Το πληροφοριακά συστήματα βρίσκουν ολοένα και αυξανόμενες εφαρμογές στο χώρο της υγείας. Μία από αυτές αποτελεί ο Ηλεκτρονικός Φάκελος Υγείας (Η.Φ.Υ), ο οποίος επεκτείνει τον παραδοσιακό ιατρικό φάκελο ασθενή σε δυνατότητες και λειτουργίες. Η παρούσα εργασία ασχολείται με το πεδίο αυτό και συγκεκριμένα με το σχεδιασμό, τη μελέτη και την υλοποίηση ενός Η.Φ.Υ. εξειδικευμένο σε πνευμονολογικές παθήσεις. Η εφαρμογή που θα υλοποιηθεί είναι διαδικτυακή και βασίζεται σε σύγχρονες τεχνολογίες όπως είναι η PHP και η SQL. Περιλαμβάνει τη διαχείριση των πληροφοριών ενός ιατρικού φάκελου αλλά και ένα προηγμένο σύστημα διαχείρισης χρηστών. Παράλληλα δε θα αναλυθούν από θεωρητική σκοπιά όλες οι πτυχές του πεδίου μελέτης, ενώ θα γίνει και μία προσπάθεια επικαιροποίησης στη σημερινή πραγματικότητα στην Ελλάδα. / The Information Systems can find more and more application in the field of Health. One of these applications is the Electronic File of Medicine (EFM) which expands the traditional file of Medicine in the abilities and the operations for each patient. This project has been written for this item and specifically for the planning, the studying and the creating of a new Electronic File of Medicine expert in pneumonologic diseases. This application which is going to be created is network-based and based on modern technologies such as the PHP and SQL. This application includes the information management for a file of Medicine and the previous User Management System.

Η Τηλεϊατρική τα τελευταία χρόνια αναπτύσσεται ραγδαία. Η άσκηση και η παροχή ιατρικών υπηρεσιών από απόσταση με τη χρήση της πληροφορικής και των τηλεπικοινωνιακών τεχνολογιών μπορεί να αποβεί σωτήρια για την ανθρώπινη ζωή, κυρίως σε περιπτώσεις απομονωμένων περιοχών (π.χ. νησιά, ορεινά χωριά), όπου η πρόσβαση σε ιατρικές υπηρεσίες είναι χρονοβόρα ή ακόμη και ανέφικτη. Ισχυρές γλώσσες προγραμματισμού, όπως η JavaME, οι οποίες υποστηρίζονται από τα σύγχρονα κινητά τηλέφωνα, δίδουν τη δυνατότητα σχεδιασμού και ανάπτυξης χρήσιμων εφαρμογών Τηλεϊατρικής. Στην παρούσα εργασία θα παρουσιαστεί η τεχνογνωσία που απαιτείται προκειμένου να μετατραπεί το κινητό τηλέφωνο σε μια συσκευή καταγραφής, επεξεργασίας, απεικόνισης και αποστολής του ηλεκτροκαρδιογραφικού σήματος. Tο ηλεκτροκαρδιογραφικό σήμα μετατρέπεται σε ηχητικό, μέσω της διαμόρφωσης εύρους, προκειμένου να αναγνωρισθεί από το κινητό τηλέφωνο και έπειτα μετατρέπεται πάλι σε ηλεκτροκαρδιογραφικό, μέσω της αποδιαμόρφωσης, προκειμένου να απεικονισθεί και να επεξεργαστεί περαιτέρω από το κινητό τηλέφωνο. Τέλος, θα παρουσιαστεί το αντίστοιχο λογισμικό που κατασκευάστηκε ώστε να επιτελεσθούν οι παραπάνω λειτουργίες. / Telemedicine is being rapidly developed. Practice and provision of medical services from distance via the use of information and telecommunication technology can prove lifesaving, especially in cases of isolated places (i.e. islands, mountainous villages), where access to medical services is time-consuming or infeasible. Powerful programming languages, such as JavaME, which are supported by the modern mobile phones, gives the possibility for designing and developing of useful telemedicine applications. In this thesis, it will be presented the know-how that is required in order to transform the mobile phone into a device able to capture, process, display and transmit the electrocardiographic signal. The electrocardiographic signal is transformed into a sound signal, through modulation, with a view to be recognizable by the mobile phone and then it is transformed again into an electrocardiographic signal, through demodulation, with a view to be further processed by the mobile phone. Also, it will be presented the software that has built so as to carry out these functions.

