers and modes of emergence in training and therapeutic groups

Ce travail de recherche explore les processus de liaisons conduisant à l’émergence des liens fraternels dans les groupes thérapeutiques et les groupes de formation. A partir d’une approche psychanalytique, nous cherchons à définir le fraternel en tenant compte des trois espaces intra-, inter- et trans- subjectifs. Les études menées au sujet des liens familiaux nous permettent de postuler que la fratrie peut servir de modèle pour décrire les processus groupaux par lesquels des liens fraternels se tissent entre des enfants qui ne sont pas familiers. Dans une première partie, les propositions théoriques exposées sont illustrées par différents objets culturels tels que des mythes et des contes. Dans une deuxième partie, nos hypothèses sont mises à l’épreuve des dispositifs de groupes de formation à l’université et d’un dispositif de groupe thérapeutique. Le traitement des observations à l’aide de « la méthode des quatre colonnes » permet d’explorer notre matériel clinique de recherche. D’une façon transitoire, nous avons vu apparaître quatre registres différents du processus de liaisons conduisant à la formation de liens fraternels. Ces registres sont organisés par des mécanismes inconscients. Ils mettent en jeu des fantasmes originaires et les défenses psychiques présidant une transition des registres archaïques vers un registre intermédiaire puis un registre fraternel. Dans une troisième partie, nos hypothèses sont discutées sur trois niveaux (culturel, théorique et clinique) et reprises pour définir les quatre registres des liens fraternels dans les différents dispositifs de groupe. / The presented research explores the processes of links leading to the emergence of fraternal bonds in therapeutic groups and training groups. From a psychoanalytic approach, we seek to place the issue in a fraternal definition which takes into account the intra-, inter- and trans- subjective space. Drawing on studies of family ties, we postulated that siblings can serve as a model for describing group processes by which fraternal links forged between children who are not familiar. In the first part, exposed theoretical propositions are first compared to the results of analyzes made upon different cultural objects such as myths and tales. In the second part, our hypotheses are tested by a setting of groups of university training and a therapeutic group. Treatment of observations using "the method of the four columns" allows the exploration of our clinical research material. On a transitional basis, we saw the emergence of four different registers of the process of connections leading to the formation of fraternal ties. These records are organized by unconscious mechanisms. These involve primal fantasies and psychic defenses presiding over a transition of archaic records towards an intermediate register and finally to a fraternal register.

The rise and fall of fraternal methods of social insurance : a case study of the Independent Order of Oddfellows of British Columbia sickness insurance, 1874-1951

Most descriptions of the rise of the Canadian welfare state emphasize that traditional social welfare institutions were always inadequate and it took the Depression to demonstrate the need for better, more comprehensive arrangements. Beyond the enlightening influence of the Depression, the rise of current welfare state institutions is attributed to the efforts and influences of politicians, unions, social reformers, and intellectuals. This myth about the rise of the welfare state has been so widely accepted that there has been little effort expended in examining pre-welfare state social insurance arrangements. The findings of this case study of the Independent Order of Oddfellows of British Columbia (IOOFBC) sickness insurance indicate that before 1930 there was an extensive formalized system of social insurance. The study also reveals that rather than having demonstrated the inadequacies of the traditional welfare mechanisms, the severe economic conditions of the Depression devastated the existing social insurance arrangements which created the impetus for the rise of the welfare state. Up until 1930, membership in a fraternal organization, like the Independent Order of Oddfellows (IOOF), was one of the most important sources of sickness/health insurance and life insurance throughout the world. Fraternal insurers were able to provide insurance at a lower cost than commercial insurers through their non-profit motive and their use of screening and peer monitoring practices to alleviate problems of adverse selection and moral hazard. Until 1930, 13% of the population in British Columbia had sickness/health insurance coverage through fraternal membership. Critics of fraternal insurance argued that while fraternal insurers may have had low costs, they led a financially precarious existence due to their hazardous pricing practices. An analysis of IOOFBC lodges for the period 1891 to 1950 reveals that this is not a good explanation of the decline of fraternal insurance. Even with the most hazardous of pricing practices, IOOFBC lodges had almost no probability of being bankrupted by high claims. Early in lodge operations, surplus revenues were invested in assets, like the lodge hall, which generated revenues that subsidized lodge operations and benefit payments. Given that fraternal insurers were viable, why was fraternal membership not an important source of insurance coverage after the Depression? An analysis of the memberships of four IOOFBC lodges over the period 1891 to 1980 reveals that the devastation of fraternal memberships in the Depression promoted the growth of alternative (commercial and government) insurance arrangements. In the 1930s, more workers than ever before were without sickness insurance coverage. As the probability of suspension for non-payment of dues tripled, workers would have had little expectation of remaining in a fraternal membership long enough to collect sick benefits that were primarily needed after age 45. Through and after the Depression, IOOFBC lodges abandoned their insurance functions. Lodge memberships had "aged" so much between 1930 and 1945 that the increased lodge benefit liabilities made reforming the IOOFBC's beneficial systems impractical. If changes were not made to the IOOFBC subordinate lodges' dues and benefits, lodges would have had to potentially draw on their assets to meet their inflated obligations. Over the nineteenth and early twentieth centuries they blossomed around the English speaking world, attracting millions of members. Combining archival research and quantitative methods, this thesis is the first multi-national economic history of the friendly societies and fraternal associations. How effective were these organisations as insurers? Were they able to overcome the problems of moral hazard and adverse selection? Were they significant in generating 'social capital'? How were they affected by the emergence the welfare state?

