Causes and predictors of 30‐day readmission in patients with syncope/collapse: a nationwide cohort study

Background Syncope accounts for 0.6% to 1.5% of hospitalizations in the United States. We sought to determine the causes and predictors of 30‐day readmission in patients with syncope. Methods and Results We identified 323 250 encounters with a primary diagnosis of syncope/collapse in the 2013-2014 Nationwide Readmissions Database. We excluded patients younger than 18 years, those discharged in December, those who died during hospitalization, hospital transfers, and those whose length of stay was missing. We used multivariable logistic regression analysis to evaluate the association between baseline characteristics and 30‐day readmission. A total of 282 311 syncope admissions were included. The median age was 72 years (interquartile range, 58-83), 53.9% were women, and 9.3% had 30‐day readmission. The most common cause of 30‐day readmissions was syncope/collapse, followed by cardiac, neurological, and infectious causes. Characteristics associated with 30‐day readmissions were age 65 years and older (odds ratio [OR], 0.7; 95% confidence interval [CI], 0.6-0.7), female sex (OR, 0.9; 95% CI, 0.8-0.9), congestive heart failure (OR, 1.5; 95% CI, 1.2-1.9), atrial fibrillation/flutter (OR, 1.3; 95% CI, 1.3-1.4), diabetes mellitus (OR, 1.2; 95% CI, 1.2-1.3), coronary artery disease (OR, 1.2; 95% CI, 1.2-1.3), anemia (OR, 1.4; 95% CI, 1.4-1.5), chronic obstructive pulmonary disease (OR, 1.4; 95% CI, 1.3-1.4), home with home healthcare disposition (OR, 1.5; 95% CI, 1.5-1.6), leaving against medical advice (OR, 1.7; 95% CI, 1.6-1.9), length of stay of 3 to 5 days (OR, 1.5; 95% CI, 1.4-1.6) or >5 days (OR, 2; 95% CI, 1.8-2), and having private insurance (OR, 0.6; 95% CI, 0.6-0.7). Conclusions The 30‐day readmission rate after syncope/collapse was 9.3%. We identified causes and risk factors associated with readmission. Future prospective studies are needed to derive risk‐stratification models to reduce the high burden of readmissions. / Revisión por pares

Someone once told me: ""In order to write well, you have to write what you know"" - this piece of advice seems obvious and simple and yet - as it turned out, it was the most difficult piece of advice to process and follow. The reason: what I know was that I was a young, female, Cape Malay filmmaker. My experiences, has largely taken place within the Cape Malay community. Religious documentaries aside, I had never before seen any representation of myself, or may community on screen. The task I had set myself seemed impossible. I had researched Thirld world theory in cinema and the weight of expectation that I felt afterward seemed a burden too hard to carry. This was not because I was in unchartered territory, but because this path has been walked by many before me: African-American filmmakers; Non-resident Indian filmmakers; Pilipino-American filmmakers; Mexican-American filmmakers; Afro-Brazilian filmmakers; Asian-American filmmakers. The list goes on and on. The one common thread is that these people felt a need to provide another point of view other than the ones seen in mainstream film and television. Their films have been like voices for the token characters that we've seen in countless mainstream films.

Vitellogenin induction in male Japanese medaka exposed to the chemotherapeutics: oxytetracycline, Romet-30, and copper sulfate

The specificity of a medaka vitellogenin (Vtg) enzyme-linked immunosorbent assay (ELISA) kit was tested on adult hybrid tilapia to determine if vitellogenin could be successfully measured in tilapia using this method. Adult hybrid (Orechromis niloticus x Oreochromis mossambicus x Oreochromis aureus) tilapia were injected with 17â-estradiol (E2) at 5 ìg/g body weight (b.w.) in two separate experiments and exposed to 2.5 ìg/g b.w. E2 through aquaria water in a third experiment. All fish were exposed to solvent controls (50/50 PBS/ethanol, or 59/41 PBS/ ethanol) proportional to the volume of estradiol solvent used. Experiment 1 was terminated due to a decrease in fish health and unexpected mortalities. Experiment 2 was continued though there was again a decrease in fish health and mortalities before experiment was completed. According to one-way ANOVA, Experiment 2 treatment groups were significantly different (P<0.05); however, Tukey's pairwise comparisons showed no differences between treatment groups. Experiment 3 results showed statistical differences between some treatment groups (P<0.001). However, maximum vitellogenin levels reached 1250 ng/mL Vtg, which were much lower than induced vitellogenin levels from literature (0.002 mg/mL to 6 mg/mL). Typical induction trends should be as follows: male control<female control< induced male<induced female; however, these trends were not observed in this study. For these reasons, the medaka vitellogenin ELISA kit was determined not specific to tilapia vitellogenin. This research was performed due to the fact that tilapia are farmed worldwide, and to increase research with these fish, in order to better understand their response to xenobiotic chemicals, easier methods are desired for measuring their vitellogenin. Vitellogenin induction from exposure to the chemotherapeutics: oxytetracycline, Romet-30®, and copper sulfate was evaluate using Japanese medaka (Oryzias latipes). Medaka were exposed to E2 (0.0552 ìg/L) in aquaria water as a positive control. Medaka were fed oxytetracycline medicated feed at 0.0174 g, 0.0348 g, and 0.087 g, and Romet-30® medicated feed at 0.0261 g, 0.0522 g, and 0.1305 g daily for 11 days and 5 days as recommended. Medaka were exposed to copper sulfate at 1.85 mg/L for one hour (for 1 day, 7 consecutive days, and every three to five days for a total of 7 doses). Vitellogenin levels were measured using a sandwich ELISA. As expected, Vtg levels were induced from exposure to E2 (P<0.01); however, exposure to the chemotherapeutic compounds did not induce elevated Vtg levels. Alternately, copper sulfate exposure for one hour for 1 day did show a significant decrease in vitellogenin (P<0.01). Biological variation accounted for large portions of the variation within treatment groups in medaka vitellogenin in fish exposed to chemotherapeutics (as high as 41.01%). From this research we suggest that these chemicals do not have estrogenic properties capable of inducing vitellogenin in medaka. / Master of Science

Estas explicações foram levantadas com base em dois elementos explicativos encontrados na literatura, que corresponde: a explicação institucionalista e a contingencial e em cima destas será elaborada a resposta sobre o problema em tela. Este trabalho ao dar atenção a este tema traz luz a um problema ainda não investigado pela ciência política brasileira e abre um leque para a emergência de uma nova agenda de pesquisa na Ciência Política brasileira. / In this dissertation I will investigate the Brazilian party framework configuration in the period after the revolution of 1930 and I aim to seek for explanations about this party rearrangement. These explanations were raised in connection with two explanatory elements found in the literature: the institutionalism and contingency explanations, over these two theories the answer to the research problem will be given. This work pretends to give attention to a very important problem that wasnt investigated by Brazilian political science and consequently this will open a new research agenda inside Brazilian political science.