Ανάπτυξη εφαρμογών τηλεϊατρικής με βιοσήματα

Η παρούσα διπλωματική εργασία εκπονήθηκε στα πλαίσια της ενασχόλησης με Έργο ανάπτυξης Δικτύου Τηλεϊατρικής. Το Δίκτυο Τηλεϊατρικής συνίσταται από φορητές μονάδες ικανές να λαμβάνουν και να μεταδίδουν τα ζωτικά σήματα των ασθενών σε κάποιο Κέντρο Συντονισμού. Το κέντρο αυτό, στελεχωμένο από κατάλληλο ιατρικό προσωπικό, έχει τη δυνατότητα αφού αξιολογήσει τα λαμβανόμενα δεδομένα, να δώσει χρήσιμες συμβουλές παροχής πρωτοβάθμιας βοήθειας στον ασθενή, έως ότου μεταφερθεί σε κάποιο νοσοκομείο. Η διπλωματική εργασία επικεντρώνεται στο σχεδιασμό και στην ανάπτυξη της εφαρμογής Θέσεως Εργασίας, η οποία είναι βασικό τμήμα του Κέντρου Συντονισμού. Στελεχώνεται από εξειδικευμένους ιατρούς και έχει ως σκοπό, χρησιμοποιώντας όλα τα παρεχόμενα ιατρικά δεδομένα, να προσφέρει τη βέλτιστη προ-νοσοκομειακή φροντίδα στους ασθενείς, οι οποίοι βρίσκονται στον τόπο του φορητού τερματικού. Μέσω του ολοκληρωμένου Δικτύου Τηλεϊατρικής, η Θέση Εργασίας προσπελαύνει όλα τα στοιχεία ενός τρέχοντος περιστατικού τηλεϊατρικής, όπως ζωτικά σήματα, ιστορικό από το Ολοκληρωμένο Πληροφοριακό Σύστημα (Ο.Π.Σ.) των νοσοκομείων, φωτογραφίες καθώς και άλλα ιατρικά δεδομένα. Χρησιμοποιεί, επίσης, την υπηρεσία τηλεδιάσκεψης για να έχει οπτικοακουστική επαφή με το φορητό τερματικό και για να εκμεταλλευτεί το συμβουλευτικό ρόλο των Περιφερειακών Αιθουσών Τηλεδιάσκεψης. Η Θέση Εργασίας μετά το πέρας των περιστατικών ενημερώνει τους ιατρικούς φακέλους των ασθενών στο Ο.Π.Σ., ενώ έχει τη δυνατότητα να εκτελέσει σωρεία λειτουργιών εκτός σύνδεσης, ώστε να ανατρέξει σε χρήσιμα δεδομένα του Δικτύου, όπως παλιά περιστατικά, και να ολοκληρώσει την εξυπηρέτηση των περιστατικών. Η ανάπτυξη της εφαρμογής πραγματοποιήθηκε σε γλώσσα προγραμματισμού Visual Basic .NET με χρήση της πλατφόρμας Visual Studio 2005 Professional Edition. Η υλοποίηση περιλαμβάνει τη δημιουργία Κεντρικής Βάσης Δεδομένων Τηλεϊατρικής, για την οποία επιλέχθηκε η Oracle 10g, ενώ για τη διαχείριση κι εποπτεία της βάσης χρησιμοποιήθηκαν οι εφαρμογές Oracle Client 10g και Oracle JDeveloper 10g / The present thesis was carried out through participation in a project concerning the development of a Telemedicine Network. The Telemedicine Network consists of mobile units capable of obtaining and transmitting patients’ vital signs to a Coordination Center. This centre is occupied by specialized medical personnel and has the capacity, after evaluating the received data, to provide useful healthcare advice prior to the patients’ transfer to a hospital. This thesis focuses on the design and development of the Treatment Workstation application, which is a basic part of the Coordination Center. It is operated by doctors and aims to provide, through the use of all the available information, an optimum pre-hospital care to the patients placed near the mobile unit. Utilizing the resources provided by the Telemedicine Network, the Treatment Workstation accesses all the data of a current telemedicine incident, e.g. vital signs, medical history by the hospitals’ Integrated Information System (I.I.M.), photos and other medical information. It also uses the teleconference service to establish an audiovisual communication with the mobile unit and to take advantage of the advisory contribution of the Peripheral Teleconference Room. After the incidents’ termination, the Treatment Workstation updates the patients’ medical files at the I.I.M. and is also able to perform a series of offline actions, in order to access Network’s useful data (e.g. old incidents) and to complete pending incidents. The application was developed in Visual Basic .NET through the platform Visual Studio 2005 Professional Edition. The implementation includes the creation of a Central Telemedicine Database, using Oracle 10g, while the management and supervision of the database was performed with the use of Oracle Client 10g and Oracle JDeveloper 10g.