O presente estudo tem como objetivo estudar a importância do laço fraterno na constituição do sujeito e quais são suas repercussões no ambiente intensivista pediátrico, tendo em vista que alguns hospitais realizam visitas dos irmãos às crianças internadas. É contemplada a importância da fratria na construção da subjetividade, além do valor da fraternidade e dos laços de amizade e solidariedade constatados a partir da experiência clínica e corroborado pelas discussões clínicas recentes. / [en] Fraternal relationships among siblings as well as fraternity itself have been an object of study by many contemporary psychoanalysts. The objective of this study is to analyze the importance of fraternal bonds and its repercussion in the pediatric intensive-care environment, considering that some hospitals support children s visits to hospitalized siblings. This paper also addresses the importance of the fraternal relationships in the construction of subjectivity in addition to the value of fraternity, friendship and solidarity bonds which were observed from clinical experience and confirmed by recent clinical discussions.

Dans le cadre d’une médecine de plus en plus technique et spécialisée, les médecins coopèrent de plus en plus ensemble au cours du parcours de soin du patient. Les relations confraternelles entre médecins sont réglementées partiellement par le Code de déontologie. Dans la pratique, il existe parfois des difficultés pouvant altérer la qualité de vie au travail des médecins. À partir de ce constat, l’objet de ce travail est de proposer une réflexion éthique pour ces relations. Après une approche méta-éthique pour définir quelle théorie éthique pourrait aider à conceptualiser l’agir relationnel confraternel, il nous est apparu que l’éthique des vertus était la plus appropriée pour répondre à des situations archétypales de difficultés pratiques. La première partie de ce travail fonde une éthique de la vertu dans le cadre d’une téléologie du bon soin choisie par le médecin au moyen de son intégration au corps médical, ce qui forge son identité médicale, dans un processus que nous appelons iatrologie et s’intégrant dans une approche de l’ordre narratif. La deuxième partie du travail développe trois vertus particulières de la confraternité, désignées par un travail de sciences humaines autour de l’analyse d’entretiens et de questionnaires réalisés auprès de jeunes médecins. Enfin, la troisième partie pose la question des rapports des vertus entre elles en vue de mettre en exergue une vertu architectonique. À partir des notions d’amitié et de justice, abordées à partir d’Aristote et de MacIntyre, nous proposons une vertu de justice se spécifiant dans ce que nous appelons la correction confraternelle. / Within the framework of a more and more technical and specialized medicine, the doctors cooperate more and more together during the course of care of the patient. The fraternal relations between doctors are partially regulated by the Code of ethics. In the practice, there are sometimes difficulties that can affect the quality of life at work for doctors. From this observation, the purpose of this work is to propose an ethical reflection for these relationships. After a meta-ethical approach to define which ethical theory could help to conceptualize confraternal interpersonal skills, it appeared to us that the ethic of virtues was the most appropriate to answer archetypal situations of practical difficulties.The first part of this work establishes an ethic of virtue as part of a teleology of the good care chosen by the physician through his integration into the medical profession, what forges its medical identity, in a process we call iatrology and which is integrated into an approach of the narrative order. The second part of the work develops three particular virtues of the fellowship, appointed by a work of human sciences around the analysis of conversations with young doctors and questionnaires filled in by young doctors. Finally, the third part raises the question of the relation of the virtues to one another to highlight an architectural virtue. From friendship and justice, approached from Aristotle and MacIntyre, we propose a virtue of justice that is specified in what we call the confraternal correction.

