É estruturada por diferentes pontos de referência: acontecimentos, pessoas/personagens e lugares, para complementar a memória e para que os acontecimentos não sejam esquecidos. Existe uma relação entre memória e as instituições que produzem e acumulam documentos. Os centros de documentação estão presentes em vários cenários e áreas do conhecimento, trabalham com informações especializadas e os documentos neles presentes possuem a função de informar. Por meio desse trabalho busca-se responder o seguinte problema de pesquisa: como o Centro de Documentação e Memória "Pe. Antão Jorge" e os documentos ali presentes contribuem para a construção da memória? A pesquisa é de natureza qualitativa, caracterizada como exploratória, cujo método aplicado é o estudo de caso. O universo de pesquisa é constituído pelo Centro de Documentação e Memória "Pe. Antão Jorge" e os documentos presentes, localiza-se na cidade de Aparecida, estado de São Paulo. Com resultado desse trabalho, compreendemos que o CDM possui uma grande quantidade de documentos que conta uma história de quase 300 anos, que possibilita conhecer a cidade de Aparecida, o Santuário Nacional e a devoção à Nossa Senhora Aparecida, está relacionado à ideia de identidade, memória e construção da memória. / Abstract: The memory is associated with the action of remembering and forgetting and is a field that involves disputes and conflicts. It is structured by different reference points: events, people/characters and places to complement the memory and that the events are not forgotten. There is a relationship between memory and the institutions that produce and accumulate documents. Resource centers are present in various scenarios and areas of knowledge, work with specialized information and documents contained therein have to inform function. M.A. (Social Work) / The importance of the role of the social worker in the Court in connection with the preparation of pre-sentence reports necessitated an investigation into the expertise with which these reports are being submitted to the Court. " The objectives of the study were to evaluate deficiencies as well as positive elements of pre-sentence reports prepared by social workers and, were necessary, to lay down guide lines to ensure that a- properly prepared and authoritative product could be presented to the user (the Court). During the investigation a qualitative approach was followed and the study can be described as exploratoryjdescriptive. Firstly, a study of literature was done through which deficiencies in and guidelines for pre-sentence reports were pointed out. Secondly, structured and semi-structured interviews were conducted with 23 interested parties. These interested parties were probation officers and supervisors of three public service departments. Regional Court Prosecutors and Magistrates of the Johannesburg Magistrate's Office, a State Advocate of the office of the public defender, and five legal practitioners who are regularly exposed to pre-sentence reports during proceedings for extenuation of sentence. The "purpose of the semi-structured interviews was to determine the SUbjective experience by interested parties of pre-sentence reports. A brief and structured interview schedule was utilised in order to gather certain general information which was useful for the interpretation of data. It was found that deficiencies and positive elements obtained from literature accorded to a large extent with the perception of interested parties of pre-sentence reports. After deficiencies and positive elements in pre-sentence reports had been abstracted, reasoned conclusions were drawn. In addition guide lines were drawn for the preparation and writing up of pre-sentence reports.

M.A. / Information plays a very significant role in the economic development of the enterprises. It is increasingly used as a corporate resource to improve productivity, efficiency and effectiveness. This means that information in the form of records, is a strategic weapon to gain competitive advantage for the enterprise that creates and receives them. The study explores the problems that are facing terminology in the study of information management and records management. The problems that exist in terms of terminology are so intense that there is yet no universally acceptable definition for records and records management. The basic elements of records management are explored in attempt to position records management as an information management function. It demands records as vital information resources be managed throughout their life cycles, at a strategic level in much the same way as other economic resources such as people, money and land. In managing records effectively, the education and training of records managers is regarded as imperative. In order to test the information management function of records management, a survey is administered within Enterprise ABC to observe the records management practices amongst the records staff. The findings of the survey reveal that the records management practices within Enterprise ABC are chaotic in the sense that there are no standards and procedures that govern the flow of records within the enterprise.