Darbo tikslas: Įvertinti pacientų ir medikų pasitenkinimą ir paklusnumą naudojantis ir teikiant telekardiologijos paslaugas. Tyrimo uždaviniai: 1. Įvertinti pacientų paklusnum�� naudojantis telemedicinos paslauga. 2. Palyginti gydytojų ir pacientų pasitenkinimą taikyta telekardiologijos sistema klinikinėje praktikoje. 3. Įvertinti pacientų pasitenkinimą gydymu, teikiant telemedicinos paslaugą. Tyrimo metodai: Tyrime dalyvavo Kauno medicinos universiteto klinikų Kardiologijos skyriaus pacientai, kuriems pastebėti vainikinės širdies ligos požymiai ir paskirta AKJO operacija. Pacientai buvo suskirstyti į tiriamąją ir kontrolinę grupes. Tiriamosios grupės pacientai naudojosi telemedicinos paslauga, o kontrolinė grupės pacientai – nesinaudojo. Taip pat dalyvavo telemedicinos paslauga besinaudojusius pacientus konsultavę gydytojai-kardiologai. Tyrimo klausimynams naudota Likerto skalė. Duomenys apdoroti ir buvo analizuojami naudojant statsitinį duomenų analizės paketą SPSS 12.0. Hipotezės apie dviejų požymių nepriklausomumą buvo tikrinamos naudojant Mann-Whitney U testo kriterijaus p reikšmę (p<=0,05). Rezultatai: Pilotinis tyrimas atskleidė naujųjų technologijų silpnąsias puses, kurios turėtų būti tobulinamos, norint trečio lygio ligoninėse teikti kokybiškas ir kvalifikuotas sveikatos priežiūros paslaugas. Išvados: Taikant telekardiologijos paslaugas pacientams sergantiems išemine širdies liga buvo lengviau tiksliai įvykdyti visus gydytojo nurodymus, 83% respondentų nekilo... [toliau žr. visą tekstą] / The aim of this study was to evaluate patients and medics satisfaction and obedience using and rendering facilities of cardiology. The tasks: 1. To evaluate patients obedience using facilities of telemedicine. 2. To compare patients and medics satisfaction rendering cardiological system during clinical practice. 3. To evaluate patients satisfaction of treatment, rendering facilities of telemedicine. Methods of research: In the research were participated patients from Kaunas medicine university clinics Cardiology department, who had symptoms of coronary disease of heart and who was prescribed AKJO surgery. Patients were dispended to exploratory and share majority groups. Patients of exploratory group were using facilities of telemedicine, patients of share majority group weren’t. Cardiologists, who consulted patients, who were using facilities of telemedicine, participated too. Licert’s scale was used to compose questionnaire. Information was processed and analyzed using statistical pack SPSS 12.0 which analyses information. Hypothesis about independence of two features was verified using criterion mean p (p<=0,05) of Mann-Whitney U test. Results: Pilot study uncovered blind sides of newest technologies, which should be improved to render qualified facilities. Conclusion: Rendering facilities of telecardiology for patients with ischemic disease of heart were easy to fulfill all medics’ prescriptions accurate, 83 percent of respondents did not have any problems using gadget... Many medical imaging applications have been developed so far; however, many of them do not support collaboration and are not remotely accessible (i.e., Telemedicine). Medical imaging applications are not practical for use in clinical workflows unless they are able to communicate with the Picture Archiving and Communications System (PACS). This thesis presents an approach based on a three-tier architecture and provides several components to transform medical imaging applications into collaborative, PACS-based, telemedical systems. A novel method is presented to support PACS connectivity. The method is to use the Digital Imaging and COmmunication in Medicine (DICOM) protocol and enhance transmission time by employing a combination of parallelism and compression methods. Experimental results show up to 1.63 speedup over Local Area Networks (LANs) and up to 16.34 speedup over Wide Area Networks (WANs) compared to the current method of medical data transmission.