Essa tese investiga a função do semelhante enquanto um fundamento para o trabalho com grupos heterogêneos. A função do semelhante diz respeito à relação horizontal entre as crianças e suas especificidades, ou seja, o que somente uma criança poderia fazer por outra. Iniciamos com uma apresentação sobre as principais problemáticas que envolvem o campo grupal, que é o dispositivo clínico que utilizamos como campo da pesquisa. Em seguida, apresentamos e discutimos os grupos heterogêneos em Educação Terapêuticas. São grupos compostos por crianças com diferentes questões subjetivas, o que aumenta a diversidade e as possibilidades de encontros inéditos entre as crianças. Após, realizamos um levantamento teórico do lugar do semelhante para três autores psicanalistas de referência: Sigmund Freud, Jacques Lacan e René Kaës. Esse levantamento permitiu uma caracterização da função do semelhante, feita também através de três vinhetas apresentadas e analisadas. Com isso, queremos sustentar a tese de que, nos grupos heterogêneos, a função do semelhante seria um operador de leitura e de intervenção que pode nortear o trabalho da coordenação e possibilitar a construção da alteridade. Verificamos que, do encontro com o semelhante, é que pode surgir um espaço para a alteridade / This thesis investigates the function of the similar while a foundation for work whit heterogeneous groups. After, we carried out a theoretical survey of the place of the similar for three authors psychoanalysts: Sigmund Freud, Jacques Lacan and René Kaës. This survey allowed a characterization of the function of the similar, also made through three vignettes presented and analyzed. Therefore, the jungian concepts and systemic approach have been used as base to analyze fictitious experiences shown by the movies in three stories: The Wedding Date , In her Shoes and Mystic Pizza . The methodological choice has been made based on theoretical verification that relationship among female siblings brings up several feelings and members of sisterhood tend to guard themselves or attack each other on their speeches, but the authors tend to be more authentic regarding their feelings in their work compared to biographies. In this analysis, it has been verified that this unique bond provides better conditions for the shadow to act out, but it depends on the ego´s structure which allows a better or worse shadow integration, fostering family and sibling´s pattern changes, or keeping up with rooted patterns / Este trabalho tem como objetivo compreender a vivência sombria no vínculo fraterno feminino, observando ainda a importância da irmã no processo de individuação da mulher. Para isso utilizou-se do referencial teórico junguiano e da abordagem sistêmica, buscando aproximações através da análise de vivências ficcionais trazidas pelo cinema em três filmes: Muito Bem Acompanhada , Em seu Lugar e Três Mulheres, Três Amores . A escolha da metodologia deu-se pela constatação teórica de que a relação entre irmãs suscita muitos afetos e que os membros da fratria tendem a resguardar-se ou atacar-se em relatos, mas nas obras culturais, os autores tendem a atribuir a seus personagens grande autenticidade de afetos, mais que em biografias. Verificou-se na análise que o vínculo diferenciado dá sustentação para a atuação da sombra, mas que é a estrutura de ego que permite ou não maior ou menor integração da sombra, promovendo mudanças de papéis familiares e na fratria, ou levando à perpetuação dos padrões arraigados