Cancer profile in an urban hospital of the Eastern Cape Province

Magister Public Health - MPH / The availability of information on profile and trends of cancer in South African populations is important for the development of appropriate cancer control strategies, as well as monitoring the efficacy of the existing cancer control programmes. Yet, generally there is a scarcity of systematically analysed reports on hospital cancer cases in South Africa, even for urban hospitals. The aim of this study was to describe the cancer profile of patients diagnosed at Frere Hospital‟s Oncology and Radiation Department and estimate the incidence of cancer among Buffalo City (BFC) urban area residents, for the 19-year period 01 January 1991 to 31 December 2009 based on the clinical administrative data system maintained by the department. The study was a descriptive case series study based on a retrospective review of Frere Hospital‟s Oncology and Radiation Department patient records from 1991 to 2009. Permission was obtained to retrieve records of cancer cases for the 19-year period from the database. Data were extracted from the customized administrative system to an excel spread sheet. Variables for each case retrieved included: socio-demographic details; age at diagnosis, sex, race, place of residence and medical aid information, tumor information; site and date of diagnosis. Data cleaning incorporated techniques such as checking of completeness and accuracy of patient information details. Dates were formatted into month-day-year sequence and checked so that the date of birth precedes the date of diagnosis of the patient and the date last seen. Age less than zero and greater than ninety nine was replaced as missing. Geographical areas were coded according to the South African Population Census. Duplicates and cases with missing diagnosis were excluded.

Developing an Electronic Hospital Trigger for Bleeding – The Ottawa Hospital ETriggers Project

Background Bleeding can be an adverse side effect from hospital treatment. The aim was to develop an electronic identification method for patients who are bleeding within The Ottawa Hospital. Methods A retrospective exploratory cohort (N=1000) was used to identify potential candidate markers for bleeding. Electronic data were extracted to evaluate candidate identifiers. Data which were associated with bleeding events were assessed in a model derivation cohort (N=700). Multivariate analysis was used to establish the best model for identifying all bleeding events and in-hospital bleeding events. Results Overall 38% of the exploratory cohort had bleeding. In the model derivation set 29% had bleeding. The model predicting all bleeding included number of transfusions, admitting specialty, re-operation and endoscopy (C-statistic 0.82, 95%CI 0.79-0.86). The model predicting in-hospital bleeding included number of transfusions, admitting specialty and re-operation (C-statistic 0.78, 95% CI 0.73-0.84). Conclusion We have developed two models for identifying hospital bleeding events from The Ottawa Hospital electronic medical records. These should be validated prospectively on the hospital-wide population.

The purpose of this thesis is to verify the SAP RM configuration's possibility to fulfill the requirements of MoReq2 standards. The theoretical part revealed the characteristics of Records Management, MoReq2 standard and SAP Records Management software. Records Management is introduced in the connection with Enterprise Content Management and Document Management. There are other standards mentioned, but Moreq2 due to its importance for the practical part of the thesis is being described closely. SAP Records Management component is explored from its conceptual and technological aspects. The practical part is composed of analysis of MoReq2 requirements in SAP RM environment and of the description of the system configuration, which enables MoReq2 requirements' fulfillment. The configuration is described in details only if SAP RM fulfills MoReq2 requirements.

Study of the archival record and its context : meaning and historical understanding

The claim of archivists to be a scholarly profession is dependent upon their ability to methodically study and understand the meaning of the records in their care. Without such contextual information about the record as the name of its creating agency, the reason for its creation, and the authority by which it was created, archivists and researchers are in a poor position to assess the value and validity of its informational content. Without knowledge of the relationship of the record to other record series, they are likely to overlook additional supporting and/or contradictory documentation and thereby miss a part of the truth they seek. This thesis is directly concerned with the means by which archival sources might be assessed to determine the value of the historical evidence they contain. It proposes a conceptual framework by which study of the original, primary, and secondary meanings of the archival record might be approached. Examples are drawn from close examination of the records of the Corporation of the City of Nanaimo surviving from the period 1875-1904. While acknowledging that extensive study of the significance of documentation might be impossible for archivists in their daily work, this thesis concludes that closer attention must be paid to sources documenting the contextual environment of the record.