An applied model for implementation of innovative IT-solutions for telehealth into the healthcare system

Today, new technologies are introduced to the market every day, and constantly changing our way of living. Especially in the healthcare sector, the change process is approaching a point where doctors can benefit from the use of, for example, connected portable reading devices instead of paper-based medical record systems. The information and communication technology is promoting the evolution of a new pathway of care delivery, a paradigm shift that alters the fundamental relationship between a doctor and its patient. The concept is defined as telehealth and formulates the provision of care at a distance and provides the possibility to treat patients in their home environment instead of at the hospital. This master’s thesis has been performed on the request of Cenvigo AB, a company active in the implementation of new IT-solutions into the healthcare and eldercare. Cenvigo AB are the owners of the Parkinson’s Digital Assessment (PANDA) application. The application has been developed through research at Dalarna University and Uppsala University Hospital.  This project will initiate the launch of PANDA and also create a model for implementation of innovative IT-solutions into the healthcare systems. The model is founded in a theoretical framework and shaped with interviews related to the implementation of technology with a focus on telehealth applications. Interviews has been performed with healthcare professionals, technology developers and users to acquire a complete picture and opinions regarding the introduction of innovation in healthcare today. From the acquired information, a model is formulated as a stepwise and chronological linear process were identified key activities are included to promote a successful implementation process. The model is connected to the practice through the implementation of PANDA. In the process of implementing PANDA into the Swedish healthcare system, a collaboration with the innovation centre at Uppsala University Hospital as a healthcare organization stakeholder, has been initiated. The model consists of five phases; Assessment, Dissemination, Adoption, Implementation and Continuation. The phases are seen as transitional steps in the innovation process, critical barriers to overcome towards a successfully implementation in a mainstream routine setting. Each phase includes a number of activities and to achieve progression in each phase, these activities must be performed in order to advance to the next phase. In the case of PANDA, the process of progression has passed assessment and are currently involved in activities related to the dissemination phase. The purpose of the model is to be used both for existing and future applications in the segment of medicine technology sector. The structure of the model is designed to promote a co-design or a common value principle of development and practice regarding an innovation. By connecting actors from both technology and healthcare in close relationships the actual needs of healthcare professionals could more effectively be identified and developed into a solution, a result from the amplification of a two-way engagement. The outmost aim is to serve as a catalysing factor, complementing the implementation models of healthcare in Sweden today. Through this study, a need for facilitating the implementation process of new technology into the healthcare systems has been identified. This model offers the necessary input that many technology companies lack. The recommendation to Cenvigo AB is to continue to develop the model during the last step in the process of launching PANDA, and parallel use this model as a business model mainly for technology start-ups and larger foreign companies that has not yet established pathways into the Swedish healthcare system. / <p>TVE 14 036</p>

The Role of Telemedicine in the Management of Stroke Patients and Knowledge Sharing among Health Care Providers in Afghanistan

Focusing on the potential use of telemedicine among other efforts for better treatment of stroke patients, this study explored the role of telemedicine in the management of stroke patients and knowledge sharing among health care providers in Afghanistan. To this end, fourteen semi-structured interviews were conducted with physicians, specialists, neurologists, and decision makers from the Afghan Ministry of Public Health (MoPH). Actor-network and diffusion of innovations theories provided a theoretical framework for this exploratory qualitative study. The study was intended: 1) to find out about the major challenges and problems associated with managing stroke patients in Afghanistan; 2) to explore the perceptions of Afghan health professionals about the application of telemedicine as a means of improving the delivery of health services for stroke patients in Afghanistan; and 3) to understand the perceived barriers to knowledge sharing and to ascertain the potential role of telemedicine in knowledge sharing among health care providers in Afghanistan. The findings of this study demonstrated that almost all of the participants were optimistic about the potential positive role telemedicine could play in the management of stroke patients and knowledge sharing among health care providers in Afghanistan. Some important existing organisational, socio-economic, geographical, security, and cultural barriers to the management of stroke patients and knowledge sharing among health care providers in Afghanistan were revealed. To the best knowledge of the researcher, there has been no study of this kind conducted in Afghanistan yet; thus, the findings of this study will likely contribute to the development of health communication in the context of Afghanistan, and could likely be used as a resource for future research about the applications of telemedicine in various medical specialities.
